1.Attitudes of Medical Students Toward Congenital Anomalies and Prenatal Diagnosis: Implications for Education in Medical Ethics

Attitudes of medical and co-medical students toward congenital anomalies and prenatal diagnosis were investigated. After problem-based learning in medical genetics and embryology, students were shown a short film of a fetus with spina bifida. The students were then asked by questionnaire to answer the following question: “If you were told at 20 weeks' gestation that your fetus had spina bifida, what would you do?” About one third of the female medical students and half of the male students said they would terminate the pregnancy. The students' written comments suggested that female students are more receptive to delivery and to handicapped children. In contrast, male students' attitudes were more passive. Responses of female public health nursing students were similar to those of female medical students, and those of student midwives were much more receptive. These results will provide a basis for education in medical genetics and medical ethics.
2.Physicians’ and Nurses’ Attitudes toward Infectious Diseases in Terminally-ill Cancer Patients: What Determines Their Variabilities?

Clinical decisions on infectious diseases in terminally-ill cancer patients are often made based on physicians’ and nurses’ personal medical experiences, which may lead to an intra-team conflict. We conducted a qualitative analysis with prospectively gathered, semi-structured interviews in order to identify physicians’ and nurses’ beliefs and perceptions contributing to the variabilities in their attitudes toward patient care and the conflicts during decision-making process. We obtained the following categories with respect to beliefs and perceptions: estimated prognosis; patient’s discomfort due to tests and treatment; agreement to physician’s treatment plan; patient’s and family’s wish for tests and treatment; and patient’s benefits by tests and treatment. The intra-team conflicts resulted from disagreement on patient’s condition, and difficulty understanding mutual intent or opinion among physicians and nurses. Our findings may help improve team-based communication and the quality of care in terminally-ill cancer patients with infectious diseases.
3.Stress- and Aging-Associated Modulation of Macrophage Functions

Effects of environmental (cold) stress and aging on cells in monocyte/macrophage lineage were investigated. We demonstrated that immune suppressive states seen in acute cold-stressed mice (8-10 weeks of age) is attributable to FcγRIIbright suppressor macrophages. Serum corticosterone levels were markedly increased in acute cold-stressed mice. In addition, expression of glucocorticoids (GC) receptor mRNA was observed in FcγRIIbright cells from these mice. The increase of FcγRIIbright cells in peritoneal exudate cells caused by acute cold stress was inhibited by adrenalectomy or administration of a saturating amount of the GC antagonist RU 38486 (mifepristone). On the contrary, administration of the GC agonist, dexamethasone, markedly increased the proportion of FcγRIIbright cells in peritoneal exudate cells of control mice. These results suggest that the generation of FcγRIIbright suppressor cells of monocyte/macrophage lineage by acute cold stress was mediated by action of GC through the GC receptor. We likewise found that the proportion of FcγRIIbright suppressor macrophages is increased in aged mice (22-24 months of age). Meanwhile, activated macrophages which function as antigen presenting cells were decreased in aged rats. Both the basal corticosterone concentrations in serum and the expression of mRNA for GC receptor in peritoneal macrophages increased significantly in aged animals, suggesting that these populational and functional changes of macrophages in aged animals were mediated, in part, by the increased basal levels of GC. This is probably being responsible for immunosenescence.

4.Reflection on the Past 30 Medical Education Seminars and Workshops Held by the Medical Education Development Center

The Medical Education Development Center has organized seminars and workshops for medical education 30 times from 2000 through 2008 throughout Japan. The participants have numbered more than 3200 and have included educators, physicians, students, and simulated patients. The topics of seminars and workshops have included problem-based learning tutorial systems, medical interview skills, objective structured clinical examinations, evidence-based medicine, coaching technology, simulations, and community medicine in response to educational needs. A questionnaire survey of 1793 participants of the 11th to 30th seminars and workshops (response rate, 85.6%) showed a high degree of satisfaction (4.33±0.13 of 5 points). Invited lecturers and directors from throughout the country often organized these workshops. The results of workshops are reported in our annual monograph, "Trends in Medical Education."

6.A Study of the Tutorial System at Gifu University School of Medicine. Part 2: Evaluation by Physicians in Community Hospitals.

To assess the effectiveness of a problem-based learning tutorial system introduced at Gifu University School of Medicine in 1995, we conducted a questionnaire survey of medical knowledge, attitudes about learning, communication ability, and social behavior in sixth-year medical students. The questionnaire was given to instructors and attending physicians at community hospitals who were involved in clinical education. Many of the evaluators felt that students who trained with the tutorial system showed improved understanding, a more active attitude toward learning, and a better attitude toward patients than did students who received traditional, lecture-based education.
7.A Preliminary Validation of Computer-Based Testing by the Common Achievement Tests Organization in Japan: An Early Report.

The Common Achievement Tests Organization performed the first nationwide trial of computer-based testing (CBT) used to assess students entering the clinical phase of medical education. Seventy-seven medical schools participated in the trial. We compared performance on the national CBT with performance on preclinical tests administered at Gifu University School of Medicine. Despite some methodologic differences between the national CBT and our system, the overall results correlated well. Students who did poorly on the national CBT also did poorly on Gifu University's preclinical test. Correlation of these two performance scales suggests that nationwide CBT could be used to accurately assess preclinical skills.
8.Experience with 10 Seminars and Workshops for Medical Education

We have organized the Seminar and Workshop for Medical Education (SWME) 10 times from 2000 through 2003. More than 1, 200 persons have participated, including teachers, physicians, students, and simulated patients. The themes of the SWME have included a problem-based learning tutorial system, medical interview skills, objective structured clinical examinations, medical ethics, advanced cardiac life support/basic life support, evidence-based medicine, coaching technology, medical English education, and crisis management education. Invited lecturers from throughout the country organized most of these workshops. Advantages of the SWME are two-fold:(1) improving the medical teaching skills of each participant and (2) scouting for good young lecturers. Workshop reports are published in our annual monographs and other materials. The present paper is a historical review of the SWME and also describes the nationwide scope of faculty development.
9.Practices, Recommendations, Knowledge, and Sources of Information for Primary Care Physicians regarding Vaccinations: A Nationwide Survey in Japan, 2019

Introduction: Vaccine policies have changed in recent years in Japan. In the present study, we assessed practices, recommendations, knowledge, and sources of information among primary care physicians (PCPs) regarding vaccinations in Japan. Methods: We conducted a nationwide cross-sectional study targeting PCPs in Japan. We used a web-based self-administered questionnaire targeting physicians that were members of the Japan Primary Care Association in 2019. We analyzed respondent administrations, recommendations, knowledge, and sources of information regarding vaccinations. Results: We received responses from 1,084 PCPs (20.1%) and invited 981 participants for the analysis. The rates at which physicians gave routine and voluntary vaccines in their own practices were 23.3-95.5% and 13.2-94.4%, respectively. The active recommendation rates for routine and voluntary vaccines were 41.6-92.0% and 13.6-75.5%, respectively. Furthermore, among routine vaccines, human papillomavirus vaccine was the least administered and recommended. PCPs working at clinics had the most accurate knowledge about vaccinations, and PCPs utilized academic organizations most readily as an information resource. Conclusion: We clarified practices, recommendations, knowledge, and sources of information regarding vaccinations among PCPs in Japan.
10.Awareness of the Patients with Senile Cataract. Experience in the Preoperative Orientation.

A total of 78 patients were operated on for senile cataract in our hospital in 1985. The number almost doubled to 142 in 1992, more than 90 percent of whom received the intraocular lens. We asked each patient postoperatively to submit a questionnaire, and found the greatest inconvenience suffered before surgery was the difficulty in reading the letters. Patients with senile cataract have anxiety due to extremely weak eyesight, therefore, we believe it is very important for them to receive orientation before the operation. Until recently, we used the B5-sized (10.12″×7.17″) leaflet for a guide to the operation, but the type was too small for them to read. We renewed the guide by using a F8-sized (17.95″× 14.96″) sketch book. We put them in the sickrooms Just before they patients left our hospital, we asked them for their opinion about the guide. 90 percent of the patients said that they were satisfied with the size of the type, all of them said that the contents are easy to understand, and 50 percent said they read the guide more than twice.
In conclusion, we found that the sketch book, full of illustrations, was quite convenient for senile patients and read repeatedly. It was also a great help to them, because it gave them encouragement and made it easy for them to prepare themselves for the operation.
