1.Videourodynamic Studies of Patients with Micturitional Disturbance after Cerebrovascular Accident

Objective To study the effect of cerebrovascular accident on lower urinary tract.Methods A retrospective review of non-complete synchronized videourodynamic recordings of 19 patients with cerebrovascular accident and 19 cases with thoracic spinal cord injury in synchronization was performed.Results The patients with cerebrovascular accident were found mainly detrusor hyperreflexia,low bladder compliance,sphincter relaxation,and the detrusor hyperreflexia was noted primarily in lesions of the basal ganglia lobe as well as the frontal.These characteristics were different from the cases with thoracic spinal cord injury.Conclusion The videourodynamic findings are mainly detrusor hyperreflexia and sphincter relaxation in patients with micturitional disturbance after cerebrovascular accident.It is different from findings in patients with thoracic spinal cord injury.
2.Endovascular treatment for TASC Ⅱ C/D femoropopliteal arterial disease

ObjectiveTo evaluate endovascular stenting for TASC TASC Ⅱ C/D femoropopliteal arterial disease.MethodsBetween January 2008 and June 2011,Endovascular treatments of 51 limbs TASC Ⅱ C/D femoropopliteal artery occlusions in 46 patients(27 male and 19 female) were retrospectively reviewed.Mean age was (70 ± 8 ) years ( range,52 years to 88 years).Severe intermittent claudication,rest pain,minor tissue defect and foot ulceration or gangrene were in 29 limbs,12 limbs,6 limbs and 4 limbs,respectively. Technical success rate,ankle brachial index (ABI),complication rate, and cumulative primary patency rate were evaluated. ResultsTechnical success rate was 90.2%.93 stents were placed in 46 limbs of the 42 patients.Postoperative ABI was increased in all patients.42 patients(46 lower limbs) were followed up for a mean period of 14.6 months.Primary,assisted-primary,and cumulative patency rates at 6 months were 81.0%,88.1% and 90.5%.Primary,assisted-primary,and cumulative patency rates at 12 months were 66.7%,71.8% and 79.5% respectively. ConclusionsEndovascular treatment of TASC Ⅱ C/D femoropopliteal arterial lesions is microinvasive,safe and effective.
3.The correlation between postprandial blood glucose level and the coronary artery disease

Objective To investigate the correlation between the level of postprandial blood glucose (PBG)and the incidence of coronary artery disease(CAD). Methods Eighty-five patients performed coronary angiography with abnormal blood glucose levels were divided into 3 groups according the blood glucose levels: impaired fasting glucose(IFG)group(28 cases), impaired glucose tolerance(IGT)group(29cases)and diabetes mellitus(DM)group(28 cases). The detection rate and the extent of CAD were observed. The risk factors of CAD in 3 groups such as C-reactive protein(CRP),carotid artery intima-media thickness(CIMT), blood lipids, blood pressure, body mass index(BMI)and the correlation with CAD were analyzed. Results Sixty-two cases were confirmed CAD by coronary angiography. The incidence rate of CAD in IGT group[79.3%(23/29)]and DM group[85.7%(24/28)]was significantly higher than that in IFG group[53.6%(15/28)](P < 0.01). The incidence rate of CAD in DM group was higher than that in IGT group, but there was no significant difference(P > 0.05). The incidence rate of two-lesion and three-lesion in IGT group and DM group were significantly higher than those in IFG group(P< 0.01). The incidence rate of two-lesion and three-lesion in DM group were higher than those in IGT group, but there was no significant difference(P>0.05). The levels of CRP, CIMT, BMI, triacylglycerol(TG)and systolic blood pressure(SBP)were higher and HDL-C was lower in IGT group and DM group than those in IFG group(P < 0.01 or < 0.05).The levels of CRP,TG and SBP were higher in DM group than those in IGT group(P<0.05). Correlation analysis showed, in IGT group and DM group,2 h PG had significantly positive correlation with CRP, CIMT,B MI, TG(P<0.05 or<0.01), and had significantly negative correlation with HDL-C(P<0.05 or <0.01).Conclusions PBG is closely related with the development of CAD.IGT patients should be intervened as early as possible, which can be effective in preventing cardiovascular events.
4.Prognostic effect of number of lymph nodes sampled in patients with stage Ⅰ non-small cell lung cancer

Objective To investigate the prognostic effect of quantity of lymph node(LN)resected in operations of patients with stage Ⅰ non-small cell lung cancer(NSCLC).Methods The clinical, pathological and follow-up data of 74 patients with stage Ⅰ NSCLC who were treated with surgery from January 1998 to December 2002 Beijing Friendship Hospital, Affiliated to Capital Medical University were reviewed retrospectively.Grouping the patients, according to the quantity of lymph node resected, the Kaplan-Meier method and Cox proportional hazards model was used for univariate analysis and multivariate analysis of factors with prognostic effect.Results The five year survival rate and disease-free survival(DFS)rate of these 74 patients were 64.9% and 47.3%.The univariate analysis showed that tumor size(P =0.016),T-staging (P =0.008)and extent of lymph node dissection(P =0.013)could influence the survival rate.The 5-year OS and DFS rates of patients with less than 6 LNs resected were less than the other group(more than 6 LNs)apparently.The multifactorial analysis indicated that other than staging, the quantity of lymph node resected was also an influence factor of prognosis.Conclusions The OS rate of patients has positive correlation with quantity of lymph node resected in operations.Six LNs must be resected leastways in operations of patients with stage Ⅰ NSCLC.
5.Application of immunophenotypes in pathologic diagnosis of follicular lymphomas

Objective To investigate the value of immunophenotypes in pathologic diagnosis of follicular lymphoma (FL) and the differential diagnosis between neoplastic follicle (NF) and reactive hyperplastic follicle (RHF).Methods 50 cases of FL and 10 cases of RHF as control were studied by clinical data, the expressions of CD20, bcl-2, CD3, CD10, bcl-6, CD21 and Ki-67 were detected by EnVision immunohistochemical method for the immunphenotypical pattern of FL and RHF.Results Among 50 FL cases , there were 24 male cases and 26 female cases, with median age of 50 years old (10-80 years old), including 32 cases (64 %) involved predominantly neck lymph nodes.The histologic grades were 1-2 in 16 cases (32 %) and ≥3 in 34 cases (68 %).CD20, CD10 and bcl-6 were positive in NF tissues of FL, with irregular forms of NF and loss of the mantle area without clear demarcation, and infiltrating to NF, irregular shape and no clear perimeter.The bcl-2 was positive in all of grade 1-2 NF, and it was positive in 68 % (23/34) cases and was partial or total negative in 32 % (11/34) cases in grade≥3 NF.Those were poorly demarcated contours.The Ki-67 proliferative index of NF were <30 % in grade 1-2, and >30 % in grade ≥ 3, with the highest to 90 %.In FL, the CD21 staining result showed follicular dendritic cells (FDC) network were synchronized with NF, and in some cases of grade ≥ 3, the FDC network were ruptured.In RHF, the CD20 was positive for round or oval nodules with clear demarcation.The CD10, bcl-6 and CD21 were positive and bcl-2 was negative in all germinal center without invasive immunophenotype.The Ki-67 index was high in germinal center, and sometimes polarity may be seen.Conclusion The immunophenotypical differences between FL and RHF include invasive characteristics, loss of follicular mantle area and germinal center pattern in the NF, while the normal follicular immunophenotype in the RHF.
6.Effect of residual renal function on prognosis of maintenance dialysis in patients with end-stage renal disease

Objective To discuss the effect of residual renal function on prognosis of maintenance dialysis in patients with end-stage renal disease ( ESRD) .Methods 90 cases of patients of maintenance hemodialysis were selected and divided into group A and group B based on different eGFR levels at the beginning of dialysis, 51 cases in group A of eGFR≤5 mL/( min· 1.73 m2 ) , while 39 cases in group B of eGFR>5 mL/( min· 1.73 m2 ).The renal function of two groups were observed,all patients underwent a five-year clinical follow-up and mortality rates, hemoglobin (Hb), serum albumin ( Alb) , C reactive protein ( CRP) of patients in both groups were observed.Results The renal function of urea nitrogen, serum creatinine, blood uric acid, urinary protein and urine creatinine in group B before dialysis was better than those in group A (all P<0.05).The Hb and Alb levels of group A were significantly lower than those of group B, while the CRP levels was significantly higher than that of group B (all P<0.05).The mortality rate of group A was 43.14%,compared with 20.51% of group B,and there was no significant difference.The two main reasons of death was cardiovascular and pulmonary infection, accounted for 50.00% and 33.33%, respectively.Conclusion Residual renal function of patients with ESRD before dialysis has important clinical significance for prognosis judgment.When eGFR level is lower before dialysis, the mortality rate of patients is higher in five years, prone to cardiovascular and cerebrovascular, with poor nutritional status, which is more vulnerable to infection.
7.Clinical characteristics and risk factors of young patients with coronary heart disease complicated metabolic syndrome

Objective:To study clinical characteristics and risk factors of young patients with coronary heart disease (CHD) complicated metabolic syndrome (MS).Methods: A total of 200 CHD young patients were selected from our hospital, including 100 patients with MS (CHD+MS group) and 100 patients without MS (CHD control group).Lesion severity was evaluated according to coronary lesion score.General data were compared between two groups,and risk factors were assessed CHD+MS group.Results: Compared with CHD control group, there were significant rise in body mass index (BMI), levels of total cholesterol (TC), 2h postprandial blood glucose (2hPG), fasting blood glucose (FBG), triglyceride (TG), blood pressure, serum uric acid (UA) and creatinine (Cr) and homeostasis model-insulin resistance index (HOMA-IR), and significant reduction in level of high density lipoprotein cholesterol (HDL-C) in CHD+MS group, P<0.01 all.Compared with CHD control group, there were significant rise in incidence rate of multi-vessel coronary disease (6% vs.32%) and Gensini score of coronary angiography [(5.1±2.0) scores vs.(8.4±5.3) scores] in CHD+MS group, P<0.01 both.Logistic regression analysis indicated that low HDL-C, HOMA-IR, hypertension and diabetes mellitus history were risk factors for CHD complicated MS in young patients(OR=1.097~2.246,P<0.01 all).Conclusion:There are much risk factors and abnormal metabolism,and coronary disease are more serious in young patients with CHD complicated MS.
8.Construction of phage random eight-peptide library

AIM: To construct random eight-peptide library for the study on atherosclerosis and restenosis. METHODS and RESULTS: Random oligodeoxynucleotides encoded eight peptides were synthesized and amplified by polymerase chain reaction(PCR). The product was cloned into phage surface display vector fUSE5 in Sfi I site and electroporated into competent MC1061. The library was identified through PCR, hybridization, DNA sequencing and affinity biopanning of streptavidin. Because the upstream primer is complementary to part vector clone site sequences and part exogenous gene sequences, and the other one complementary to pIII gene of vector, thus only clones inserted exogenous gene could be amplified easily. Additionally we used the probe oligodeoxynucleotide complementary to vector clone site sequences to identify clones which were not inserted exogeneous genes. Furthermore, two hybridizing positive clones were sequenced. Their sequences are consistent with two oligodeoxynucleotide probe sequences. As a result, 2.1?108 special clones were obtained. Affinity biopanning proved that the libraries could be amplified steadily. CONCLUSION: The eight-peptide library is reliable.
9.Extraction-purification of pueraia isoflavone and soyisoflavones from Radix Puerariae lobatae by inner ebullition

AIM:To extract pueraia isoflavone and soyisoflavones with n-butanol by means of inner ebullition from Radix puerariae lobatae.METHODS:A little ethanol was first used to saturate the powder of Radix puerariae lobatae to make the pueraia isoflavone desorbed adequately,and then hot n-Butanol solution was added to make the ethanol inside Radix puerariae lobatae begin to boil and the extraction was intensified.Based on different solubility of pueraia isoflavone and soyisoflavones in n-Butanol,when solvent vaporized,the soyisoflavones were separated and then the pueraia isoflavone by step vaporizeing on centrating crystal.RESULTS:80 mL of 60% ethanol,saturating time 20 min,600 mL n-Butanol including 20% water,at the temperature of 90 ℃,the yield of total isoflavone was 11.3 %.When 90% solvent vaporized,the ratio of quality of the soyisoflavones and pueraia isoflavone was 1∶3.The puerarin and daidzin's purity reached 46.0% and 6.12% by the HPLC respectively.CONCLUSION:The technology of inner ebullition extraction and solvent vaporizing crystal separation are simple,which provide industry production basis for pueraia isoflavone and soyisoflavones.
10. Research progress on role of exosomes in acute kidney injury

Exosomes are extracellular vesicles with plasma membrane. They are produced intracellularly and then secreted out of the cells. Almost all types of cells in human bodies can produce and excrete exosomes. Exosomes carry a variety of signal proteins, mRNA, miRNA, lncRNA and their degradation fragments. Due to the protection of lipid membrane, these components have biological activity and get involved in the process of cell migration, apoptosis and proliferation, and can modulate the function of the recipient cells. As an important cell-cell signal carrier, exosomes are involved in the development of many kidney diseases. This article reviewed the biological characteristics of exosomes and their roles in acute kidney injury.
