1.Clinical Observation of Biyuan Tongqiao Granule Combined with Triamcinolone Acetonide Spray in the Treatment of Chronic Rhinosinusitis

OBJECTIVE:To observe the clinical efficacy and safety of Biyuan tongqiao granule combined with Triamcinolone acetonide spray in the treatment of chronic rhinosinusitis. METHODS:120 patients with chronic rhinosinusitis were randomly divid-ed into observation group and control group,with 60 cases in each group. Control group was given Triamcinolone acetonide spray 220 μg,qd,in first week,decreasing to 110 μg,qd;observation group was additionally given Biyuan tongqiao granule 15 g,tid, on the basis of control group. Both groups were treated continuously for 10 weeks. The clinical efficacy of 2 groups were observed as well as the levels of MTR,IL-5 and IL-8,SNOT-20 score,VAS score and Lund-Mackey sinus CT score before and after treat-ment. The incidence of ADR was compared between 2 groups. RESULTS:Total effective rate of observation group was 93.3%, which was significantly higher 85.0% of control group,with statistical significance(P<0.05). There was no statistical significance in IL-5,IL-8,MTR,SNOT-20 score,VAS score and Lund-Mackey sinus CT score between 2 groups before treatment(P>0.05). IL-5 and IL-8 levels of 2 groups decreased significantly,while MTR,SNOT-20 score,VAS score and Lund-Mackey sinus CT score were decreased significantly;the observation group was better than the control group,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Biyuan tongqiao gran-ule combined with Triamcinolone acetonide spray is effective in the treatment of chronic rhinosinusitis,and can effectively improve the quality of life and relieve clinical symptom with good safety.
2.The Clinical Significance of Cerebrovascular Reserve and Its Determination Methods

Cerebrovascular reserve is an important parameter for evaluating cerebrovascular disease,and it has great clinical significance.There are a number of determination methods for cerebrovascular reserve,such as position emission tomography(PET),single photon emission computed tomography(SPECT),Xenon-133 inhalation technique,Xenon CT,perfusion CT, magnetic resonance imaging(MRI),transcranial Doppler(TCD),transcranial harmonic perfusion imaging(HPI),near infrared spectroscopy(NIR),and Laser Doppler vibrometry(LDV), however,there have been no unified criteria up to now.The provocation test that makes cerebral blood flow increase includes CO_2 inhalation test,breath holding test,and acetazolamide test,etc. The article focuses the discussion on the advantages and disadvantages of the above determination methods.

4.Transdifferentiation of alternatively activated macrophages into lymphatic endothelial cells induced by VEGF-C

Objective To explore whether alternatively activated macrophages (aaMphi) can transdifferentiate into lymphatic endothelial cells (LEC) under the inducement of VEGF-C, and to investigate the possible mechanisms involved in aaMphi-induced lymphangiogenesis. Methods An aaMphi model constructed by treating mouse macrophage cells RAW264.7 with mouse recombinant IL-4 for 24h was treated with different concentrations of recombinant mouse vascular endothelial growth factor-C (VEGF-C). After the clustering formed and the tube-like structures were detected in the matrigel, the aaMphi transdifferentiation system was finally decided to be constructed with the VEGF-C in the concentration of 100ng/ml. The mRNA expression of LEC specific markers, VEGFR-3 and Prox1, and aaMphi specific marker, Fizz1, were detected by real time quantitative RT-PCR on the 0, 7th, 14th, and 28th day, respectively, after VEGF-C stimulation. Formation of tube-like structure in the matrigel was observed with inverted phase contrast microscope in 28 consecutive days. Results The VEGF-C induced transdifferention system, incubated in EBM-2 medium and sustained by the matrigel, was successfully established. In this system, it was found that the mRNA expression of VEGFR-3 and Prox1 gradually increased, whilst that of Fizz1 decreased. The mRNA expression of VEGFR-3 and Prox1 reached the peak value, whilst that of the Fizz1 went down to the nadir, on the 14th day. No significant difference in values was found between the 14th day and the 28th day. During the period of the 7th day to the 28th day, distinct tube-like structures were gradually formed in the matrigel and the numbers increased in a time-dependant manner. Conclusion VEGF-C can induce the transdifferentiation of aaMphi into LEC by up-regulating the mRNA expression of VEGFR-3 and Prox1 in aaMphi, which is one of the possible mechanisms involved in aaMphi-induced lymphangiogenesis.
5.Prevention and treatment of endotoxemia by recombinant human growth hormone in murine experimental obstructive jaundice

Objective To evaluate the effect of recominant human growth hormone(rhGH) in the prevention and treatment of endotoxemia(ETM) in murine experimental obstructive jaundice (OJ) Methods Twenty OJ rats received subcutaneously given rhGH of 0 75*!IU/kg daily for 2 weeks (rhGH group). Blood endotoxin(ET) was measured, and intestinal mucosa was observed under electron microscope at 5th week.Results In rhGH group, ET level 〔(0 40?0 02)*!EU/ml〕 was significantly lower than OJ rats〔(0 77?0 03)*!EU/ml, n =20〕 ( t = 6 237, P 0 05). Under electron microscope bowel villi in OJ group were distroyed, epithelial cells were degenerated and necrotic, whereas in rhGH group the ultrastructural pathology was much less evident and similar to that in sham operation rats. Conclusion rhGH has significant effect on protecting the injured mucosa barrier in OJ,and decreases ETM significantly.
7.Cultivation strategy and path analysis on big brand Chinese medicine for small and medium-sized enterprises.

Small and medium-sized enterprises (SMEs) are important components in Chinese medicine industry. However, the lack of big brand is becoming an urgent problem which is critical to the survival of SMEs. This article discusses the concept and traits of Chinese medicine of big brand, from clinical, scientific and market value three aspects. Guided by market value, highlighting clinical value, aiming at the scientific value improvement of big brand cultivation, we put forward the key points in cultivation, aiming at obtaining branded Chinese medicine with widely recognized efficacy, good quality control system and mechanism well explained and meanwhile which can bring innovation improvement to theory of Chinese medicine. According to the characters of SMEs, we hold a view that to build multidisciplinary research union could be considered as basic path, and then, from top-level design, skill upgrading and application three stages to probe the implementation strategy.

8.Relationship between anemia and the severity of coronary artery lesions

Objective: To investigate the relationship between anemia and severity of coronary artery lesions.Methods: A total of 227 consecutive patients with coronary artery disease(CAD) were studied.According to their hemoglobin levels,all patients identified positive by selective coronary angiography(CAG) were divided into two groups,anemic group and non-anemic group,their clinical types and severity of coronary artery lesions were compared.Results: The proportion of anemia in unstable angina and infarction group was higher than those in stable angina group,similarly the proportion of anemia was also high in severe stenosis,occlusion and three-branch stenosis groups than that in other groups.Conclusion: Anemia is possibly one of the predictive factors suggesting CAD's clinical types and deterioration of coronary artery stenosis.
9.Hemodynamic Study on Central Retinal Artery in Patients with Ischemic Cerebrovascular Disease by Color Doppler Flow Imaging

Objective To explore the feasible of diagnosing ischemic cerebrovascular diseases by detecting central retina artery sclerosis(CRA) with color Doppler ultrasonography.Methods210 patients with ischemic cerebrovascular disease(420 eyes) and 80 healthy volunteers(160 eyes) were examined by color Doppler flow imaging(CDFI) to detect double sides blood frequency spectrum of CRA and hemodynamic parameters of CRA including peak systolic velocity(PSV),end of diastolic velocity(EDV)and resistant index(RI).ResultsThe blood frequency spectrum of CRA showed three apices and two vales or slanting triangle in healthy volunteers.But in patients with ischemic cerebrovascular disease,the blood frequency spectrum changed,and PSV and EDV were slower and RI increased compared with those in the healthy volunteers(PP<0.05~0.001).ConclusionIn patients with ischemic cerebrovascular disease,the blood flow of CRA is slower and RI increases compared with health individuals.The color Doppler ultrasonography can used for diagnosis of CRA sclerosis.
10.The antidiabetic and hepatoprotective effects of magnolol on diabetic rats induced by high-fat diet and streptozotocin.

The effects of magnolol (Mag) on hyperglycemia and hyperlipemia, hepatic oxidative stress and cytochrome P4502E1 (CYP2E1) activity of diabetic rats induced by high-fat diet (HFD) and streptozotocin (STZ) were studied. After oral administration of Mag (25, 50 and 100 mg x kg(-1) x d(-1)) for continuous 10 weeks, the blood glucose and lipids (TC, TG and LDL-C) levels, as well as the hepatic CYP2E1 activity and MDA content of diabetic rats, decreased significantly (P < 0.05 or P < 0.01), whereas the oral glucose tolerance and hepatic antioxidant enzymatic activities (CAT and GSH-Px) of diabetic rats, increased significantly (P < 0.05 or P < 0.01). The results indicated that Mag was effective against the hepatic oxidative damage, hyperglycemia and hyperlipemia of diabetic rats induced by HFD and STZ, and the inhibition of Mag on hepatic CYP2E1 activity could be an important mechanism of Mag against hepatic insulin resistance and oxidative damage.
