3.Physiological changes and implications during the fetal-neonatal transition
Journal of Clinical Pediatrics 2016;34(3):223-226
During the fetal-neonatal transition, the body must undergo many important physiological changes to adapt the extrauterine environment. After birth, the blood and energy supply through placenta is stopped with clamping of the umbilical cord and, meanwhile, the pulmonary ventilation function is established when exposure to the air, which results in a series of changes in the respiratory, circulatory and endocrine systems and energy metabolisms, etc. The physiological transition can be relfected in heart rate, blood pressure, oxygen saturation, temperature, and other physiological indicators. The changes of these indicators can be used as references for prevention, diagnosis and treatment of neonatal diseases. This review provides an overview of physiological changes and implications in the lung function, circulatory and endocrine systems, and energy metabolism during the transition at birth as well as intervention measures for abnormal fetal-neonatal transition.
4.Total cost of hospitalization after internal fixation of intertrochanteric fracture
Chinese Journal of Orthopaedic Trauma 2016;18(2):138-143
Objective To analyze the factors influencing the total cost of hospitalization after internal fixation of intertrochanteric fracture. Methods Enrolled in this study were 142 patients with intertrochanteric fracture who had been treated at our department between March 2013 and August 2014.They were 54 men and 88 women, aged from 24 to 95 years (average 69.8 years).Intramedullary fixation was adopted in 135 cases while extramedullary fixation in 7.By AO classification,32 cases were type A 1,75 type A2 and 35 type A3.We analyzed their demographic data,fracture-associated factors,bleeding-associated factors and surgery-associated factors. For univariate analysis, the independent sample t test was performed between 2 groups.One-Way ANOVA analysis was performed among several groups. For multivariate analysis, multivariate linear regression analysis was performed.P < 0.05 was regarded as statistically significant. Results The independent sample test revealed that the total cost of hospitalization for those with preoperative deep venous thrombosis (6.9 ± 1.0 × 105 (¥)) was significantly higher than for those without (5.0 ± 0.8 × 105 (¥)) and the total cost for those undergoing intramedullary fixation (5.5 ± 1.2 × 105 (¥)) was significantly higher than for those undergoing extramedullary fixation (4.4 ±0.7 × 105 (¥)) (P < 0.05).The multiple linear regression analyses showed that preoperative deep venous thrombosis (t =11.750,P < 0.001),perioperative transfusion (t =2.803,P =0.006),hospital stay (t =5.949,P < 0.001) and intraoperative blood loss (t =2.885,P =0.005) were the independent factors influencing the total cost of hospitalization. Conclusions Preoperative deep venous thrombosis, perioperative transfusion, intraoperative blood loss, and hospital stay may influence the total cost of hospitalization for patients undergoing internal fixation of intertrochanteric fracture.More attention should be paid to control of the total cost of hospitalization in addition to ensuring the perioperative life safety.

6.An Improved Experimental Myocardial Ischemia Reperfusion in Rat Model
Journal of Zhejiang Chinese Medical University 2007;0(01):-
[Objective] To improve the way of establishing myocardial ischemia reperfusion rat model.[Methods]30 SD rats,quality(220?20)g,male,randomly split into normal model group and improved model group, improved established model ligation with a 0.6mm diameter line, and drawn after 30min.Compare such a method with normal model.[Results] In the context of keeping experimental stability, the reduced heart rate, wake time and survival rate of the way of improved established model respectively are(50?20)/min,(15?5)min,86.67%,but it is just(120?20)/min,(80?20)min,66.67% of the normal model group.[Conclusion]The improved myocardial ischemia reperfusion model offers significant superiority to the former one.
7.Effects of resveratrol on the expression of osteoprotegerin and receptor activator for NF-?B ligand in femurs of ovariectomized rats
Chinese Journal of Clinical Pharmacology and Therapeutics 2000;0(03):-
AIM:To study effects of resveratrol on osteoprotegerin(OPG)and receptor activator of NF-?B ligand(RANKL)expression in femur of ovariectomized rats.METHODS:48 female SD rats(3 months old)were assigned into 6 groups randomly:sham-operated group(SHAM);ovariectomized group(OVX);17?-estradiol replacement therapy group(ERT,0.1 mg?kg-1?d-1,sc);high-dose resveratrol-treate group(RH,40 mg?kg-1?d-1,i.p.);middle-dose resveratrol-treated group(RM,20 mg?kg-1?d-1,i.p.);low-dose resveratrol-treated group(RL,10 mg?kg-1?d-1,i.p.).Except for SHAM group, rats in other groups were operated by bilateral ovariectomy.Administration started at the 8th day after operation and lasted 8 weeks. At the end of the 9th week after operation, all rats were sacrificed. The bone mineral density(BMD)and vitodynamics properties(including ELASTIC,M-STRESS,M-LORD and STIFFNESS)of femurs were measured. The expression of OPG and RANKL in femurs of rats were observed by immunohistochemical staining methods.RESULTS:Compared with OVX group, resveratrol treatment(40 mg?kg-1?d-1 and 20 mg?kg-1?d-1)increased expression of OPG in femurs while decreased expression of RANKL in ovariectmized rats.In RH,RM,and ERT group,BMD,ELASTIC,M-STRESS of femurs were higher than those of OVX group.CONCLUSION:Resveratrol increased expression of OPG in femur while decreased expression of RANKL in ovariectmized rats, which was probably one of mechanism that resveratrol improve osteoporosis.
8.Metabolic disturbance and cardiovascular events in primary aldosteronism:recent advance
Chinese Journal of Practical Internal Medicine 2003;0(01):-
Metabolic disturbances such as hypertension and insulin resistence,as well as cardiovascular events are more frequent in primary aldosteronism patients than in essential hypertension patients. When the diagnosis of primary aldosteronism is made, more attention should be paid to the associated metabolic disturbances and cardiovascular events, which may benefit the prognosis of patients.
9.Clinical analysis of continuous ambulatory peritoneal dialysis complicated by serum hypoproteinemia in 30 patients
Clinical Medicine of China 2010;26(11):1183-1185
Objective To test the serum albumin level in patients having the peritoneal dialysis, and investigate its relationship with C-reactive protein(CRP)level or other inflammatory complications. Methods Sixty patients having peritoneal dialysis treatment were divided into two groups based on the serum albumin(ALB)level, patients had normal ALB level were included into group A,while the patients with serum hypoproteinemia were included into group B. The serum level of CRP,transferin(TF)and nutritional status were compared between two groups, and their relationship with clinical complications were evaluated. Results The serum CRP level in patients of group B(8.40 ± 1.32)mg/L was significantly higher than group A(2. 14 ±0.87)mg/L(P <0. 01). Patients from group B had more inflammation complications, such as peritonitis(6. 7%),pneumonia(13. 3%),cardiovascular disease(16.7%). Conclusions The continuous ambulatory peritoneal dialysis patients accompanied with hypoproteinemia are more likely to have inflammation complications and cardiovascular diseases, which should cause more attention on their malnutrition-inflammation- CVD(MIA)syndrome diagnosis and treatment.
10.The anal verge: localization with multi-slice spiral CT
Chinese Journal of Radiology 2010;44(8):819-822
Objective To determine and evaluate the method of localization of anal verge by mutislice spiral CT. To provide an imaging reference for operative guidance of low-rectal cancer. Methods Forty eight consecutive adult patients suspected of abnormalities other than rectal disease were evaluated with abdominal and pelvic CT scans since August, 2009. They were divided into two groups based on sex and age. There were 23 men and 25 women. The ages of young group were 28 to 50 years and the average age was 41 years. The ages of elderly group were 52 to 81 years and the average age was 64 years. A small cotton ball dipped with contrast media was put at the anal verge as a marker and CT scans were performed with 64-slice spiral CT scanner. The distances between the cotton balls and the lower margin of the pubis combination (La), the lower margin of the 5th sacral vertebra (Lb), the inferior aperture of minor pelvis(Lc) and the lower margin of the basement of external anal sphincter (Ld) were measured on the midsagittal images obtained by MPR. The averages, the standard deviations(s), the 95% and 80% confidence intervals of La, Lb, Lc and Ld were calculated. We took the intervals of ± 1.96 s or ± 1.28 s < 5 mm as the standard of meeting the clinical requirement. The distances meeting the clinical requirement between different sex groups or age groups were compared with t test. Results The Ld was (9.8 ± 1.2) mm, ofwhich intervals of ± 1.96 s and ± 1.28 s were 4. 8 mm and 3.0 mm respectively, meeting the clinical need. This distance was also measured conveniently. There were no significant differences ( P > 0.05 ) between two different sex groups [ male group, ( 10.0 ± 1.2) mm], female group, (9.6 ± 1.2) mm and between two age groups [ young group, (9.6 ± 1.2) mm, elderly group, ( 9.9 ± 1.3 ) mm ]. Conclusions The lower margin of the basement of external anal sphincter was a useful anatomic lanmark for localizing the anal verge, and could be definitely identified on the middle sagittal pelvic CT image. The distance between the structure and anal verge is constant enough and can be used in measuring distance from low rectal lesion to the anal verge.
