1.The expression and clinical significance of miR-98 in peripheral blood mononuclear cells in asthmatic children

Objective To explore the role of miR-98 in peripheral blood mononuclear cells (PBMC) in the pathogenesis and development of childhood asthma.Methods A total of 43 cases of asthmatic children and 30 cases of healthy controls were enrolled in the study.Peripheral blood mononuclear cells were isolated in both healthy subjects and asthmatic children in acute attack and remission stages.The expressions of miR-98 and interleukin-4(IL-4) and IL-13 mRNA were detected by real-time quantitative PCR.Results The miR-98 levels of asthmatic children in attack stage were significantly lower than those in remission stage and control group (P<0.01).The IL-4 and IL-13 mRNA levels of asthmatic children in attack stage were significantly higher than those in remission stage and control group (P<0.01).There was no significant difference of miR-98,IL-4 and IL-13 mRNA between asthmatic children in remission stage and the controls (P>0.05).Furthermore,a negative correlation was found between the expression of miR-98 and IL-5(r=-0.794,P<0.01) and between the expression of miR-98 and IL-13 mRNA (r=-0.804,P<0.01) in asthmatic children in attack stage.A positive correlation was also found between IL-4 and IL-13 mRNA in asthmatic children in attack stage (r=0.853,P<0.01).Conclusion The expression of miR-98 decreased in asthmatic children,and miR-98 might be involved in the pathogenesis and development of asthma.
2.The clinical value of multiple-slice spiral CT in apical hypertrophic cardiomyopathy with giant negative T wave

Objective To investigate the clinical value of multiple-slice spiral CT (MSCT) in identifying apical hypertrophic cardiomyopathy (AHCM) with giant negative T wave.Methods Sixteen patients with AHCM and giant negative T wave (AHCM group) underwent MSCT,electrocardiogram,echocardiography,coronary angiography and left ventriculography.Thirty patients without myocardial hypertrophy were enrolled as control group.Measurement results of two groups were compared.Results MSCT confirmed all patients with AHCM,but echocardiography missed 10 patients.In the end of diastole phase,left ventricular apical thickness (LVA) was (21.3 ± 3.6) mm and LVA/left ventricular posterior wall thickness (LVPW) was 2.2 ± 0.4 in AHCM group,which was (8.5 ± 1.6) mm and 0.9 ± 0.2 in control group.The level of LVA and LVA/LVPW in AHCM group were significantly higher than those in control group (P <0.01).Conclusion MSCT is a accurately diagnostic modality for AHCM with giant negative T wave,and the cardiac anatomy,function and coronary artery are also assessed simultaneously.

The epidemic of coronavirus disease 2019 (COVID-19) puts higher demands on critical care medicine. Lots of studies have been conducted to solve COVID-19-related problems. Therefore, we reviewed the annual progress for COVID-19-related issues including antivirals threapies, respiratory support and immunomodulatory therapies and other critical issues, including the effect of antibiotic on mitochondrial damage and its relationship with sepsis, the goal and direction of antimicrobial de-escalation, drug prophylaxis of constipation, bleeding in gastrointestinal disorders and management of critical illness in the informalization era and so on. We hope to provide reference for clinical and scientific research work of the intensivists.

Objective:
Schizophrenia (SCZ) is one of the most common and severe mental disorders. Modified electroconvulsive therapy (MECT) is the most effective therapy for all kinds of SCZ, and the underlying molecular mechanism remains unclear. This study is aim to detect the molecule mechanism by constructing the transcriptome dataset from SCZ patients treated with MECT and health controls (HCs).
Methods:
Transcriptome sequencing was performed on blood samples of 8 SCZ (BECT: before MECT; AECT: after MECT) and 8 HCs, weighted gene co-expression network analysis (WGCNA) was used to cluster the different expression genes, enrichment and protein-protein interaction (PPI) enrichment analysis were used to detect the related pathways.
Results:
Three gene modules (black, blue and turquoise) were significantly associated with MECT, enrichment analysis found that the long-term potentiation pathway was associated with MECT. PPI enrichment p-value of black, blue, turquoise module are 0.00127, <1×10-16 and 1.09×10-13, respectively. At the same time, EP300 is a key node in the PPI for genes in black module, which got from the transcriptome sequencing data.
Conclusion
It is suggested that the long-term potentiation pathways were associated with biological mechanism of MECT.

6. Prevalence of inpatients with liver disease in infectious diseases department of three comprehensive hospitals in Yunnan: a multi-center retrospective analysis

Objective:
To study the constitutional features of diseases spectrum of inpatients with liver disease in infectious diseases department of three comprehensive hospitals to provide resource allocation proposition for the construction of Department of Infectious Diseases.
Methods:
Inpatients data were extracted from the department of infectious diseases of three comprehensive hospitals (Kunming General Hospital of the People's Liberation Army, Yuxi People's Hospital and Dali People's Hospital) between January 2010 to December 2015, and were retrospectively analyzed. The distribution of patients with viral hepatitis (A, B, C, E) and severe liver disease (Severe hepatitis, cirrhosis, liver cancer) was further analyzed in hospitalized patients. Data were analyzed by one-way analysis of variance. A chi-squared test was used for comparison between groups. The change trends of disease constituent ratio in different years were checked by 2 test.
Results:
Liver disease, tuberculosis and AIDS were the three common diseases of three comprehensive hospitals in Yunnan, accounting for 58.61% of all admissions. However, an inpatients with liver diseases (17.25%, 3555/20606, 95% CI 16.73%-17.77%) were significantly lower than tuberculosis inpatients (33.98%, 7002/20606, 95% CI 33.34%-34.62%). An observations from different hospitals and at different time points showed that the proportion of patients with liver disease was lower than that of tuberculosis patients. OBJECTIVES:
Shelter hospital was an alternative way to provide large-scale medical isolation and treatment for people with mild coronavirus disease 2019 (COVID-19). Due to various reasons, patients admitted to the large shelter hospital was reported high level of psychological distress, so did the healthcare workers. This study aims to introduce a comprehensive and multifaceted psychosocial crisis intervention model.
METHODS:
The psychosocial crisis intervention model was provided to 200 patients and 240 healthcare workers in Wuhan Wuchang shelter hospital. Patient volunteers and organized peer support, client-centered culturally sensitive supportive care, timely delivery of scientific information about COVID-19 and its complications, mental health knowledge acquisition of non-psychiatric healthcare workers, group activities, counseling and education, virtualization of psychological intervention, consultation and liaison were exhibited respectively in the model. Pre-service survey was done in 38 patients and 49 healthcare workers using the Generalized Anxiety Disorder 7-item (GAD-7) scale, the Patient Health Questionnaire 2-item (PHQ-2) scale, and the Primary Care PTSD screen for the Diagnostic and Statistical Manual of Mental Disorders, 5th edition (PC-PTSD-5). Forty-eight healthcare workers gave feedback after the intervention.
RESULTS:
The psychosocial crisis intervention model was successfully implemented by 10 mental health professionals and was well-accepted by both patients and healthcare workers in the shelter hospital. In pre-service survey, 15.8% of 38 patients were with anxiety, 55.3% were with stress, and 15.8% were with depression; 16.3% of 49 healthcare workers were with anxiety, 26.5% were with stress, and 22.4% were with depression. In post-service survey, 62.5% of 48 healthcare workers thought it was very practical, 37.5% thought more practical; 37.5% of them thought it was very helpful to relief anxiety and insomnia, and 27.1% thought much helpful; 37.5% of them thought it was very helpful to recognize patients with anxiety and insomnia, and 29.2% thought much helpful; 35.4% of them thought it was very helpful to deal with patients' anxiety and insomnia, and 37.5% thought much helpful.
CONCLUSIONS
Psychological crisis intervention is feasible, acceptable, and associated with positive outcomes. Future tastings of this model in larger population and different settings are warranted.
