1.Correlation between Liver and Kidney Toxicity and Cyclosporin A Blood Concentration in 1236 Samples of Patients with Aplastic Anemia

Objective:To analyze the correlation between cyclosporin A blood concentration and drug-induced liver and kidney in-jury in the patients with aplastic anemia, investigate the significance of cyclosporin A concentration in the monitoring of liver and kidney toxicity, and provide theoretical basis for clinical individualized drug use. Methods:A total of 149 patients with aplastic anemia trea-ted with cyclosporin A as the main therapeutic drug were selected, and after 3-day treatment, the blood concentration of cyclosporin A was detected by an HPLC-UV method 10 minutes before the administration and 2 hours after the administration. The liver and kidney function were measured at the same time. The correlation between the different concentration of cyclosporine A and the change of liver and kidney function was analyzed. Results:All the patients were monitored cyclosporine A blood concentration with 1236 samples, and 34 patients (22. 82%) were with drug-induced liver injury and 51 patients (34. 23%) showed drug-induced kidney toxicity. The average C0 of liver injury patients was (297. 92 ± 74. 14) μg·L-1 , and C2 was (944. 47 ± 148. 47) μg·L-1 , while the average C0 of kidney injury patients was (311. 41 ± 52. 80)μg·L-1, and C2 was (926. 25 ± 136. 02) μg·L-1. The function indices of liver (ALT, AST, TBIL) and kidney (SC, BUN, UA) were significantly higher than those in the normal group (P<0. 05), and the blood concentration of cyclosporin A was significantly correlated with the liver,and kidney function. Conclusion:There is a certain correla-tion between cyclosporine A -induced toxicity and its blood concentration and C2 may be a more valuable predictor for drug -induced liver injury.
2.Autologous platelet-rich plasma promotes premaxillary suture distraction osteogenesis in rabbits

BACKGROUND: Platelet-rich plasma containing various high-concentration growth factors can promote new bone formation and accelerate bone healing. But its effects on distraction osteogenesis remain unclear. OBJECTIVE: To observe the effects of autologous platelet-dch plasma on premaxillary distraction osteogeaesis in rabbits. DESIGN, TIME AND SETTING: A randomized controlled animal experiment was performed at the Laboratory of Physiology, North Campus,Sun Yat-sen University between July and August 2007. MATERIALS: Sixteen healthy rabbits, the same number for male and female, aged 3-5 months,weighing 1. 4-1.7 kg,were randomly and evenly divided into an experimental group and a control group. Premaxillary incisor band,outside front distraction mask, and distraction rubber band were self-made.Coagulant was prepared by dissolving 1000 u bovine thrombin in 1 mL of 10% calcium chloride.METHODS: Titanium nails (1.5 mm in diameter) were separately inserted on two sides of left premaxillary suture in rabbits. A self-made distraction device was used. Gelatum-like substance [Vplatelet-rich plasma: V coagulant=9:1] was injected into the left premaxillary suture of experimental group rabbits immediately prior to distraction.In each group, one-week sustained distraction was performed in 4 rabbits,and three-week sustained distraction was performed in another rabbits. MAIN OUTCOME MEASURES: The increase of nail-nail distance on the two sides and histological results following distraction osteogenesis.RESULLTS: Rabbit premaxillary bone moved anterior in the two groups. The experimental group showed greater increase of nail-nail distance, faster bone formation and mineralization, more blood vessels, and thicker and more mature bone trabecula in the distraction interspace in comparison with the control group. CONCLUSION: Platelet-rich plasma will help bone tissue regeneration and promote premaxillary distraction osteogenesis.
3.Determination of Paclitaxel and Docetaxel in Plasma by Crossing Internal Standard Method and its Clinical Application

Objective:To establish an HPLC method for the determination of paclitaxel and docetaxel in plasma to provide refer-ence for the individualized treatment regimen and the evaluation of curative effect and adverse reactions. Methods:Paclitaxel and do-cetaxel were used as the internal standard for each other. The samples were precipitated by acetonitrile and separated on a DikMA Dia-monsil C18 column with a mixture of acetonitrile-water (55: 45) as the mobile phase. The flow rate was 1. 2 ml·min-1 . The column temperature was set at 25℃. Paclitaxel and docetaxel were detected by UV-detection (λ= 227 nm). Results: A linearity was ob-tained within the range of 0. 078-10. 0 mg·L-1 for paclitaxel and docetaxel. The limit of quantitation was 0. 039 mg·L-1 . The aver-age recovery of paclitaxel and docetaxel was 99. 85% and 100. 35%, respectively. The inter- and intra-day RSD were both less than 5% and the RSD for freeze-thaw stability was below 10%. The plasma concentration of paclitaxel in clinical samples was within the range of 0. 18-6. 16 mg·L-1 and obvious individual difference was shown. Conclusion:Therapeutic drug monitoring is very important due to the obvious differences in plasma concentration of paclitaxel and docetaxel. The established method is sensitive, accurate, con-venient and rapid in r the therapeutic drug monitoring, and is useful for the adverse drug reactions monitoring and pharmacokinetic study.
4.Clinical study of unrelated cord blood transplantation in patients with hematologic malignancies in single center

Objective To retrospectively analyze the engraftment, transplant-related complications and survival after unrelated cord blood transplantation (UCBT) in patients with hematologic malignancies. Methods Fifty consecutive patients with hematological malignancies (median age, 19 years; median weight, 53 kg) were treated with UCBT in single center from April 2000 to August 2009. Thirty-nine patients were high-risk or refractory. Double UCB grafts were used for 26 patients, while single UCB graft for 24 patients. Myeloablative conditioning was given to 45 cases and non-myeloablative regimens to 5 cases. All patients were given a combination of cyclosporin A (CsA) and mycophenolate mofetil (MMF) for graft-versus-host disease (GVHD) prophylaxis. Results The median total nucleated cell (TNC) dose was 4.0 (range, 1.95-16.24)×10~7 TNC/ kginfused, and CD34~+ cell dose was 2.74(range, 0.67-29.28)×10~5/kginfused. Forty-two of 50 patients acquired engraftment with implantation rate being 86%. The median time to engraftment (absolute neutrophil count>500/mm~3 and platelets 20 000/L) was 19 and 34 days. The cumulative incidence of neutrophil engraftment by day 42 was 86.3%(95% confidence interval [CI] 0.769-0.957); the cumulative incidence of platelets engraftment by day 120 was 72.3% (95% CI 0.620-0.821). Twenty cases developed acute GVHD, and the incidence of acute GVHD of grades Ⅲ/Ⅳ by day 100 was 7.1%. The incidence of chronic GVHD within 2 years was 17.4%. During a median follow-up period of 22 months (range 4-116), Overall 6-month, 1-year and 2-year survival rate was 66.2%(95% CI 0.590-0.734), 57.4%(95% CI 0.496-0.652), 54.2%(95% CI 0.462-0.622), respectively. For the patients with non-advanced hemotologic malignancies, 6-month, 1-year and 2-year survival rate was 73.2% (95% CI 0.659-0.805), 66.1% (95% CI 0.579-0.743), and 62.2% (95% CI 0.542-0.682) respectively. Five cases relapsed. The cumulative incidence of relapse within 2 years was 16.2% (95% CI 0.099-0.225). Twenty-one cases died mainly due to infection. Conclusion UCBT could be safely and effectively used for adult patients with hematologic malignancies.
5.Incidence analysis of malignant cancer in Shanxi cancer registration areas in 2011

Objective To explore the cancer incidence in registration areas in Shanxi Province. Methods Data of 8 cancer registration areas in 2011 were taken into account and cancer incidence in different areas with different ages was compared with that in other domestic areas. Results 8 395 new cases in Shanxi all cancer sites were reported in 2011, including 4 810 male and 3 585 female. The incidence of malignant cancer of Shanxi was 207.53/100 000, and the standardized incidence of Chinese population and world population were 125.20/100 000 and 165.72/100 000, respectively. In urban areas, the incidence of Shanxi and the standardized incidence of Chinese population were 202.49/100 000 and 112.81/100 000, respectively. In rural areas, incidence rate of Shanxi was 211.96/100 000 and the standardized incidence of Chinese population was 138.43/100 000. In Shanxi Province, the major malignant cancer sites for males involved stomach, lung, esophagus, liver and colorectum, and cancer sites for females were more on cervix, lung, breast, stomach and esophagus. Conclusions Upper gastrointestinal cancer and uterine cervix cancer are the major cancers in Shanxi registration areas. The incidence of stomach cancer and uterine cervix cancer in Shanxi Province are much higher than national average.
6.The comparative study of neurological function score and subgroups analysis for rats at different ages after CPR modeling

Objective The purpose of this study is to select suitable ages of rats for the CPR ( cardiopulmonary resuscitation) animal model.The neurological function score and subgroups analysis are evaluated in 2 month old and 4 month old animal groups.Methods Based on the evaluation of physiological indexes including ECG, blood pressure and neurological function defect score ( NDS) and subgroup analysis, the stability of CPR rats model was compared between 2 month old and 4 month old animal groups.Results The results showed that, the model rate of the ventricular fibrillation was induced by electrical stimulation , the 4 month old group was 87.5%, significantly higher than the 2 month old group, however, there was no significant difference between the two groups in the mortality rate;For the changes of blood pressure during the process of CA( cardiac arrest) induced by electrical stimulation, the 4 month old group was significantly lower than the 2 month old group (P <0.01); for the NDS at each time point after CPR, there was no significant difference between the two groups; however, the NDS subgroup analysis at different time points showed that there were different degrees of differences between the two age groups ( P <0.05) .Comparing with the 2 month old group, the 4 month old group had a stable process during the animal model preparation, had an obvious cerebral blood hypoperfusion phenomenon and aggravation of brain injury after CPR.Conclusion The 4 month old rats are more suitable for preparation of CPR animal mode , the model rate is high, the brain injury aggravate.It is more suitable evaluation for basic research and treatment of CPR.
7.Studv on the mental health status for 260 patients with breast cancer

ObjectiveTo explore the mental health status of patients with breast cancer and social support on their influence. Methods260 cases of patient with breast cancer were investigated by using the questionnaire of demographic characteristics and symptom check list 90(SCL-90)and social support rating scale(SSRS). ResultsBreast cancer patients ' mental health condition was worse than normal healthy people and their mood changes were obviously observed. Breast cancer patients achieved good social support.However, the urban patients with breast cancer got better social support than the rural patients and the difference were statistically significant(t =7.91, P =0.000). SCL-90 total score and its factors such as forced symptoms, sensitive interpersonal relationship, depression, anxiety, hostile, terror, paranoid,psychoticism,were negatively related with the social support (r =-0.278,-0.259,-0.165,-0.294,-0.215,-0.177,-0.175,-0.167,-0.219,all P < 0.05).ConclusionThe social support influences breast cancer patients'mental health,therefore,the patients with breast cancer should be given a better social support in order to improve the psychological health level and improve recovery.
8.Relationship between microRNAs and diffuse large B-cell lymphoma and its effect

Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma with a high degree of heterogeneity.Studies have shown that microRNAs (miRNAs) are involved in many biological processes and are closely related to lymphoid hematopoietic system and play an important role in various stages of B-cell differentiation and malignant transformation.miRNAs are increasingly concerned as a potential biomarker,which are closely related to the classification of DLBCL.miRNAs are of great values for the diagnosis of the disease and help to determine the prognosis of different patients,which may become new therapeutic targets.
9.Correlation between Acute Drug-induced Liver and Kidney Injury and HD-MTX Blood Concentration in Patients with Osteosarcoma

Objective: To analyze the correlation between HD-MTX blood concentration and acute drug-induced liver and kidney injury in the patients with osteosarcoma, and investigate the significance of HD-MTX concentration in the monitoring of liver and kidney toxicity. Methods: A total of 56 osteosarcoma patients treated with HD-MTX were selected, and after HD-MTX treatment, the blood concentration of MTX was detected by an HPLC-UV method in 48 h and 72 h after the administration. The liver and kidney function were measured at the same time. The correlation between the different concentrations of MTX and the change of liver and kidney func-tion was analyzed. Results: All the patients were monitored MTX blood concentration at different time points. After the 48-hour HD-MTX treatment, 4 patients (7. 14% ) were with acute drug-induced liver injury and 13 patients (23. 21% ) showed drug-induced kid-ney injury. The average C48hof liver injury was (2.90 ±0.78) μmol·L-1, and the average C48hof kidney injury was (1.65 ±1.12) μmol·L-1. After the 72-hour HD-MTX treatment, 7 patients ( 12. 50% ) were with drug-induced liver injury and 16 patients (28.57%) showed drug-induced kidney injury. The average C72hof liver injury was (0.30 ±0.17) μmol·L-1, while the average C48hof kidney injury was (0. 29 ± 0. 29) μmol·L-1. The function indices of liver ( ALT, ALP and TBIL) and kidney ( SCr) were significantly higher than those in the normal group (P<0. 05), and the blood concentration of MTX was partly significantly correlated with those indicators. Conclusion: There is a certain correlation between MTX induced injury and the blood drug concentration at par-ticular points, and C48hmay be more valuable to predict drug-induced liver and kidney injury.
10.Postmenopausal osteoporosis treated with acupoint injection of salmon calcitonin:a randomized controlled trial.

OBJECTIVETo verify the clinical efficacy on postmenopausal osteoporosis treated with acupoint injection of salmon calcitonin.
METHODSNinety patients of postmenopausal osteoporosis were randomized into three groups, 30 cases in each one. In the acupoint injection group, Shenshu (BL 23) and Zusanli (ST 36) were selected bilaterally. The injection 4 mL was prepared with salmon calcitonin 100 U (1 mL) and 0.9% sodium chloride injection. Each acupoint was stimulated with the injection, 1 mL. In the blank group, 0.9% sodium chloride injection was applied to bilateral Shenshu (BL 23) and Zusanli (ST 36), 1 mL at each acupoint. In the intramuscular injection group, salmon calcitonin 100 U was injected at gluteus maximus. The treatment was given once every two days in the patients of the three groups and lasted for 2 months. The levels of bone mineral density (BMD), bone alkaline phosphatase (NBAP), C-terminal telopeptides of typeⅠcollagen (CTX), urine calcium/creatinine (Ca/Cr) and the symptom score of osteoporosis were detected in the patients of the three groups before and after treatment.
RESULTSIn the patients of the three groups, NBAP and BMD in lumbar vertebra after treatment were higher than those before treatment (all<0.05); CTX, Ca/Cr and symptom score were lower than those before treatment (all<0.05). After treatment, NBAP was (32.7±2.5) μg/L in the acupoint injection group, higher than those in the blank group and the intramuscular injection group (both<0.05). In the acupoint injection group, CTX was reduced to (239.7±63.6) μmmol/L and Ca/Cr was reduced to 0.525±0.274, apparently lower than those in the blank group and intramuscular injection group (both<0.05). After treatment, in the acupoint injection group, BMD of lumbar vertebra was (0.731±0.062) g/m, higher than the level of the rest two groups (both<0.05). After treatment, the symptom score was 5.2±0.6 in the acupoint injection group, lower than those in the blank group and intramuscular injection group (both<0.05).
CONCLUSIONSSalmon calcitonin injec-tion at Zusanli (ST 36) and Shenshu (BL 23) achieves significant efficacy on postmenopausal osteoporosis, stimulating osteoblast activity and inhibiting bone absorption of osteoclast.
