Objective To investigate the influence of continuous renal replacement treatment on the therapy of severe acute pancreatitis(SAP) associated with kidney injury(AKI) and inflammatory mediators.Methods Clinical data of 54 SAP patients complicated with AKI,who received conventional treatment (control group) or conventional therapy combined with CRRT(observation group) were retrospectively analyzed.The clinical efficacy between the two groups before and after treatment was compared.Results The mechanical ventilation proportion in control group was significantly higher than in the observation group (x2 =4.464,P ＜ 0.05).The mortality rate between the two groups had no statistical di.fference(P ＞ 0.05).In observation group,TNF-alpha and IL-6 levels were significantly lower than before treatment(P ＜0.01),APN was significantly increased (P ＜ 0.01).APN also increased in the control group,but the extent was less than the observation group.The levels of TNF-αand IL-6 in the control group were significantly higher than before treatment(P ＜ 0.01).The urine output of observation group increased significantly after 72h therapy (P ＜ 0.01).And blood urea nitrogen (BUN),creatinine (SCr),APACHE Ⅱ score were significantly decreased(P ＜ 0.01),in the control group,urine output also increased (P ＜ 0.01),but the extent was less than the observation group,APACHE Ⅱ score had no significant difference compared with before treatment(P ＞0.05).In control group,BUN and SCr levels significantly increased than before treatment (P ＜ 0.05).Conclusion CRRT for the treatment of SAP patients complicated with AKI can significantly improve renal function and reduce mortality,and its mechanism may be clear a large number of inflammatory mediators and metabolites in body,reduce the body inflammatory response.

Objective To analyze the clinical relationship of progesterone(P) and estradiol(E2) for embryo arrest in early pregnant patients.Methods 118 patients with embryo arrest in early pregnancy were selected and divided into two groups,research group (58 cases,embryo arrest in early pregnancy) and control group (60 cases,threatened abortion but embryo keeping growth by tocolysis).Serum P and E2 were measured and compared between two groups.Receiver operating characteristic curves(ROC) of all indexes were drawn and the areas under the ROC curves were calculated to evaluate diagnostic value.Results Compared to control group,the serum P and E2 were lower in research group(P ＜ 0.05).To diagnosis embryo arrest,the threshold for P was less than 118.4nmol/L,with 93.9％ sensitivity and 87.3％ specificity; while the threshold value for E2 was less than 1110pmol/L,with 95.6％ sensitivity and 90.7％ specificity.Conclusion Detection of serum P and E2 may be helpful for the diagnosis of embryo arrest in early pregnancy,and the serum E2 had the best diagnostic value.

Objective:To analyze therapeutic effect of atorvastatin combined clopidogrel on patients with unstable an‐gina pectoris (UAP) and its influence on prognosis .Methods :A total of 215 UAP patients were randomly divided in‐to atorvastatin group (n=106 ,received atorvastatin based on routine treatment ) and combined treatment group (n=109 ,received atorvastatin combined clopidogrel based on routine treatment ) .Therapeutic effect ,levels of serum high sensitive C reactive protein (hsCRP) ,blood lipids and interleukin (IL)‐6 etc .were compared between two groups .Results:Total effective rate of combined treatment group was significantly higher than that of atorvastatin group (94.5% vs .76.4% ,P<0.01) .Compared with before treatment ,there were significant reductions in levels of hsCRP ,IL‐6 and total cholesterol (TC) in both groups after treatment ,P<0.01 all;compared with atorvastatin group ,there were significant reductions in levels of hsCRP [ (5.35 ± 1.37) mg/L vs .(3.36 ± 1.25) mg/L] ,IL‐6 [ (17.36 ± 4.58) pg/ml vs .(12.28 ± 3.24) pg/ml] ,TC [ (7.69 ± 1.20) mmol/L vs .(4.35 ± 1.20) mmol/L] and TG [ (3.35 ± 1.02) mmol/L vs .(1.29 ± 0.25) mmol/L] in combined treatment group , P<0.01 all .Total inci‐dence rate of adverse cardiac events in combined treatment group was significantly lower than that of atorvastatin group (3.7% vs .16.0% ,P=0.002) .Conclusion:Atorvastatin combined clopidogrel cansignificantly enhance ther‐apeutic effect , effectively inhibit inflammation ,reduce incidence rate of adverse cardiac events in patients with un‐stable angina pectoris .

Objective To investigate the effect and safety of Qizheng Qingpeng ointment in the treatment of eczema.Methods Ninety -eight patients of eczema were randomly divided into two groups, who received either Qizheng Qingpeng ointment plus hydrocortisone butyrate cream or only hydrocortisone butyrate cream at the first 2 weeks,and all patients were treated of Qizheng Qingpeng ointment plus hydrocortisone butyrate cream at the further 2 weeds.Results At 1 week and 2 week post-treatment,the efficacy rates were 68.52%,87.03% for the study groups and 27.91%,58.13%for the control groups,the efficacy rate of the study groups was statistically better than that of the control groups (χ2 =14.21,8.99, all P<0.01 );At 4 week post -treatment, the efficacy rates were 94.44%for the study groups and 90.70%for the control groups,the difference of the efficacy rates in the two groups were no statistically significant (χ2 =0.11,P>0.05) .Conclusion The clinical efficacy of Qizheng Qingpeng oint-ment associated with corticosteriod for treatment of eczema was better than corticosteriod alone.

Heme oxygenase(HO) is the rate-limiting enzyme in the degradation of heme.Three HO isoforms have been identified:HO-1,HO-2 and HO-3.The substances of heme degradation,carbon monoxide(CO),biliverdin and free iron(Fe~(2+)),have an important function in anti-oxidant and protecting cells.And the anti-oxidant function of induction HO-1 expression is mediated in the signal transduction of HO-1 gene expression and the promoter of HO-1 gene in the cells.So the new way is provided in the relevent liver-injury which is caused in anti-oxidant stress or non-oxidant stress.

Objective To develop a port expander for simple network cable tester,which can raise the testing efficiency of the computer network twisted-pair.Methods Several RJ-45 sockets were used to connect the eight points separatedly according to the same arrangement order.One socket was connected to testing port by short twisted-pair,while other sockets were used as expander port.Results In practical test,the operation only needs one person,who could test the computer network twisteded-pair conveniently without going back and forth frequently between the two terminals of the tested line.Conclusion It is proved by testing that the port expander for the simple network cable tester can raise the testing efficiency of the computer network twist-pair.

Currently, Lemology in PLA is facing two challenges: i) the threat of emerging infectious diseases (EID), bioterrorism and biological warfare (BW); ii) professional staffs are concerned over their development while the armed forces are undergoing re-organization. Whereas military lemology meets several opportunities such as: Ⅰ) the discipline occupies\ a\ prominent portion in military medicine; Ⅱ) an outbreak of EID and the threat of bioterrorism are closely related to military medicine; Ⅲ) infectious diseases are still common diseases prevalent in both servicemen and civilians; Ⅳ) the present status of lemological study in PLA is in the leading position in the whole nation; Ⅴ) once there should be another outbreak of SARS epidemic such as occurred in 2003 in Beijing, the most expeditions and effective approach is still to deploy the specialists in PLA organizing a medical team to enter the epidemic area. The development strategy of this discipline in PLA is proposed as follows: i) expanding the scope of the discipline; ii) further the construction of the team; ii) perfecting the system for prevention and treatment of EID. Finally, the presumption for the 21st century of the subject is following suggestions are the contemplation for the development of this discipline in the 21st century: i) further emphasize the study of this discipline is beneficial for both military and civilian; ii) to simplify the scope of the discipline is not against its enhancement; iii) the interchanges and collaboration among different disciplines should be encouraged and strengthened.

Objective To analyze the situation of secondary hypertension caused by different etiology and its treatment status so as to improve its treatment. Methods Totally 580 patients with secondary hypertension selected with strict criteria were divided into 4 groups in accordance with the etiology. The data were dealt with SPSS 13.0. We analyzed the sex, age, systolic pressure, diastolic pressure and positive incidence of family history of all the secondary hypertension patients in each group, and compared their usage of every unilateral preparation. Results The average age of renal parenchymal disease was more advanced than that of adrenal disease and other diseases. The administration of diuretics and angiotensin-converting enzyme inhibitor (ACEI) in renal parenchymal disease group, αβ receptor blocking agents and calcium antagonists in kidney-artery disease group, a antagonists in adrenal disease and compound antihyportensive agents in other diseases groups all affected advantage of the ratio. Conclusion Diuretics and ACEI are suitable for hypertension caused by renal parenchymal disease, αβ receptor blocking agents and calcium antagonists are suitable for that caused by kidney artery disease, but a receptor blocking agents are suitable for that caused by adrenal disease.

Osteoporosis is a common problem of the elderly. It is important to carry out community-based rehabilitation, care and education to prevent and treat osteoporosis.

Objective:To investigate the expression and clinical significance of glycosylated albumin(GA) and glycosylated hemoglobin(HbA1c) in patients with diabetic microangiopathy(MVCD).Methods:A total of 295 patients with type 2 diabetes admitted to Chaoyang Central Hospital of Liaoning Province from May 2017 to July 2019 were selected as case group, and they were divided into simple diabetes group (217) and microvascular disease group (78) according to whether the patients had microvascular lesions.Two hundred healthy people were selected as normal control group.The levels of GA and HbA1c were compared between the case group and the normal control group, and the levels of GA and HbA1c in the diabetic group and the microvascular disease group were compared.The risk factors of microangiopathy in patients with type 2 diabetes mellitus were analyzed by logistic regression analysis.Results:The levels of GA and HbA1c were (23.9±10.1)% and (8.6±2.3)% in the case group, which were (13.4±1.2)% and (5.0±0.6)% in the normal control group.The differences between the two groups were statistically significant( t=14.628, 21.636, all P<0.01). The levels of GA and HbA1c were (22.8±9.7)% and (8.4±2.0)% in the simple diabetes group and (25.4±10.3)% and (9.2±2.4)% in the microvascular disease group.The differences between the two groups were statistically significant( t=1.997, 2.869, P=0.047, 0.004). Logistic regression analysis showed that GA and HbAlc were risk factors for microangiopathy in patients with type 2 diabetes ( OR (95% CI) values were 1.088 (1.030-1.157) and 15.251 (1.756-131.157), P=0.003 and 0.014). Conclusion:The occurrence of MVCD is related to GA and HbA1c.Logistic regression analysis showed that GA and HbA1c levels were risk factors of MVCD, and could be used as predictive factors of MVCD.
