1.Femoral head replacement and clover titanium plate fixation in the repair of unstable intertrochanteric fractures in elderly patients

BACKGROUND:The unstable intertrochanteric fractures are common in the elderly patients. Artificial femoral head replacement is a common treatment method, and different ways can be selected for reconstruction. OBJECTIVE:To explore the clinical effect of femoral head replacement combined with clover titanium plate fixation for unstable intertrochanteric fractures of the elderly. METHODS:A retrospective analysis was performed in 64 elderly patients with unstable intertrochanteric bone in the Jiangyou 903 Hospital from December 2011 to December 2012. They were given femoral head replacement treatment, and were divided into control group (32 cases) and experimental group (32 cases) according to reconstruction method. They received steel wire tension band and reconstruction and clover titanium plate fixation. RESULTS AND CONCLUSION:There were no significant differences in operation time and bleeding volume in operation process, postoperative recovery time of joint activities, time of out-of-bed activity, and bone calus formation time between the two groups (P > 0.05). However, compared with the control group, therapeutic effect was better and the incidence of complications was low in the experimental group (P < 0.05). These data suggest that femoral head replacement combined with the implementation of clover titanium plate fixation treatment can obtain good results for treatment of unstable intertrochanteric fracture in elderly patients.
2.Effect of PEG-PEI/Fe_3O_4 Nano-magnetic fluid-TK on apoptosis of liver cancer cells(HepG2) in vitro

Objective:To investigate the effect of PEG-PEI/Fe3O4 Nano-magnetic fluid-TK on Apoptosis of liver cancer cells(HepG2)in vitro.Methods:Construcr ecombinant plasmid PEGFP-AFP-TK was constructed,which was delivered by nano-magnetic fuilds into AFP positive HepG2 cells and AFP negative SMMC7721 cells.The fluorescence was detected in order to evaluate the transfection rate 12h after transfection RT-PCR and Western blot were used to detect expression of HSV-TKgene at 48 h after transfection.MTT assay was used to evaluate the effect of HSV-TK on the proliferation of HepG2 cells,Flow cytometry was used to analyze the apoptosis of HepG2 cells.Results:Nano-Magnetic fluids delivered plasmid PEGFP-AFP-TK into HepG2 cells and HSV-TK gene was successfully expressed.The transfection eficacy of nano-magnetic fluids was superior than lipofectamine in HepG2 cells.RT-PCR and Western blot demonstrated that TK gene was expressed in HepG2 cells after being transfected with nano-magnetic fluids/PEGFP-AFP-TK.MTT and flow cytometry showed HSV-TK gene exerts a cell-killing effect.Conclusions:Nano-Magnetic fluids could successfully deliver PEGFP-AFP-TKinto HepG2 cells and induce expression of HSV-TK,it may become a promising gene vector for liver cancer gene therapy.AFP enhancer can specifically enhance the expression of target gene within the cell with positive AFP.

5.A binary logistic regression analysis of deep venous thrombosis after hip fracture surgery

Objective To explore the incidence, etiological and protective factors, and preventive countermeasures of deep venous thrombosis (DVT) in patients undergoing hip surgery. Methods Four hundred and forty-one patients who had hip fracture surgery in our department from January, 2005 to May, 2010were analyzed retrospectively. They were 223 males and 218 females, aged 65.7 years. (range, 22 to 87). A binary logistic regression was performed, using gender, age, duration of surgery, surgery type, anesthesia type, blood transfusion, physical therapy, thrombo-prophylaxis and complication as covariates, taking DVT incidence as the dependant variable. Results The patients obtained a median follow-up of 17.2 months (range, 3 months to 4 years and 7 months). Sixty-nine patients developed DVT (incidence, 15.6%).Analysis of multiple variables showed that duration of surgery, anesthesia type, blood transfusion, hypertension and diabetes were risk factors to DVT. Physical therapy and thrombo-prophylaxis were protective factors.Age, gender and surgery type had no statistically significant influence on DVT (P > 0.05). Conclusions The DVT incidence after hip fracture surgery is high. In order to decrease the DVT incidence, surgeons should try their best to estimate risk factors for every patient perioperatively, avoid general anesthesia or blood transfusion, shorten operating duration, effectively control complications, actively perform physical therapy or thrombo-prophylaxis, and prolong the anticoagulant therapy for patients with higher risk factors.
6.Multivariate Analysis of the Survival of 109 Lung Adenocarcinoma Patients with Brain Metastases

Objective: To investigate prognostic factors of survival of lung adenocarcinoma patients with brain metasta-ses.Methods: The clinical data of 109 lung adenocarcinoma patients with brain metastases were reviewed.Fifty-senven pa-tients survived more than 1 year, and 52 patients survived less than 1 year.The data were analyzed by SPSS 15.0 statisti-cal software.Results: Both the univariate analysis and multivariate analysis showed that patients' PS score, the interval be-tween diagnosis and brain metastasis, and multimodality treatment were independent prognostic factors for the survival of lung adenocarcinoma patients with brain metastases (P<0.05).Patients' gender, age, metastatic symptoms and extracrani-al metastases didn't significantly affect patient survival.Conclusion: Patients with PS score 0-1, the interval of more than 1 year between diagnosis and brain metastasis, and combined therapy have longer survival.The research provides an impor-tant guidance for the treatment of advanced lung adenocarcinoma patients with brain metastases.
7.Stereotactic radiation therapy in the era of precision medicine for cancer.

Unlike conventional radiation therapy, stereotactic radiation therapy (SRT) is an emerging tumor-ablative radiation technology with a high-dose delivery to targets while dramatically sparing adjacent normal tissues. The strengths of SRT involve noninvasive and short-course treatment, high rates of tumor local control with a low risk of side effects. Although the scientific concepts of radiobiology fail to be totally understood currently, SRT has shown its potential and advantages against various tumors, especially for those adjacent to less tolerable normal organs (spinal cord, optic nerve, bowels, etc.). Nowadays, the clinical efficacy of SRT has been widely confirmed in certain patients, especially for those medically inoperable, unwilling to undergo surgery, medicine ineffective with tumor progression. Moreover, SRT could be properly used as palliative treatment aiming at relieving local symptoms and pain, and eventually achieving a potential survival benefit of several months. However, the weaknesses of SRT relate to inevitable radiation-induced toxicities as well as the inaccessibility of prophylactic irradiation. In general, one flaw cannot obscure the splendor of the jade. The emergence and development of SRT has opened the new era of precision radiation therapy, and SRT will probably step gloriously onto the remarkable stage for precision medicine.

8.Chinese medical syndromes of rheumatoid arthritis: a clinical literature study.

OBJECTIVETo explore the distribution features of Chinese medical syndromes of rheumatoid arthritis (RA) by literature retrieval.
METHODSPertinent articles on treating RA by syndrome differentiation were retrieved from China National Knowledge Infrastructure Databases (CNKI), VIP Chinese Biomedical Journal Database, Guizhou Digital Library, and Duxiu Chinese Academic Periodicals full papers database from January 2000 to December 2011.
RESULTSA total of 33 documents were recruited covering 4 233 cases. Damp-heat blocking collaterals syndrome occupied the top in the occurrence frequency (20 times, 60.61%), followed by deficiency of Gan and Shen syndrome (18 times, 54.55%), intermingled phlegm and blood-stasis syndrome (17 times, 51.52%), wind-cold-damp impediment syndrome (15 times, 45.45%), cold-damp blocking collaterals syndrome (14 times, 42.42%), wind-damp-heat impediment and deficiency of qi and blood syndrome (10 times, 30.30%), and intermingled cold and heat syndrome (9 times, 27.27%). According to the case number of patients, it was sequenced as damp-heat blocking collaterals syndrome syndrome (768 cases, 18.14%), wind-damp-heat impediment syndrome(666 cases, 15.73%), wind-cold-damp impediment syndrome(584 cases, 13.80%), cold-damp blocking collaterals syndrome syndrome (517 cases, 12.21%), intermingled cold and heat syndrome (415 cases, 9.80%), intermingled phlegm and blood-stasis syndrome (364 cases, 8.60%), deficiency of Gan and Shen syndrome (235 cases, 5.55%),asthenia of healthy energy due to lingering arthralgia syndrome (223 cases, 5.27%). The case numbers of remaining syndromes did not exceed 5%.
CONCLUSIONDamp-heat blocking collaterals syndrome was the main syndrome in RA patients, followed by wind-cold-damp impediment syndrome,wind-damp-heat impediment syndrome,cold-damp blocking collaterals syndrome,intermingled phlegm and blood-stasis syndrome, and deficiency of Gan and Shen syndrome.

9.Effects of soil factors on yield and quality of Angelica dahurica var. formosana

Objective To study the effects of the soil factors on the yield and quality of Angelica dahurica originated from Sichuan Province.Methods The yield and contents of imperatorin,isoimperatorin,and other 11 different inorganic elements of A. dahurica,gathered from 15 different habitats of Suining in Sichuan Province were determined.And the basic soil nutrients and other 11 different mineral elements of the soil were analyzed.The effects of the soil factors on the yield and quality were analyzed through correlation analysis.Results The weight,length,circumference,and the contents of imperatorin and isoimperator in the roots of A.dahurica from different habitats were different.And the basic soil nutrients and other 11 different mineral elements of soil were different.The positive correlation between root length of A.dahurica,and soil available P content,root circumference of A.dahurica,and soil organic matter content,root weight of A.dahurica,and soil available P and K contents were all significant.The soil total nitrogen content had negatively significant correlation with the content of isoimperatorin.The correlation among the imperatorin,isoimperator contents of A.dahurica,and 11 other soil inorganic elements was not significant.Conclusion The soil factors have effects on the yield and quality of A.dahurica to some degree.
10.Laparoscopy in treatment of renal cysts

Objective To evaluate the effect of laparoscopy in treatment of renal cysts.Methods Twenty- eight and thirty-five patients with renal cysts were performed with transperituneal and retroperitonel laparoscopy,re- spectively.Results A total of 62 patients were treated successfully,the average operation time was 60 minutes,no complications presented,and there was no recurrence in follow-up.Conclusion Laparoseopy survey had the advan- tages of less trauma and fewer complications and rapid recovery in treatment of renal cysts,which was suggested for clinical applications.
