2.Experience on Laparoscopic Total Extraperitoneal Repair of Inguinal Hernia

Objective To discuss the value of laparoscopic total extraperitoneal repair(TEP) for inguinal hernia.MethodsBetween 2005 and 2007,82 consecutive patients with inguinal hernia at 97 sides underwent TEP by laparoscopy in our hospital.Among the cases [76 men and 6 women with a mean age of 52 years(21 to 88)],9 had unilateral direct inguinal hernia,50 showed unilateral indirect inguinal hernia,and 9 suffered from bilateral indirect inguinal hernia,6 cases were found having bilateral indirect inguinal hernia complicated with direct hernia,and 8 were recurrent cases of indirect inguinal hernia.Results In the patients,5 showed massive postoperative abdominal adhesion;one of them was converted to open surgery because of severe injury to the peritoneum,the other 4 were treated by continuous suture using 5-0 absorbable thread.The operation time for laparoscopic surgery ranged from 30 to 180 minutes(58 minutes for unilateral operation,and 97 minutes for bilateral operation on average).No patient needed analgesics after the procedure.The mean hospital stay for this series was 7 days(range: 4 to 12).Nine patients developed seromas of the scrotum,and was then cured by local drainage and physical therapy.The 82 patients were followed up for 13 to 38 months with a mean of 26 months,during which no one had recurrence.Conclusion As a safe and minimally invasive procedure,which leads to less trauma and pain,as well as quick recovery,TEP is especially suitable for patients with recurrent or bilateral hernias.
3.Nursing care of patients with cervical fracture-dislocation and spinal cord injuries during the perioperative period

[Objective]To investigate the methord of nursing care of patients with cervical fracture-dislocation and spinal cord injuries during the perioperative period.[Method]Fifty-six patients with cervical fracture-dislocation and spinal cord injuries experienced preoperative training,psychology nursing and posture exercise,and postoperative position nursing and founctional exercising.The improvement of spinal cord founction was observed.[Result]All patients were followed up from 6 to 31 months,with an average of 17.6 months.According to JOA scores,the preoperative scores was from 0 to 13,with an average of 7.76 points.The postoperative score was 6-17 with an average of 10.3.The relief rate was 68.5%.Satisfaction rate was 74.1%.[Conclusion]The perioperative nursing is important for preventing and lessening complications,achieving good operative effect and promoting rehabilitation.
4.Mechanical analyses on urethra wall after suburethral slings for female pig stress urinary incontinence

Objective To analyze the mechanical mechanism of suburethral slings by measuring the mechanical properties of female pig urethra wall and polypropylene mesh.Methods The uniaxial tension tests of 49 female pig urethra wall strips and 27 polypropylene mesh samples were performed to test the mechanical properties.Results The stress and strain of urethra wall displayed a parabola shape and its elastic modulus was proportional to the square root of stress as well as the strain.The maximum calculated elastic moduli of the urethra wall were 0.806 MPa and 0.818 MPa correspondingly calculated with stress and strain.The elastic modulus of polypropylene mesh was(6.977?1.095) MPa.The ultimate stress of polypropylene mesh was significantly higher than that of the urethra wall,and the ultimate strain of polypropylene mesh was significantly lesser than that of the urethra wall.The calculation results suggested that the suburethral slings treatment of poly-propylene mesh decreased the deformation to 1/3 of the untreated stress urinary incontinence.Conclusion Polypropylene mesh has special mechanical properties that suit suburethral slings and enhance the support to the urethra,and improve the continence.
6.Clinical characteristics and genetic analysis of two cases with Leigh syndrome with acute pulmonary hemorrhage as predominant manifestation.

OBJECTIVETo analyze clinical and imaging features and genetic characteristics of Leigh syndrome with emergent pulmonary edema.
METHODThe clinical features and imaging data of 2 cases (1 male, 1 female) seen in Anhui Provincial Children's Hospital from 2012 to 2014 were analyzed and summarized. Venous blood samples were sent to Guangzhou Jinyu Medical Examination Center for genetic analysis. Peripheral blood DNA was extracted and amplified, then sent to a sequencing facility for presence of genetic mutation by comparing with the reference sequence (NC_012920.1).
RESULT(1) The first patient was a 7 months old boy. The second patient was a 7 months and 21 days old girl. They were presented with abnormal respiration and pulmonary hemorrhage required mechanical ventilation. The first patient had a similar attack after 4 months of his birth, whose psychomotor development was normal, and no abnormal neurological findings. The value of blood lactate was 1.58 mmol/L. The value of pyruvic acid was 0.25 mmol/L. The value of cerebrospinal fluid lactate was 6. 4 mmol/L, which was an abnormal increase. The second patient had abnormal nervous system development, which included motor development retardation and hypotonia. The value of blood lactate was 6. 8 mmol/L, pyruvic acid was 0.31 mmol/L. Cerebrospinal fluid lactate was 8.2 mmol/L. (2) Imaging data: chest X-ray revealed double lung effusion. Bilateral caudate nucleus and lentiform nucleus had high signal, and bilateral internal capsule forelimbs were affected in DWI sequence of head MRI. Hemispheres, basal ganglia, cerebral peduncle, cerebellum, pons, and splenium of corpus callosum had multiple abnormal signals in head MRI of the second patient. NAA peak showed significantly reduced lesion area in magnetic resonance blood-flow scanning, and Cho peak increased significantly, which were double lactate-peak. (3) Genetic testing: ATPase6 m.9185 t > C mutation was found in case 1 that was consistent with Leigh syndrome pathogenesis. Hybrid mutations (m. 10191 t > C) in mitochondrial DNA was found in case 2. Two cases with the diagnosis of Leigh syndrome was clear. They were given combined therapy, such as mechanical ventilation, limited fluid to alleviate lung exudation, coenzyme Q10, and L-carnitine. The illness of case 1 relapsed after discharge. But in case 2, there was no improvement. They both died after treatment was given up.
CONCLUSIONNeurological symptoms were common in Leigh syndrome, in which acute lung hemorrhage was rarely reported. Timely ventilator support can temporarily save lives, but fatality rate is high and prognosis is poor.

7.The effects of different training regimens on motor function recovery and nerve-muscle morphology after spinal cord injury in rats

Objective To investigate the effects of three different motor training regimens on motor function improvement after spinal cord injury (SCI) in rats.Methods Forty-five healthy,adult Sprague-Dawley rats (female),weight 260-300 g,were included.Six rats were selected as the normal group.A model of incomplete SCI at the T9 level was induced in the others using a modification of Allen's method.Nine rats died after the injury and were excluded.The other 30 rats were randomly divided into 5 groups:a 7 d control group,a 35 d control group,a bodyweight-supported-treadmill-training (BWSTT) group,a swimming training group and a wheel running group,with 6 rats in each group.The three training groups began exercising at the 8th day post surgery,30 min per day,5 days a week for 4 weeks.Locomotor function was evaluated by inclined plane tests,modified Tarlov scores,and the BassoBeattie-Bresnahan (BBB) scale before the operation and on the 1st,7th,14th,21st,28th,and 35th day post surgery.Histomorphological changes of the T9 level spinal cord and the gastrocnemius muscle were observed with light microscopy and electron microscopy,and the cross sectional areas and diameters of the gastrocnemius muscle fibers were calculated.Results ①In the BWSTT group and the swimming training group,locomotor function scores increased significantly at all time points compared with the two control groups.There was no significant difference in rehabilitative effect between the BWSTT group and the swimming training group.But compared with both control groups,improvement in the wheel running group was not significant.②After 4 weeks of training,histomorphological observation of the injured T9 spinal cord tissue and gastrocnemius muscle fibers showed that the improvement in the BWSTT group was the most significant.The edema of the injured T9 level spinal cord tissue had decreased,cell vascular degeneration had lessened,the morphology of the neurons and glial cells trended to perfect recovery and nerve fibers had proliferated significantly.③Comparing the cross sectional areas and diameters of gastrocnemius muscle fibers showed that the improved amyotrophy in the BWSTT group was the most significant.The average cross sectional area (55.34 + 14.46 μm2) and diameter (8.32 + 0.99 μm) were close to the normal group (55.49±13.84 μm2 and 8.37 +1.13 μm).The swimming training group also had great improvement (46.05±8.50 μm2 and 7.68 + 0.76 μm) compared with the 35 d control group,while improvement in the wheel running group was not significant.Conclusions All three motor training regimens can improve locomotor and neurologic function,but the effects of BWSTT and swimming are better than that of wheel running.

9.Studies on the Biotransformation of Poria cocos by Ganoderma lucidum

Objective: To study the biotransformation of Poria cocos by Ganoderma lucidum and the changes of total polysaccharide and total triterpenes contents of the biotransformation products. Method: The experiment uses the microwave method to extract and sulfuric acid-phenol method to measure total polysaccharide content. And the ultrasonic wave method was used to extract total triterpenes and the content was measured by vanillin-perchloric method. Result: After treatment by G. lucidum, the total polysaccharide and triterpenes content of the biotransformation products were significantly higher than the non-conversed P. cocos (P<0.01), showing that the original performance of P. cocos was changed by way of biotransformation of G. lucidum; P. cocos had a promotion effect on the growth of G. lucidum. Conclusion: Biotransformation of P. cocos for Increasing the total polysaccharide and triterpenes contents by using G. lucidum has advantages of simple, low cost and so on. This method has a certain application value in modernization of Chinese traditional medicine.
10.The effect of butorphanol on vascular reactivity in lipopolysaccharide-pretreatment isolated rabbit pulmonary arteries

Objective To explore the regulatory roles of butorphanol on isolated rabbit pulmonary arteries by lipopolysaccharide(LPS)pre-incubation and assay the protective effect of ketamine on clinic sep tic shock patients.Methods Vascular ring tension detection technique was applied to observe the effect of butorphanol on the normal or lipopolysaccharide incubated pulmonaryvascular.Results In normal group, the final concentration of butorphanol(0.1 μmol,1μmol,10μmol,100μmol)were used to make pretreat ment- phenylephrine(PE)pulmonary artery relaxation.In LPS group,the effect of vasodilation of butor phanol was enhanced.Conclusions LPS can enhance the effect of butorphanol on pulmonary artery relaxa tion reaction.And the direct effect of butorphanol on pulmonary arteries was vasodilation.
