1.The research about relationships among the balance between nurse's work and family, the nursing practice environment and demission intention

Objective To determine the relationships among the balance between nurse's work and family,the nursing practice environment and demission intention.Methods A total of 351 nurses were recruited from 5 hospitals in Zhengzhou by convenience sampling method.They were investigated using the Questionnaires for balance between nurse's work and family,the nursing practice environment and demission intention.Results The mean scores of the balance between nurse's work and family,the nursing practice environment and demission intention was (3.09±0.46),(2.86±0.48),(2.51 ±0.57).The predictors of demission intention of nurses were rational allocation of resources,age,work disturbs family,and family promotes work.Conclusions The levels of the balance between nurse's work and family,the nursing practice environment and demission intention are all in the medium.The balance between nurse's work and family and the nursing practice environment have direct positive influence on demission intention.
2.The construction of a nursing layer classification standard system based on Delphi method

Objective To establish the nurse layer classification standard of Henan province.Methods Nursing experts were selected within the scope of Henan province,nursing layer classification standard system was established through two rounds of experts enquiry with Delphi method.Results According to two rounds of consultation,the index system was divided into five layers:the base layer,growth layer,the professional layer,backbone layer and the experts level,which corresponded to the five classes:N0 assistant nurses,N1 registered nurse,N2 primary nurse,N3 specialized nurses/nursing managers,N4 clinical nursing specialist/nursing researchers/nursing management experts.Various levels of nurses had corresponding requirements and responsibilities framework.The various levels of nurses had corresponding requirements.Conclusions This study built in Henan province has good concentration,such as the representation of the expert advice and high credibility,which can provide decision-making reference for the nurse station management of Henan province.
3.The association of fasting insuline and insuline resistance with benign prostatic hyperplasia

Objective To reveal the effect of fasting insuline(FINS) and insuline resistance(IR) in the process of benign prostatic hyperplasia(BPH). Methods One hundred and seventeen outpatients( ≥60 ys)with BPH from geriatric department were enrolled into the study. The patients were divided into groups according to their FINS and insulin resistance index (HOMA-IR). The indices of BPH, including volume of prostate ( PV ),prostate specific antigen( PSA ), international prostate symptom score (IPSS), course of BPH were analyzed in both groups. Results The PV ( [ 56. 46 ± 26. 88 ] ml vs [ 44. 84 ± 17.66 ] ml, P = 0. 017 ) and the course ( [ 18. 00 ± 6. 91 ] years vs [ 13.93 ± 7. 74 ] years, P = 0. 031 ) were significantly greater in BPH combined hyperinsulinemias(HINS) group than the BPH with normal FINS group;but we found no significant differences in the comparisons of serum PSA level or IPSS between two groups. The PV( [54. 17 ± 25.38 ] ml vs [42. 26 ±14. 15]ml,P =0. 004)and the course([ 16.58 ±7. 65] years vs [13.49 ±7. 59] years,P = 0. 036) were also significantly greater in BPH combined insuline resistance gruop than the insulin sensitivity group, again we found no significant differences in the comparisons of serum PSA level or IPSS between two groups. Conclusion FINS and IR are risk factors of progressed BPH and can promote the progress of BPH.
4.Research on the relationship among nurses′ perceptions of patient safety culture, nursing practice ;environment and the report barrier of nursing adverse events

Objective To explore the relationship among nurses′perceptions of patient safety culture, nursing practice environment and the report barrier of nursing adverse event. Methods Totally 513 registered nurses from six hospitals in Zhengzhou were investigated with Patient Safety Culture Assessing Scale, Work Environment Questionnaire and Report Barriers Questionnaire. Results The mean scores of nurses′perceptions of patient safety culture, nursing practice environment and report barriers of nursing adverse events were (3.85±0.55), (3.84±0.53) and (2.47±0.55). The nurses′perceptions of patient safety culture were positively correlated to nursing practice environment (r=0.393,P<0.01), negatively correlated to report barriers of nursing adverse events (r=-0.372,P<0.01). The influencing factors of nurses′perceptions of patient safety culture were colleague relationship, punitive culture, allocation of medical resources, marital status, quality control, significance of report, gender and education, which could explain 33.8 percent for the variances. Conclusions The level of nurses′perceptions of patient safety culture is higher than the average level. The nursing managers should build supportive nursing practice environment and the non-punitive reporting system of nursing adverse events. Cultivate the beliefs and attitudes of nursing safety and focus on the impact of occupational stress to patient safety in order to enhance safety nursing.
5.The research progress of nurses hierarchical management

In order to improve the utilization efficiency of nursing human resources, stable nursing troop and enhance the quality of care, at present, the post management experiences for nurses of domestic and international are adopted by many hospitals and certain results are achieved. Study about nurses stratified management is reviewed, the approach, basis, effects, features and the advantages and disadvantages of hierarchical management experiences for nurses in different countries and regions is summarized and some thinking and prospect in studying is proposed in this article, as well as the follow-up research, to explore a constructive policy of further promoting the development of the level management for nurses.
6.CRISPR-mediated downregulation of PD-1 expression on T cells

Objective To investigate the feasibility of using clustered regularly interspaced short palindromic repeats ( CRISPR)-mediated genome editing to downregulate the expression of programmed cell death protein 1 (PD-1) on primary T cells by using a lentivirus delivery system. Methods Lentivirus vec-tors pLentiCRISPR A1-A6 containing different PD-1 genomic DNA sequences as single guide RNA ( sgRNA) for Cas9 targeting were constructed individually. The lentivirus vectors were tranduced into primary CD4 T cells. Flow cytometry analysis was performed to detect the expression of PD-1 for evaluating the knockout ef-ficiency. Results The lentivirus vectors pLentiCRISPR A1-A6 carrying six different target sites were con-structed and respectively tranduced into primary CD4 T cells. The expression of PD-1 accompanied with the activation of T cells. Co-expression of CD25 and PD-1 was observed on activated T cells. All of the six sites could be targeted by Cas9, of which A2 and A6 sites were more efficient in knocking out the gene encoding PD-1 with a rate of 19% and 29%, respectively. Conclusion This study suggests that it is feasible to knock out the expression of PD-1 on primary T cells by using CRISPR.
7.Impacts of emotional health and quality of life on the cognitive functions of epileptics

Objective At present, the risk factors for cognitive impairment in epilepsy patients are not quite clear.This study was to explore the impacts of the clinical features, emotional health and quality of life (QOL) on the cognitive function of the adult patients with mild cognitive impairment (MCI).Methods Using the Montreal Cognitive Assessment (MoCA) and Mini Mental State Examination (MMSE) scales, we evaluated the cognitive functions of the 109 adult epileptics of the outpatient clinic of neurology in Jinling Hospital.We assessed their emotional health with Hamilton Depression Scale-24 (HAMD-24), estimated their QOL with Quality Of Life in Epilepsy-31 (QOLIE-31), and collected their baseline clinical data by questionnaire survey.Results There were 67 cases of MCI (61.5%) among the 109 patients.The residential area was the strongest predictor of MCI in the adult epileptics (OR=0.226, 95% CI: 0.082-0.627).Among other risk factors of post-epileptic MCI were the total scores of HAMD-24 (OR=0.770, 95% CI: 0.644-0.921) and QOLIE-31 (OR=0.712, 95% CI: 0.575-0.880), QOL (OR=1.070, 95% CI: 1.015-1.128), cognitive function (OR=1.120, 95% CI: 1.043-1.203), and social function (OR=1.103, 95% CI: 1.035-1.175).Conclusion The incidence of MCI is high in adult patients with epilepsy.The development and progression of post-epileptic MCI can be delayed by more emphasis on the evaluation of cognitive function, emotional health, and quality of life.
8.The relationship among job satisfaction,work engagement and organizational citizenship behavior of nurses

Objective To explore the relationship among job satisfaction,work engagement and organizational citizenship behavior (OCB) of nurses.MethodsA total of 497 nurses in 5 tertiary level general hospitals in Zhengzhou selected by convenience sampling,were investigated by Minnesota satisfaction questionnaire,nurses' OCB scale and the Utrecht Work Engagement Scale.ResultsThe scores of job satisfaction,work engagement,OCB were (78.63±11.34),(38.31±7.78),(106.33±10.26) respectively.Nurses' job satisfaction was positively correlated with work engagement and OCB(r=0.529,0.481,P<0.01),and work engagement was positively correlated with OCB(r=0.456,P<0.01).The job satisfaction and work engagement had a positive prediction effect on OCB(changed R2=34.8%),and work engagement played a partial moderation effect between job satisfaction and OCB,the mediating effect accounted for 30.77% of the total effect.ConclusionManagers should take measures to improve job satisfaction and work engagement to promote OCB of nurses.
9.Significance of intensive glycemic control on early diabetic nephropathy patients with microalbuminuria

Objective To investigate the therapeutic effect of intensive glycemic control on patients with early diabetic nephropathy. Methods A total of 41 type 2 diabetes patients who developed microalbuminuria were divided into two groups randomly. Patients in Group A received intensive glycemic control and the blood glucose in Group B was regularly controlled. Glycemic monitoring and control were followed for 12 weeks to observe the changes of microalbuminuria in both groups; meanwhile the levels of serum lipids and coagulation indices were also recorded. Results The urine albumin excretion rate (UAER) in Group A decreased significantly from (47.91±13.86)mg/24h to (35.31±14.56)mg/24h after 12 weeks (P<0.05), and this decrease was significantly greater than that in Group B. However, Group B had no significant difference in UAER decrease [(48.93±13.32)mg/24h to (40.48±19.62)mg/24h, P>0.05]. The decrease of triglyceride (TG) and low-density lipoprotein cholesterol (LDL cholesterol), and the increase of high-density lipoprotein cholesterol (HDL cholesterol) showed no significant differences (P>0.05). And the level of plasma fibrinogen (FIB) showed no significant decrease after 12 weeks, either (P>0.05). Conclusion Intensive glycemic control reduces the level of microalbuminuria and may ameliorate the progression of early diabetic nephropathy.
10.Effect evaluation of quality control circle on shortening DNT in patients with cerebral ischemic stroke

Objective To evaluate the application effect of the quality control circle(QCC) on shortening DNT in patients with cerebral ischemic stroke. Methods QCC activity group had been established and the reasons for the DNT extension in patients with cerebral ischemic stroke were analyzed so as to identify key improvement links and to formulate and to implement countermeasures. Fifteen cases with cerebral ischemic stroke in our hospital before the implementation of QCC from March to April in 2016 were selected as the control group, and 32 cases of patients with cerebral ischemic stroke after the implementation of QCC activities from May to August in 2016 were selected as the observation group. DNT in the two groups of thrombolytic cases were compared, and the more time consuming of emergency, blood test and signature communication were compared and optimized. Results The average DNT of patients with acute cerebral stroke in the observation group was significantly shorter than that in the control group, and the difference was statistically significant (P<0. 05). There were 5 cases with more than 110min DNT in the control group and no case with more than 110min DNT in the observation group, and the difference was statistically significant (P<0. 05). The average time consuming in emergency treatment, blood specimen test and signature communication in the observation group was significantly shorter than that in the control group, and the differences were statistically significant(P<0. 05). Conclusion Through the implementation of QCC can effectively regulate the emergency treatment process of stroke and shorten the DNT in patients with cerebral ischemic diseases.
