Urticaria pigmentosa is a form of urticaria characterized by abnormality of cutaneous mast cells. No prediction for either gender has been reported but it usually occurs in children. When it develops in an adult cutaneous manifestations often follows recalcitrant and persistent course. Though tendency of genetic inheritance with an autosomal dominant pattern has been suggested most of patients have no family history of mastocytosis. We report a case of adult-onset urticaria pigmentosa with references.

Seoul, Korea Cutaneous mastocytosis is primarily a disease of children and its onset in adults life is rare. When it develops in adult, lesions appear as urticaria pigmentosa in most cases, and rarely as telangiectasia macularis eruptiva perstans. We report herein a case of cutaneous mastocytosis which appeared for the first time around the age of 40 with mixed or intermediate form of urticaria pigmentosa and telangiectasia macularis eruptiva perstans clinically, and histologically.

We report three cases of atypical skin eruptions in leukemias presenting as scrotal ulcers, herpes simplex and urticaria pigmentosa that are unique in their morphology and site of presentation. Therefore any skin lesion that develops in patients with leukemia should be examined by biopsy.

A patient with long-standing urticaria pigmentosa presented with a pea-sized reddish to purplish papule on the posterior part of the right ear. Histopathologic examination revealed numerous dilated vascular structures in the upper dermis and mast cell infiltrations throughout the whole dermis, consistent with combined mastocytoma-hemangioma. The mast cells were strongly positive with Giemsa stain.

Urticaria pigmentosa (UP) is the most common variant of cutaneous mastocytosis. Primarily a disease of childhood, in over one-half of the cases onset is before 2 years of age, and in 90%, the disease is confined to the skin. UP precedes the diagnosis of systemic mastocytosis and is more common in adults than in children. Therefore, systemic examination should be performed in patients with UP. We report a case of adult-onset UP with systemic involvement.

Urticaria pigmentosa developed in twins at the age of 6 weeks. They have numerous round to oval shaped, discrete or confluent reddish brown colored macules over whole body surface and neither twin had vomiting, diarrhea, flushing or irritability. Their relatives are unaffected. Biopsy specimen of. back lesion showed. pooling of mast cells in the upper dermis, and electron microscopic examinattion revealed normal mast cells.

Mastocytosis is a heterogenous group of disorders characterized by abnormal growth and accumulation of mast cells in one or more organ systems. Urticaria pigmentosa represents the most common form of cutaneous mastocytosis in children and adults. We report a case of adult-onset urticaria pigmentosa in a 53-year-old man. The patient had pruritic multiple scattered well-demarcated brownish macules and papules on whole body for 4 years. Histopathologic examination showed diffuse and perivascular infiltration of lymphocytes and cuboidal mast cells in the upper dermis. Any sign or symptom of systemic involvement was not detected.

We observed 2 cases of urticaria pigmentosa with synchronous onset and concordant manifestation in a pair of identical male twins. At the age of 12 months both had developed multiple, round-to-oval, discrete brown-colored macules on the trunk and extremities. They had no systemic manifestations. Biopsy specimens in both cases showed the presence of a number of mast cells in the papillary and superficial dermis. We, herein, describe a pair of monozygotic twins with urticaria pigmentosa.
