1.Packing effects of different hemostatic materials during functional endoscopic sinus surgery: histocompatibility comparison

BACKGROUND:At present, the most common materials for clinical hemostasis are absorbable tampons and Vaseline gauze. OBJECTIVE:To compare the packing effects of absorbable tampons and Vaseline gauze in functional endoscopic sinus surgery. METHODS:Totaly 100 sinusitis patients, 57 males and 43 females, aged 16-61 years, with a history of 5 months to 23 years, were randomized into two groups: patients in the observation group were treated with absorbable tampons and patients in the control group treated with Vaseline gauze during functional endoscopic sinus surgery. Patient's discomfort, bleeding when the filer was extracted at 48 hours, and nasal bleeding within postoperative 24 hours were compared between the two groups. RESULTS AND CONCLUSION: No adverse reaction related to absorbable tampons occurred in the observation group, indicating that the absorbable tampon has better histocompatibility. Patient's discomfort, bleeding when the filer was extracted at 48 hours after operation, and nasal bleeding within postoperative 24 hours were significantly improved in the observation group compared with the control group (P < 0.05). These findings suggest that the absorbable tampons has a better hemostatic function, which can effectively reduce postoperative nasal bleeding and relieve postoperative discomfort in patients with sinusitis.
2.Clinical Study:Concurrent Interventional Cisplatin Ieucovorin Fluorouracil Hydroxy Camptothecin Chemotherapy Plus Radiotherapy in Patients with Ⅳ Stage Middle-lower Section Esophageal Cancer

Objective To investigate the efficacy of concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy in patients with Ⅳ stage middle-lower section esophageal cancer.Methods The subjects were all the patients with Ⅳ stage middle-lower section esophageal cancer.Among of them,42 patients,via artery,had been enrolled for concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy.40 patients were given irradiation.Results The incidence rate of Ⅰ grade esophageal X-ray in the former group was 57.1%,and that in the later group was 25.0%,and the parameters had significant difference(?0.05).Conclusion Concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy may be attributed to progression of Ⅳ stage middle-lower section esophageal cancer.
3.Influential factors to the prognosis of senile patients with bladder cancer

Objective To investigate the influential factors to the prognosis of senile patients with bladder cancer. Methods One hundred and forty-five senile patients with bladder cancer confirmed by pathology from 1998 to 2008 were clinically investigated and analyzed using measuring scales of Functional Living Index-Cancer (FLIC), Functional Assessment of Cancer Therapy-General (FACT-G) and Cancer Rehabilitation Evaluation System short form (CARES-SF). 4.Endoscopy nasal microsurgery and multiple techniques in treatment of nasopharyngeal angiofibromas

Objective To explore the methods for endoscopy nasal microsurgery and multiple techniques in treatment of nasopharyngeal angiofibromas. Methods CT, MRI and digial substraction angiography (DSA) and endoscopic examination had been used. ALL 12 patients accepted preoperative feeding artery embolum, hypotention anesthesia and injected adrenaline-lidocaine. Microelectrotomy-electrocoagulation had been used to remove tumor. Results All patients were treated by endoscopic nasal surgery. No complications had occurred. No recurrence were found in follow-up period. Conclusion The important factors to reduce amount of introperative bleeding are to select proper operative patients and use multiple techniques. Endoscopy nasal surgery has dominance in reducing complications, wound, tumor recurrence, operative time and nasal function.
5. Compensatory activation of estrogen receptor α signaling in acquired resistance to lapatinib of HER2-overexpressing/ERα-positive breast cancer cells

Objective To investigate the role of compensatory activation of estrogen receptor α (ERα) signaling in acquired resistance to lapatinib of breast cancer cells BT474 and the related mechanism. Methods Real-time PCR and Western blotting analysis were used to determine the changes of HER2 and ERα pathways in BT474 cells treated by lapatinib. Acquired resistant model of rBT474 cellswas induced with increasing concentrations of lapatinib (from 0. 25 μmol/L to 5 μmol/L); the apoptosis in rBT474 cells was determined by flow cytometry. Western blotting analysis was used to evaluate the differences between BT474 and rBT474 in the HER2 and ER pathways. The growth of rBT474 cells treated by lapatinib and/or fulvestrant was detected by MTT assay, andcolony formation was used to observe the possibility of preventing acquired resistance to lapatinib in BT474 cells by double targets therapy. Results The results of real-time PCR and Western blotting analysis showed that Lapatinib inhibited phosphorylation of HER2 and induced expression of forkhead-box protein O 3A (F0X03a) and progesterone receptor (PR) in BT474 cells. Acquired resistance cell model of rBT474 was established in a 5 μmol/L lapatinib condition. Western blotting analysis showed that the phosphatidylinositol 3-kinase/protein kinase B (PI3K/AKT) pathway was inhibited in rBT474 cells compared with that in the BT474 cells, while the mitogen-activated protein kinase (MAPK) pathways, especially the ER pathway, were activated in the BT474 cells. MTT results showed that, Lapatinib combined with fulvestrant had a significantly greater inhibition on rBT474 cell vitality compared with lapatinib alone (P<0. 01). Colony formation results also showed that combination of lapatinib and fulvestrant had a significantly greater inhibition effect against colon formation of BT474 cells compared with each drug alone and DMSO (P<0. 01), showing a possible prevention ability against acquired resistance. Conclusion Compensatory activation of estrogen receptor a signaling might be one of the mechanisms of acquired resistance to lapatinib in HER2-overexpressing/ERcrpositive breast cancer cells, and inhibition of PI3K/AKT and activation of MAPK might be the main reason for compensatory activation of ERα.
6.Research of modified skin flap of external canal in auditory canal shaping to enlarge.

OBJECTIVE:
To investigate the new method of modified skin flap of external canal in auditory canal shaping to enlarge at radical mastoidectomy in endaural incision.
METHOD:
From 1999 to 2009, 412 patients with 428 ears in operation were randomly divided into two groups. Patients underwent open mastoidectomy and some patients were performed concurrent tympanoplasty. A group (control group)were operated in traditional skin flap of external canal, B group is modified group. Bleed and effusion from the wound surface, the time of dry ear and the change of diameter of extermal acoustic meatus after one year were observed after the surgeries and deal the results with SPSS.
RESULT:
The difference had statistical significance in bleed and effusion from the wound surface, the time of dry ear and the shrink of external acoustic meatus in 12 months among two groups.
CONCLUSION
Modified skin flap of external canal in auditory canal shaping to enlarge can help to make a no-wound dehiscence by wiping out the raw surface of external acoustic meatus. It is better than traditional skin flap of external canal.

7.Verruciform Xanthoma of the Penis.

Verruciform xanthoma is an uncommon benign lesion. The most common presentation occurs in the oral cavity; however, it has also been described in other sites, especially the penis. Herein is reported the first case of verruciform xanthoma of the penis in China, the fourteenth case in the literature. Clinically, genital verruciform xanthoma is often confuses with papillomas, verrucous carcinomas and squamous cell carcinomas, and therefore a histopathological diagnosis is necessary.

8.Application of EABR and ESRT in children with inner ear malformation after cochlear implantation

OBJECTIVE To investigate the characteristics and laws of changes in electrically evoked auditory brainstem responses(EABR),electrically evoked stapedius reflex(ESRT) after cochlear implantation in children with inner ear malformation.METHODS 88 cases of Australian 24 multi-channel cochlear implants were divided into normal group and inner ear malformation group,The EABR and ESRT thresholds were measured at different periods within one year after operation.Behavior auditory responses,T-level and C-level were measured 1 year postoperatively to analyze their characteristics and changes.RESULTS The EABR and ESRT thresholds in the patients with inner ear malformation were significantly higher than those in the normal cochlear group(P<0.05).The changing trend of EABR and ESRT were the same in the 2 groups,the overall trend is that low frequency value is low,high frequency value is higher.EABR,ESRT threshold increased gradually after 1 year,EABR was significantly correlated with T-level,and ESRT was significantly correlated with C-level.CONCLUSION The changes of EABR and ESRT thresholds in the cochlear implantation of the inner ear malformation group were similar to those of the normal cochlear group,the thresholds can be used to guide mapping for the inner ear malformation of the cochlear implant.
9.Effects of Tanshinone II A on the myocardial apoptosis and the miR-133 levels in rats with heart failure.

OBJECTIVETo explore the effects of Tanshinone II A (Tan II A) on the myocardial apoptosis in rats with heart failure and its mechanisms for regulating the miR- 133 levels.
METHODSThe heart failure rat model was established by thoracic aorta constriction (TAC). Tan II A Injection was applied for 12 successive weeks. Meanwhile, partial heart failure rats were subcutaneously implanted with osmotic pump of antagonist to observe its inhibition on the miR-133 level. Twelve weeks later, the hemodynamic conditions, the myocardial apoptosis (using TUENL method), myocardial pro-apoptotic genes (Bax and Caspase-3), and the expressions of anti-apoptosis genes (Bcl-2) (using Western blot and RT-PCR method) were analyzed.
RESULTSCompared with the sham-operation group, TAC operation could deteriorate the heart function (except the mean arterial pressure), elevate the myocardial apoptosis level, increase the protein and mRNA levels of Bax and Caspase-3, and down-regulate the protein and mRNA levels of miR-133 and Bcl-2. TAC rats treated by Tan II A could significantly improve all indices with statistical difference except the heart rate. Subcutaneously pumping of antagonist could partially abolish the anti-apoptosis effect of Tan II A.
CONCLUSIONTan II A could decrease the myocardial apoptosis level of heart failure rats, which was possibly realized by up-regulating the miR-133 level.

10.A study of histopathological factors increasing postage-specific antigen

Objective To evaluate the cause of evaluated postage-specific antigen(PSA)in patients performed transrectal uhrasonography(TRUSG)guided prostate biopsy beeause of high PSA levels.Methods In a retrospective study 504 prostate biopsies performed between January 1998 and December 2001 were evaluated and the levels of serum PSA were determined in samples obtained immediately before sextant biopsy was performed.All patients underwent 6 or 13 cote primary prostate needle biopsies.Results 185 prostate cancer,109 NIH-Ⅳ prostatitis and 210 patients with benign prostate hyperplasia(BPH)were identified.The difference in free-PSA,total-PSA,f/t-PSA levels between prostate cancer,BPH and NIH-Ⅳ prostatitis was significant(P<0.05).No significant difference was found in age,transrectal ultrasonography and digital rectal examination.In multivariate analysis,free-PSA,total-PSA,f/t-PSA was the significant predictors of histology in prostate cancer(P<0.05).A significant correlation was found between the serum total and free PSA levels and the grade and stage of prostate cancer(P<0.05).Preoperative variables predictors of histology in BPH were TPSA and FPSA(P<0.05).In multivariate analysis,TPSA was the only significant predictors of histology in BPH(P<0.01).The best cutoff value was constructed to differ pathology type in prostate diseases:tPSA≥4 ng/ml,fPSA≥0.85 ng/ml and f/t-PSA≤0.16(P<0.05).Conclusions High serum PSA levels may correlate with asymptomatic inflammatory prostatitis,prostate cancer and BPH.The factors contributing to elevated serum PSA concentrations include cell proliferating,glandular epithelial disrupt.
