1.The effect of intravascular low energy laser irradiation on renal function of patients with early diabetic nephropathy as revealed by the SPECT

Objective To investigate the effects of intravascular low energy laser irradiation on blood (ILIB) on renal function of the patients with early diabetic nephropathy. Methods Fifteen patients with type 2 diabetes mellitus complicated with early diabetic nephropathy, were treated with ILIB in addition to the conventional drugs, and observed the maximal time(Tmax)of renal concentration,the variation of 10 minutes residual rate and 15 minutes residual rate, respectively, by using the SPECT of isotone 99mTc-EC before and after ILIB treatment, and compared the results obtained before and after ILIB treatment. Results There were significant differences with regard to the 10 minutes residual rate (both the left and right kidney,P0.05). Conclusion ILIB could improve the excretion and secretion function of renal tubule in patients with the type 2 diabetes mellitus complicated with early diabetic nephropathy, and ILIB could be used as an effectively therapeutic method for patients with early diabetic nephropathy.
2.Experience in the Management of the Facilities and Equipment of Pharmacy Intravenous Admixture Ser-vice in Our Hospital

OBJECTIVE:To provide a reference for the management of the facilities and equipment of the pharmacy intrave-nous admixture service(PIVAS). METHODS:The experience in the management of the facilities and equipment of PIVAS in our hospital was introduced in respect of system establishment,archives management and the maintenance of important equipment,and the performance of core purification equipment was tested to evaluate management effect. RESULTS & CONCLUSIONS:The PIV-AS in our hospital established a practical system by developing and improving 7 management systems,9 sets of operational proce-dures and 5 contingency plans,increased the awareness of archives management by perfecting 27 archive files and standardized and conducted the maintenance of key equipment. In 2014 and 2015,after implementing management,the passing rates of all test items of the purification equipment were 100%. It is suggested that facilities management should focus on active preparation of an intelli-gent monitoring system,the improvement of the information system and the training of professional management personnel.
3.Detection of circulating tumor cells from peripheral blood in patients with esophageal squamous cell carcinoma and its clinical significance

ObjectiveTo explore the relationship between circulating tumor cells(CTCs) in the peripheral blood of patients with esophageal squamous cell carcinoma(ESCC) and the physiopathological characteristics of esophageal neoplasms.MethodsUsing negative selection system,we depleted red blood cells(RBCs) in red blood cell lysis buffer,depleted white blood cells (WBCs) with Miltenyi magnetic beads and enriched the rare cells from ESCC patients'peripheral blood.Immunofluorence staining (IF) was adopted to identify CTCs.ResultsCirculating tumor cells in the peripheral blood of patients with esophageal squamous cell carcinoma was closely related to cell differentiation grade,the invasion of primary cancer,lymph node status,P-TNM stages,and was rarely related to the sex,age or the location of tumor.ConclusionThe results suggest that circulating tumor cells in the peripheral blood of patients with esophageal squamous cell carcinoma may express the development of esophageal cancer and may be served as a tumor marker to evaluate the biological behavior of esophageal cancer.
4.Carcinoma of the lung and multiple primary malignant tumors.

BACKGROUNDTo evaluate the incidence of multiple primary malignancies combined with lung and other organs and their relationship.
METHODSAll patients who consecutively treated for lung cancer from December 1964 to December 1992, were retrospectively analyzed regarding the recurrence of a synchronous primary tumor elsewhere in the body.
RESULTSOf total 1019 patients reviewed, 21 (2.1%) had associated carcinoma of other organs. In these 21 patients, the incidence of associated aerodigestive tract cancer was 71.4% (15/21) and that of other organs was 28.6% (6/21). Five patients had simultaneous cancer with the lung cancer and 16 had metachronous cancer.
CONCLUSIONSMultiple primary tumors in patients with carcinoma of the lungs is not a rare phenomenon. Surveillance programs for detection of second primary tumors should focus on these patients.
5.Inhibitory Effect of Endostar on Lymphangiogenesis in Non-small Cell Lung Cancer and Its Effect on Circulating Tumor Cells

Background and objective It is unclear how could endostatin effect tumor lymphangiogenesis? hTe aim of this study is to explore inhibitory effect of recombinant human endostatin injection (endostar) on lymphangiogenesis in non-small cell lung cancer (NSCLC) tissue and its effect on circulating tumor cells (CTC) in peripheral blood. Methods Tu-mor-bearing model nude mice were divided into eight groups randomly (n=7), including control group, cisplatin group, several concentration endostar groups and endostar plus cisplatin groups. Continuous administration of Endostar for two weeks, ob-served one week atfer the end of administration. Using HE staining and immunohistochemical staining to diagnose the tumor tissue and suspect metastasis lymph nodes, detected vascular endothelial growth factor (VEGF)-C, VEGF-D, VEGF receptor (VEGFR)-3 expression level and microlymphatic vessel density (MLVD) of tumor tissue. Enrichment of circulating tumor cells in peripheral blood used immunomagnetic negative selection strategy, used immunolfuorescence staining to diagnose and count CTCs. Results Microlymphatic vessel density and the positive expression rate of VEGF-C, VEGF-D, VEGFR-3 in three endostar groups and three endostar plus cisplatin groups were signiifcantly less than those in control group and cisplatin group. Microlymphatic vessel density and the positive expression rate of VEGF-C, VEGF-D, VEGFR-3 in endostar plus cisplatin group and endostar group with high endostar concentration were signiifcantly less than those with low endostar concentration;hTere was a signiifcant positive correlation between microlymphatic vessel density and the positive expression rate of VEGF-C, VEGF-D, VEGFR-3. hTe number of circulating tumor cells in endostar plus cisplatin groups were signiifcantly less than that of endostar or cisplatin alone. Conclusion Endostar could inhibit tumor lymphangiogenesis and reduce tumor cells into the bloodstream through the lymphatic. Inhibitory effect concerned with drug concentrationwith a dose-dependant.
6.Efficacy of docetaxel plus carboplatin combination chemotherapy for advanced non-small cell lung cancer.

BACKGROUNDChemotherapy is one of the important treatment methods for advanced non-small cell lung cancer (NSCLC). The aim of this study is to evaluate the efficacy of docetaxel combined with carboplatin in treatment of advanced NSCLC.
METHODSSixty-four stage IIIB/IV NSCLC patients were treated with docetaxel (75 mg/m² intravenously, on day 1) and carboplatin (AUC=5 intravenously, on day 2).
RESULTSThe overall response rate (RR) was 42.6%, median survival time (MST) was 14 months, and 1-year survival rate was 45.23%. In initial treatment group, 1-year survial rate was 48.84% and MST was 14 months, and 37.89% and 12 months respectively in retreatment group (P=0.0233). The 1-year survial rate and MST of stage IIIB patients were 44.86% and 15 months, and 39.75% and 12 months respectively in stage IV patients (P=0.0354). There was no significant difference in efficacy between squamous cell carcinoma and adenocarcinoma patients. The major adverse effects were granulopenia, fatigue, nausea, vomiting and alopecia.
CONCLUSIONSThe combination of docetaxel and carboplatin has a high response rate and tolerable side effects in treatment of advanced NSCLC, which can be adopted as both the first-line and second-line treatment.

Objective To explore the clinical efficacy of arc-edge needle therapy based on Zaodian theory in the treatment of refractory tennis elbow.Methods 64 patients with refractory tennis elbow were collected and randomly divided into a control group and an observation group using a random number table method.Among them,32 cases in the control group were treated with local pain point injection.32 patients in the observation group were treated with arc-edge needle therapy once a week,with two treatments per group.The Number rating scale(NRS)Mayo,elbow joint performance score(MEPS)and Disabilities of the arm,shoulder and hand(DASH)score were recorded and compared between the two groups before treatment and at 1,2,4 and 24 weeks of follow-up after treatment,disease efficacy score(WDES)at 24 weeks of follow-up.Results During the treat-ment process,neither group of patients experienced serious adverse reactions.At each stage after treatment,two groups of NRS MEPS,DASH,and WDES at 24 weeks of follow-up all improved compared to before treatment.After one week of treatment,there was no statistically significant difference in NRS scores between the two groups(P>0.05).At follow-up visits at weeks 2,4,and 24,the NRS scores in the observation group were lower than those in the control group.After 1,2,4,and 24 weeks of follow-up after treatment,the DASH score was lower in the observation group than in the control group,and the MEPS score was higher in the observation group than in the control group,with statistically significant differences(P<0.05).After 24 weeks of treatment,the WDES of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Local pain point injection and arc-edge Zaodian release can alleviate pain and improve elbow joint function in the early stage of refractory tennis elbow.However,the long-term effect of local pain point injection treatment is not good,and the treatment effect of the observation group gradually improves.
9.Well-designed board sanding can improve the upper extremity motor functioning of hemiplegic stroke patients

Objective:To observe the effect of well-designed board sanding training on the upper extremity motor functioning of hemiplegic stroke survivors.Methods:Sixty stroke survivors with hemiplegia were randomly divided into an observation group (30 cases) and a control group (30 cases). All received conventional rehabilitation. The observation group′s training involved intelligent board sanding, while the control group′s training involved traditional sanding.Results:After the treatment, significant improvement was observed in the Fugl-Meyer upper extremity scores, modified Barthel index scores and reported shoulder pain in both groups, with the observation group′s averages significantly better than those of the control group. After the intervention, the average scores of both groups on the modified Ashforth scale had also improved significantly.Conclusions:Supplementing conventional rehabilitation treatment with intelligent board sanding can significantly improve upper extremity motor function and ability in the activities of daily living of stroke survivors with hemiplegia while somewhat relieving shoulder pain. The effect is better than with traditional board sanding.
10.The value of plaque-to-aorta CT value ratio in differentiating coronary lipid and fibrous plaques

Objective To explore the value of plaque-to-aorta CT value ratio(standardized CT value)in differentiating coronary lipid and fibrous plaques,and to preliminarily analyze the stability of the cutoff.Methods Patients who underwent coronary computed tomography angiography(CCTA)and intravascular ultrasound(IVUS)within 1 week were included.The plaque CT value was obtained by measuring the all,four and two short-axis planes,respectively.The CT value of the ascending aorta was measured and standardized(plaque-to-aorta CT value ratio).The receiver operating characteristic(ROC)curves of the standardized and the traditional CT values were drawn.Results A total of 60 patients with 74 plaques were included,35 lipid and 39 fibrous plaques were diagnosed by IVUS.The aorta CT value was significantly correlated with the plaque(r=0.420,P<0.01);the cutoffs for the CT value of all,four and two plaque slices were 55 HU,48 HU and 52 HU,respectively,and all there of the cutoffs of standardized CT value were 0.149;the sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)of four-slice traditional and standardized CT values to differentiate lipid and fibrous plaques were 69%,87%,83%,76%and 91%,82%,82%,91%,respectively.Conclusion Compared with traditional CT value,the standardized CT value can greatly improve the sensitivity and NPV in differentiating coronary lipid and fibrous plaques,while maintaining modest to high specificity and PPV.Furthermore,the cutoff is stable.
