1.Observations on the Therapeutic Effect of Acupoint Application on Senile Hypertension of Yin Deficiency and Yang Excess Type

Objective To investigate the clinical efficacy of acupoint application in treating senile hypertension of yin deficiency and yang excess type.Methods Sixty-six old patients with hypertension of yin deficiency and yang excess type were randomly allocated to treatment and control groups, 33 cases each. The control group took nifedipine controlled release tablets and the treatment group received acupoint application in addition. The quality of life score was recorded in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.Results The total efficacy rate in reducing blood pressure was 90.0% in the treatment group and 84.8% in the control group; there was no statistically significant difference between the two groups (P>0.05). The total efficacy rate in improving the symptoms was 90.9% in the treatment group and 69.7% in the control group; there was a statistically significant difference between the two groups (P<0.05). There was a statistically significant pre-/post-treatment difference in the quality of life score in the two groups (P<0.05). There was a statistically significant post-treatment difference in the quality of life score between the treatment and control groups (P<0.05).Conclusions Acupoint application is an effective way to treat senile hypertension of yin deficiency and yang excess type. It can improve the quality of life in the patients.
2.Effects of sedation induced with dexmedetomidine versus propofol on intracranial pressure and cerebral oxygen metabolism in patients with permissive hypercapnia

Objective To compare the effects of sedation induced with dexmedetomidine and propofol on intracranial pressure and cerebral oxygen metabolism in patients with permissive hypercapnia. Methods Twentyfour patients with acute respiratory distress syndrome (ARDS) were randomly divided into 2 groups ( n = 12 each) :dexmedetomidine group (group D) and propofol group (group P) . Their APACHE Ⅱ scores were 11-18. The patients were mechanically ventilated (VT 5-7 ml/kg, RR 12-17 bpm, PEEP 6-10 cm H2O, FiO2 40-60%). PaCO2 was maintained at 50-65 mm Hg. Radial artery was cannulated for direct BP monitoring and blood sampling. Right internal jugular vein was cannulated and the catheter was advanced cephalad until jugular bulb. Continuous infusion of dexmedetomidine was started at 0.5 μg· kg-1· h-1 and TCI of propofol was started at target plasma concentration (Cp) of 0.4 μg/ml. The infusion of both drugs was gradually increased until Ramsay score (1= fully awake, 6 =asleep, unresponsive to loud verbal stimulus) reached 3,4,5. Transcranial Doppler monitoring was used to determine cerebral blood flow velocity (CBFV), pulsatility index (PI) and resistance index (RI) before administration of dexmedetomidine and propofol (T0 ) and at 30 min after the 3 levels of sedation were reached (T1-3) . Meanwhile blood samples were taken from radial artery and jugular bulb for blood gas analyses. Cerebral O2 metabolic rate (CMRO2), cerebral A-V O2 content differences (Da-jvO2) and cerebral O2 extraction rate (CERO2) were calculated .ResultsCBFV, PI, RI and CMRO2 were significantly decreased at T1-3 as compared with the baseline values at T0 in both groups. CBFV was positively correlated with CMRO2 in both group D (r = 0.80) and group P ( r = 0.76) . CBFV, PI and RI were significantly lower at T1-3 in group D than in group P. There was no significant change in Da-jvO2 and CERO2 at T1-3 as compared with the baseline values at T0 in both groups. Conclusion At different sedation levels, dexmedetomidine results in lower intracranial pressure than propofol and maintains the balance between cerebral O2 supply and demand in patients with permissive hypercapnia.
3.Fluorimetric analysis of camptothecin in Chinese herbal medicine common Camptotheca fruit.

Three-dimensional (3D) fluorescence spectra and thin layer fluorescence chromatogram of common Camptotheca fruit (CCF) crude drug, camptothecin (CPT) and 10-hydroxycamptothecin (HCPT) have been studied, and a novel fluorimetric method for determination of CPT in CCF crude drug has been established. In 3D fluorescence spectra, CPT presented 3 fluorescence peaks with excitation wavelengths lambdaex of 215, 255 and 365 nm, separately, and all peaks with emission wavelength lambdaem of 430 nm. HCPT presented 4 fluorescence peaks with lambdaex of 220, 265, 325 and 375 nm, separately, and all peaks with lambdaem of 555 nm. The fluorescence of CPT is much stronger than that of HCPT. Comparison of 3D fluorescence spectra and analysis of thin layer fluorescence chromatogram revealed that the main fluorescent component of CCF is CPT. HCPT and other components basically do not interfere with the fluorescence of CPT. Under the condition of pH 3.0-6.7, CCF aqueous solution can produce strong and steady fluorescence. Using methanol as solvent, the extracting solution of CCF was prepared, and diluted properly with water, then measured fluorescence intensity at lambdaex/lambdaem = 365/430 nm to determine the content of CPT. A linear calibration curve covered the concentration range 0.002 35-0.235 microg x mL(-1). The regression equation was IF = 9 + 30,844 c, with correlation coefficient r = 0.999 (n=9). The method has been applied to the analysis of CPT in a CCF sample, with a result of 0.127% and a spiked recovery rate of 102%. The reliability of the method has been verified by a thin layer chromatography-fluorescence scanning method. Experimental results demonstrated that this method can be used for quality evaluation of CCF crude drug.

5.Changes of nitric oxide synthase in hippocampus of rats tolerant to and dependent on the anticonvulsant action of flurazepam

Aim To study the role of nitric oxide by measuring the expression and activity of nitric oxide synthase in hippocampus of rats tolerance and dependence to the anticonvulsant action of flurazepam.Method Animal models of toleran and dependence to flurazepam were established. Nitric oxide synthase in hippocampus was measured using Western blot and immunohistochemistry, and its activity was evaluated using colorimetry. Result Hippocampal nNOS increased in rats tolerance to flurazepam. In rats dependence to flurazepam, hippocampal nNOS increased but without statistical significance. Total NOS activity increased dramatically both in flurazepam tolerane and dependene rats.Conclusion NO may play a role in rats tolerance and dependence to the anticonvulsant action of flurazepam.
6.Study on the effects of psychological intervention and multimodal drug therapy on analgesia and stress response in department of orthopedics

Objective To explore the effect of psychological intervention combined with drug multimodal therapy on orthopedic analgesia and its effect on stress response, and to provide reference for clinical application.Methods 88 cases treated with open reduction and internal fixation for lower limb fracture as the object of study were collected from March 2015 to March 2017 in Baoji traditional Chinese medicine hospital, which were received multi-mode drug treatment.According to whether to receive psychological intervention, 88cases were divided into the intervention group(45 cases) and the control group(43 cases).Postoperative pain was assessed by visual analogue scale(VAS) and simplified McGill questionnaire(SF-MPQ), the changes of plasma CRP and IL-6 levels were compared before and after operation.Results There was no significant difference in VAS score, SF-MPQ score, preoperative plasma CRP and IL-6 levels between the two groups.One and 24 hour after operation, VAS score and SF-MPQ score were higher than those at admission(P<0.05).The VAS score and SF-MPQ score in the intervention group were lower than those in the control group at different time points(P<0.05).One and 24 hour after operation, plasma CRP and IL-6 levels in the two groups were significantly higher than those before operation(P<0.05), the levels of plasma CRP and IL-6 in the intervention group were significantly lower than those in the control group(P<0.05).Conclusion Psychological intervention combined with multimodal drug therapy can effectively reduce the degree of postoperative pain and stress reaction, and is conducive to postoperative rehabilitation.
7.Effect of psychological intervention on quality of life of chemotherapy patients with advanced non-small cell lung cancer

Objective To explore the effect of psychological intervention on the quality of life in patients with advanced non-small cell lung cancer chemotherapy. Methods A total of 60 patients with advanced non-small cell carcinoma chemotherapy in our hospital from January 2015 to January 2017 were enrolled and divided into two groups according to different intervention methods. The control group was treated with Shengmai injection alone. The observation group was treated with psychological intervention + compound Kushen injection.The changes of scores of depression (SDS), anxiety (SAS) and pain (VAS), quality of life (QLQ-CCC) score and adverse reactions were recorded and analyzed before and after intervention in two groups. Results The clinical intervention effect of the observation group was better than that of the control group, the stability of the tumor focus, the degree of pain, the quality of life and other aspects were better than the control group, the difference between the two groups was statistically significant (P<0.05). Conclusion Patients with advanced non-smal cel lung cancer chemotherapy choose psychological intervention +compound sophora injection combined treatment effect is remarkable, which can effectively improve the patient's clinical symptoms and quality of life, is worth widely used clinically.
8.Analysis and consideration of mechanism reform in graduate student training

This article described the importance of mechanism refonil in the graduate student training and presented its contents.Also,It summarized the characteristics of the reforill proposals in some universities,and analyzed the problems required to attend and solve ia the refogln.The characteristics of the proposals included to set up innovative fund by university,to perform the student awarded police of tutor leading,to establish the scientific system in recruiting students.By analyzing the problems developed during the reform four measures were lannched,which included to establish evaluating system in assessing students,to create a good relationship between students and tutors,to improve the ability and morality of tutors,and to adjust the management method of research funding.It would be possible to reach the objective of perfecting the reform proposal during the implementation of the measures.
9.Determination of Natural Benzoic Acid in Different Mesona Chinensis Benth

Objective:To develop a method for the determination of natural benzoic acid in Mesona chinensis Benth. with different production places and storage time. Methods:HPLC was adopted, the column was an Agilent ZORBAX SB-C18 (250 mm × 4. 6 mm, 5 μm) column, the mobile phase was methanol-0. 02 mol·L-1 ammonium acetate (5∶95) at the flow rate of 1. 0 ml·min-1 , the col-umn temperature was 35℃, the detection wavelength was 230 nm, and the injection vocume was 10μl. Results:Benzoic acid showed good linearity over the concentration range of 0. 1-10. 0 μg·ml-1(r=0. 999 9). The average recovery was 99. 59%(RSD=1. 57%, n=9). Natural benzoic acid in powered Mesona chinensis Benth. was different because of various production places. The maximum content of natural benzoic acid in Mesona chinensis Benth. was 141. 8 mg·kg-1, while the minimum content was 73. 8 mg·kg-1. The content of natural benzoic acid in Mesona chinensis Benth. stored for two years was declined obviously. Conclusion: The method with simple operation is accurate and reproducible. It can provide a basis for the further study of Mesona chinensis Benth. resources, and provide a reference for the content determination of natural benzoic acid in other plants.
10.Clinical efficacy of improved method of inverted L-shaped incision combined with medial canthal tendon plication for moderate and severe epicanthus correction

Objective To explore the clinical effect of inverted L-shaped incision combined with medial canthal tendon plication for moderate and severe epicanthus correction.Methods Improved method of inverted L-shaped incision combined with medial canthal tendon plication were performed in 42 cases of moderate and severe epicanthus.The epicanthus covered more than 50% lacrimal caruncle with single-fold eyelids or unconspicuous double-fold eyelids.Among them,40 cases received doubleeyelid plasty simultaneously.Results The follow-up time of 38 cases ranged from 6 to 24 months.37 cases received double-eyelid plasty simultaneously.All the patients obtained satisfactory results aesthetically,the contours of two eyelid fissures and the double-eyelid radian looked more natural postoperatively.There was no reoccurrence of epicanthus.Mild scar proliferation was observed in all of the patients in the early 1-2 months of postoperative period and then would fade within three months.Conclusions It is effective to correct epicanthus by improved method of inverted L-shaped incision combined with medial canthal tendon placation.
