1.Evaluation of the timing of orthodontic arch expansion and graft in cleft lip and palate.

Maxillary transverse growth is inhibited by congenital cleft, early surgical scar strain, and oppression of lipmuscles in patients with cleft lip and palate. Clinical manifestations have shown severely constricted maxilla, insufficientmaxillary width, mismatch of upper and lower dental arches, and crossbite. Alveolar bone graft and arch expansion can effectively correct the deficiency in maxillary width. This paper discusses the timing and success rate of alveolar bone graft, as wellas the relationship between alveolar bone graft and arch expansion. Secondary alveolar bone graft is optimally performed beforepermanent canine eruption, especially when the teeth have formed between half and three quarters of their roots. Rapid maxillaryexpansion prior to alveolar bone graft is beneficial because this process increases the gap of the cleft, expands bone graft, andreduces the difficulty. However, the stability of this process remains controversial. Small-scale studies have reported that rapidmaxillary expansion after alveolar bone graft can open the midpalatal suture without bone graft loss. Slow maxillary expansioncan provide continuous light forces to reconstruct the bone. However, these studies are coordinated with fixed orthodontictreatment. Further research is necessary to determine the effects of maxillary expansion on long-term stability of teeth.

2.Bronchioloalveolar Carcinoma:Imaging Classification and Dynamic Changing

Objective To research the imaging classification and dynamic changing of bronchioloalveolar carcinoma (BAC). Methods The imaging types of BAC proved by operation and pathology in 32 cases were divided into single nodules, single segment or lobe consolidation, multiple segments or lobar consolidation and multiple diffuse nodules according to the features of plain chest films and CT at first examination. The spreading lesions in lung were recorded.The developing course and changes in size of lesions on the followed-up plain films and CT were observed, then the developing rules of different types of BAC were analyzed. Results The single noduli were enlarged and became poor defined opague shadows gradually. The single segment or lobar consolidation enlarged until the whole lobe. All single lesions progressed and coexisted with diffused nodular or opaque pulmonary involvement eventually. Multiple segments or lobar consolidation developed from peripheral to center of the segment or lobar, diffused pneumonia-like shadows appeared simultaneous. Multiple extensive nodules showed a trend of getting larger and then fused.Conclusion Different types of BAC have theirown developing rule,but they finally form a consequence that the primary lesion coexisted with the spread ones from different way.
3.Peer assessment rating analysis of the influencing factors of treatment outcomes of 80 cases underwent ortho-dontic therapy

Objective:To analyze some clinical factors influencing the treatment outcomes by Peer assessment rating(PAR).Meth-ods:80 cases underwent othordontic therapy were included.The clinical data and treatment effects were analyzed by PAR,the behav-ioral and psychological evaluation of the subjects was collected and analyzed by statistics.Results:Improvement and great improvement were achieved in 42.5% and 56.3% of the patients.There was no significant difference in different groups of gender,bone-facial con-tour classification and tooth extraction,but there was a significant difference in different groups of Angle classification in terms of pre-treatment PAR and reduction of PAR.There was a linear regression between pre-treatment PAR and reduction of PAR and percentage reduction of PAR,respectively.PAR showed more decrease in the patients of Angle class III classification,than in those with Class II and Class I.Conclusion:PAR is less influenced by gender,bone-facial contour,classification and tooth extraction.PAR evaluation shows that the more severe initial Angle malocclusion the patients have,the more significant improvement they may finally obtain.

Objective To detect the levels of homocysteine (Hcy) and anti-phospholipid antibodies (APLA) in the hematoplasma of the patients with deep venous thrombosis (DVT), discuss the reason of DVT recurrence and search for the predictors of it. Methods Sixty cases with DVT in our department from January 2001 to April 2003 were collected, which were divided equally into two groups as primary and recurrent, and first degree relative of the 30 DVT recurrent patients were also collected. The author established a control group using 30 cases of out-patient clinic without varicose veins of lower extremity or insufficient venae profundae. Hcy was detected with fluorescence polarization immunoassay (FPIA) and APLA 〔anticardiolipin antibody, ACLA (IgG, IgM); lupus antibody (LA)〕were detected with enzyme linked immunosorbent assay (ELISA). Odds ratios (OR) were also calculated to assess the relative risk of each study group. Objective To improve the diagnostic quality of tumors composed of smooth muscles in alimentary tract.Methods The clinical and X-ray date of 40 cases of smooth musles tumors confirmed by surgery and pathological examination.Results Fifteen cases in the esophagus (including leiomyoma 14,leiomyosarcoma 1)showed the following X-ray appearence:filling defect,unfolding of mucous membrane,widening of the canal and pericanal mass 25 cases in the stomach and intestine(including leiomyoma 6,leiomyoplastoma 7,leiomyosarcoma 12)showed the following X-ray changes depending on the way of tumor,grows,cyst formation caused by tumor necrosis,compression of the stomach or intestine and adjacent organs,rarely seen calcification in tumor.The X-ray changes of small intermascular leiomyoma were short of specific X-ray signs.Conclusion X-ray exeamination is a reliable diagnostic method for smooth muscles tumors in alimentary tract.

7.Studies on the reform of payment system of grass-roots health care institutions in China

The paper presented the payment system theory of the primary health care service in China,the current status of the health service system,and analyzed main challenges for the time being.Based on such studies,the authors made the following policy proposals.The first is adjustment of the fiscal payment method of the government to such institutions in line with the classification guidance principle; the second is to shift the payment method of primary public health services to the post payment of fee-for-service; the third is reform of the performance-based salary system to link payment to medical workers directly with the amount and quality of their services; the fourth is a set of result-oriented performance appraisal indicators,with rising proportion of performance pay; the fifth is to integrate the outpatient clinic fund covered by medical insurance and the primary public health service fund,into a capitation payment.
8.Hypotensive resuscitation for traumatic hemorrhagic shock: a systematic review

Objective To systematically review the efficacy of hypotensive resuscitation for traumatic-hemorrhagic shock.Methods Randomized controlled trails (RCTs) or quasi-Randomized controlled trails (qRCTs) were searched in Pubmed,Embase and the Corchrane Library from inception to August 2015.Two reviewers respectively picked out the useful data and performed quality evaluation.Metaanalysis was carried out with RevMan 5.3 software,risk ratio (RR) and its 95% confidence interval (CI) were pooled to estimate the enumeration data,and GRADE 3.6.1 software was used to rate the level of evidence.Results The results of meta-analysis and GRADE rating system which included 4 studies showed that:compared with conventional resuscitation,hypotensive resuscitation was associated with lower total mortality [RR =0.77,95% CI:0.62-0.95,P =0.01;n =984,GRADE rating:moderate],and 24-hour mortality [RR =0.47,95% CI:0.24-0.91,P =0.03;n =281,GRADE rating:moderate],but the subgroup analysis of total mortality showed that there were no significant differences in mortality between the subgroup of blunt or penetrating trauma and the subgroup of penetrating trauma.Conclusions Hypotensive resuscitation reduced total mortality and 24-hour mortality,and the quality of the evidence was moderate.The future studies should do further research to explore the efficacy of hypotensive resuscitation for different types of trauma.
