1.Analysis of semen quality of 1 865 outpatients in male infertile clinic in Foshan city

Objective To analyze the semen quality in the outpatients from 3 male infertile clinics of Foshan city.Methods The semen from 1 865 outpatients with male infertility was detected and analyzed routinely by the CASA technology in strict accordance with the WHO operation technical specifications.The outpatients were divided into the normal group,mild,moderate and severe as-thenospermia groups according to the sperm vitality.Results There were 524 (28.1%)cases of abnormal sperm concentration,826 (44.3%)cases of abnormal sperm viability,985 (52.8%)cases of abnormal sperm motility,713 cases (38.2%)of teratosperm and 652 (35.0%)cases of leukocytospermia in all outpatients;the motion parameters were decreased along with the decrease of activity in each group,in which,the 6 indicators of VSL,VAP,VCL,LIN,STR and WOB were significantly lower than those in the normal activity group with statistical differences(P <0.05),furthermore,the sperm vitality had staitstcally significant differences among the mild,moderate and severe asthenospermia groups (P <0.05).Conclusion In the routine parameter indicators,the sort order of the percentage of abnormal results in turn were sperm motility、sperm viability、sperm deformity rate、semen WBC、sperm concentra-tion、semen liquefaction time、semen pH.With reduced sperm motility,the sperm motion parameters of VSL,VCL,VAP,LIN,STR and WOB were decreased,the results of each group were significantly lowered (P <0.05).
2.Methodological comparison on two kinds of method for detecting serum human epididymis protein 4 and bias evaluation

Objective To compare the performance of 2 kinds of quantitative detection method the chemiluminescence immu-moassay and the enzyme-linked immunosorbent assay(ELISA)for detecting serum human epididymis geneproduct 4(HE4),and to explore the accuracy and practical value of the two methods.Methods In accordance to the document EP9-A2 of NCCLS,the Roche automatic electrochemical immunoassay system as the comparative method(X)and the ELISA as the testing method(Y )were per-formed according to the requirements of the detection kits.Results The serum HE4 level could be accurately reflected by the two methods.The correlation coefficient(r)in comparing these two kinds of method was ≥0.975,which showed that there was a high correlation between them.The detection results of the two methods were compared by paired sample t-test,and no statistically sig-nificant difference was found between them(P >0.05).Conclusion Because of CLIA′88 giving no specific medical decision level of HE4 and clinical acceptable level,therefore the expected bias of credibility interval can not be calculated or whether being clinically acceptable is unable to be judged.Based on the comparison of paired sample t test,these two kinds of detection methods can be used interchangeably to a certain extent.
3.The study of daily glucose change in insulin resistance with polycystic ovary syndrome women

Objective To explore the clinical metabolic characteristics and the blood glucose variability in the insulin resistance polycystic ovary syndrome (PCOS) patients.Methods Totally 69 cases of PCOS patients were divided into two groups:PCOS with insulin resistance (PCOS-IR) group and PCOS without insulin-resistance (PCOS-NIR) group.Their clinical and metabolic characteristics were analyzed,and continuous glucose monitoring system (CGMS) was used to monitor the glucose concentration of subcutaneous interstitial fluid to reflect the blood glucose level.Mean blood glucose level (MBG) and its standard deviation (SDBG),mean amplitude of glycemic excursion (MAGE),peak level of postprandial plasma glucose as well as its peaking time during a 48-hour period CGMS were calculated.Results MBG,SDBG,large amplitude of glycemic excursions (LAGE),MAGE level in PCOS-IR group,were significantly higher than those in PCOS-NIR (P < 0.05).Body mass index (BMI),triglycerides,glycated hemoglobin Alc (HbAlc),fasting blood glucose (FBG),2 hours glucose,free androgen index (FAI) level in PCOS-IR group were higher than in PCOS-NIR group (P < 0.01).The difference of total cholesterol,low-density lipoprotein,mean of daily difference (MODD) level were not statistically significant (P > 0.05).Conclusions PCOS-IR patients suffer from more serious metabolic disorder and blood glucose variability.
4.Mechanisms analysis of the reinforcing effect of Epimedii Folium on kidney in the treatment of polycystic ovarian syndrome with network pharmacology

Objective:This study by using the method of network pharmacology to screen the active constituent and related targets of Epimedii Folium aims to explore the mechanisms of the reinforcing effect of Epimedii Folium on kidney in the treatment of PCOS. Methods:By retrieving data from TCMSP datebase, screened out the active constituent of Epimedii Folium and the information of the targets corresponding to each active constituent; by using the gene database of NCBI, translated the information of the targets into gene names; by retrieving data from GeneCards datebase, extracted the genes related to PCOS; related targets of Epimedii Folium in the treatment of PCOS were obtained by Venn tool; by using Cytoscape 3.7.2 software, constructed a network diagram of Epimedii Folium-active constituents-targets-PCOS; by using STRING database, constructed the protein interaction network; then carried out GO enrichment analysis of related targets by Geneontology database and carried out pathway enrichment analysis of related targets by KEGG database. Results:There were 23 active constituents of Epimedii Folium and 132 related targets treating PCOS. The Epimedii Folium could play the reinforcing effect on kidney mainly by regulating the biological processes like steroid hormone receptor activity, as well as KEGG pathways such as Estrogen signaling pathway, GnRH signaling pathway, GnRH secretion, HIF-1 signaling pathway and VEGF signaling pathway in treating PCOS. Conclusion:From the perspective of network pharmacology, this study preliminarily analyzed the related targets and pathways of reinforcing effect on kidney of Epimedii folium in the treatment of PCOS, providing reference for further experiments and application inclinics.
5.A retrospective study of 140 forensic pathology cases caused by both injury and disease

It is a challenge to determine the cause of death in cases caused by both injury and disease in forensic pathology examinations.Here,we retrospectively analyzed 140 such cases enrolled in the Sun Yat-sen University Forensic Identification Center from 2013 to 2021.It was found that the cases caused by both injury and disease accounted for 2.89%of the total number of cases during the same period.The male-to-female ratio was 5.09.Cardiovascular disease accounted for 71.43%of the death-leading diseases,and coronary heart disease accounted for 49.29%.Among the three types of injury and disease relationship,the proportion of disease-based and injury-assisted cases accounted for more than half(65.86%),and the proportion of injury and disease with the same effect cases was the least(12.14%).Medical treatment was involved in as more as 82.86%of the cases.Re-identification were carried out in 10.71%of the cases,53.33%of which were with a≤24-hour-survival time,higher than the proportion(20.80%)of this survival time in initial identification cases(x2 = 13.84,P = 0.000 2).This study revealed the epidemiological characteristics of cases caused by both injury and disease,and supplied useful data for improving the level of forensic identification in such cases.
6.Sclerosing polycystic adenosis of the parotid gland: a case report and literature review

Objective :
To explore the clinicopathological features, diagnosis, treatment and prognosis of sclerosing polycystic adenosis (SPA) and provide a reference for clinics.
Methods :
A case of sclerosing polycystic adenosis of the parotid glands was retrospectively analyzed, and the relevant literature was reviewed.
Results :
A 57-year-old female patient presented with a tumor, which she had noticed for half a month, on the left side of the lower ear, with occasional paroxysmal numbness and no complaint of other discomfort. Resection of the left submandibular area tumor was performed, and the tumor specimen pathological results showed sclerosing polycystic adenosis of the left parotid gland, with no recurrence after six-months follow-up. Sclerosing polycystic adenosis is rare, occurs in the parotid gland and is characterized by a frequently painless, slow-growing mass of the parotid gland. Imaging examination and fine needle aspiration biopsy can only be used as a reference; the diagnosis must include a pathology examination. Histological manifestations showed that abundant sclerotic collagenous stroma was permeated by ductal and acinar lobules, and cystic dilatation of the duct was accompanied by epithelial hyperplasia and diverse ductal cells. Immunohistochemistry of the ductal and acinar cells showed positive expression of cytokeratin (AE1-3 and CAM5.2) and S100 protein. The ducts filled with hyperplastic and dysplastic epithelium were surrounded by an intact myoepithelial layer that was positive for SMA, p63, and calponin, with a Ki-67 index less than 3%. Treatment comprised mainly surgical resection, with a good prognosis. However, one-third of cases relapse: low-grade malignant tumors may occur, with at least one report of invasive cancer.
Conclusion
Sclerosing polycystic adenosis of the salivary gland is rare and has a good prognosis, but patients may relapse easily after surgery. The diagnosis depends primarily on pathological examination. The main treatment is surgical resection, the prognosis is good, and follow-up should be strengthened after surgery.
