2.Treatment of Henoch-Sch(o)nlein purpura nephritis in children

Henoch-Sch(o)nlein purpura(HSP) is one of the most common vasculitides in children with many manifestations including skin purpura,arthritis,abdominal pain and renal involvement.The renal involvement(Henoch-Sch(o)nlein purpura nephritis,HSPN) is the principal cause of morbidity and major risk factor for prognosis in HSP.The paper reviewed the updated treatment strategy for HSPN.Accordingly,steroids,cyclophosphamide,mycophenolate mofetil,mizoribine,cyclosporine,triptolide,angiotensin converting enzyme inhibitors (ACEI) / angiotensin receptor blockers(ARB),plasma apheresis and tonsillectomy are possible indicator for the further therapy of HSPN.Furthermore,prospective randomized controlled treatment studies on children with severe HSPN would be needed.
3.Review on Stem Cells Used on Heart Regeneration

[Objective] This article mainly discussed the potential of myocardial cellregeneration and the possible mechanisms involved. [Method] To study the mechanism of cardiac regeneration ability HSCs, this review studyed the relationship of the differentiation of cardiac stem cells, hematopoietic stem cells, myocardial cells and myocardial regeneration and the possible mechanisms involved. [Result] Self-renewing, differentiation both in vitro and in vivo of the c-kit positive CSCs had been seen. No matter in physiological condition or after damage, the c-kit positive CSCs had clonality and the potential of multi-directional differentiation. Hematopoietic stem cells can generate cardiomyocytes and coronary blood vessels for human celltherapy. The ability of dedifferentiation cells to maintain and improve the ventricular function of a damaged heart is very limited. [Conclusion] CSCs and HSCs play an important role in reversing chronic ischemic and non-ischemic heart failure.
4.Clinical study on treating of the knee osteoarthritis by using glucosamine capsule and injecting sodium hyaluronate combined with appropriate functional exercise

Objective To investigate the clinical effect of three capsule combined with Function injection combined with functional exercise in the treatment of knee osteoarthritis.Methods120 patients with knee osteoarthritis were randomly divided into two groups, the study group took orally hydrochloride capsule combined with joints injection combined with functional exercise therapy, and the control group only oral glucose capsules.two groups of patients after a period of treatment.ResultsIn the VAS score, lequesne index and serum inflammatory molecular level of the two groups were significantly better than the control group (P<0.05).ConclusionWhich the clinical study on treating of the knee osteoarthritis by using glucosamine capsule and injecting sodium hyaluronate combined with appropriate functional exercise has a significant effect on knee osteoarthritis, and more favorable in improving the quality of life of patients.
5.Neuroprotective effect of PTEN inhibitor BPV on cerebral ischemia-reperfusion injury in rats and its mechanism

Objective To investigated the neuroprotective effect of PTEN inhibitor BPV on cerebral ischemia-reperfusion injury in rats and its mechanism. Methods Male Sprague-Dawley rats were used to induce a reperfusion model of middle cerebral artery occlusion for 1 h. During the reperfusion, the BPV solution (0. 2 mg/kg daily) or the equal volume of saline was injected intraperitonealy immediately. The neurological deficit scores were conducted at day 1, 3,5, and 7 after ischemia-reperfusion. At day 4, triphenyl tetrazolium chloride staining was used to assess cerebral infarction volume. Enzyme-linked immunosorbent assay was used to detect the levels of interleukin 10 (IL-10) and tumor necrosis factor α(TNF-α) in cortical ischemic border zones. Real-time quantitative polymerase chain reaction was used to detect the expression level of PTEN mRNA. Western blotting was used to detect the expression levels of PI3K, Akt, and p-GSK-3β. At day 7, Bielschowsky silver staining was used to detect the axonal distribution in the ischemic border zone of the striatum. Immunohistochemical staining was used to detect the expression of myelin basic protein (MBP). Results At day 4 after ischemia-reperfusion, the infarct volume (32. 27% ± 1. 71% vs. 45. 49% ± 2. 12% ; P < 0. 001), TNF-α concentration in the cortical ischemic border zones (134. 17 ± 10. 38 pg/ml vs. 264. 17 ± 24. 84 pg/ml; P < ), and PTEN mRNA level (1. 19 ± 0. 08 vs. 2. 50 ± 0. 06; P < 0. 001) in the rats of the BPV group were al significantly lower than those of the normal saline group. The IL-10 concentration (186. 83 ± 10. 83 pg/ml vs. 147. 83 ± 11. 62 pg/ml; P < 0. 001), and the expression levels of PI3K (0. 43 ± 0. 08 vs. 0. 26 ± 0. 06; P = 0. 004), Akt (0. 52 ± 0. 05 vs. 0. 40 ± 0. 04;P = 0. 001), and p-GSK-3β (0. 75 ± 0. 08 vs. 0. 38 ± 0. 06; P < 0. 001) were al significantly higher than those of the normal saline group. At day 7 after ischemia-reperfusion, the neurological deficit score (4. 83 ± 0. 41 vs. 6. 33 ± 0. 52; P < 0. 001) in the rats of the BPV group was significantly lower than that of the normal saline group. The axon densities in the ischemic border zones (35. 51% ± 2. 45% vs. 25. 31% ± 2. 79% ; P < 0. 001) and the expression level of MBP (32. 56% ± 3. 46% vs. 27. 81% ± 4. 18% ; P = 0. 037) were significantly higher than those of the normal saline group. Conclusions BPV has neuroprotective effect for cerebral ischemia-reperfusion injury in rats. Its mechanism may be associated with the up-regulation of PTEN downstream proteins PI3K, Akt and p-GSK-3β expression to regulate inflammatory mediators and reduce the inflammatory response.
6.Quality Standard for Anweiyang Capsules

Objective:To improve the quality standard for Anweiyang capsules. Methods: TLC was used to identify Glycyrrhiza inflata Bat. Licochalcone A was determined by RP-HPLC. Using a C18 Column, the mobile phase consisted of acetonitrile-0. 1% phos-phoric acid(40:60), and the detection wavelength was at 376 nm. Results:The herb could be identified by TLC. For licochalcone A, the linear range was from 25. 563 to 1 533. 798 ng, and the average recovery was 99. 8%(n=9). Conclusion:The method is simple and accurate, which can be used to improve and control the quality of Anweiyang capsules.

8.High-absorbing chitosan dressings for hemostasis and wound healing

BACKGROUND:High liquid-absorbing property, high-capacity imbibition property and high antibacterial property of chitosan dressings can be promoted by chemical modification.
OBJECTIVE:To observe the effect of high-absorbing chitosan dressings on hemostasis and wound healing.
METHODS: (1) Venule bleeding experiment: symmetry oozing wounds were produced on the rabbit skin, which were respectively covered by high-absorbing chitosan dressings and sterile gauze. Then, hemostatic time and bleeding volume were recorded. (2) Deep II degree burn wound healing experiment: 40 Sprague-Dawley rats were selected to prepare deep II degree burn wounds on the back, and then model rats were randomly divided into two groups: experimental group with high-absorbing chitosan dressings and control group with vaseline gauze. After 3, 7, 14, 21 days, wound tissues were taken and analyzed histopathologicaly.
RESULTS AND CONCLUSION: (1) Venule bleeding experiment: The bleeding amount and hemostatic time were significantly lower in the high-absorbing chitosan dressing group than the sterile gauze group (P < 0.05,P < 0.01). (2) Deep II degree burn wound healing experiment: the unhealed wound areas were significantly lower in the experimental group than the control group at days 7, 14, 21 after repair (P < 0.05). After 3 days of wound repair, necrosis of the squamous epithelial layer and the structural damage of the hair folicle in the corium layer and skin appendages were found in both two groups. Meanwhile, the infiltration caused by varying numbers of neutrophils and lymphocytes inside the damaged skin tissues was visible. After 21 days of repair, the epithelium healed wel in the control group, with a little lymphocyte infiltration and formation of crust on the skin surface; while in the experimental group, the epithelium healed wel, with complete squamous epithelial layer and no inflammatory cel infiltration. Thus, these experimental findings reveal that the high-absorbing chitosan dressing has an excelent effect on hemostasis and wound healing.
9.Treatment of lumbar disc herniation by prosthetic disc nucleus replacement

[Objective]To evaluate short term clinic results of prosthetic disc nucleus(PDN)replacement for the treatment of lumbar disc herniation.[Method]Twenty cases of lumbar disc herniation(including one case of recurrent lumbar disc herniation)were treated with PDN from June 2003 to November 2003,including 13 males and 7 females with average age of 40.5 years.All cases were implanted with a single PDN,in which 7 cases with PR725,8 cases with PW725,and 5 cases with PDN-SOLO-7 respectively.[Result]After surgery,one patient accepted the revision operation to remove the PDN because of device migration,the others experienced pain relief.The nineteen cases were followed-up for 23~29 months(mean 26.4 months).Compared with preoperative height of intervertebral disc,it gained increase of 17.2%(P
10.Clinical results of anterior decompression surgery for cervical myelopathy in aged patients

Objective To investigate clinical features and anterior surgical results of cervical myelopathy in patients more than 70 years of age. Methods Twenty aged patients with cervical myelopathy who underwent anterior decompression with bone graft surgery were reviewed. Neurological function was assessed using a scoring system proposed by the Japanese Orthopaedic Association (JOA score). The clinical results and complications were compared with the control group which included 31 patients with multisegmental cervical myelopathy and less than 69 years old and underwent the same operation. Results The preoperative mean JOA score was 9.3 (ranged from 3 to 14) and the JOA score at latest follow-up averaged 13.4 (ranged from 8 to 17) in the aged patient group. 68 percent patients had achieved an excellent or good result and the recovery rate was 58%. 71 percent patients had achieved an excellent or good result and the recovery rate was 67% in the control group. No statistical difference was found in the excellent and good result rate or the recovery rate of JOA score between the aged group and the control group (?字2=0.04, P=0.85;t=1.12, P=0.138). The incidence of surgical related complications in the aged group was 35% (7 cases), which was considerably higher than that in the control group (3 cases, 10%) but without statistical difference (?字2=3.47, P=0.06). In the preoperative flexion-extension stress lateral radiographs, the incidence of cervical instability was higher in the control group (8 cases, 26%) than that in the aged group(1 case, 5%). Conclusion 1) Multilevel lesion induced by overcompensation for cervical instability are the probable cause of myelopathy in aged patients. 2) Anterior decompression is beneficial to improve the neurological function and life style in the aged patients with cervical myelopathy, but associated with a high incidence of surgical complications.
