1.Effect of Continuous Passive Motion on Hand Spasm and Hand Function of Hemiplegic Patients

Objective To observe the effects of hand function training with continuous passive motion (CPM) on hand spasm and function of the hemiplegic patients.Methods 60 hemiplegic patients after stroke or brain trauma and with hand spasm were randomly divided into the treatment group and control group with 30 cases in each group. The cases in the treatment group were treated mainly with the method of CPM together with the routine rehabilitation procedures, while the those in the control group were only treated with the routine rehabilitation procedures. The changes of hand spasm and function of two groups were observed.Results After four weeks' treatment, the efficacy of the treatment group was obviously better than that of the control group ( P<0.05).Conclusion The rehabilitation training is an effective method for treatment of hemiplegia, and CPM is an important auxiliary method in the treatment. These two Methods combined together can shorten the course of treatment, and strengthen cure efficacy.
2.Clinical study of Shiwei-Jinhuang Ointment and Chinese herbal external washing prescription for acute paronychia

Objective:To evaluate the clinical efficacy of Shiwei-Jinghuang Ointment combined with Chinese herbal external washing prescription in the treatment of acute paronychia. Methods:A total of 126 patients with acute paronychia in Xiyuan Hospital of China Academy of Chinese Medical Sciences from January to December 2019, who met the inclusion criteria, were divided into 3 groups according to the random number table method (42 in each group). The treatment group was treated with Shiwei-Jinghuang Ointment combined with Chinese herbal external washing prescription. The control group 1 and group 2 were simply applied with Chinese herbal external washing prescription and mupirocin ointment, respectively. All groups were treated for 2 weeks and followed up 2 months. Serum CRP level was determined by ELISA method, and neutrophil percentage (NEU%) was measured by automatic hematology analyzer. The subsided time of red swelling and inflammatory exudation in nail groove of the paronychia patients were observed and recorded. Results:The total effective rate was 90.5% (38/42) in the treatment group, 71.4% (30/42) in the control group 1 and 66.7% (28/42) in the control group 2. The statistical difference were significant among three groups ( χ2=7.350, P=0.025). After treatment, the serum CRP level and NEU% in three groups were decreased ( P<0.01), and the serum CRP and NEU% level in treatment group were significantly lower than those in the control group 1 and group 2 ( F values were 6.482, 33.619, respectively, all Ps<0.001). After 2 months of follow-up, the incidences of chronic paronychia were 50.0% (6/12) and 57.1% (8/14) in control group 1 and group 2, respectively, but no chronic paronychia occurred in the treatment group. The difference among three groups was statistically significant ( χ2=9.355, P=0.009). Conclusion:The combination of Shiwei-Jinghuang Ointment and Chinese herbal external washing prescription can promote paronychia red swelling, reduce the inflammatory exudation time and the incidence of chronic paronychia, effectively improve the in vivo decrease inflammation index and clinical efficacy.
3.Advances in research on influencing factors of visual impairment in the elderly

The definition of visual impairment, the current status of visual impairment in the elderly, influencing factors related with visual impairment and management control modes are reviewed.There are three international standards of visual impairment including the WHO standard in 1972, the United States standard, and the WHO standard in 2003. The domestic standard is consistent with the WHO standard in 1972. Currently, with the global aging, the international and the Chinese situations of visual impairment are severe,and it is urgent to reduce the incidence of visual impairment. The visual impairment of the elderly is affected by various factors. A variety of management and control modes of visual impairment for the elderly have begun to bear fruit. The aim of the review is to provide the basis to screen for high-risk groups of visual impairment and provide timely intervention to reduce the prevalence of visual impairment in the older population.
4.Changes of IL-17 and IL-21 in glucocorticoid therapy of active TAO and its relation with the results

AIM:To discuss the changes of IL-17, IL-21 in glucocorticoid therapy of active thyroid associated ophthalmopathy (TAO) and analyze the relation with results.METHODS:The 67 patients (134 eyes) of TAO in our hospital, were divided into activity group(32 patients 64 eyes) and atypical activity group(35 patients 70 eyes) according clinical activity score (CAS) grading standard, and 30 cases of healthy as control group at the same time.The activity group were treated by glucocorticoid therapy treatment, and proceed the CAS scores before and after treatment, measured the degree of exophthalmus and width of palpebral fissure.At the same time, compared the expression level of IL-17 and IL-21 in all groups, and analyzed the correlation between the IL-17 and IL-21 and CAS score.RESULTS:Compared with control group, expressions of IL-17 and IL-21 in TAO patients were significantly higher (P<0.05).The expressions of IL-17 and IL-21 in active period TAO patients were higher than atypical activity (P<0.05).After glucocorticoid treatment, the expressions of IL-17 in active period TAO patients decreased significantly(P<0.05), and it was significantly positive correlation with CAS score (before treatment:r=0.8847,P=0.042;after treatment:r=0.8886,P=0.0439) the expression of IL-21 in active period TAO patients was significantly positive correlation with CAS score (before treatment:r=0.8893, P=0.0435;after treatment:r=0.8876,P=0.045).CONCLUSION:IL-17 and IL-21 is closely related to the TAO disease activity, and glucocorticoids impact treatment by reducing IL-17 and IL-21 in activity TAO, IL-17 and IL-21 can be used as one of indexes of predicted curative effect and condition in patients with TAO.
5.Research progress on early diagnosis of primary open angle glaucoma

Glaucoma is a progressive optic nerve disease, it can cause structural changes of the optic nerve, resulting in irreversible damage of visual function. Early diagnosis is the key to treat the disease at an early stage to stop or delay the progression of visual functional defects. New technologies, including optic nerve and nerve fiber layer structural tests and visual functional tests, increased the diagnosis rate of primary open angle glaucoma (POAG) at an early stage.

Therapies for uveitis include drug treatment and surgery.Medical therapies including proper administration of corticos- teroids,immunosuppressive drugs,nonsteroidal anti-inflammtion drugs and anti-infective drugs are important in management of the in- traocular inflammation and decreasing the risk of complications,furthermore,are the basis for surgical treatment of uveitis.
7.Substitute popliteal vein valve operation for primary deep vein insufficiency of lower limbs

Objective To investigate the clinical value of substitute popliteal vein valve operation (SPVSO) for primary lower limb deep vein insufficiency(DVIS). Methods The clinical data of 859 cases(907 legs) of DVIS who underwent SPVSO in the past 10 years in our hospital were analyzed retrospectively. Results Symptoms of edema and fatigue distinctly reduced or disappeared after the operation.Pigmentation in the lower portion of the leg began to disappear in 1 week, and nearly or totally disappeared within 2 weeks after the operation.Ulcer in the lower portion of the leg recovered three weeks after the operation. Patients were followed up for 6 to 120 months. The effective rate within 60 months was 96.8%, and the effective rate over 60 months was 87.2%. Conclusions The curative effect of SPVSO on primary deep vein insufficiency of lower extremity is definite and it is worthy to be clinical practice.
8.Immune function affer postoperative parenteral nutrition enriched with arginine in patients with gastrointestional carcinoma

Objectives:In order to study the immune function after postoperative parenteral nutrition enriched with arginine in patients with gastrointestinal carcinoma. Methods:Forty patients who underwent radical operation of gastrointestinal carcinoma were randomly divided into experimental group( n= 20),in which patients received TPN enriched with arginine,and control group (TPN, n =20).At day 1 before operation,day 1 and day 8 after operation,IgG,IgA,IgM,T cell subgroups.NK cell activity and IL 2 were dectected. Results:The immune function of patients with gastrointestinal carcinoma was depressed at different levels.At day 1 after operation,IgG,IgM,IgA,CD4 +,CD4 +/CD8 +,Nk cell activity and IL 2 decreased more significantly than those before operation.The immunefunction recovered rapidly and exceeded the level before preoperation in experimental group.The difference was significant( P
9.Evaluation of efficacy and safety of different doses Octreotide combined with Lansoprazole in the treatment of hepatocirrhosis with upper gastrointestinal bleeding

Objective To evaluate the efficacy and safety of different doses Octreotide combined with Lansoprazole in the treatment of hepatocirrhosis with upper gastrointestinal bleeding.Methods Eighty hepatocirrhosis patients with upper gastrointestinal bleeding during May.2013 to May.2016 were treated with Octreotide combined with Lansoprazole,and were divided into high dose group (40 cases) and low dose group (40 cases) according to different doses Octreotide.Before and after treatment,portalvenousmaximumvelocity (PVV) and portalveindiameter (PVD) were detected by color Doppler ultrasound respectively,and haemostatic time,blood transfusion cases and volume,rehaemorrhagia cases were recorded,and adverse reactions were observed.Results After treatment,PVV and PVD of the two groups were significantly ameliorative (P < 0.05),and there were no significantly different between two groups.Haemostatic time,blood transfusiorate,blood transfusiorate volume and rehaemorrhagia rate of the high dose group were significantly less than the low dose group (P < 0.05).The clinical effective rate of the high dose group was significantly higher than that of the low dose group (P < 0.05).The rate of adverse reactions of the high dose group was not significantly different from that of the low dose group.Conclusions High doses Octreotide combined with Lansoprazole in the treatment of hepatocirrhosis with upper gastrointestinal bleeding have a gain on hemostatic effect,and cannot increase adverse reactions significantly.
