1.Therapeutic Observation of Acupuncture plus Chinese Herbs for Sensorineural Hearing Loss

Objective To observe the clinical efficacy of acupuncture plus Chinese herbs in treating sensorineural hearing loss.Method Eighty patients with sensorineural hearing loss were randomized into a treatment group and a control group, 40 cases in each group. The control group was intervened by conventional medication, while the treatment group by acupuncture in addition to the intervention given to the control group. The two groups were intervened once a day, 30 days as a treatment course. The serum interleukin (IL)-2, IL-6, soluble interleukin-2 receptor (sIL-2R) levels and pure tone hearing threshold value were observed before and after consecutive 2 treatment courses, and the clinical efficacies in the two groups were compared.Result The serum levels of IL-2, IL-6 and sIL-2R and pure tone hearing threshold value were significantly changed after the treatment in the two groups (P<0.05). There were significant differences in the serum IL-2, IL-6, sIL-2R levels and pure tone hearing threshold value between the two groups after the treatment (P<0.05). The total effective rate was 94.6% in the treatment group versus 84.2% in the control group, and the difference was statistically significant (P<0.05).Conclusion Acupuncture plus Chinese herbs is an effective approach in treating sensorineural hearing loss.
2.Effect of Acupuncture on Anxiety and Depression Due to Vertebrobasilar Insufficient Vertigo

Objective To observe the clinical efficacy of acupuncture in treating anxiety and depression due to vertebrobasilar insufficient vertigo (VBIV).Method Eighty-two patients with anxiety and depression due to VBIV were randomized into a treatment group and a control group, 41 cases in each group. The control group was intervened by conventional medication, while the treatment group was by acupuncture in addition to the treatments given to the control group. The Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), and Treatment Emergent Symptom Scale (TESS) were evaluated before and after intervention.Result The HAMA, HAMD, SAS, and SDS scores were significantly changed after intervention in both groups (P<0.05). After treatment, the HAMA score in the treatment group was significantly different from that in the control group (P<0.05). The TESS scores were significantly changed after treatment in both groups (P<0.01). After intervention, the TESS score in the treatment group was significantly different from that in the control group (P<0.01).Conclusion Acupuncture is an effective approach in treating anxiety and depression due to VBIV, and it can reduce the occurrence of adverse reactions.
3.Analysis of related factors on anastomotic leak after total mesorectal excision in middle and lower rectal cancer

Objective To investigate the reasons and prevention measures of anastomotic leak after total mesorectal excision (TME) in middle and lower rectal cancer.Methods Clinical data of 216 cases of middle and lower rectal cancer patients from March 2005 to March 2013 were retrospectively analyzed.Results All the 216 cases,anastomotic leak occurred in 19 cases,which from March 2005 to February 2009 surgery in 96 cases,13 cases of anastomotic leak,from March 2009 to March 2013 surgery in 120 cases,6 cases of anastomotic leak.Time of the anastomotic leak occurrence was postoperative 4-10 d with an average of 7.3 d.There was no correlation between the occurrence of postoperative anastomotic leak and gender,Dukes stage,histological type and incomplete intestinal obstruction (P > 0.05).However,there was correlation with the age,tumor distance from the anal margin lower end,anemia,hypoproteinemia,diabetes and the stapler hand-sewn anastomosis strengthen (P < 0.05 or < 0.0 1).Sixteen cases were healed by conservative treatment,3 patients with severe anastomotic fistula underwent transverse colostomy surgery to cure.Conclusions Anastomotic leak after TME in middle and lower rectal cancer is caused by a variety of factors,fine operation technique,good agreement,full-effective drainage and anal is the key factor to reduce the occurrence of anastomotic leak.Anemia,hypoproteinemia,diabetes and low anastomotic are the risk factors for the anastomotic leak,most cases of anastomotic leak are able to cured by conservative treatment,and if the conservative treatment fails,then choose the right time to colostomy.
4.Construction of recombinant adenoviral vector carrying the LEDGFp52 gene by homologous recombination in bacteria and its expression in vitro

AIM: To construct recombinant adenoviral vector carrying the LEDGFp52 gene by homologous recombination in bacteria and to detect its expression in vitro.METHODS: The LEDGFp52 gene was cloned to adenoviral shuttle plasmid pAdTrack-CMV. Then, the resultant pAdTrack-CMV-LEDGFp52 was cotransfected into BJ5 183 bacteria with the adenoviral backbone plasmid pAdeasy-1. The adenoviral plasmid carrying LEDGFp52 was generated with homologous recombination in bacteria, and the adenoviruses were produced in 293 cells. These 293 cells were then infected with adenoviruses, and the expression of LEDGFp52 was detected by CPE (cytopathic effect) and western blot.RESULTS: The titer of Ad-LEDGFp52 adenoviruses was up to 5×1012 pfu/L after proliferation in 293 cells. LEDGFp52 was expressed efficiently in 293 cells after infection.CONCLUSION: The recombinant adenoviruses vector expressing LEDGFp52 was constructed successfully and can be used in further gene transfection experiments.
5.Construction and identification of the eukaryotic expression vector carrying specific siRNA of LEDGF p52 gene

AIM: To construct and identify LEDGFp52 eukaryotic expression vector for RNA interference.METHODS: Recombinants were designed and established by targeting gene LEDGFp52 and plasmid pGensil-1 based on LEDGFp52 cDNA sequences of Genomes. Two pairs of oligonucleotides were synthesized according to the Tuschl principle and inserted into plasmid pGenSil-I to generate siRNA eukaryotic expression vector. DH5α strains were transformed, plasmids were extracted, and recombinant vectors were identified by the restriction map and the sequence analysis. The cultured cells were transfected by the recombinant plasmid (pGensil-1-RNA. LEDGFp52-1). At 48 hours after transfection, the whole cell protein was extracted, and the protein level was detected using Western blotting with mouse anti-human LEDGFp52 monoclonal antibody.RESULTS: Recombinant plasmids completely concord with the designs by the restriction map and the sequence analysis,the proteinlevel of LEDGFp52 was down regulated at 48hours after transfecting pGensil-1- LEDGFp52-1 expression vector into HeLa cells, the recombinant eukaryotic expression vectors were successfully constructed.CONCLUSION: siRNA recombinant can be successfully constructed by RNAi technique to inhibit the expression of LEDGFp52.
6.Construction of prokaryotic expression vector of rhLEDGFp52 gene、 inducing expression and purification of its protein

AIM: To construct the prokaryotic expression vector of rhLEDGFp52 gene,to obtain the rhLEDGF p52 protein.METHODS: rhLEDGFp52 gene was constructed into a prokaryotic expression vector pET30a (+) by recombinant DNA techniques and was identified by enzymatic digestion and sequence analysis. rhLEDGFp52 protein was induced expression by IPTG in E.coli BL21 (DE3), it was tested by Western blot and was purified by Ni-NTA His. Bind. Resin.RESULTS: We Successfully constructed the prokaryotic expression vector of rhLEDGFp52 gene and obtained its expression in E.coli BL21 (DE3),it was expressed in a soluble form and detected up to 34.63% of the total bacterial protein expressed in E.coli BL21 (DE3).Western blot analysis demonstrated that rhLEDGFp52 protein could spicifically integrate with LEDGF-ab. After purified by Ni-NTA His. Bind. Resin, the ultimate concentration of purified rhLEDGFp52 protein was 520μ g/ml and its purity was 87.93%.CONCLUSIONS: rhLEDGF p52 protein was obtained that provides an experimental basis for the further study of the biological function of rhLEDGFp52 protein.
7.Therapeutic Observation of Acupuncture plus Medication for Sudden Hearing Loss

Objective To observe the clinical efficacy of acupuncture plus medication in treating sudden hearing loss.Method Sixty patients with sudden hearing loss were randomized into a treatment group and a control group, 30 cases in each group. The treatment group was intervened by acupuncture plus medication, while the control group was intervened by medication alone. The two groups were treated once a day, 10 d as a treatment course. The pure-tone hearing threshold, plasma endothelin (ET) content, comparative and absolute counting of endothelial progenitor cells (EPCs) were observed before the treatment and after 2 treatment courses, and the clinical efficacies were compared. Result The pure-tone hearing threshold, plasma ET content, comparative counting and absolute counting of EPCs were significantly changed after the treatment in both groups (P<0.05). After the intervention, the pure-tone hearing threshold, plasma ET content, comparative counting and absolute counting of EPCs in serum of the treatment group were significantly different from those of the control group (P<0.05). The total effective rate was 92.6% in the treatment group versus 82.1% in the control group, and the between-group difference was statistically significant (P<0.05).Conclusion Acupuncture plus medication is an effective method in treating sudden hearing loss.
9.Clinical and MRI features in pediatric multiple sclerosis

ObjectiveTo investigate the clinical and MRI features of multiple sclerosis in children,including the clinically isolated syndrome (CIS) and relapse.MethodsIn total,16 cases of pediatric multiple sclerosis were included in this study.Of them,11 patients were female and 5 were male,with the mean onset age of 10.1 years.They were followed up for 4 months to 7 years and found to have 1-5 relapses.The clinical manifestations of CIS and relapse were analyzed by a pediatric neurologist.An experienced neuroradiologist reviewed the MRI images of CIS and relapse.Information on the location,size,and pattern of the lesions was gathered.The location of lesions included subcortical,central,and periventricular white matter,cortex,deep gray matter,brain stem,and cerebellum.ResultsCIS episode presented acute onsets in 13/16 cases,with symptoms of cortice in 10 cases and visual impairment in 6 cases.Relapse occurred in 14/16 cases within one year.The incidence of symptoms of cortice was less frequent and severe in the second episode of MS,whereas the visual impairment had a high incidence.All patients had full recovery after the last episode.MRI of CIS showed confluent subcortical white matter lesions in 13/16 cases,abutting on central white matter lesions.The most frequently involved brain part was the frontal lobe,followed by the parietal lobe.Cortical involvement was observed in 9/16 cases.In 6 cases,periventricular white matter lesions were detected.Bilateral deep gray matter was abnormal in 4 cases.Other abnormalities included brain stem lesions in 5 cases,cerebellum lesions in 3 cases,optic nerve involvement in 3 cases,and pyramidal tract lesions in 2 cases.MRI of relapse revealed more small lesions in the subcortial and periventricular white matter in the patients.In the second episode,only 2 cases presented cortical involvement.Lesions were found in the brain stem in 4 cases and in the cerebellum in 5 cases.Pyramidal tract involvement was identified in 6 cases,serving as “railway track sign”.ConclusionsCIS in children is characterized by large confluent lesions in subcortical white matter coalescing with central white matter.MRI of relapse showed distinctive “railway track sign”.It is important to integrate MRI and clinical features in the diagnosis of pediatric multiple sclerosis.
10.The Effects of Inhalation of Aerosolized Low Molecular Weight Heparin on the Clotting Time in Guinea Pigs and Bleeding Time in Mice

AIM:To study the effects of inhalation of aerosolized low molecular weight heparin(LMWH)and standard heparin(SH) on the clotting time and bleeding time in guinea pigs and mice.METHODS:Guinea-pigs and mice inhaled aersolized LMWH and SH for 7 days,and the clotting time in guinea-pigs and tail bleeding time in mice were observed.RESULTS:Compared with vehicle,LMWH 800IU/L,400IU/L and SH 800IU/L,400IU/L had no significant effects on the clotting time in guinea-pigs and tail bleeding time in mice(P>0.05);There were no significant differences between LMWH 800IU/L,400IU/L and SH 800IU/L,400IU/L(P>0.05).CONCLUSION:Inhalation of aerosolized LMWH and SH had no significant effects on clotting time and bleeding time in animals.
