1.Domesitical botulinum toxin type A injection in the treatment of post-stroke patients with upper extremity spasticity

Objective To investigate the efficacy and safety of intramuscular injection botulinum toxin A (CBTX-A) made in China. Methods 43 post-stroke patents with upper extremity refractory spasticity were recruited in the study. The patients were randomly divided into two groups: injection combined with rehabilitation therapy (treatment group) and simple rehabilitation therapy (control group). CBTX-A was injected to the biceps brachii. The efficacy was assessed by modified Ashworth scale (MAS), range of motion (ROM),Fugl-Meyer Assessment-upper limb (FMA-upper limb) and motor-Functional Independence Measure-upper limb (mot-FIM-upper limb) at the baseline,2-week,4-week,8-week and 12- week after treatment by a physical therapist who was blind so as to which treatment they received. Results In the treatment group,85.7%(18/21) patients were benefited from the CBTX-A injection. Both groups improved on MAS,ROM,FMA-upper and mot-FIM-upper post-treatment. The score of MAS of treatment group were respectively 1.74?0.41,1.62?0.35,1.60?0.41 at weeks 2,4,8,while that of control group were respectively 2.50?0.51,2.27?0.53,2.18?0.55. The differences were significant(P0.05). After treatment,the ROM of treatment group were respectively 57.81?57.60,66.43?64.38,68.14?65.99 at 4,8, 12-week, and that of control group were respectively 27.91?30. 13,30.73?34.03, 33.73?34.50. The differences were significant (P0.05). Conclusion The result of the present study indicates CBTX-A injection combined with rehabilitation therapy decreases in local spasticity,enlarges the ROM of elbow joint. It is efficient,safe and well-tolerated. CBTX-A injection provides an additional alternative procedure for treatment of adult acquired refractory focal spasticity.
2.Combination detection of serum squamous cell carcinoma antigen and carcino-embryonic antigen for the diagnosis of cervical carcinoma and evaluation of effect

Objective To explore the clinical significance of combination detection of serum squamous cell carcinoma antigen (SCC-Ag) and carcino-embryonic antigen (CEA) for the diagnosis,effect evaluation,monitoring and prognosis in cervical carcinoma.Methods Four hundred and twenty-four cases of cervical carcinoma patients diagnosed by pathology and 400 binign gynaecology patients were selected to detect levels of SCC-Ag and CEA by ELISA.Besides,the pelvic color ultrasound results of cervical carcinoma patients were collected and analyzed.Results The positive rates of serum SCC-Ag,CEA and combined detection of cervical carcinoma patients were 55.9 %(237/424),26.4 %(112/424),61.8 %(262/424),which were higher than those of binign gynaecology patients [12.0 %(48/400),12.5 %(50/400),14.5 %(58/400)],and the differences were statistical significance (all P< 0.05).The sensitivity of combined detection of SCC-Ag and CEA for diagnosing cervical carcinoma (61.8 %) was better than that of detection of alone SCC-Ag or CEA (55.9 %,26.4 %),and the difference was statistical significance (P < 0.05).The differences of pretreatment serum SCC-Ag and CEA levels in the cervical carcinoma patients who were dependent on clinical stage,pathological type,depth of invasion,lymph node metastasis were statistically significant (all P < 0.05).The combined detection of SCC-Ag and CEA in patients with stage Ⅰ and Ⅱ had higher positive rates [45.5 % (71/156),73.2 %(139/190)] than those of the ultrasound examination [33.3 %(52/156),51.1%(97/150)] (P< 0.05).After surgery or concurrent chemoradiotherapy or surgery combined radiation and chemotherapy,SCCAg levels of patients in post-treatment (median 0.34,0.51,0.33 ng/ml) were much lower than those of pretreatment (median 0.48,3.44,1.29 ng/ml),and CEA levels of post-treatment (median 0.75,0.81,0.71 ng/ml)were also lower than those of pre-treatment (median 1.22,1.53,1.50 ng/ml) (P < 0.05).After treatment,cervical cancer patients with the raised or not decreased to the normal range levels of SCC-Ag and CEA had recurrences or metastasis rates of 33.3 %(6/18),56.5 %(13/23) and 46.1%(35/76).Survival rate of the patients with positive SCC-Ag was significantly lower than that of patients with negative (P < 0.05),while the survival rate of patients with positive or negative CEA had no difference (P > 0.05).Conclusions Combined detection of SCC-Ag and CEA can be used as an important indicator to estimate clinical stage,pathological type,depth of invasion,lymph node metastasis,curative effect and prognosis of cervical carcinoma.Combined detection of SCC-Ag and CEA had higher positive rates than pelvic color ultrasound examination for cervical carcinoma patients in stage Ⅰ and Ⅱ.
5.A Research on the Transformation of Traditional Chinese Medicine Language System into SKOS Ontology

Objective To transform knowledge organization systems (KOS) about traditional Chinese medicine (TCM) domain into standardized ontologies, in order to achieve the publication, sharing, and fusion of TCM KOS on the Web, and to further improve the organization and management of TCM knowledge. Methods Simple knowledge organization system (SKOS), a technical specification recommended by World Wide Web Consortium, was used to transform TCM KOS into publishable ontologies on the Web. Results Traditional Chinese medicine language system was transformed into an SKOS ontology, which was stored into a knowledge base that could support SPARQL (standard query language of the Semantic Web) query access, and provide concept, terminology, and knowledge services for Semantic Web applications, such as semantic wiki and semantic search via Web service interfaces. Conclusion This research preliminarily verifies the applicability of SKOS in TCM domain, and exemplifies the process of transforming TCM KOS into SKOS ontologies.
6.EFFECTS OF N-ACETYLCYSTEIN AND OTHER AGENTS ON THE ROLE OF MATRIX METALLOPROTEINASES IN AIRWAY REMODELLING OF COPD RAT MODELS

Fifty three Wistar rats were randomly divided into healthy control group, COPD model group and NAC group, PKC inhibitor H7 intervention group and TGF ? monocolonal antibody group(TGF ? MA group).To study the role of matrix metalloproteinases(MMPs) and the tissue inhibitor of MMPs(TIMP 1) in the airway extracellular matrix(ECM) remodelling of COPD rat models and to observe the effects of NAC, H7 and TGF ? MA intervention on the regulation of MMPs and TIMP 1 and on ECM remodelling of the airway walls. Compared with control group, the airway collagen, the hydroxyproline(Hy) content of lung homogenates ,the number of fibroblasts(Fb) , the protein and/or mRNA expressions of MMP 2, MMP 9, TIMP 1 and TGF ? Ⅰ, Ⅱ receptor and the enzyme activities of MMP 2 (72kD), MMP 9 (92kD) were significantly increased in COPD model group. In all the drug intervention groups, the expressions of the above parameters were significantly decreased than those in model group except for Hy and Fb in H7 group, protein expression of TGF ? I receptor in NAC group and MMP 9 in TGF ? MA group. The results suggested that increased MMPs as matrix degrading enzymes might be responsible for the excessive degradation of ECM in airway, whereas incresed TIMPs might promote excessive ECM synthesis and deposition. The imbalance of MMP 9/TIMP 1 was related to the airway ECM remodelling. An antioxidant NAC and TGF ? MA might regulate the MMPs/TIMP 1 expression and reduce the airway fibrosis.H7 had strong collagenase inhibitory action,resulting increse in Hy and Fbs. The data may be helpful for searching effective prevention and treatment of airway ECM remodelling.
7.Effect of the family history of cardiovascular disease and diabetes on birth weight and insulin sensitivity in neonates

Objective Examining the potential for genetic influences on the association of fetal growth restriction (FGR) with increased occurrence of coronary heart disease, hypertension and diabetes in later life. Methods The study group consisted of 75 neonates of small-for gestation age (SGA) and 244 neonates of adequate for gestational age (AGA), whose mothers did not have diabetes mellitus. Anthropometric measurements were performed at birth. Fasting glucose and insulin levels were measured on the 3rd day after birth. The history of cardiovascular disease and diabetes mellitus was recorded in their parents and grandparents. Results The prevalence of coronary heart disease, hypertension and diabetes mellitus was significantly higher in parents and grandparents of SGA group than that of AGA group (20.0%, 30.7%, 12.0% vs 9.3%, 14.7%, 3.5%,P0.05). Conclusions Genetic factor may promote both FGR and susceptibility to occurrence of coronary heart disease, hypertension and diabetes.
8.Research on High-level Framework for Traditional Chinese Medicine Informatics Standards

The basic construction of three-dimensional profiling framework includes TCM business domain, elements of TCM informationization and specificity level. The clinical informatics standards development was stated as an example to demonstrate the application method of this three-dimensional profiling framework.
9. Effects of helicid on hippocampal neurogenesis in chronically stressed mice

OBJECTIVE: To explore the effects of helicid on the hippocampal neurogenesis in chronically stressed mice and its possible action mechanism of antidepressants. METHODS: Mice were exposed to chronic stress and isolated breed for 28 consecutive days with coadministration of helicid (10, 20, 40 mg · kg-1, ig) or fluoxetine(2.6 mg · kg-1, ig). The proliferation of hippocampal progenitor cells and level of brain derived neurotrophic factor (BDNF) were measured by immunohistochemistry and Western blot analysis. RESULTS: Immunohistochemistry showed decreased expression of BrdU and BDNF in hippocampal dentate gyrus in chronic stress mice. Compared with chromic stress group, helicid and fluoxetine significantly increased expression of BrdU and BDNF. CONCLUSION: The antidepressant mechanisms of Helicid in chronically stressed mice is probalblely related to promote neurogenesis in dentate gyrus and induce up-regulation of BDNF. Various central nerve system lesions involved cerebral hemisphere and nerve fiber tract such as cerebral infarction and hemorrhage can cause Wallerian degeneration,which is similar to the pathological change of peripheral nerve cut.There is a clear correlation between the degree of recovery and the degree of Wallerian degeneration.This article reviews the advances in feature of poststroke Wallerian degeneration and the correlation between post-stroke Wallerian degeneration and rehabilitation outcome.
