1.Epidemiological study of post-stroke depression of the elderly in the Wanshou Road Community in Beijing

ObjectiveTo study the prevalence rate of post-stroke depression (PSD) of the elderly in the community and analyze risk factors of them. MethodsThe elders of the age of 60 or over were sampled as subjects from a community. Zung (SDS) was used as screening instruments and Hamilton rating scale for depression (HRSD-24) as evaluating scales. The prevalence rate of PSD was surveyed. A structured questionnaire was used and physical examination was carried out. Risk factors measured included baseline information, daily habits, and history of main diseases (hypertension, coronary heart disease, diabetes mellitus, myocardial infarction, and so on). The relationship between risk factors and PSD was analyzed.ResultsThe prevalence of PSD was 9.5%. The risk factors for PSD were life quality and age. ConclusionPost-stroke depression disturbs elder's health and rehabilitation. The prevalence rate and harm of PSD are higher than depression without stroke.

ObjectiveTo elucidate the clinical characteristics and the outcome of SARS in elderly pat ients. Methods Clinical data of 66 elderly patients(≥60 years) with SARS were analysed and compared with that of 238 younger adult SARS patients(18-59 years) in the sa me period. Results Sixty-six out of 304 patients(21 7%) with SARS were more than 60 years. Th e mortality rate of SARS in elderly patients was 22 7%. Objective To extract the loci of murine MHC gene and construct plasmids.Methods The RNA of mice was extracted and reversely transcribed into cDNA.By using nested PCR,the products were connected with T vector,cloned,and sequenced.Subsequently,the genes were digested by endonucleases,connected with expression vector,and sequenced again to choose the correct clones.Results After the nested PCR,the products were approved by sequencing.After being connected with the vectors,they were approved again by sequencing and the correct clones were chosen.Conclusion All of the loci of the MHC gene can be obtained by nested PCR.The plasmids from the correct clone can be used in the further experiments of transferring the gene to mitigate the transplantation rejection.
5.Total flavonoids of litsea coreana decreases the cytotoxicity of oxaliplatin in TM3 Leydig cells via enhancing the function of gap junction.

OBJECTIVETo investigate the effects of total flavonoids of Litsea Coreana (TFLC) on the gap junction (GJ) intercellular communication in TM3 testicular Leydig cells and whether TFLC can reduce the cytotoxicity of oxaliplatin (OHP) in vitro.
METHODSWe detected the effect of TFLC on the dye spread of the in vitro cultured TM3 cells by parachute assay, observed changes in the expression of connexin 43 (Cx43) total protein in the TFLC-treated TM3 cells by Western blot, and determined the effects of TFLC on the expression of Cx43 on the membrane of the TM3 cells by immunofluorescence assay and on the cytotoxicity of OHP by MTT assay.
RESULTSTFLC obviously enhanced the GJ function with the increasing of the TFLC concentration in the TM3 cells. Western blot and immunofluorescence assay confirmed that TFLC significantly enhanced the expression of Cx43 total protein and Cx43 expression on the membrane of the TM3 cells. MTT assay showed that at a high cell density (confluent with GJ formation), 20 microg/ml TFLC enhanced the GJ function of the TM3 cells and reduced the cytotoxicity of OHP (P < 0.05), while at a low density (preconfluent with no GJ formation), TFLC exhibited no effect on the cytotoxicity of OHP (P > 0.05).
CONCLUSIONTFLC increases the Cx43 expression and GJ function in normal TM3 Leydig cells, and the enhancement of GJ function reduces the cytotoxicity of OHP.

6.Anti-thrombopoietin antibodies in systemic lupus erythematosus with thrombocytopenia

Objective To investigate the existence and significance of circulating autoantibodies to thrombopoietin (TPO) in sera from patients with systemic lupus erythematosus (SLE).Methods Fifty-six consecutive patients with SLE,twenty patients with idiopathic thrombocytopenic purpura (ITP),and twenty normal individuals were involved in this study.The characteristics of all patients with SLE were analyzed.Antibodies to TPO were detected using an enzyme-linked immunosorbent assay (ELISA).For normal distribution count data,x2 test or Fisher exact test was used,t test was used for measurement data,and Wilcoxon's rank test for non-normally distributed data which was represented by M(Q).Results A higher frequency of antibodies to TPO were observed in SLE patients than those in healthy controls (39.3% vs 0,x2=11.058,P=0.001).Moreover,anti-TPO antibodies were detected in 15 (57.7%) of 26 SLE patients with thrombocytopenia,compared with that in 7 (23.3%,x2=6.894,P=0.009) of 30 patients without thrombocytopenia.Furthermore,the patients with antibodies to TPO exhibited more severe thrombocytopenia (t=3.010,P=0.004).Finally,anti-TPO antibodies seemed more likely to occur in patients with arthritis (x2=5.959,P=0.015),anti-dsDNA antibodies (x2=5.959,P=0.015).Conclusion The high incidence of antibodies to TPO in SLE patients with thrombocytopenia suggests that anti-TPO antibodies might play a vital role in SLE patients developing thrombocytopenia.Thus,there might be a clinical value by detecting anti-TPO antibodies in SLE patients with thrombocytopenia.
7.Clinical therapy of paradoxical herniation

Objective To explore the treatment of paradoxical herniation after bone flap decompression surgery,and improve people's understanding of paradoxical herniation.Methods From April 2014 to July 2017,the clinical data of 12 patients with paradoxical herniation were retrospectively analyzed.The treatment efficacies of position adjusting,sufficient hydration,and cranioplasty were explored.Results Nine patients (75%) had paradoxical herniation within two weeks of surgery,two(16.67%) developed paradoxical herniation from two weeks to one month of surgery,and one (8.33%) developed paradoxical herniation at two months after operation.But no all the patients had sunken and collapsed skin flap.After position adjusting,sufficient hydration,and cranioplasty,12 patients had periodic improvement;the neurological function of 9 patients accepted cranioplasty recovered obviously.Conclusion Paradoxical herniation is reversible after effective treatments,as position adjusting,sufficient hydration,and cranioplasty,enjoying good results.
8.Efficacy and safety of intravenous thrombolysis with different doses of rt-PA in the treatment of acute anterior circulation cerebral infarction with atrial fibrillation

Objective To explore the Efficacy and safety of intravenous thrombolysis with different doses of rt-PA in the treatment of acute anterior circulation cerebral infarction with atrial fibrillation.Methods Retrospective analysis of 61 cases of patients with anterior circulation of cerebral infarction with atrial fibrillation from October 2009 to October 2014 in the First Affiliated Hospital of Xiamen University, the incidence within 4.5 hours of intravenous thrombolysis,and divided into two groups by rt-PA usage,19 cases in adequate group,received 0.9 mg/kg rt-PA intravenous thrombolytic therapy,42 cases in low dose group, received 0.6 mg/kg rt-PA intravenous thrombolysis.Before and after thrombolysis 1,7 and 30 days,NIHSS score was measured, the indexes of coagulation were observed at before thrombolysis and 1,7 days after thrombolysis,,CT scans were performed at 1, 7, and 14 days after thrombolysis,and Rankin (MRS) scores were compared at 90 days after thrombolysis.Results NIHSS 1,7,30 days scores of 2 groups were significantly decreased after thrombolysis(P<0.05),there was no statistically significant at at each time point after thrombolysis.Plasma prothrombin time increased significantly at 1 day and 7 days after thrombolysis,fibrinogen was significantly lower,compared with the low dose group, the difference was significant (P<0.05).There was no significant difference between the two groups in clinical outcome and mortality.The rate of mucosal bleeding in low dose group was lower than that in adequate group (P<0.05).Conclusion Low-dose rt-PA group intravenous thrombolysis with anterior circulation of atrial fibrillation is more safe,can reduce the risk of bleeding, reduce neurological deficits and improve the quality of life of patients.
9.Intra-arterial thrombolysis with r-tPA for the treatment of acute cerebral infarction 6 to 9 hours after onset

Objective To determine the safety and efficacy of intra-arterial recombinant tissue plasminogen activator (r-tPA) for the treatment of acute cerebral infarction (ACI) in patients under the guidance of computed tomography perfusion-based selection within a 6-9 hour window.Methods Sixtythree ACI patients selected by using computed tomography perfusion imaging (CTPI) identifying thresholds for salvageable penumbra were randomly (random number) assigned to the group treated with intra-arterial thrombolysis with r-tPA (group A,n =30) or to the group managed with conventional anti-platelet aggregation agent (group B,n =33) within a 6-9 hour window.The National Institutes of Health Stroke Scale (NIHSS) and the modified Rankin Scale score (mRS) and Barthel Index (BI) were used for evaluating therapeutic efficacy.Global brain digital subtraction angiography (DSA) was done pre-and posttreatment to observe the recanalization of occluded vessels in the group A.All patients were monitored with CT scan within 24 hours to determine the cerebral hemorrhage,an unexpected complication of thrombolysis.Results Compared with pre-treatment,there were significant differences in NIHSS 24 hours after treatment in the group A and 7 days after treatment in both groups (P < 0.01).However,there were no significant differences in NIHSS 24 hours after treatment in the group B.More improvements in NIHSS at 24 hours and 7 days after treatment were observed in the group A than those in group B (P < 0.01),and more patients with favorable outcomes identified by mRS and BI in the group A than those in the group B (P =0.017 and P =0.016,respectively).In addition,twenty patients were showed successful recanalization in the group A and there were 2 cases of cerebral hemorrhage occurred in the group A,and there was no significant difference in the incidence of cerebral hemorrhage within 24 hours between the two groups (P > 0.05).Conclusions Intra-arterial thrombolysis with r-tPA for treatment of acute cerebral infarction was safe and effective within a 6-9 hour window under the guidance of CTPI.
10.Application of temporal and frontotemporal puncture or frontotemporal craniotomy for hypertensive intracerebral hemorrhage in basal ganglia region

Objective To investigate the application of temporal and frontotemporal puncture or frontotemporal craniotomy for hypertensive intracerebral hemorrhage in basal ganglia region .Methods From January 2013 to April 2017,eighty patients with basal ganglia region hypertensive intracerebral hemorrhage in Shanxi Da Hospital were divided into observation group and control group according to random number table method ,with 40 cases in each group.The two groups were respectively given puncture of temporal and frontal temporal region and craniotomy with frontal and temporal bone flap for evacuation of hematoma and decompression of bone flap .The complications and treatment effect after surgery of the two groups were compared.Results Compared with the control group , the emptying rate of postoperative hematoma 1 day in the observation group was decreased [(60.1 ±9.7)%vs.(81.0 ± 9.9)%,t=-9.537,P<0.05];the operation time was shortened [(68.5 ±7.7) min vs.(110.2 ±9.1) min,t=-22.124,P<0.05];the use of postoperative mannitol decreased [(672.1 ±84.2) g vs.(1427.2 ±99.6) g,t=-36.617,P<0.05];the incidence rate of intracranial pneumatosis in the observation group was increased (7.5%vs. 30.0%,χ2=6.646,P<0.05).Conclusion Monitoring of intracranial pressure in lateral ventricles combined with puncture of temporal and frontal temporal region used in hypertensive intracerebral hemorrhage in basal ganglia region is helpful to reduce the surgical trauma and accelerate the recovery of postoperative patients ,and can be widely used.
