2.Clinical significance of the detection of myocardial enzymes and high sensitive C- reactive protein in chil-dren with herpangina

Objective To evaluate the clinical significance of myocardial enzymes and high sensitive C-reactive protein( hs-CRP) in children with herpangina. Methods A total of 80 children with herpangina( obser-vation group,n=80),at the same time,50 healthy children as control group(n=50)were assigned for the detec-tion of myocardial enzymes(AST,LDH,CK,CK-MB)and hs-CRP. Results The elevated CRP levels were found in 45(56. 25%)patients of observation group and 2(4%)patients of the control group,showing statistical significance(χ2 =31. 55,P<0. 01). Abnormal myocardial enzyme levels were observed in 59(73. 75%)cases, which included AST 51. 6%, CK 40. 1%, CK-MB 21. 4%, LDH 72. 2%. Abnormal myocardial enzyme levels were observed in 6(12%) cases of control group,which included AST l6%,CK 10%,CK-MB 4% and LDH 12%. The significant differences were observed between the two groups ( P<0. 01 ) . Conclusion Herpangina children may have different degrees of myocardial damage. Myocardial enzymes and hs-CRP are useful indicators for myocardial injury which may effectively provide reference for clinical diagnosis and treatment.
3.Clinical study of surface-enhanced laser desorption/ionization time-of-flight mass spectrometry technology in detecting the anxiety of patients with breast cancer

Objective To investigate the incidence of anxiety in patients with breast cancer,and further explore the surface-enhanced laser desorption/ionization time-of-flight mass spectrometry (SELDI) technology' s clinical value in the diagnosis of anxiety.Methods 121 breast cancer patients were selected as research objects by using convenience sampling method.Self-rating anxiety scale (SAS) was used to evaluate the anxiety on the first day of admission and SELDI was used on the next day to detect the serum from patients' fasting venous blood sample got in the morning after their consent,then anxiety-related protein fingerprints spectrums were selected.The SAS score and SELDI result were finally compared and analyzed.Results SAS score in 121 patients was 53.45±9.78,anxiety occurred in 63 cases (52.07 %).Established the diagnostic model between 15 000+H and 16 800+H protein fingerprints abundance ≥5 %,that was to say,abundance ≥5 % was judged as positive,otherwise negative.On this basis,patients can accurately be distinguished between anxious group and non-anxious group.Correct rate (total coincidence rate) was 91.74 %,specificity and sensitivity respectively were 89.66 % (52/58) and 93.65 % (59/63),the positive predictive value was 90.77 % (59/65),and negative predictive value was 92.86 % (52/56).Conclusions Breast cancer patients have a high incidence of anxiety.SELDI technology has showed high sensitivity and specificity in anxiety detecting.As an objective assessment tool,it could have better prospects for clinical use.
4.Study on the neuroprotective role and its mechanism of p-nerve growth factor in rats with focal cerebral ischemia

Objective To investigate the dynamic changes of Trk(tropomyosin receptor kinase) and Akt(protein kinase B), and to explore the neuroprotective role and its mechanism of β-NGF (β-nerve growth factor) after focal cerebral ischemia. Methods A focal cerebral ischemia rat model was made and β-NGF was injected intra abdominally. The phosphorylation of Trk and Akt were tested by immunocytochemistry and Western blot assay. Results Trk receptor was increased obviously in the penumbra area after 8 h of infarction. The level of phosphorylated Trk(p-Trk) was increased after 2 h of infarction, while the level of phosphorylated Akt(p-Akt) had a decrease and then recovered gradually. The level of p Akt in cerebral ischemia group was decreased by 76.5% compared with that in control group after 8 h of infarction(P<0. 01). After the injection of β-NGF, the level of p-Trk was increased by 74.4% after 12 h of infarction(P<0.01), while the level of p-Akt was recovered significantly after 8 h of infarction, and had no statistical difference compared with the control group after 24 h of infarction (P>0.05). Conclusions Ischemia induces the activation and increased expression of Trk receptor, andβ-NGF may play a protective role in cerebral ischemia by increasing the phosphorylation of Trk and regulating the phosphorylation of Akt.
5.MR1 siRNA suppresses proliferation of human hepatoma BEL-7402 cells

Objective To identify the function and mechanism of MR1 in proliferation of BEL-7402 cells.Methods siRNA targeting MR1 and negative control siRNA were synthesized and transfected into BEL-7402 cells using Lipofectamine 2000.The silencing effect of MR1 siRNA was determined by semi-RT-PCR.SRB assay,colony formation assay and growth curve assay were used to investigate whether MR1 siRNA regulated cellular proliferation.Cell cycles were assessed by flow cytometry.G2 arrest reagent nocodazole was used to show the potential effect of MR1 siRNA on G1 arrest.The expression of Cyclin D1 was determined by Western blotting.Results(1)MR1 mRNA significantly decreased in BEL-7402 cells 24 h after MR1 siRNA transfection.(2)MR1 siRNA induced the down-regulation of cell growth.The expression of Cyclin D1 in MR1 siRNA tranfected BEL-7402 cells decreased significantly.(3)Flow cytometry results showed that MR1 siRNA markedly decreased G2 phase population with nocodazole treatment,and distinctly increased G1 phase population.Conclusion The gene MR1 is involved in the proliferation of BEL-7402 cells.MR1 siRNA causes inhibition of the proliferation of BEL-7402 cells.One of the mechanisms ofMR1 siRNA on the proliferation of BEL-7402 cells is the induction of G1 arrest.
6.Diagnosis study of pathological findings,CT and MRI in primary centralnervous system lymphoma

Objective To investigate the pathological findings and diagnosis value of CT and MRI in detec-tion of primary centralnervous system lymphoma (PCNSL)so as to improve diagnostic accuracy of the condition. Methods The results of CT and MRI for 32 patients with PCNSL were analyzed retrospectively,and the results were compared with the finally obtained surgical-pathological examination.Results Of 32 cases,the foci were single in 29 cases and multiple in 4 cases,and there were 41 foci in total.Most foci were located near the surface or middle line of brain.On CT scan appearances,most foci were hyperdensity or slightly hyperdensity masses with the well definite margin,on MRI,it showed low signal of T1 -weighted imaging (T1 WI)and slightly increased non-uniform signal of T2 -weighted imaging (T2 WI).All the foci were equally and markedly enhanced after enhancement.The diagnostic coincident rate of combination diagnosis (93.8%)was significantly higher than that of CT (71.9%)and MRI (75.0%)(χ2 =7.234 6,P=0.003 2;χ2 =6.953 8,P=0.003 7).Conclusion CT,MRI have characteristic mani-festation for PCNSL,and according to the manifestation of CT,MRI,predilection site,mass effect,and combined with pathological examination,which can effectively improve the sensitivity and accuracy of diagnosis.
7.Application of trans-umbilicus two-port laparoscopic procedures in pediatric surgery

Objective To evaluate the feasibility of trans-umbilicus two-port laparoscopic procedures in pediatric surgery.Methods Sixty children (age, 3 months~14 years; mean, 3.5 years) with indirect inguinal hernia and 40 children (age, 2.5~14 years; mean, 6.3 years) with appendicitis underwent supra-high ligation and appendectomy, respectively, by using mini laparoscope from January 2003 to December 2003. During the supra-high ligation, the camera and forceps were respectively introduced via the left and right side of umbilical ring, and a hernial-ring needle with suture was brought in at the site of body surface projection of internal ring to apply external knotting. During the appendectomy, laparoscopic instruments were put through the site of right McBurney’s point, and explorations for contralateral hernia or other digestive tract malformations were applied simultaneously. Results All the operations were completed smoothly. The mean operating time was 8.5 min and 55.7 min in hernia repair (per side) and appendectomy, respectively. The mean postoperative hospital stay was 1~2 days in children with indirect inguinal hernia and 5~7 days in children with appendicitis. Follow-up for 3~12 months showed no recurrence of hernia except for 1 case of intraoperatively misdiagnosed latent hernia, which relapsed 2 months after the surgery. Postoperative abdominal cavity infection took place in 2 cases of perforated appendicitis. Conclusions Trans-umbilicus two-port laparoscopic technique is simple to perform, safe and minimally invasive. The technique has fewer complications and gives satisfactory cosmetic results, being highly recommended.

9.Assessment of SCT Multiplanar Reformation in Dignosing and Staging of Cervical Cancer

Objective To evaluate the value of multiplanar reconstruction (MPR) of spiral CT in diagnosing and staging of cervical cancer .Methods SCT scan with a 3~5 mm collimation thickness and MPR in 1.5~2.5 mm reconstruction interval were performed in 20 patients with cervical cancer , confirmed by biopsy pathology. Results The sagittal imaging could show entirely the growing character of a carcinomal mass;the coronal and transverse imagings showed clearly parametrial invasion and pelvic lymph node metastases. Conclusion MPR images in the pelvic could show entirely the character of growing and invasion of a carcinomal mass. It is an important value in dignosing and staging of cervical cancer.
