1.The feasibility analysis of hypofractionated high-dose three-dimensional conformal radiotherapy in the treatment of esophageal carcinoma

ObjectiveTo evaluate the instant response and side effects of hypofractionated high-dose 3-dimensional conformal radiotherapy (3-DCRT) for esophageal carcinoma treatment.MethodsSixty-nine patients with esophageal carcinoma were treated by 3-DCRT and 91 patients with routine radiotherapy.The prescribed dose was from 3000 to 4500 cGy in 3-DCRT group, with fractionated dose 300 cGy, one dose a day and five doses a week.The reference dose curve was the 90% of the isodose curve.For the routine radiotherapy group,the prescribed dose was from 4000 to 6600 cGy,fractionated by 180 ~200 cGy a time,once a day and 5 times a week.Results At the end of the radiotherapy, 3-DCRT produced CR 42.0% (29/69) , PR 49.3% (34/69) and instant response rate 91.3%, while routine radiotherapy produced CR 35.2% (32/91), PR 59.3% (54/91) and instant response rate 94.5%, respectively.The difference was not significant between the two groups(P > 0.05) .The survival rates one year after the treatment were 82.6.0% (57/69)and 84.6% (77/91) ,respectively.The acute radiation esophagitis rate was 66.7% (46/69) in 3-DCRT group and was 64.8% (59/91) in routine radiotherapy grouprespectively.ConclusionHypofractionated high-dose 3-DCRT is feasible for treatment of esophageal carcinoma.
2.Relationship of cholesterol ester transfer protein TaqIB gene polymorphism with type 2 diabetes mellitus in Chinese Han nationality patients

Objective To explore the relationship between plasma cholesterol ester transfer protein(CETP) and TaqIB gene polymorphism and type 2 diabetes mellitus(DM).Methods The genotypes of the first intron of CETP TaqIB gene in 102 patients with type 2 DM and 103 healthy controls were analyzed by polymerase chain reaction-restricted fragment length polymorphism(PCR-RFLP),and plasma CETP were measured by enzyme-linked immunosorbent assay(ELISA).The association of gene polymorphism with plasma lipids,lipoproteins and apolipoproteins was also analyzed.Results No significant differences in both of the alleles frequencies and genotypes in type 2 DM were noted in comparison with controls.There were no relationships between CETP TaqIB gene polymorphism and gender,family history,smoking and body mass index(BMI).No significant differences of plasma lipid levels were observed in healthy controls with different genotypes.However,significant differences in level of HDL-C or apoAI among different CETP TaqIB genotype groups were observed,and significant correlation between low high-density lipoproteinemia and B1 allele in type 2 DM was observed.Conclusion CETP TaqIB gene polymorphisms associated with lipid metabolism,which may be an important genetic factor for abnormal lipid metabolism in type 2 DM.
3.Preparation,Quality Control and Clinical Application of Xiaozhong Zhitong Cataplasma

OBJECTIVE:To prepare Xiaozhong zhitong cataplasma,establish a method for its quality control and to observe its clinical efficacy on orthopedic patients(30 cases).METHODS:Based on the precompounded prescription order of Xiaozhong zhitong powder with water soluble polymer substance as base material which were evenly mixed with the drug extractives before being applied on non-woven fabric to prepare Xiaozhong zhitong cataplasma.The clinical efficacy was compared between the trial group(Xiaozhong zhitong cataplasma) and the control group(Xiaozhong zhitong powder).RESULTS:The prepared Xiaozhong zhitong cataplasma had smooth surface and uniform thickness,with a satisfactory stability achieved within 3 months at a dosage of 0.715 6 g?100 cm-2.The total effective rate in the trial group(93.3%) was significantly higher than in the control group(76.7%)(P
4.Application of Antibiotics in 843 Inpatients and Analysis of Irrational Drug Use

OBJECTIVE:To investigate the use of antibiotics in the inpatients of our hospital.METHODS:The medical records of inpatients from Jan.2004 to Dec.2006 randomly selected in our hospital were analyzed statistically in respect to the irrational use of antibiotics.RESULTS:Of the total 1,120 inpatient medical records examined,843(75.3%) used antibiotics,56 involved irrational use of antibiotics,which manifested as improper in dosage,dosage regimen,drug combination or solvents selection etc.CONCLUSION:The irrational use of antibiotics is quite common,thus the role of clinical pharmacists should be strengthened to promote rational use of antibiotics in clinical practice.
5.Ultrasound diagnosis of occult inguinal hernia in children

ObjectiveTo evaluate ultrasound diagnosis of occult inguinal hernia in children. MethodsThe contralateral site of 132 established unilateral inguinal hernia cases was checked by ultrasound and 33 outpatient suspected occult inguinal hernia cases underwent B-mode ultrasonography. [WT5”HZ]Results Occult inguinal hernia was diagnosed in 15 cases (11.4%) in the 132 children,and diagnosis of occult inguinal hernia was founded by ultrasound in 17 out of the 33 outpatient cases. No opposite inguinal hernia developed in the ultrasound negative 117 patients during follow up of 2 to 12 months,and hernia developed in one out of the 16 outpatient cases initially judged as negative by ultrasound. ConclusionUltrasonography is useful for the diagnosis of occult inguinal hernia in children.
6.CONSTRUCTION OF CD34 EXTRACELLULAR REGION ENCODING cDNA CLONE, AND EUKARYON EXPRESSION VECTOR AND ITS PRELIMINARY APPLICATION IN MONOCLONAL ANTIBODY PREPARATION BY GENE IMMUNIZATION

Objective To clone CD34 extracellular region encoding cDNA and to construct its eukaryon expression vector to explore the feasibility of its monoclonal antibody preparation by gene immunization. Methods Total RNA was extracted from KG-1a cell lines. CD34 extracellular region encoding genes were amplified by RT-PCR method and then confirmed by enzymatic lysis and DNA sequencing, and its eukaryon expression vector was constructed as pcDNA3.1-CD34. Twelve BABL/c mice aged 4-6 weeks were selected and randomly divided into 3 groups. Before immunization, 50?l 25% saccharin solution was injected into mouse musculus quadriceps femoris. Fifteen minutes later, blank control PBS, PBS diluted empty vector pcDNA3.1(50?g), or PBS diluted pcDNA3.1-CD34 was injected into the same site of the above three groups, respectively. Immunization was taken every two weeks for a total of three times. The antibody was detected regularly by FACS using tail blood from immunized mice. Results The results demonstrated that the length of CD34 cDNA was 886bp which was identical to the theoretical value and its sequence was confirmed by DNA sequencing which was identical to the registered sequence. The accuracy of CD34 expression vector recombination was confirmed by restriction enzymatic lysis. Hind III and EcoRI restriction enzymatic lysis sites were introduced into 5 and 3 terminals of amplified cDNA sequence, respectively. Terminate code TGA was also introduced into CD34 extracellular encoding cDNA. The expression vector possessing target genes was named as pcDNA3.1-CD34. FACS detection indicated that only 1/4(25%) immunized mice had a lower titer CD34 antibody in their tail vein serum 2-6 weeks after immunization, and the others did not show no antibody production. Conclusion The eukaryon expression vector pcDNA3.1-CD34, which express CD34 extracellular region, has been constructed. The feasibility of CD34 McAb preparation can primarily be confirmed by gene immunization.

Chronic rhinosinusitis (CRS) is a common disease in otolaryngology, and with the development of the therapy technology, most of the cases have reached a cure. But there are still some intractable cases in clinic, signs and symptoms of chronic rhinosinusitis of which still persist after the standardized drugs and technically adequate endoscopic sinus surgery, and this part is clinically named refractory chronic rhinosinusitis (RCRS). In recent studies of the pathogenesis of RCRS, bacterial biofilm (BBF) is attracting more attention. The main emphasis of this review will be to highlight the biological characteristics of BBF, the relationship of BBF and CRS and treatment strategies of BBF positive CRS.

9.Assessing Radiosensitivity of Lung Cancer with the Expression ofγH2AX

Objective To observe the relationship between expression changes of γH2AX and the radiosensitivity of lung cancer cells in vitro. Methods The radiosensitivity of lung cancer cell lines A549 and SBC-3 was measured by clone forming assay. The DSBs damage of lung cancer cell lines A549 and SBC-3 was determined by Western blot assay. Re-sults The clone forming rates of lung cancer cell lines A549 and SBC-3 were gradually decreased with the increased radia-tion dose.γH2AX expression was related to the cell radiosensitivity 1 hour and 6 hours after radiated. Conclusion The phosphorylated histoneγH2AX is a powerful tool to monitor DNA DSBs and to predict the radiosensitivity in lung cancer ra-diotherapy.
10.Influence of Medication on the Oscillatory and Dynamic Characteristics of Subthalamic Local Field Potentials in Patients with Parkinson's Disease.

The dysfunction of subthalamic nucleus is the main cause of Parkinson's disease. Local field potentials in human subthalamic nucleus contain rich physiological information. The present study aimed to quantify the oscillatory and dynamic characteristics of local field potentials of subthalamic nucleus, and their modulation by the medication therapy for Parkinson's disease. The subthalamic nucleus local field potentials were recorded from patients with Parkinson's disease at the states of on and off medication. The oscillatory features were characterised with the power spectral analysis. Furthermore, the dynamic features were characterised with time-frequency analysis and the coefficient of variation measure of the time-variant power at each frequency. There was a dominant peak at low beta-band with medication off. The medication significantly suppressed the low beta component and increased the theta component. The amplitude fluctuation of neural oscillations was measured by the coefficient of variation. The coefficient of variation in 4-7 Hz and 60-66 Hz was increased by medication. These effects proved that medication had significant modulation to subthalamic nucleus neural oscillatory synchronization and dynamic features. The subthalamic nucleus neural activities tend towards stable state under medication. The findings would provide quantitative biomarkers for studying the mechanisms of Parkinson's disease and clinical treatments of medication or deep brain stimulation.
