1.Effect of thalidomide combined with gefitinib on quality of life andsurvival in advanced non-small cell lung cancer

Objective To investigate the effect of thalidomide combined with targeted therapy of gefitinib on quality of life and survival in patients with advanced non-small cell lung cancer. Methods 90 cases with non-small cell lung cancer were selected in our hospital, according to the treatment were divided into observation group and control group, the observation group were treated by thalidomide combined with gefitinib treatment, patients in the control group received only gefitinib treatment, treatment effects was compared between the two groups, the remission rate, quality of life and survival were compared between two groups of patients. Results The effective rate of the observation group patients was 55.6%, compared with 33.3% in the control group, the difference was statistically significant (P<0.05), the life quality of the observation group of general health (general health, GH), physical functioning (PF), role –physical (RP), role-emotional (RE), mental health (MH), social functioning (SF), bodily pain (BP) and vitality (VT) were significantly better than the control group (P<0.05), there was no significant difference in 1, 2, 3, 4 years survival rate between two groups, while 5 years survival rate in observation group was higher than the control group(P<0.05). Conclusion Thalidomide combined with gefitinib targeted therapy is helpful to improve the quality of life and 5 years survival rate of non-small cell lung cancer.
2.Change of serum metalloproteinases and metalloproteinases inhibitors in exfoliation syndrome in Uyghur population

Background Exfoliation syndrome is a systemic disease with abnormal accumulation of extracellular matrix.Oxidative stress and imbalance between matrix metalloproteinases(MMPs) and tissue inhibitor of matrix metalloproteinases(TIMPs)may play an important role in the pathogenesis of exfoliation syndrome.Researches showed that the incidence of exfoliation syndrome is higher in Uyghur nationality than that in Han nationality.However,whether the imbalance of serum MMPs and TIMPs is associated with pathogenesis of different ethnic groups is unclear.Objective The aim of the study was to discuss the change of serum MMP-9 and TIMP-2 in Uyghur patients.Methods This study protocol was approved by Ethic Committee of Xinjiang Medical University and followed Declaration of Helsinki.A prospective cohort study was performed.Forty Uyghur nationality (46 eyes) with exfoliation syndrome were collected from March 2012 to May 2013 in Affiliated First Hospital of Xinjiang Medical University and First People Hospital of Kashi.Forty cases(40 eyes)age-and gender-matched normal volunteers were included in the same duration.The peripheral blood was collected under the informed consent.Serum MMP-9 and TIMP-2 levels were detected by ELISA,and the results between the two groups were compared using independent samples t test.Results Slit-lamp examination found that part of pigmentation was depigmented and white dandrufflike substance attached in the pupil margin in all the patients,and stripping white dandruff-like substance was deposited in the front surface of the lens capsule which distributed in 3 zones.The pupils were disk-shaped pupil,and the surrounding area was the ring granular and the middle was transparent area without ablative material after dilation.Serum MMP-9 levels were (57.88±18.63)μg/L in the exfoliation syndrome group and (9.35±2.78)μg/L in the normal control group;serum TIMP-2 levels were (17.36±4.66) μg/L in the exfoliation syndrome group and (25.73±3.59) μg/L in the control group.The ratios of MMP-9/TIMP-2 were 3.57± 1.45 in the exfoliation syndrome group and 0.37±0.11 in the control group,with statistically significant differences in serum MMP-9 and TIMP-2 levels as well as ratio of MMP-9/TIMP-2 between the two groups(t=11.52,-6.36,9.87,all at P=0.00).Conclusions The upregulation of serum MMP-9 and downregulation of serum TIMP-2 are found in Uyghur patients with exfoliation syndrome.It is verified that the imbalance of serum MMP-9 and TIMP-2 contributes to the pathogenesis of exfoliation syndrome in Uyghur patients.
3.Effects of pulmonary rehabilitation exercise on mood and subjective and objective sleep quality of elderly patients with chronic obstructive pulmonary disease

Objective:To investigate the effects of pulmonary rehabilitation on anxiety/depression and subjective/objective sleep quality of elderly patients with stable chronic obstructive pulmonary disease(COPD).Methods:From February 2018 to February 2019, 120 elderly patients with stable COPD were selected and randomly divided into the experimental group (pulmonary rehabilitation exercise combined with conventional COPD treatment) and the control group (simple COPD conventional treatment). Sixty cases in each group were intervened for 8 weeks. Before and after treatment, Hamilton anxiety scale (HAMA) was used to evaluate anxiety, Hamilton depression scale(HAMD)was used to evaluate depression, Pittsburgh sleep quality index(PSQI)and sleep log were used to evaluate subjective sleep quality, and objective sleep quality was monitored by multi-channel sleep monitor.SPSS 21.0 software was used to analyze and process the data. Chi square test, independent sample t test and paired t test were used for statistical analysis. Results:After 8 weeks of intervention, the HAMA and HAMD scores of the experimental group were lower than those of the control group (HAMA: (7.57±3.19) vs (10.15±4.89), t=-3.428, P=0.001; HAMD: (8.22±4.73) vs (10.60±6.49), t=-2.300, P=0.023). COPD patients with anxiety decreased (χ 2=7.566, P=0.006). After treatment, the subjective sleep latency of the experimental group was shorter than that of the control group ((42.00±9.88)min vs (47.25±10.27)min, t=-2.854, P=0.005). The subjective sleep efficiency was higher than that of the control group ((76.00±4.50)% vs (74.00±5.20)%, t=2.272, P=0.025), and the objective sleep latency was shorter than that of the control group ((28.02±5.59)min vs (32.95±6.21)min, t=-4.575, P<0.05). Conclusion:Pulmonary rehabilitation exercise can improve the anxiety and depression of elderly patients with stable COPD, and improve the subjective and objective sleep quality.
4.The Effectiveness of Self-efficacy and Education Intervention for Exercise Adherence of Patients with Chronic Stable Heart Failure

Objective To evaluate the effectiveness of exercise rehabilitation intervention by using self-efficacy theory on patients with chronic stable heart failure, and to provide evidences for rehabilitation nursing of patients. Methods Using the method of randomized controlled studies, 132 patients with chronic stable heart failure from Nanshitou Street and Shayuan Street communities in Haizhu, Guangzhou, Guangdong were randomly divided into two groups according to the community's name. The patients in the intervention group received self-efficacy and education intervention while the control group received health education only. The patients' exercise self-efficacy scores and six minute walk distances were separately measured by exercise self-efficacy scale and six minute walk test before and after intervention. Results The scores of exercise self-efficacy and six minute walk distances increased more significantly in the intervention group than in the control group after intervention (P<0.01). Conclusion Self-efficacy and education intervention can effectively improve the exercise self-efficacy and six minute walk distance of patients with heart failure.
5.Detection of TORCH infection in pregnant women by using reverse phase protein array

Objective To evaluate whether the reverse phase protein array (RPPA) method can be used for detecting TORCH infection in pregnant women .Methods The RPPA method was established for detecting TORCH infection .The positive coinci‐dence rates of TORCH infection detected by the RPPA method and ELISA method in 2000 fresh serum samples from pregnant women were compared for evaluating the feasibility of RPPA in TORCH detection .Results The positive coincidence rates of estab‐lished RPPA and ELISA for detecting TORCH infection was 100 .0% ,91 .1% ,97 .2% ,91 .3% and 93 .0% respectively ,indicating that the detection results of various indexes by RPPA and ELISA had better consistency (P>0 .05) ,but the positive detection rates of RPPA for Rubellavirus ,CMV and HSV‐1 ,2 were higher than those of correspondent ELISA method .Conclusion RPPA method for detecting TORCH infection has the advantages of simpleness ,rapidness ,high sensitivity and strong specificity ,is an effective method of auxiliary diagnosis for bearing and rearing better children in clinical ,and is worthy of being promoted and used in the fu‐ture .
6.Application of NRS 2002 and PG-SGA in Patients With Gastrointestinal Malignancies

Background:At present,domestic guidelines and consensus recommend the use of nutritional risk screening 2002(NRS 2002)and patient-generated subjective global assessment(PG-SGA)for nutritional risk screening and assessment of patients with gastrointestinal cancer during the perioperative period.However,PG-SGA has higher professional requirements,complex content and time-consuming.In the current busy situation of medical staff,NRS 2002 is more used for screening alone.Aims:To explore the consistency of NRS 2002 and PG-SGA in the assessment of nutritional status and clinical outcomes in patients with gastrointestinal malignancies,and to explore the accuracy of screening using NRS 2002 alone,so as to provide guidance for the establishment of clinical nutritional screening and assessment standards.Methods:A retrospective analysis was conducted on 157 patients with gastrointestinal malignancies who underwent radical operation in the Department of Gastrointestinal Surgery of Xiangya Hospital,Central South University from January 2020 to October 2022.Nutritional screening and evaluation were performed by NRS 2002 and PG-SGA scales and demographic data and nutrition-related laboratory indicators were collected to observe short-term postoperative clinical outcomes.Results:Patients with nutritional risk or malnutrition had lower body mass index(BMI),lymphocytes and prealbumin(P<0.05).The correlation and consistency of NRS 2002 and PG-SGA scales were good(r=0.728,κ=0.46)and the areas under the curve(AUC)for predicting postoperative complications were 0.691 and 0.702,respectively.In addition,nutritional risk and postoperative complications were significantly increased in patients with malnutrition(P<0.05).Conclusions:Therefore,gastrointestinal surgeons can only use NRS2002 to perform nutritional screening of patients and make corresponding nutritional treatment according to the screening results in the case of busy clinical work.

The latest epidemiological data suggests that the situation of adult diabetes in China is severe, and metabolic diseases have become significant chronic illnesses that have a serious impact on public health and social development. After more than six years of practice, the National Metabolic Management Center(MMC) has developed distinctive approaches to manage metabolic patients and has achieved a series of positive outcomes, continuously advancing the standardized diagnosis and treatment model.
