1.Application of gabapentin in the treatment of neuropathic pain

As an anti-epilepsy drug,gabapentin plays an important role in the treatment of neuropathic pain through a variety of central and peripheral mechanisms.Studies have shown that gabapentin has better efficacy for post-stroke pain,spinal cord injury-related pain,postherpetic neuralgia,and pain in diabetic peripheral neuropathy.For mild adverse effects,good tolerance and lack of interactions,gabapentin is an ideal drug for the treatment of neuropathic pain.
2.Effect and safety of gamma globulin combined with glucocorticoid pulse therapy on serum CRP in children with refractory mycoplasma pneumonia

Objective To investigate the effect of gamma globulin combined with glucocorticoid pulse therapy in the treatment of children with refractory mycoplasma pneumonia in CRP and safety.Methods 46 cases of children with refractory mycoplasma pneumonia from February 2012 to February 2016 were selected and randomly divided into the control group and experimental group,23 cases in each group.On the basis of the regular treatment,the control group was given azithromycin,10mg/kg,qd,intravenous drip,on the basis of the control group treatment,the experimental group was treated with methylprednisolone 2mg/kg+human immunoglobulin 400mg/kg,qd,intravenous drip.The levels of serum C-reactive protein(CRP),white blood cell count(WBC),lymphocyte count(LC),therapeutic efficiency and safety were measured before and after treatment.Results After treatment,compared with the control group,the serum levels of CRP、WBC、LC were lower in the experimental group,the difference was statistically significant(P<0.05).The treatment efficiency of the experimental group(91.31%)was significantly higher than that of the control group(65.22%),the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion The gamma globulin combined with glucocorticoid pulse therapy can significantly reduce the serum levels of CRP,WBC and LC in children with refractory mycoplasma pneumonia,improve clinical efficacy and the safety were good.
3.Clinical observation on treatment of mycoplasma pneumonia in children by modified San'ao Decoction combined with Azithromycin

Objective To observe the clinical effect of Sangao Decoction combined with Azithromycin in the treatment of mycoplasmal pneumonia in children.MethodsFrom August 2013 to August 2014, 76 children with mycoplasma pneumonia were enrolled in this study.The patients were randomly divided into observation group and control group.The control group was treated with azithromycin alone, and the observation group was given the treatment of Sanao Decoction on the basis of the control group.Comparison of the two groups of clinical efficacy, fever time, cough time, tonsil disappearance time, lung rales disappear time, length of stay and the incidence of adverse reactions.ResultsThe effective rate was 94.37% in the observation group and 76.32% in the control group, which was statistically significant (P<0.05).(2.45±0.90) d, duration of coughing (5.37±1.67) d, duration of disappearance of tonsil (6.23±2.47) d, disappearance of rales (7.18±1.58) d, duration of hospital stay (10.26±1.65) d were shorter than the control group, which was statistically significant (P<0.05).The incidence of adverse reactions was not statistically significant.ConclusionThe effect of Sangao Decoction combined with azithromycin in the treatment of mycoplasma pneumonia in children can significantly shorten the symptoms and signs of the patients, the time of disappearance of the signs and the length of hospital stay, promote the rapid recovery of children, high safety, and have good clinical application value.

6.Clinical study on lipid-lowering therapy with simvastatin in elderly patients with lower extremity atherosclerosis disease

ObjectiveTo evaluate the therapeutic effects of simvastatin in elderly patients with lower extremity atherosclerosis disease. MethodsOne hundred and twenty-eight patients were divided into 2 groups:64 cases with intermittent claudication(group l)and 64 asymptomatic cases with intermittent claudication(group 2).Sixty-four normal cases were served as control group(group 3).THe patients of group l were given simvastatin for 18 months.The differences in levels of blood lipids and C-reactive protein(CRP),the size of plaque and the changes between pre-and post-treatments were observed among three groups. ResultsTC,LDL-C and CRP levels were significantly higher in group l than those in group 3 before the treatment[(6.3±1.7)mmol/L vs. (5.2±1.3)mmol/L,(3.8±1.3)mmol/Lυs.(2.1±1.3)mmol/L,(5.7±1.5)mmol/Lυs.(3.3±1.4)mmol/L,respectively,all P<0.05].TC,LDL-C and CRP levels significantly decreased in group0.053.There were no differences in plaque size and in plaque number in group 1 between pre-and post -treatments(both P>0.05),but the clinieal symptoms were relieved or disappeared.There were significant differences in plaque size,blood lipid level and CRP level in group 2 after 18 months treatment(all P<0.05). ConclusionsSimvastatin can delay the development of artherioscIerosis in the patients with lower extremity atherosclerosis disease.CRP can be used as an early index of disease development and illness monitoring.
7.Application and effectiveness evaluation of PBL in integrated nursing course for senior vocational nursing students

Objective In order to explore the application and effects of PBL in integrated nursing course for senior vocational nursing students. Methods Nursing students were randomly divided into the experimental group( 122 students) and the control group( 189 students). PBL was applied to the experimental group while traditional method was applied to the control group during the course. All the students fulfilled the Critical Thinking Disposition Inventory-Chinese Version (CTDI-CV) before and after the course. Theory and practical examinations were administrated after the course. Results The experimental group was better in theory and practical examinations than the control group. There were significant differences in total score and some subscale scores of CTDI-CV between the two groups after the course, and also between the beginning and the end of PBL for the experimental group. Conclusions The application of PBL in integrated nursing course can effectively promote the mastery of knowledge and skills, and can improve the critical thinking ability.
8.Incision choice of nipple-areola complex-sparing mastectomy

With the development of surgical techniques, the use of nipple-areola complex (NAC)-sparing mastectomy (NSM) has been increasing. Except for oncologic safety, the aesthetic outcome of surgery is greatly concerned by clinicians and patients. So far, there has been no consensus on the incision of NSM. This article reviewed the literature so as to provide some evidence for the selection of NSM incision.
9.Combining bone marrow mesenchymal stem cells with new nerve conduits for repair of peripheral nerve defects

BACKGROUND:With the continuous development and progress of medical model, the treatment and rehabilitation recovery from peripheral nerve defects needs higher requirements; therefore, stem cel culture-based nerve tissue engineering technology provides a new strategy for nerve defect repair. OBJECTIVE:To explore the feasibility of combining bone marrow mesenchymal stem cels and new nerve conduits to repair peripheral nerve defects. METHODS:Fifty New Zealand white rabbits, clean grade, were randomly divided into experimental group and control group, with 25 rabbits in each group. Animal models of peripheral nerve defects were made about 15 mm distant to the middle segment of the radius of the rabbit foreleg. At 1 week after modeling, bone marrow mesenchymal stem cels/new nerve conduit composite material was implanted into the defect in the experimental group; and in the control group, only bone marrow mesenchymal stem cels were transplantation. At 4 weeks after transplantation, a 5-mm nerve fiber was taken from the defect site, stained with hematoxylin-eosin and observed under scanning electron microscope. Density and diameter of regenerated nerve fibers were compared between the two groups. RESULTS AND CONCLUSION:Compared with the control group, the density of nerve fibers was significantly higher in the experimental group, but the diameter of nerve fibers was significantly lower (P < 0.05). There were more cels with good growth, large size and many processes on the surface of regenerated nerve tissues in the experimental group than the control group. Moreover, in the experimental group, interconnected cels were woven into a mesh and the axons were longer and thicker, both of which were the performance of typical neuron-like cels. Taken together, the bone marrow mesenchymal stem cels/new nerve conduit composite material can be used for peripheral nerve repair and present exact achievements.
10.Influence of chronic renal failure complicating obstructive sleep apnea syndrome on cognitive function

Objective To evaluate the change situation of cognitive function in the patients with chronic renal failure (CRF) complicating obstructive sleep apnea syndrome(OSAS) .Methods Sixty‐two inpatients with non‐dialysis CRF in our hospital from September 2013 to January 2015 were selected and divided into the group O (complicating OSAS ,31 cases) and group N(without complicating OSAS ,31 cases) ,and contemporaneous 33 individuals undergoing the physical examination were taken as the control group(group C) .The cognitive function was evaluated by the Montreal cognitive assessment (MoCA) .The apnea hypopnea index (AHI) and lowest pulse oxygen saturation(LSpO2 ) at night were measured with a portable sleep monitor .The comparative analysis was performed .Results In the group O ,there was 14 cases of patients(45 .16% ) whose MoCA total score was less than 26 points , while there was only 3 cases(9 .68% ) in the group N ,the difference was statistically significant (P<0 .05) .The MoCA total score , scores of visuospatial and executive function ,attention ,delayed recall and orientation in the group O and N were lower than those in the group C ,while the MoCA total score ,attention and delayed recall in he group O were lower than those in the group N ,the score of language was lower than that in the group N and C ,the differences were statistically significant (P< 0 .05) .The MoCA total score was negatively correlated with serum creatinine and AHI (r= -0 .374 ,r= -0 .408 ,P<0 .05) ,and positively correlated with the GFR level and night LSpO2 (r=0 .382 ,r=0 .519 ,P<0 .05) .Conclusion The patients with CRF complicating OSAS increase the risk of cognitive impairment .
