Background and purpose:Epithelial-mesenchymal transitions(EMTs) occur as key steps during embryonic morphogenesis,and are now implicated in the progression of primary tumors towards metastases.We have found that the transcription factor Twist,a master regulator of EMT,was expressed markedly in human lung carcinoma cell line(A549).This study is to investigate the effects of short hairpin RNA(shRNA) targeting Twist on EMT and invasion ability of A549 cells in vitro.Methods:RNA interference plasmid that can express short hairpin RNA(shRNA) targeting Twist(positive RNAi plasmid) or express shRNA that does not match any known human coding mRNA(negative RNAi plasmid)was designed,constructed,and transfected into A549 cells line.Positive RNAi A549 cells(Pi group) expressing Twist suppressed or negative RNAi A549 cells(Ni group) expressing Twist uninfluenced was selected by neomycin resistance.In normal(control,C),Pi and Ni group A549 cells,Twist,alpha-smooth muscle actin(?-SMA) and E-cadherin expression were examined by reverse transcription-polymerase chain reaction(RT-PCR) and Western blot,invasion ability were examined by Boyden chamber model.Results:1,In C group,both Twist and ?-SMA expressions were strongly positive,but E-cadherin was poorly expressed.The number of A549 cells permeating septum of Boyden chamber(NCS) was equal to 57.4?3.4.2,In Pi group,compared with that in C group,Twist and?-SMA expression,and NCS were all decreased,but E-cadherin expression was significantly increased(P
2.Study on application value of invasive intracranial pressure monitoring in children with severe craniocerebral trauma

Objective To explore the application value of invasive intracranial pressure (ICP) monitoring in children with severe traumatic brain injur (sTBI).Methods A total of 158 children with sTBI in our hospital from January 2012 to March 2015 were collected and retrospectively analysed.According to whether the child recieved invasive ICP monitoring or not,they were divided into the monitoring group (80 cases) and the control group (78 cases).Children in the monitoring group were embed of invasive ICP monitoring device within first 24 h after surgery,and were treated with appropriate measures according to the ICP level.In the control group,the determination of whether needing to recieve head CT and adjusting the treatment program was made based on the clinical symptoms and surgeon′s experiences.The prognosis of children in the two groups were evaluated by Glasgow coma scale (GOS) and compared at 3 months after injury.At the same time,the prognosis of children in the monitoring group with different ICP levels were compared.Results The favorable prognosis rates in the monitoring group and control group were 61.2% and 41.0% respectively,and there was statistically significant difference between the two groups(P<0.05).The mortality rates of children in the monitoring group had statistical difference among different ICP levels(P<0.05).Conclusion For children with sTBI,the invasive ICP monitoring is contribute to dynamically observing the changes in ICP,early detecting changes of conditions,guiding clinical treatment,and improving the prognosis.

4.Effect of health literacy on stroke education in patients with ischemic cerebrovascular disease and their family members

Objective To investigate the effect of health literacy on stroke education in patients with ischemic cerebrovascular disease and their family members.Methods Health literacy questionnaire was used to conduct the health literacy questionnaire survey for patients with ischemic cerebrovascular disease and their family members.The respondents received stroke education randomly through neurologists' explanation or self-reading.The corresponding questionnaire was used to evaluate the mastering of the content of stroke education for the respondents.Results The complete data of 42 patients with ischemic cerebrovascular disease (34 ischemic stroke and 8 transient ischemic attack) and 57 family members of the patients with ischemic cerebrovascular disease were collected and enrolled in the analysis.The good health literacy score group (< 10) and the poor health literacy score group (≥ 10) were 61 and 38 subjects respectively.The proportions of younger age (x2 =16.410,P <0.001) and high school or above (68.85% vs.23.68% ;x2 =19.25,P < 0.001) of the former were higher than those of the latter,and the proportion of patients (32.79%vs.57.89%;x2 =6.043,P=0.014) was significantly lower than that of the latter,but there was no significant difference in the proportion of the males between the two groups (55.74% vs.44.74% ;x2 =1.134,P =0.287).Multivariate logistic regression analysis showed that health literacy was independently associated with the age of respondents (≤40 years:odds ratio [OR] 7.753,95% confidence interval [CI]1.208-49.740; P=0.031) and the level of education (high school and above:OR,3.993,95% CI 1.305-12.218; P=0.015),and it was not correlated with gender and role.Those with good stroke education score (≥7)and poor stroke education score (<7) were 57 and 42,respectively.The proportions of younger age (x2 =17.095,P <0.001),high school and above (70.00% vs.23.08% ;x2 =20.836,P < 0.001),and good health literacy score (88.33% vs.20.51%;x2 =45.968,P<0.001) were significantly higher than those of the latter,while there were no significant differences in the proportions of gender,role,and stroke education.Multivariatelogistic regression analysis showed that only the good health literacy was independently associated with the stroke education score (OR 20.147,95% CI6.089-66.655; P<0.001).Conclusions Health literacy can significantly affected the effect of the stroke education in patients with ischemic cerebrovascular disease and their family members.In the development of individualized stroke education program,it should be considered.

Objective In patients with pulmonary atresia and intact ventricular septum ( PAIVS) without right ventricular-dependent coronaries, catheter techniques including the use of a sniff wire, lasers, and radiofrequency have been the most widely used initial therapy. However, percutaneous perforation and balloon valvuloplasty were associated with higher rate of procedural failure and serious complications. Methods We report our experience with a hybrid approach for pulmonary atresia with intact ventricular septum, combining surgery and interventional catheterization techniques. Between March 2005 and March 2010, hybrid procedure was carried out successfully in 30 newboms and infants with favorable anatomy. The age ranged from 1 day to 48 months with a mean of (4.59 ±3.21) months. The heart was exposed through median sternotomy. A pursestring suture was placed in the right ventricular outflow tract 2 cm away from the pulmonary trunk. Then a 16-gauge intravenous catheter was punctured through the right ventrical and perforated the atretic PV with the guidance of echocardiography. A guide wire was then inserted into the sheath and used to guide the balloon across the PV. Sequential dilations were performed until a full opening of the PV with the guidance of epicardial echocardiography. In patients < 3 months PDA ligation was performed followed by modified Blalock-Taussig (B-T) shunt. In patients > 3 months PDA ligation was not performed. A modified B-T shunt was inserted if severe systemic oxygen desaturation occurred after PDA ligation. Bidirectional Glenn shunt was performed for severe hypoplasia. Hybrid procedure was achieved in all patients. The simultaneous procedures included 25 cases of PDA ligation. 6 newborns underwent modified B-T shunt placement (3.5 to 5 mm) after pulmonary valvuloplasty and PDA ligation, and 2 patients > 1 month underwent modified B-T shunt. Another 2 patients were selected for univentricular palliative surgery because of a diminutive monopartite right ventricle and bidirectional Glenn procedure was performed. No pericardial effusion or cardiac tamponade was observed in all patients. Another case without PDA ligation underwent a modified B-T shunt because of hypoxemia three days after hybrid procedure, and the rest patients were discharged without any further surgical intervention.During the follow-up period of 1.5 to 62.0 months, 5 patients died. 25 (83.3%) survived and were all in New York Heart Association functional class 1. Peripheral oxygen saturation increased from 0.73 ± 0.08 to 0.94 ± 0.04 (P < 0.05). One patient remains in a single-ventricle pathway, whereas 24 patients achieved a two-ventricle circulation. Results Conclusion Perventricular balloon pulmonary valvuloplasty using a hybrid approach is a safe and feasible procedure for patients with PAIVS.

Low back pain is becoming an important factor affecting people's quality of life, while the age of its onset is getting younger and younger, and the social and economic losses caused by low back pain are huge every year. Intervertebral disc degeneration (IDD) is an important cause of low back pain. Due to multiple factors, biomechanical and structural changes occur in intervertebral disc tissue, including rupture of annulus fibrosus, protrusion of nucleus pulposus, which cause compression of spinal cord and nerve root, and lower back pain. Micro-RNA (miRNA) is a kind of single-stranded non-coding small molecule RNA, with 18-24 nucleotides in length, which exists widely in eukaryotes. As one of the important regulatory molecules of gene expression, it has been proved to play a key role in the initiation and progression of many diseases, and it may also play an important role in intervertebral disc degeneration. At present, the clinical treatment for IDD is mainly surgical treatment to alleviate clinical symptoms. Even if surgical treatment can achieve good results, it will bring great physical trauma and economic burden to patients. The role of miRNA in IDD is one of the hotspots in the current academic research. Studies have shown that miRNA has abnormal expression patterns in degenerative intervertebral disc tissues and participates in a variety of pathological processes of IDD. At present, some miRNAs have been proved to be related to a variety of pathological processes in IDD, including nucleus pulposus cell apoptosis and proliferation, extracellular matrix degradation, autophagy, inflammation and cartilage endplate degeneration. The comparative study of gene chip showed that there were significant differences in the expression of some miRNAs between degenerative and normal nucleus pulposus cells. These differentially expressed miRNAs may be involved in the process of nucleus pulposus cell degeneration by regulating their respective upstream or downstream pathways. Most of the regulatory pathways are crossed and parallel, thus constructing a huge miRNA regulatory network. Understanding the target genes and mechanisms of miRNA in the pathogenic process can provide an important reference for the origin, development and prognosis of IDD. In this article, the important role of miRNA in IDD and the potential significance of clinical treatment are reviewed. With the in-depth study of miRNA and the molecular biological mechanism can provide new ideas for the diagnosis and treatment of IDD, which is likely to become a new strategy for biological treatment of IDD.

BACKGROUNDIt has been proved that hyaluronan was involved in adhesion and invasion behavior of varied tumor cells. Layilin is a receptor of hyaluronan found recently and has close relationship with cytoskeleton. The aim of this study is to investigate the effect of short hairpin RNA (shRNA) targeting layilin on adhesion and invasion behavior of human lung adenocarcinoma cell line A549 induced by hyaluronan in vitro.
METHODSRNA interference plasmid that included U6 promoter and could express shRNA targeting layilin was designed, constructed, and transfected into A549 cell line. Layilin expression was examined by RT-PCR, Western blot and immunofluorescence. Adhesive and invasive ability was examined by plate adhesion model and Boyden chamber model.
RESULTSAfter plasmid transfected, layilin expression in A549 cells obviously decreased (P < 0.01), and the numbers of adhesive A549 cells on plate and A549 cells permeating septum of Boyden chamber induced by hyaluronan also significantly decreased (P < 0.01).
CONCLUSIONSThe shRNA targeting layilin can efficiently inhibit the adhesive and invasive ability of A549 cells induced by hyaluronan in vitro.
9.Factors affecting hyperuricemia among adults in Xinjiang Uygur Autonmous Region

Objective:
To investigate the prevalence and influencing factors of hyperuricemia among adults in Xinjiang Uygur Autonomous Region, so as to provide insights into the prevention and intervention of hyperuricemia.
Methods:
Permanent residents at ages of 18 years and above in 8 counties (cities, districts) of Xinjiang Uygur Autonomous Region were sampled using a multi-stage cluster random sampling method from July 2018 to November. Demographic data, smoking, alcohol consumption, body mass index and serum uric acid were collected through questionnaire surveys, physical examination and laboratory testing. Factors affecting hyperuricemia were analyzed using a multivariable logistic regression model.
Results:
A total of 4 690 residents were collected, including 2 093 males (44.63%) and 2 597 females (55.37%), and had a mean age of (49.45±14.57) years. There were 1 795 overweight residents (38.27%), 1 198 residents with obesity (25.54%), 983 smokers (20.96%), 1 224 drinkers (26.10%), and 268 residents with hyperuricemia (5.71%). Multivariable logistic regression analysis showed that a higher risk of hyperuricemia was seen among adults who were males (OR=1.409, 95%CI: 1.036-1.917), aged 60 years and above (OR=2.092, 95%CI: 1.495-2.928), had high school education or above (high school, OR=2.003, 95%CI: 1.366-2.937; college and above, OR=4.876, 95%CI: 3.196-7.438), were overweight (OR=2.343, 95%CI: 1.618-3.395) or obese (OR=4.754, 95%CI: 3.291-6.868) and had alcohol consumption (OR=1.494, 95%CI: 1.104-2.020).
Conclusion
The risk of hyperuricemia may be associated to gender, age, educational level, body mass index and alcohol consumption among adults in Xinjiang Uygur Autonomous Region.
