1.A path analysis of psychosocial factors on job burnout in first aid medical staff

Objective To analyze the psychosocial influential factors on job burnout and their interaction models in first aid medical staff.Method A survey was carried out concerning job burnout status,job stress,job overload,work-family conflict,overall job satisfaction,coping styles and social support in 504 first aid medical staffs,by which to seek the psychosocial influential factors of job burnout status and explore their interaction models using stepwise multivariable regression and path analysis.Result (1) The stepwise multivariable regression on the job burnout showed that job overload,work stress,work-family conflict,overall work satisfaction and negative coping style were the key social-psychological influential factors in job burnout(F =90.562,P =0.001).(2) The path analysis showed that the job overload was the major event caused the work stress,and work stress played a direct and indirect roles in the level of job burnout in first aid medical staff,and the work-family conflict and overall work satisfaction was disposition factors of job burnout with direct actions and indirect actions.The negative coping style was an intermedium link between work stress and job burnout.Conclusion Decrease of work intensity and change of bad coping styles can be effective on job burnout in first aid medical staff.
2.The formation and treatment of tumor associated thrombosis

Patients with malignant tumor are easy to be complicated with venous thrombosis,which is the second leading cause of death for patients with malignant tumor.The mechanism of tumor thrombus forma-tion is related to tumor type,treatment,metastasis,heredity and tumor itself.Evaluating the risk of venous thrombosis in patients with cancer and providing targeted preventive anticoagulation measures for high-risk patients have important significances for improving the quality of life and prolonging the survival period of pa-tients with cancer.
3.Application of nasal endoscopy in styloid process truncating operation.

OBJECTIVE:
To investigate the application and the clinical effect of nasal endoscope in styloid process truncating operation.
METHOD:
Dividing 66 patients into nasal endoscope group and control group randomly, and then observe them. Nasal endoscope group were cured with nasal endoscope during the styloid process truncating operation and the control group were treated with the routine method of intraoral path. All the operations were executed by the same person. Observation index included the following aspects: the length of the truncated styloid process, the operation time, intraoperative bleeding, postoperative pain and pain degree, inflammatory reaction of raw surface and rehaemorrhagia.
RESULT:
Compared with the control group, the length of the truncated styloid process of nasal endoscope group were longer and the operation time had no significant differences. The intraoperative bleeding of intraoperative bleeding group were (85.0 +/- 35.0)ml and in control group were (95.0 +/- 40.0)ml. There are no significant differences between the result. Nasal endoscope group also had the advantages of fewer aches and less inflammatory reaction of raw surface. Both of the team had no rehaemorrhagia.
CONCLUSION
nasal endoscope can truncate a longer styloid process in styloid process truncating operation and is effective to relieve pain, and ease inflammatory reaction, it's a safe mini-trauma operation.

4.Dectin-1 mRNA expression in lung of different immune status mice infected with Aspergillus fumigatus

Objective To study dectin-1 mRNA expression in lung of different immune status mice infected with Aspergillusfumigatus, and to explore the influence of immunosuppressive agent on the expression of dectin-1 and its relationship with the progression of disease. Methods Mice were divided into four groups which were normal control, immuncompromised, immuncompromised with A.fumigatus inoculation and immuncompetent with A.fumigatus inoculation groups. We explored the kinetic mRNA expression of dectin-1 in lung of different groups by real-time quantitative PCR. Pulmonary fungal burden assessment was performed to reflect the progressing of disease during the experimental time course. Results On day 3 after inoculation, pulmonary fungal burden of the immuncompromised mice was higher than that of the immuncompetent group. On day 1 and 3, dectin-I mRNA expression in lung of the immuncompromised group was much lower than that of the normal control. On day 3, dectin-1 mRNA expression in lung of the immuncompetent mice infected with A.fumigatus was much higher than that of the immuncompromised with A.fumigatus inoculation and the normal control groups ( P < 0.05 ). Conclusion During the infection, expression of dectin-1 in lung of the immuncompetent group was strikingly increased, which may play an important role on the defence to A.fumigatus invasion. Cyclophosphamide inhibited the expression of dectin-1 in lung of mice which may be one of the mechanisms of the invasive pulmonary aspergillosis development induced by eyclophosphamide.
5.Rapid determination of phosphodiesterase 5 inhibitors in invigorative health food by paper spray ionization mass spectrometry method

Objective A rapid method was constructed for detection of 3 kinds of phosphodiesterase 5 (PDE5) inhibitors (sidenafil,vardalafil,tadalafil) in invigorative health food by paper spray ionization mass spectrometry (PSI-MS method).Methods The characteristic ions of the PDE5 inhibitors could be used for preliminary identification and semiquantitative analysis with internal standard method using PSI-MS method.Results The 24 kinds of commercially available health-care products includes capsule,tablet,pill,powder,wine,syrup and liquid were tested.Results from PSI-MS were consistent with the HPLC-UV date.The calibration curves of PSI-MS has a good linearity in a given range.The linear coefficients of analytes were higher than 0.99.The LODs of 3 kinds of PDE5 inhibitors were lower than 1.0 mg/L.The RSDs of this method ranged from 20% to 24%.Conclusion The PSI-MS method was rapid,accurate and targeted,which is compliant for quickly screening the PDE5 inhibitors in large complex matrix samples.
6.Clinical study in effect of edaravone on free radical scavenging in patients with acute cerebral infarction

Objective To investigate the clinical effect of edaravone on free radical damage after acute stroke. Methods The 60 patients with acute stroke were randomly divided into therapy group and control group. Both the groups were treated with routine approaches of dehydration, intracranial pressure reducing and blood pressure control. The treatment with balanced saline 250 ml plus edaravone 30 mg, intravenously infusion twice a day, was adopted in therapy group, while the control group received balanced saline 250 ml only, the treatment lasted for 14 days. Before and 14 days after treatment, the neurologic impairment analysis, activity of daily living scale (ADL) and serum superoxide dismutase (SOD) were checked. Results Compared with control group, after 14 dtreatment, the improvements in neurologic impairment analysis (7.5±5.4 vs. 15.9±7.9, P<0.05),ADL (58.32±11.57 vs. 43.73± 12.48, P<0.05) and SOD[(157.25±21.81)mmol/L vs. (127.08 ± 13. 14)mmol/L, P<0. 05] occurred in therapy group. Conclusions Edaravone could increase the ability of cleaning free radicals and promoting function of nerves recovery.
7.Expression and clinical significance of gonadotropin-releasing hormone receptor and epidermal growth factor receptor in gastric carcinoma

Objective To investigate the expressions of gonadotropin-releasing hormone receptor(GnRHR)and epidermal growth factor receptor(EGFR) in gastric carcinoma and to explore its clinical significance.Methods The expressions of GnRHR and EGFR in 31 cases of gastric carcinoma were observed by SP immunohistochemical method and the method of in situ quantification.Results The GnRHR and EGFR positive immunoreactive substances were located in the tumor cells of gastric carcinoma,GnRHR 28 cases (90.32% ),EGFR 15 cases ( 48.39% ),the expression of GnRHR increased in accordance with histologic differentiation,well-differentiated 14 cases,moderately differentiated 9 cases,poorly differentiated 5 cases,the expression of EGFR decreased in accordance with its increasing degree of histologic differentiation,well-differentiated 3 cases,moderately differentiated 5 cases,poorly differentiated 7 cases,( P < 0.05 ),and GnRHR was more obvious than EGFR in immunoreaction.Conclusion GnRHR and EGFR could be related to the pathogenesis and the development of the gastric carcinoma.
8.Clinical observation of effect of mifepristone combined with norethindrone in treating dysfunctional uterine bleeding of peri-menopause

Objective To analyze the efficiency of mifepristone combined with norethindrone in treating dysfunctional uterine bleeding of peri-menopause. Methods 56 patients with dysfunctional uterine bleeding of peri-menopause were randomly divided into two groups, the study group(30 cases) and the comparision group(26 cases). The patients in the study group were treated with mifepristone in small doses combined with norethindrone. And the patients in the comparision group were treated only with norethindrone. Before treating, all the patients were observed and measured symptom of dysfunctional uterine bleeding, blood routine,the function of liver and ren.the en-dometrial thickness and pelvic by ultrasonography. Through hysteroscopy or uterine curettage of diagnosis, en-dometriums were measured by pathology. Results 26 cases in the study group were evaluated as effective,2 cases as obviously effective,and 2 cases as no effective. In the comparision group, 10 patients were evalutated as effective,8 patients as obviously effective,8 patients as no effective. 10.Relationship between membrane phospholipids and CD14 expression of macrophage stimulated by LPS

Objective To investigate the effect of cellular membrane phospholipids on CD14 protein expression of macrophage stimulated by LPS. Methods Changes of CD14 protein expression and CD14 mRNA of macrophages stimulated by LPS in vitro were determined by Western blotting and RT PCR. Effects of membrane phospholipids on CD14 protein expression were also detected. Results After stimulation by LPS, CD14 expression increased at 1 h, reached the peak value at 5 h and decreased to the normal level at 8 h but CD14 mRNA reached the peak value at 3 h and decreased at 5 h. The levels of phospholipids and membrane fluidity decreased at 5 h but CD14 protein expression increased after LPS stimulation. After pretreatment with liposomes, membrane phospholipid microenvironment improved and CD14 protein expression decreased. Conclusion LPS can up regulate CD14 protein expression, which might be regulated at least at the transcriptional level of the CD14 gene. Changes of membrane phospholipid microenvironment may be an important reason for the up regulation of CD14 induced by LPS.
