1.Compare the effects of combined nerve block and epidural anesthesia in elderly patients undergoing unilaternal total hip replacement surgery

Objective To compare the effects of combined lumbar plexus-sciatic nerve block and epidural anesthesia in elderly patients undergoing unilatemal total hip replacement surgery. Methods Fortytwo patients were randomly divided in two groups (each in 21 cases),combined lumbar plexus-sciatic nerve block group (group N) and epidural anesthesia group (group EA). In group N, combined lumbar plexussciatic nerve block guided by a nerve stimulator was conducted,with 1% lidocaine 15 ml and 0.5% ropivacaine 20 ml (lumbar plexus) and 1% lidocaine 5 ml and 0.5% ropivacaine 20 ml (sciatic nerve). In group EA,epidural anesthesia was achieved by injection of 0.5% ropivacaine 10-15 ml. The variations in hemodynamics, the effect of never block and adverse reactions in both groups were recorded. Results The blood pressure was decreased significantly in group EA than that in group N at 10,20,30,60 min after epidural anesthesia (P <0.05). There was no difference in the onset time of sensory and motor blockade between two groups (P >0.05),while the duration of sensory and motor blockade was longer in group N [(436.5 ± 103.3 ), ( 307.3 ± 112.4 ) min ] than those in group EA [ (305.2 ± 110.8), ( 189.3 ± 84.5 ) min ](P < 0.05). There was 4 patients combined nausea and vomiting in group EA. Conclusion Combined lumbar plexus-sciatic nerve block in elderly patients undergoing unilaternal total hip replacement surgery has stable hemodynamics, longer analgesia duration and fewer adverse reactions.
2.Advances of taxol combinatorial biosynthesis.

Taxol is a kind of isoprenoid with strong anticancer activity. It is difficult to be obtained because of its low concentration in nature, which hinders its application in cancer treatment. Recently, biosynthesis methods for taxol production have attracted more attentions. Several systems including Escherichia coli, Saccharomyces cerevisiae, Physcomitrella patens, Arabidopsis, tomato and ginseng were explored. This review focuses on the advance in biosynthesis of taxol in different systems and features the bottleneck of scale fermentation for producing the intermediates. At the same time some advices for the further were given. At last, the future and character of Physcomitrella patens system used in taxol combinatorial biosynthesis were analyzed based on our lab's research.

3.Research and progress in tissue-engineered skin

BACKGROUND:Tissue-engineered skin is a new method to cure the massive defects of skin caused by burn or trauma.Therefore, the seed cells and scaffold on the tissue-engineered skin have become the focus today.DATA SOURCES: A computer-based online search of Pubmed database was undertaken to identify the articles about tissue-engineered skin published in English between January 1990 and January 2007 with the key words of "tissue engineered skin, stem cell". Meanwhile, we searched Chinese Journals Full-text Database for related Chinese articles published between January 1994 and January 2007 with the key words of index "tissue engineered skin, skin stem cell" in Chinese.STUDY SELECTION: The data were selected firstly to choose the full-text of articles met the criteria. Inclusive criteria:① Articles about seed cells of tissue engineered skin;②Articles about the scaffold of tissue-engineered skin. Exclusive criteria: Repetitive or analogical articles, Meta analysis or case reports.DATA EXTRACTION: Totally 97 articles on tissue-engineered skin were collected and 69 met the inclusive criteria after eliminated the repetitive or similar studies.DATA SYNTHESIS: The development of tissue engineering provides a new approach for the treatment of massive defects with skin transplantation, and gets rid of some problems such as donor site shortage, immune rejection, disease spread et al.① Skin seed cell: Embryonic stem cell and various adult stem cells can serve as seed cell of tissue-engineered skin, and embryonic stem cell is the first choice, which is transplanted on the biological scaffold to acquire the epidermis, dermis and compound skin.②Research progress of epidermis substitute.③Methods to make dermal substitute.④Research direction of compound artificial skin.⑤There are many problems in wide clinical application of tissue-engineered skin, but more and more attention has been paid on its research.CONCLUSION: Tissue-engineered skin is the best choice to cure the massive defects of skin and has a wide clinical prospect. With the rapid development of life science, operation technology, material science and many other related subjects, the ideal tissue-engineered skin can be obtained with similar function and feature to natural skin.
5.Influence of the Trial Point on the Quality of Root Canal Obturation

Objective To observe the influence of the trial point on the quality of root canal obturation.Methods 400 teeth were randomly divided into two groups. The experimental group was with the trial point,while the control group without. The quality of obturation was analyzed based on with and without testing the trial point.Results There were 201 teeth with the trial point and 199 teeth without, the rates of flush filled teeth were 86%,75% and 51% respectively in the trial point against,but its rates were 62%,49% and 26% respectively without the test in anterior, premolars and molars. 6.Application of HPLC fingerprint to optimization of extraction process of Herba Artemisiae scopariae

AIM:To optimize the extraction process of Herba Artemisiae scopariae in Yinchenhao Decoction.METHODS:On the basis of HPLC fingerprint of Herba Artemisiae scopariae uniform design was adopted to the extraction process,with four factors,including solvent concentration,solvent dosage,extraction cyde and extraction times in nine levels.Finally,the best extraction conditions were chosen according to the similarity of HPLC fingerprint and the extraction efficiency.RESULTS:The optimal extract procedure was refluxing for 90 mins with 25 times solvent of(11.14%) ethanol for three times.CONCLUSION:HPLC fingerprint is useful for the optimization of extraction process of Herb Artemisiae scopariae.
7.Surveillance of bacterial resistance at Children's Hospital of Chongqing Medical University in 2015

Objective To investigate the susceptibility and resistance profile of clinical isolates.Methods Clinical isolates were collected from Children's Hospital of Chongqing Medical University from January 1 to December 31,2015.Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems.Results were analyzed according to CLSI 2015 breakpoints.Results A total of 13 109 clinical isolates were collected from January to December 2015,of which gram negative organisms and gram positive cocci accounted for 65.3 % (8 560/13 109) and 34.7 % (4 549/13 109),respectively.Methicillin resistant strains in S.aureus (MRSA) and coagulase negative Staphylococcus (MRCNS) accounted for 29.6 % and 67.1%,respectively.Most (93.1%) MRSA strains were still susceptible to trimethoprim-sulfamethoxazole,while 80.2 % of MRCNS strains were susceptible to rifampin.No staphylococcal strains were found resistant to vancomycin,teicoplanin or linezolid.The resistance rates of E.faecalis strains to most antibiotics tested (except tetracycline) were much lower than those of E.faecium.Some strains of both species were resistant to vancomycin.No E.faecalis or E.faecium strains were found resistant to vancomycin.The prevalence of ESBLs-producing strains was 55.7 % in E.coli and 43.5 % in Klebsiella (K.pneumoniae and K.oxytoca) and 11.6 % in Proteus mirabilis isolates.ESBLs-producing Enterobacteriaceae strains were more resistant than non-ESBLs-producing strains in terms of antibiotic resistance rates.Enterobacteriaceae strains were still highly susceptible to carbapenems.Overall,less than 16.0 % of these strains were resistant to carbapenems.About 10.5 % and 9.4 % of the A.baumannii strains were resistant to imipenem and meropenem,respectively.Compared to the data of year 2014,the prevalence of extensively-drug resistant P.aeruginosa and K.pneumoniae strains increased.Conclusions The antibiotic resistance of clinical bacterial isolates is growing.The emerging and increasing prevalence of multi-drug or pan-drug resistant strains poses a serious threat to clinical practice and implies the importance of strengthening infection control.
8.Clinical study of modified Shenghua Decoction combined with ethinylestradiol and cyproterone acetate tablets for the polycystic ovary syndrome patients with blood-insufficiency congealing-cold and blood-stasis syndrome

Objective:To discuss the clinical efficacy of Shenghua Decoction combined with conventional western medicine therapy in treatment of polycystic ovarian syndrome with blood-insufficiency congealing-cold and blood-stasis syndrome. Methods:A total of 140 polycystic ovarian syndrome patients with blood-insufficiency congealing-cold and blood-stasis syndrome were selected from January 2018 to December 2019 in our hospitals, and were divided into observation group and control group according to the random indicator method, 70 cases in each group. The control group received ethinylestradiol and cyproterone acetate tablets treatment, and Shenghua Decoction was added based on the control group’s treatment in the observation group. Changes in the TCM syndrome score, modified Ferriman-Gallway (mF-G) of hirsutism, Rosenfield of acne, Testosterone (T), Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH) levels detected by ELISA of two groups were observed. The adverse events were recorded and the clinical efficacy was evaluated. Results:The effective rate of the observation group was 88.6% (62/70), which was significantly higher than control group of 60.0% (42/70) ( χ2=14.957, P<0.001). After treatment, mF-G, Rosenfield and TCM Syndrome Scores in the observation group were significantly lower than those in the control group ( t=6.524, 3.111, 2.196, P<0.01 or P<0.05). After treatment, the levels of FSH [(5.16 ± 1.76) IU/L vs. (7.19 ± 1.06) IU/L, t=2.530], LH [(6.12 ± 1.88) IU/L vs. (8.03 ± 1.78) IU/L, t=2.403], T [(1.34 ± 0.32) μg/L vs. (1.80 ± 0.36) μg/L, t=7.080] in the observation group were significantly lower than those in the control group ( P<0.05 or P<0.01) Conclusion:The Shenghua Decoction showed beneficial effect for the patients with polycystic ovarian syndrome with blood-insufficiency congealing-cold and blood-stasis syndrome.
9.Determination of schizandrol A in Mian'er'an Dripping Pills by HPLC

AIM: To establish a method for determination of schizandrol A in Mian'er'an Dripping Pills (Semen Ziziphi Spinosae, Fructus Schisandrae, etc.). METHODS: HPLC was used with hypersil C 18 column (250mm?4.6mm; 5?m), methanol water (65∶35) as a mobile phase and detection wavelength at 254nm. The flow rate was 1.0mL?min -1 and at room temperature. RESULTS: The calibration curve was linear (r= 0.9999). The average recovery was 100.2% and RSD was 2.9%, respectively. CONCLUSION: The method is simple, quick and specific, and provides a quality control for the dripping pills.

DNA polymerase γis the only known DNA polymerase in human mitochondria,and is essential for mitochondrial DNA replication and repair.DNA polymerase γ is encoded by POLG gene.POLG-related disorders resulted from mutations of POLG gene comprise a continuum of overlapping phenotypes including Alpers Huttenlocher syndrome and other five subtypes, with high prevalence rate at patients with intractable seizure.Genetic testing for POLG mutations in patients with intractable epilepsies is very important for clinical diagnostics, genetic counseling, and treatment decisions because of the increased risk for VPA-induced liver failure in patients with POLG mutations.
