1.Analysis of clinicopathological characteristics of gastric primary adenosquamous and squamous carcinoma

Objective To explore the Clinicopathological characteristics of patients with primary adenosquamous and squamous carcinoma of stomach. Methods The clinical data of 12 cases of primary squamous and adenosquamous carcinoma of the stomach were reviewed retrospectively, and the immunohistochemical staining of CK17 and CKI8 protein were performed in primary gastric adenosquamous carcinoma. Results Primary adenosquamous and squamous carcinoma of the stomach accounted for 0.28% of all the 4352 patients with gastric cancer during the same period. Of the 12 patients, 10 were adenosquamous carcinoma and other two were squamous carcinoma. There were 10 males and 2 females in this group, with their mean age being 65 years. The main clinical presentation included epigastric pain and discomfort, followed by hematemesis and melena. The definite diagnosis rate was 33% (4/12) by gastroscopy and biopsy before operation. The tumors were less than 5 cm in diameter in 3 patients, and more than 5 cm in 9 patients. The surgical procedure was radical resection in 8 patients and palliative resection in 4 patients. There were 1 case of stage Ⅰ, 5 cases of stage Ⅲ, 6 cases of stage Ⅳ. 10 patients died of tumor recurrence and metastasis within 2 years after operation, one died of other unrelated disease, and one was alive for more than 5 months. The component of both adenosquamous and squamous carcinoma were more than 30% in 4 patients with adenosquamous carcinoma who underwent palliative resection and died within 6 months after operation. Conclusions Primary adenosquamous and squamous carcinoma of the stomach were rare, and had specific clinicopathological characteristics. Having both biological behaviours of adenocarcinoma and squamous carcinoma may lead to poor prognosis in adenosquamous carcinoma of stomach.
2.Effect of portal vein occlusion and following restoration of portal vein flow on apoptosis of intestinal mucusal cells and liver function

Objective To investigate the effect of liver function with portal vein occlusion (PVO) in various phases and the following restoration portal vein flow on intestinal mucosal cells. Methods Twenty-four healthy adults white Japanese rabbits were randomized into 1 control group and 2 experimental groups (according to portal vein clamping for 30 min and 45 min). Each experimental group's blood samples were collected from caval vein 1 h before operation, by the end of portal vein oc-cluded, 30 min, 60 rain after relief of portal vein blocking, then with restoration of portal vein flow for 2 h and rabbit guts were continuously cut to sections for HE, TUNEL staining and Bcl-2, Bax protein immunohistochemical staining to observe the injury of intestinal mucosa cell apoptosis and the relation-ship between the expressions of Bcl-2 and Bax. The levels of blood glutamate-pyruvate transaminase (ALT), glutamic oxalacetic transaminase (AST) were measured and compared. Results The levels of ALT, AST in the control group did not significantly change. Compared with control group, group B did not change significantly with PVO 30 rnin in liver enzyme and they were significantly increased after portal vein occlusion relief. The levels of ALT and AST were increased obviously at 45 min with PVO, then raised again. Down-regulation of Bcl-2 expression, up-regulation of Bax expression and the increased index of apoptotic cell were found in each experimental group. Conclusion It may be more safe with PVO for 30 min. But the following restoration portal vein flow will bring about ischemia-reperfusion injury that is mainly apoptosis in the small intestine. The index of apoptosis will be raised with time prolongation of PVO.
3.Expression and clinical significance of FOXP1 in gastric MALT lymphomas

Objective To investigate the expression of forkhead box protein P1(FOXP1)in gastric mucosa-associated lymphoid tissue(MALT)lymphomas and its relationship with histological morphology and prognosis. Methods In this study.samples of 43 MALT lymphoma were studied histologically and divided into monomorphic histology group and polymorphic histology group according to their cellular features.The expressions of FOXP1 and NF-κB in gastric MALT lymphoma were evaluated immunohistochemically by two-step method of Envision,and the clinicopathological features and prognosis were analyzed retrospectively.Results The nuclear expressions of FOXP1 in 43 cases with gastric MALT lymphoma were 44%(19 of 43),including strong immunoreactivity in 7 cases and moderate immunoreactivity in 12 cases.There were 4 cases with positive immunoreactivity in moninorphic histology group and 15 cases in polymorphie histology group,and the difference was statistically significant(15%vs.88%,P<0.01).All the postoperative recurrent cases were strongly positive with FOXP1 stain,and it was closely with FOXP1 expression(P<0.01).The median survival time(26 months)in polymorphic histology group was significantly shorter than that(123 months)in monmorphic histology group(P<0.01),and the median survival time was significantly longer in negative FOXP1 expression group than that in moderate FOXP1 expression group and in strong FOXP1 expression group(115 vs.55 vs.12 months)(P<0.05).similarly,the median survival time in nuclear factor kappa B(NF-κB)expression group was significantly shorter than that in negative NF-κB expression group(26 vs.131 months)(P<0.01).The median survival time in stageⅠ(98 months)and stage Ⅱ(121 months)was significantly longer than that in stage Ⅱ E+Ⅳ(33 months)(P<0.01).By multivariate COX regression analysis.FOXP1 nuclear expression and clinical stage were independently prognostic factom. Conclusion FOXP1 expression may be used as a biomarker for the assessment of malignant transformation to diffuse large B-cell lymphoma(DLBCL)and predicting prognosis.
4.Surgical retrieval of testicular spermatozoa and intracytoplasmic sperm injection in non-obstructive azoospermic patients

Objective To evaluate the efficacy of surgical retrieval of testicular spermatozoa,and to present the preliminary results of the retrieved testicular spermatozoa used in intracytoplasmic sperm injection (ICSI) for treatment of non obstructive azoospermic infertility. Methods Forty nine cases of non obstructive azoospermia underwent open testicular biopsy and diagnostic testicular sperm extraction (TESE).Those who had sperms in diagnostic TESE underwent ICSI at least 3 months later. Results Sperms were detected in diagnostic TESE in 12 cases (24.9%). The testicular volume,FSH level and testicular pathology did not accurately predict the presence or absence of sperm in testes.Seven (87.5%) of 8 cases had sperm in re TESE, which were used in their ICSI cycles,resulting in 3 clinical pregnancies. Conclusions The diagnostic TESE should be performed to determine the presence or absence of testicular spermatozoa in testicular failure before ICSI,and the surgically retrieved testicular spermatozoa can be adopted in ICSI for treatment of non obstructive azoospermic infertility.
5.Congestion-Reperfusion Injury to Small Intestine and cell Apoptosis

Objective To investigate the effect of apoptosis with portal vein occlusion in various degrees or times and the following restoration portal vein flow on intestinal mucosal cells,to analyze its possible pathogenesis. Methods 40 healthy adults white Japanese rabbits were randomly divided into one controlled group and four experimental groups (according to portal vein half-clamping for 30 min, 45 min and portal vein full-clamping for 30 min, 45 min, divided into B1、C1、B2、C2 groups.).Each experimental group with portal vein occlusion on certain times then with restoration of portal vein flow for 2h. At choiced time rabbit guts were continuously cut to sections for HE、TUNEL staining and Bcl-2、Bax protein immunohistochemical staining,to observe the injury apoptosis in intestinal mucosa and the relationship between the expressions of Bcl-2 and Bax protein. 6.Air Circumstance in Laminar Flow Operating Room in Beijing:A Crosssection Investigation of 18 Hospitals

OBJECTIVE To investigate the management and sanitation status of the air circumstance in laminar flow operating rooms of grade two and above hospitals in Beijing.METHODS Air surveillance methods,cleaning methods of intakes,cleaning status of pipelines and frequency of renewing filter systems were investigated through interviewing charge nurse of 18 hospitals by questionnaires.RESULTS 50.0% of pipelines had never been cleaned,14.3% of intakes had never been cleaned.Air surveillance method of all hospitals was using sedimentation,with a low sensitivity.The monitoring systems were poor in 50.0% of hospitals.CONCLUSIONS Now there is only requirements of construction standard of clean surgery region,lacking routine regulations and technologies of contamination controlling standard,nor available dynamic contamination controlling system.Potential hazard exists in routine regulations of laminar flow operating room and it demands for making contamination controlling standard urgently in hospitals.
7.Review of Literature about Adverse Reactions to Astragalus Injection in41Cases

OBJECTIVE:To study the characteristics and preventive measures of the ADRs associated with astragalus in?jection.METHODS:ADRs to astragalus injection in41cases,reported in domestic medical journals from1988~2003,were col?lected and analysed.RESULTS:The ADRs to astragalus injection were independent of patient's sex and age.ADRs occured du_ ring intravenous drop in32cases(78.05%)mostly within30min after injection,and24cases(41.46%)had ADRs at a second time or repeated injection.The clinical manifestations of ADRs were varying,of which the allergic reaction was the commonest one including anaphylactic shock.CONCLUSION:Clinical physicians and pharmacists should pay attention to the ADRs of as?tragalus injection and rationally use the preparation.
8.Expressions and clinical significance of CD133 and CD44 in gastrointestinal stromal tumors tissues

Objective To investigate the expressions of CD133 and CD44 and their prognostic significance in gastrointestinal stromal tumors (GIST).Methods Streptavidin perosidase (SP) method of immunohistochemistry was used to detect expressions of CD133 and CD44 proteins in 42 cases of GIST, and the relationship between their expressions and tumor size, mitotic count were analyzed by univariate and multivariate factor analyses.Results The expressions of CD133 and CD44 proteins in GIST were 21.4% (9/42) and 78.6% (33/42), respectively.The expressions of CD133 and CD44 proteins were significantly correlated with tumor size and mitotic count (P < 0.05).Univariate factor analysis showed that the overall survival of GIST patients with positive CD133 protein (23.2 months) was shorter than that of patients with negative CD133 protein(63.1 months) (P < 0.05).The overall survival of GIST patients with negative CD44 protein (23.2 months) was shorter than that of patients with positive CD44 protein (63.3 months) (P < 0.05).Multivariate factor analysis showed that tumor size, mitotic count and CD44 protein were independent prognostic indicators for survival time after operation.Conclusions The positive expressions of CD133 and CD44 proteins might be the prognostic factors of GIST patients.
9.Comparison of the clinical efficacy of immunoglobulin and interferon in the treatment of hand foot and mouth disease complicated with viral encephalitis

Objective To investigate the clinical efficacy and safety of intravenous immunoglobulin and interferon in the treatment of hand foot and mouth disease complicated with viral encephalitis .Methods 80 cases of hand foot and mouth disease complicated with viral encephalitis were randomly divided into the three groups according to the random number table .27 cases in the control group were given comprehensive symptomatic treatment ,27 cases in the study group 1 were given gamma globulin ,and 26 cases in the study group 2 were given interferon .The clinical efficacy,improvement of disease ,incidence of adverse reactions of the three groups were compared .Results Defer-vescence time ,seizure control time ,time of skin rash subsided ,time of psychiatric symptoms relieved and the average hospitalization time in the study 1 group were (3.65 ±0.28)d,(4.04 ±0.33)d,(3.86 ±0.27)d,(5.83 ±0.36)d and (7.53 ±0.83)d,which were significantly less than those in the control group (t=8.43,8.58,9.15,9.80,8.96, all P<0.05).Those in the study 2 group were (3.92 ±0.29)d,(4.21 ±0.32)d,(4.27 ±0.30)d,(6.32 ±0.43)d and (8.10 ±0.72)d,which were significantly less than the control group (t=7.99,8.17,8.54,9.18,8.55,all P<0.05);however,there were no significant differences between the study 1 group and the study 2 group(t=1.12, 2.04,1.67,1.38,2.21,all P>0.05).The incidence rate of adverse reaction among the three groups showed no sta-tistically significant difference (χ2 =2.17,P>0.05).Conclusion Intravenous immunoglobulin and interferon have significant effect in the treatment of hand foot mouth disease complicated with viral encephalitis , which can quickly improve symptoms ,shorten treatment time and have high safety and good clinical application value .
10.Factors of ultrasonic-assisted extraction of arctiin and arctigenin from Fructus arctii

AIM: To investigate and optimize the technology of ultrasonic-assisted extraction of arctiin and arctigenin from Fructus arctii.METHODS: The effects of ultrasonic power,particle size,ratio of liquid to solid,extraction time,extraction temperature and duty factor on the extraction rates of arctiin and arctigenin were investigated,and optimized with orthogonal experiments.RESULTS: The results showed that the optimal extraction conditions were as follows: particle 80 ~ 100 mesh 14 mL/g,ratio of liquid to solid 400 W ultrasonic power,50 ℃ of extraction temperature,and 10 min of extraction time.CONCLUSION: The optimized conditions are reasonable and reliable.
