1.Curative effect observation of labetalol combined with magnesium sulfate in treating moderate and severe pregnancy-induced hypertension disease

Objective To observe the clinical effect and safety of labetalol combined with magnesium sulfate in treating moderate and severe pregnancy-induced hypertension disease.Methods 78 cases with moderate and severe pregnancy-induced hypertension disease were divided into the control group and observation group according to random number table method,39 cases were in each group.The patients in the control group were given the magnesi-um sulfate only,and the patients in the observation group were given the treatment of labetalol combined with magnesi-um sulfate.After treatment,the average blood pressure,24h urine protein,maternal heart rate,fetus heart rate and pregnancy outcomes were detected.Results After treatment,the average blood pressure[(134.5 ±9.7) vs (85.7 ± 5.9)]mmHg of observation group was significantly lower than the control group(t=4.60,7.21,all P<0.05);the 24h urine protein,maternal heart rate and fetus heart rate had no significant difference (t=0.52,0.56,0.15,all P>0.05);the termination of pregnancy(7.7%),placental abruption(0.0%),fetal distress(5.1%),incidence of neo-natal asphyxia(7.7%),postpartum hemorrhage(12.8%),low birth weight(12.8%) were significantly lower than the control group(33.3%,12.8%,20.5%,25.6%,33.3%,35.9%) (χ2 =7.86,5.34,4.13,4.52,4.62,5.64,all P<0.05);the total effective rate of observation group with 37 cases (94.9%) was significantly higher than the con-trol group with 31 cases (79.5%) (χ2 =4.13,P<0.05);the incidence of adverse reactions of observation group with 15.4%was significantly lower than the control group with 35.9%(χ2 =4.30,P<0.05).Conclusion Labe-talol combined with magnesium sulfate in treating moderate and severe pregnancy-induced hypertension disease could effectively reduce the average blood pressure,the 24h urine protein and improve the pregnancy outcomes,which was worthy of clinical popularization and application.
2.Choice and application of medical records in teaching of prescription lessons

Chinese medical records recorded the whole activities during a syndrome differentiation and treatment of disease. With the characteristics of diversity and lively, the records have important values in establishing TCM therapeutic thoughts and abilities for students. Amount of medical records have been recorded in medical books since the Western Han dynasty. In order to provide the references for teaching prescription aided by medical records, this article discusses the choice and application of medical records to teach prescription by listing examples on the basis of outlining the development of TCM medical records.
3.Emergency repair of soft tissue defect at thumb using miniature flaps

Objective To report the treatment outcomes and repair methods for soft tissue defects at thumb using miniature flaps. Methods Six different kinds of surgical flaps were used for the repair on different thumb injuries. They were radial lateral thumb reverse island flap, dorsal ulnar thumb revers flap, first metacarpal dorsal reverse flap, first digital dorsal island flap, neurovessel pedicled V-Y advanced island flap, free nail flap first foot digital, and so on. The emergency repair using miniature flaps were studied on 132 different cases, all 137 flaps. Results The flaps survived expect 5 flaps had partial epidermis necorsis in the 137 flaps. But the 5 flaps survived too after disinfecting and changing dressing. The appearances and functions of thumbs were satisfactory for the 1 to 2 years post-operation follow-up. Conclusion It will gain better results if choosing appropriate emergency repair methods using miniature flaps on different cases.
4.Three-dimensional reconstruction visualization system enhances the accuracy of lower cervical pedicle screw implantation

BACKGROUND:Pedicle screw implantation is a common method to repair many kinds of diseases of the lower cervical spine. Three-dimensional (3D) reconstruction visualization system can be used in order to improve the accuracy of the implant and improve the prognosis.
OBJECTIVE:To investigate the effect of 3D reconstruction visualization system on the accuracy of cervical pedicle screw implantation.
METHODS: The clinical data of 89 patients with cervical spine dislocation, who underwent cervical pedicle screw implantation, were analyzed retrospectively. Patients were divided into control group (46 cases) and observation group (43 cases) according to the navigation method. Patients in the control group underwent C arm X ray two dimensional navigation. Patients in the observation group underwent three-dimensional reconstruction visualization system navigation. Intraoperative placement time was observed, and the accuracy of screw placement was assessed in both groups. The cases were folowed up for 12 months, and the adverse events were recorded and compared.
RESULTS AND CONCLUSION: (1) Intraoperative implantation time was shorter in the observation group than in the control group, but no significant difference was found (P> 0.05). (2) Accuracy rate: There were no three types of nail cases in the observation group, and the accuracy rate was 93% (40/43). In the control group, two cases affected three types of nailing, and the accuracy rate was 81% (37/46). The accuracy rate was significantly higher in the observation group than in the control group (P < 0.05). (3) Postoperative folow-up: None suffered from nerve tissue injury to vertebral artery, blood vessel and spinal cord. The screws of the two groups were in stable condition, without screw breakage or loosening. (4) Results suggested that in lower cervical pedicle nail implantation, 3D reconstruction navigation system can effectively improve the accuracy of screw placement, and does not increase nailing time or lead to adverse events. It is a safe and effective navigation mode.
5.Correlation between elastography score and strain rate ratio in breast small tumor

Objective To explore the value of elastography score and strain rate ratio in the diagnosis of small breast malignant focus. Methods We retrospectively analyzed 22 patients with breast small malignant foci less than 10 mm. Ultrasound characteristics were summed up in breast small cancer. Results On elastogram, 2 patients scored 3, 14 scored 4 and 6 scored 5.The average strain rate ratio of all foci was 4.76, and there was correlation between it and elastography scores. Conclusion Ultrasonic elastography has important value in the diagnosis of breast small cancer.
