1.Analysis of sleep electroencephalograph signal based on detrended cross-correlation.

The quality of sleep has a great relationship with health and working efficiency. The result of sleep stage classification is an important indicator to measure the quality of sleep, and it is also an important way to diagnose and treat sleep disorders. In this paper, the method of detrended cross-correlation analysis (DCCA) was used to analyze sleep stage classification, sleep electroencephalograph signals, which were extracted from the MIT-BIH Polysomno graphic Database randomly. The results showed that the average DCCA exponent of the awake period is smaller than that of the first stage of non-rapid eye movement (NREM) sleeps. It is well concluded that the method of studying the sleep electroencephalograph with this method is of great significance to improve the quality of sleep, to diagnose and to treat sleep disorders.

2.Effects of interleukin-18 and hypoxia-inducible factor-1α in serum and gingival tissues of rat model with periodontitis exposed to chronic intermittent hypoxia.

OBJECTVEThis study evaluates the expression of interleukin-18 (IL-18) and hypoxia-inducible factor (HIF)-lα in rat periodontitis model exposed to normoxia and chronic intermittent hypoxia (CIH) environments. The possible correlation between periodontitis and obstructive sleep apnea-hypopnea syndrome (OSAHS) was also investigated. Methods: Thirty-two Sprague-Dawley (SD) rats were randomly assigned into four groups: normoxia control, normoxia periodontitis, hypoxia control, and hypoxia periodontitis groups. The periodontitis models were established by ligating the bilateral maxillary second molars and employing high-carbohydrate diets. Rats in hypoxia control and hypoxia periodontitis groups were exposed to CIH treatment mimicking a moderately severe OSAHS condition. All animals were sacrificed after eight weeks, and the clinical periodontal indexes were detected. The levels of IL-18 and HIF-1α in serum and gingival tissues were determined using enzyme-linked immunosorbent assay (ELISA). The correlation between attachment loss (AL) and the levels of IL-18 and HIF-lα in hypoxia periodontitis group was evaluated.
RESULTSThe levels of IL-18 and HIF-lα in hypoxia periodontitis group were significantly higher than that in normoxia periodontitis and hypoxia control groups (P<0.05). Furthermore, the levels of IL-18 and HIF-lα in serum (r-0.792, r=0.753, P<0.05) and gingival tissues (r-0.817, r=0.779, P<0.05) were positively correlated with AL.
CONCLUSIONCIH could aggravate the destruction of periodontal tissues, which is correlated with IL-18 and HIF-lα levels.

3.Advances in microbial genome reduction and modification.

This article summarized the essential genes and the methods to identify them in microorganisms, compared various strategies for microbial genome reduction, and analyzed the characteristics of some microorganisms with the minimized genome. This review shows the important role of genome reduction in constructing cellular chassis.

4.Clinical analysis of radiotherapy for 45 cases with malignant bone metastatic

Objective To analyze the effect of pain control with radiotherapy in bone metastases from malig-nancies. Methods A total of 72 lesions in forty-two cases with bone mestastases were received 6my X ray. The con-ventional radiotherapy was performed in 28 lesions in the way of 200cGy/fraction,5 fracties/week,total dose of 3000 ~5000cGy. The hypofractionation radiotherapy was.performed in 44 lesions in the way of 400cGy/fracfion,3 frac-tions/week,total dose of 2400~3600cGy. Results Overal pain relief rate was 90.28%, there were no significant differences between the two groups(P 0.05). The pain relief of hypofractionation radiotherapy was faster than anoth-er. Conclusion Radiotherapy is a very effective way to relieve pain in bone metastases from malignacies.
5.Effect of rosiglitazone on resistin level in patients with type 2 diabetes

Objective To study the effect of rosiglitazone (ROS) on resistin level in patients with type 2 diabetes. Methods 56 patients with type 2 diabetes mellitus were recruited according to WHO criterion(1999). They were randomly divided into two groups, the threatment group 36 and the control group 20 patients. Patients in control group received routine therapy,and patients in treatment group received ROS,4mg/d with routine therapy for 12 weeks. Clinical routine and plasma glucose, insulin and resistin determination were performed. Insulin sensitivity was evaluated using homeostasis model assessment(HOMA) formula. Results Levels of fasting plasma glucose,insulin,HOMA-IR and resistin were significantly decreased. Conclusion ROS can significantly reduce insulin resistance,improve the utilizing of plasma glucose, which has an important effect on level of resistin in patients with type 2 diabete.
6.A comparative study of gentamicin and triamcinolone acetonide for the treatment of primary trigeminal neuralgia

Objective To observe the therapeutic effects and adverse events of gentamicin and triamcinolone acetonide on primary trigeminal neuralgia. Methods Forty patients with primary trigeminal neuralgia were allocated into two groups to receive a peripheral nerve block with lidocaine combined with gentamicin (gentamicin group) or triamcinolone aeetonide (triamcinolone acetonide group), respectively,once a week for 5 weeks. The pain intensity were assessed using visual analogue scale (VAS) before treatment and once a week just prior to every treatment, continued to make such assessment every month until 3 months after treatment. The pain relief rate and the occurrence of adverse events were also evaluated.Results The VAS of patients in two groups decreased significantly after treatment (P < 0.01). Compared with triamcinolone aeetonide group, the VAS in gentamicin group were higher during the early 3 weeks after treatment (P< 0.05 or < 0.01). At the observing point of 2, 3 months after treatment, the VAS in gentamicin group were lower than that in triamcinolone acetonide group(P< 0.01). The effective rate in gentamicin group was lower than that in triamcinolone acetonide group during the early 2 weeks after treatment, the clinical cure rate in gentamicin group was higher than that in triamcinolone aeetonide group at the observing point of 2, 3 months (70% and 60% vs 15% and 5%). Conclusion Compared with the triamcinolone aeetonide, the therapy of the gentamicin has slower onset time and longer sedative duration, has no corticosteroids-related side effect, and also has the possibility of repeat blocking time after time.
7.Research in application of timing-work schedule mode in human resource management in the outpatiet department of island hospital

Objective To evaluate the effectiveness of application of timing- work schedule mode in hu-man resource management according to the characteristc and work load of the outpatient department in island hospital. Methods General quality of care, nurses and patients' satisfaction degree were compared before and 12 months after the application of timing- work schedule. Results Total quality of care and nurses and pa- tients' satisfaction degree with nursing had statistical significance before and after the application of timing-work schedule mode. Conclusions To adoption of timing-work schedule mode in outpatient department in island hospital is feasible, it not only saves the nursing human resource, but also can guarantee the nursing quality and improve patients' satisfaction degree. So it is worthy of clinical application.
8.Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy

Studies in recent years have suggested that the basic characteristics of the identified cerebral autosomal dominant arteriopathy with subeortical infarcts and leukoencepha-lopathy (CADASIL) are the onset of the disorder with a form of familial hereditary, middle-age onset with progressive ischemic stroke-like course and multiple diffuse white matter lesions, the identified MRI abnormal white matter signals and pathology. Molecular genetic studies have suggested that the multiple mutations of the Notch3 gene are associated with CADASIL. The combination of gene diagnosis with peripheral tissue biopsy may be the most valuable diagnostic means before death. To study CADASIL from the aspects of pathogenesis and clinical manifestations contributes to improve clinical diagnosis rate.
9.Production and clinical application of limb wound guard

Objective This experiment aimed at exploring a limb wound guard to avoid the clothing being stained by bleeding or drainage on llmb wounds, keep them dry and clean inside, enable the limbs to move conveniently and protect the wounds untouched so as to increase patients' sense of security. Methods 180 surgical patients with limb wounds were randomly divided into two groups, 94 cases in the observation group who used limb wound guards and 86 patients in the control group who did not. The treatment effect and comfort degree were compared between the two groups by X2 test and Ridit analysis. Results Statistically sig-nificant differences between the two groups could be observed in the per capita frequency of clothing replace-ment, the wound infec- tion rate, the average days of hospital stay and patients" comfort degree. Conclusions The results indicate that using limb wound guards can prevent the clothing from being polluted by wound exu-clarion, keep them dry and clean inside, make the patients feel comfortable and reduce wound infection, facili-tate the activities of the injured limbs, reduce patients" fatigue and discomfort, protect the wound untouched and therefore, increase pa-tients' sense of security.
10.Qualitative research in intensive care experience of patients receiving mechanical ventilation at inten-sive care units

Objective To understand patients' experience of intensive care with mechanical ventilation at intensive care units to provide a more holistic approach to patient care. Methods In-depth interviews were applied to 11 participants who survived from mechanical ventilation. The pa-tients were asked to describe their experience by responding to the research question "what is it like to experience mechanical ventilation treatment at an ICU?"The data were analyzed by Giorgi's ph e-nomenological procedures. Results Five mutually exclusive themes emerged, which were titled "be-ing in an unconventional environment", "suffering from physical discomfort", "encountering psycho-logical crisis", "self-encouragement", and"self-reflection". Conclusions Patients' experiences while receiving mechanical ventilation at an intensive unit were poignant and afeard, but "self-encour-age-ment" and "self-reflection" will enhance patients' self-confidence which will be beneficial for their re-covery of critical illness.
