2.Clinical analysis of ketogenic diet for 25 children with infantile spasms

Objective To assess the efficacy,compliance and safety of ketogenic diet (KD) for children with infantile spasms,thus to provide an effective basis for the treatment of infantile spasms.Methods Twenty-five patients with infantile spasms were treated with KD.The gas chromatography-mass spectrometry and high performance liquid chromatography-mass spectrometry (HPLC-MS/MS) were used,except for metabolic diseases,to detect the blood routine,urine,stool,blood Iipids,liver function,as well as electrocardiogram (ECG),electroencephalograph (EEG),ultrasound of the urinary system and other related checks of all patients.The KD antiepileptic drug therapy was given after all children were admitted to hospital,and KD started directly in all patients without fasting.The efficacy compliance and safety were followed up in this study.Results KD maintenance therapy for 1 month,3 mouths,6 months were given to 25 cases (100%,25/25 cases),to 22 cases (88%,22/25 cases),and to 10 cases (40%,10/25 cases),respectively.Among them 7 patients (28%,7/25 cases) were in complete control of seizures within 1 month.Up to Class Ⅱ,Ⅲ efficacy level accounted for 32% (8/25 cases),12% (3/25 cases),respectively.Thirteen cases of children had varying digestive symptoms,10 cases of them developed asymptomatic hypoglycemia,and 9 cases of children had sleep trouble during KD treatment,but after symptomatic treatment,children's adverse reactions almost subsided.Only 4 cases of children discontinued treatment due to adverse reactions.All children's seizures were fully controlled.Conclusions The effect of KD is sure in the treatment of infantile spasms.Although KD has some adverse reactions,but it can be tolerated;serious body organ dysfunction treated by KD + antiepileptic drugs combination therapy for 1 to 6 months is not found yet,so the therapy is worthy of promotion clinically.
3.Observing the efficacy of azelastine hydrochloride nasal spray combined with budesonide nasal spray in treating persistent allergic rhinitis

OBJECTIVE To observe clinical efficacy of azelastine hydrochloride nasal spray plus budesonide nasal spray on persistent allergic rhinitiss(AR). METHODS 120 patients with persistent AR were devided randomly into group A(n=40), group B(n=40) and group C(n=40). Group A were treated with azelastine hydrochloride nasal spray and budesonide nasal spray, group B received only azelastine hydrochloride nasal spray and group C received budesonide nasal spray only, the treatment process were 4 weeks. Clinical efficacy were compared among 3 groups 4 weeks later. RESULTS 1. VAS scores: VAS scores of nasal symptoms in group A were lower than those both in group B and in group C after 4 weeks, the difference were statistically significant(P<0.05); 2. RQLQ scores: RQLQ scores in 3 groups were all significantly improved after 4 weeks, the RQLQ scores of sleep, nonnose/eye symptoms, nasal symptoms and emotions in group A were improved more significantly than those in group B and in group C, the difference were statistically significant(P<0.05); 3. Clinical efficacy: the significant effective rate and the total effective rate were 50.00% and 93.33% in group A, but were 30.00% and 75.00% in group B and 27.50% and 77.50% in group C, the differences between group A and group B or group A and group C were statistically significant(P <0.05). CONCLUSION Azelastine hydrochloride nasal spray plus budesonide nasal spray can rapidly relieve nasal symptoms and improve the life quality of the patients with persistent AR.

5.Clinical evaluation of Montelukast plus Budesonide nasal spray and Desloratadine citrate disodium in treating moderate and severe persistent allergic rhinitis.

OBJECTIVE:
To investigate the clinical efficacy of montelukast plus budesonide nasal spray and desloratadine citrate disodium tablets on moderate and severe persistent allergic rhinitis.
METHOD:
Senenty patients with moderate and severe persistent allergic rhinitis were devided randomly study group (n = 35) and control group (n = 35). The study group were treated with montelukast sodium tablets combined with budesonide nasal spray and desloratadine citrate disodium tablets for 4 weeks, the control group received budesonide nasal spray and desloratadine citrate disodium tablets for 4 weeks. Comparing visual analogue scale (VAS) scores of nasal symptoms, rhino conjunctivitis quality of life questionnaire (RQLQ) scores and total effective rate in two groups at baseline and after treatment.
RESULT:
(1) VAS scores of nasal symptoms: the difference of total nasal symptoms VAS scores or single nasal symptom VAS scores from both groups at 2 weeks and 4 weeks after treatment were statistically significant (P < 0.05); (2) RQLQ scores: the difference of RQLQ scores of 2 group's at baseline and 4 weeks after treatment were statistically significant, the difference of RQLQ scores about nasal symptoms in two groups at 4 weeks after treatment were statistically significant (P < 0.05); (3) The total effective rate was 94.29% in study group but 80.00% in control group, the differences were statistically significant (P < 0.05).
CONCLUSION
Montelukast plus budesonide nasal spray and desloratadine citrate disodium tablets can work together better on relieving clinical syptoms quickly and promoting the life quality of patients with moderate and severe persistent allergic rhinitis.

6.Choice of surgical treatment of low lumbar spine burst fracture

[Objective]To approach the unique characteristics and the choice of surgery of low lumbar spine burst fractures.[Method]In this group 23 patients with low lumbar spine burst fractures were included.They were classified into three types : type one: single vertebral body fracture,were treated with short segmental pedicel screw system.type two:multiple vertebral body fracture,treated with long segmental pedicel screw system;type three:type one or type two combined with sacral vertebra fractures,treated with lumbar-sacral-ilio combination pedicel screw system.The intervertebral height of the fractured vertebral body and lumbar lordosis were evaluated both before and after operation on X-ray.The spinal impingement of bone was evaluated on CT scan,the neurofunction was evaluated with ASIA system and the clinical outcome were evaluated with Charles system.[Result]All patients were followed up for 1~5 years.The intervertebral height of the fractured vertebral body increased from preoperative 34%(20%~50%) to postoperative 88%(70%~95%).The lumbar lordosis angel increased from preoperative 35??7?to postoperative 38??10?.The spinal impingement of bone area decreased from preoperative 55%(35%~90%)to postoperative 6%(0%~10%).ASIA score increased by 1-3 degree,the clinical outcome reached 85% according to Charles classification.[Conclusion]Instable low lumbar spine burst fractures should be treated with surgery as early as possible,according to different fracture segment,it can be treated with short-long segmental pedicel screw system or lumbar-sacral-ilio combination pedicel screw system respectively.
7.Clinical features and gene mutations analysis in 62 children with early - onset epileptic encephalopathy of un-known causes

Objective To study the clinical features and gene mutations of early - onset epileptic encephalo-pathy(EOEE)of unknown causes and to identify pathogenic mutations of EOEE by next generation sequencing. Methods The clinical data of 62 cases diagnosed with unexplained EOEE between June 2013 and June 2015 were ob-tained and analyzed. Specimens were collected from the selected children and their parents. Next generation sequencing was used to detect epilepsy - related genes,and Sanger sequencing was performed to verify the results and confirm the source of the parents,further to identify suspected pathogenic mutations of EOEE. Results Among 62 cases with unex-plained EOEE,37 cases(61% )were diagnosed as non - specific EOEE,17 cases(27% )with West syndrome,6 ca-ses(10% )with Dravet syndrome,1 case(1% )with Ohtahara syndrome,1 case(1% )with early myoclonic epileptic encephalopathy. The pathogenic mutations were not detected among 17 cases with West syndrome and the early myoclonic epileptic encephalopathy. Among 37 cases with non - specific EOEE,suspected pathogenic mutations were detected in 7 cases. Three cases of missense mutations for PCDH19 gene,1 case of frame - shift mutation and 1 case of splice site mutation for CDKL5 gene,1 case of denovo nonsense mutation for KCNQ2 gene,and 1 case of missense muta-tion for GRIN2A gene were detected. Among 6 children with Dravet syndrome,2 cases of frame - shift mutations and 1 case of missense mutation for SCN1A gene were detected,of which 2 cases were of frame - shift mutations,1 case was denovo mutation,1 case of missense mutation for SCN1A gene and 1 case of missense mutation for SCN1A combined with SCN9A gene were detected. One case of denovo nonsense mutation for STXBP1 gene was detected. After treatment, 22 cases with clinical seizures were under control,and 40 cases were out of control. Conclusions The clinical pheno-types for children with unexplained EOEE were varied. SCN1A,SCN9A,STXBP1,PCDH19,CDKL5,KCNQ2 and GRIN2A genes detected in China are in accordance with those reported internationally and some gene sites are denovo mutations which have not been reported. The SCN9A gene may be the new pathogenic mutation for Dravet syndrome. And the KCNQ2 gene nonsense mutation may be the lethal mutation.

10.Activated Caspase-8 Regulates Bcl-2 Expression in Fas-and Actinomycin D-induced Apoptosis in Bel-7402 Cells

Objective To investigate whether caspase-8 regulates Bcl-2 expression in Fas and Actinomycin D(AD) induced apoptosis in Bel-7402 cells.Methods Bel-7402 cells were divided into four groups randomly,that is:control,Fas group,Fas-AD group and Ac-IEFD-cho group,and treated with Fas-AD in the presence/absence of caspase-8 inhibitor Ac-IEFD-cho.The expressions of caspase-8 and Bcl-2 were determined with Western Blot and RT-PCR,respectively.Results Compared with control group and Fas group,after Bel-7402 cells were treated with Fas-AD,typical apoptosis morphology was clearly observed,the expression of activated caspase-8 increased significantly,and the Bcl-2 expression decreased apparently.However,after Bel-7402 cells were treated with Fas-AD in the presence of the caspase-8 inhibitor Ac-IEFD-cho,the expressions of activated caspase-8 decreased while the Bcl-2 expression increased.Conclusion Activated caspase-8 can regulate Bcl-2 in Fas-and AD-induced apoptosis in Bel-7402 cells.
