1.Research progress of red distribution width in atrial fibrillation

Red blood cell distribution width is an index reflecting the size heterogeneity of circulating red blood cells, which is usually used for diagnosis and treatment of blood system diseases.In recent years, more and more evidences show that the increase of red blood cell distribution width level is closely related to the occurrence, development and prognosis of cardiovascular diseases.We focused on the relationship between red blood cell distribution width and atrial fibrillation (AF) and its possible mechanism, in order to provide some reference value for the clinical treatment of AF.
2.Application of DWI with high b value in staging of cervical cancer at 3.0T MRI

Objective To evaluate the value of 3.0T MRI diffusion-weighted imaging (DWI)in diagnosis and staging of cervical cancer. Methods A total of 65 cervical carcinoma patients were enrolled and performed T2 WI,DWI and LAVA-Flex dynamic contrast-enhancement before operation.MR images were analyzed by two radiologists to evaluate the staging performance.Results All the cervical cancers were detected in DWI,while three lesions were missed in T2 WI and one lesion was missed in LAVA-Flex dynamic contrast-enhanced MRI.Respectively,the accuracy of staging with DWI,T2 WI and LAVA-Flex dynamic contrast-enhanced MRI was 90.8%,78.5%and 87.7%.Accuracy of DWI was significantly higher than that of T2 WI(P =0.04),while there was no significant difference of accuracy between DWI and LAVA-Flex dynamic contrast-enhanced sequence (P =0.39).Conclusion DWI shows relatively higher accuracy than T2 WI in the staging of cervical cancer which makes it an ideal method to replace LAVA-Flex dynamic contrast-enhanced MRI for exact staging.
3.Detection of lymphocyte subsets in peripheral blood of patients with drug eruption and its significance

Objective To analyze the changes of lymphocyte subsets in peripheral blood of patients with drug eruption . Methods 18 newly diagnosed patients were served as the drug eruption group ,and were subdivided into cephalosporin group (n=9) ,penicillin group(n=5) and Chinese medicine group(n=4) according to different sensitizing drugs .20 healthy people were taken as the control group .Flow cytometry were utilized to detect the percentages and absolute counts of T lymphocytes (CD3+ ,CD3+CD4+ and CD3+CD8+ ) ,B lymphocytes ,natural killer cell(NK) and natural killer T lymphocytes(NKT) in their peripheral blood . Results Differences of percentages of T lymphocytes (CD3+ ,CD3+ CD4+ ) ,B lymphocytes ,NKT cells between the drug eruption group and the control group showed statistical significant (P<0 .05) .Difference of percentages of CD3+ CD8+ lymphocytes of pa-tients between the drug eruption group and the control group demonstrated no statistical significant (P>0 .05) ,while that of abso-lute counts of T and B lymphocytes of patients was statistical significant between the drug eruption group and the control group (P<0 .05) .Conclusion The percentages of CD3+ ,CD3+CD4+ lymphocytes of patients with drug eruption decrease ,while those of NKT cells increase ,which may be related to the patients′immune regulation .
4.Principle of multi-echelon medical care for the injured in Chinese Wenchuan earthquake

Objective To discuss the application value and improvement of principle of multi-echelon medical care in emergent rescue of the injured in Chinese Wenchuau earthquake. Methods The author analyzed and evaluated the medical rescue that was done at disaster site, in the front line hospital and higher level hospitals during earthquake. Results A total of 4 689 patients were treated at disaster site, including 413 patients with severe injury, of whom 3 died. Different kinds of operations including debridement were performed at disaster site, with infection incidence of open wound was nearly 80%. In the front hne hospital, 1 400 patients were treated, with 200 operations done. Of all, 110 patients with severe trauma were treated emergenfly, with an amputation rate of 3.0% and postoperative infection incidence of 66.8%. In the station hospitals, 125 patients received definite surgeries, with 1-5 surgeries per injury site. There was no postoperative cross infection, amputation or death. Conclusions The multi-echelon medical care is the basic mode for medical rescue of large number of patients in natural disaster rescue. First aid at disaster site should be performed as early as possible. Transportation is crucial for successful rescue and an improved patient grading system can help increase the efficiency of rescue. The front line hospitals should mainly provide life support, debridement and fixation of simple fracture, while the specific treatment and definite surgery should be carried out in the station hospitals.
5.Clinical and pathological characteristics of small renal mass analysis

Objective To explore the clinicopathological characteristics of small renal mass .Methods We reviewed the medical records of patients who had received partial nephrectomy ( PN) or radical nephrectomy ( RN) in 2011 in the tumor hospital of harbin medical university .We identified xxx patients who were treated with PN or RN for renal masses within a size of 4 cm.All small renal masses were localized ,sporadic,solid,and had reports of renal carcinoma measurement by CT or MRI .Results Of the 36 SRMs,there were 32(88.89%)clear cell renal cell carcinoma,3(8.33%)papillary renal cell carcinoma,1(2.78%)begin tumor.Of the 36 patients, 34 patients received radical nephrectomy ( RN) and only 2 male patients received partial nephrectomy ( PN) .The mean tumor size of 36 SRMs was 2.9 cm.Conclusion The majority of 36 SRMs are categorized to clear cell re-nal cell carcinoma by histological type .Most of the 36 patients receive radical nephrectomy ( RN) .
6.Early signs of cognitive impairment in patients with obstructive sleep apnea hypopnea syndrome: an event-related potential study.

This study seeks to explore the early signs of cognitive impairment in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). According to polysomnography, twenty patients diagnosed with OSAHS and twenty normal controls underwent event-related potential (ERP) examination including mismatch negativity (MMN) and P300. Compared with normal controls, OSAHS patients showed significantly prolonged latency of MMN and P300 at Cz. After controlling age and body mass index (BMI), MMN latency positively correlated with apnea hypopnea index (AHI), oxygen reduction index, stage N1 sleep and arousal index, while MMN latency negatively correlated with stage N3 sleep and mean blood oxygen saturation; and P300 latency positively related to AHI and oxygen reduction index; no relationships were found among MMN latency, MMN amplitude, P300 latency and P300 amplitude. These results suggest that the brain function of automatic processing and controlled processing aere impaired in OSAHS patients, and these dysfunction are correlated with nocturnal repeatedly hypoxemia and sleep structure disturbance.

7.Red cell distribution width correlates with prognosis and severity of critical illness among medical patients

Objective To investigate the prognostic value of red cell distribution width (RDW) and relationship between RDW and severity of critical illness patients and prognostic role of RDW.Methods This was a prospective observational and diagnostic test study.From February 2011 to May 2013,196 medical patients in ICU were enrolled to study.Blood samples were taken within 24 hrs after admission to ICU for blood routine test and other chemical routine test,including RDW.Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score were also calculated.The patients were divided into survival group (n =126) and death group (n =70) based on their hospital outcomes.Differences in RDW levels and APACHE Ⅱ score between survival and death groups were compared with One-way ANOVA.The correlation between RDW and APACHE Ⅱ score were analyzed by Spearman correlation test.Receiver operator curve (ROC) test was used to determine the predictive value of RDW and APACHE Ⅱ for hospital death.Results Both RDW level and APACHE Ⅱ score were significantly higher in death group than those in survival group,whichwere (13.35 ±2.21) vs.(11.34 ±1.32) and (20.43 ±6.41) vs.(13.94 ±6.77) respectively (both P < 0.01).RDW significantly correlated with APACHE Ⅱ score (r =0.309,P <0.01).To predict hospital death area under curve (AUC) of ROC curve by RDW and APACHE Ⅱ score were 0.792 (0.725,0.860) and 0.754 (0.686,0.822) respectively.The best cut-off value was determined by Yoden Index were 12.35 for RDW and 17.5 for APACHE Ⅱ score.The Accuracy of prediction was 76.5% and 70.4% respectively.In addition,all the 12 paticnts whose RDW level > 15.4 were dead.Conclusion RDW level at admission to ICU correlate well with the severity of critical illness.RDW level has an additional predictive value for the prognosis of critical ill patients.
8.Effects of FXR gene deletion on neurobehaviors and neurotransmission systems in female mice

Aim To investigate how Farnesoid X re-ceptor (FXR),a nuclear hormone receptor,acts on neurological behaviors such as emotion,social behav-ior,memory and so on.Methods FXR’s function in central nervous system was evaluated by conducting a battery of behavioral tests including elevated plus maze test (EPMT),forced-swimming test (FST),social in-teraction test (SIT ), and passive avoidance test (PAT),and the contents of neurotransmitters were de-termined by the LC-MS /MS method in FXR knockout (KO)female mice and their wild-type controls.Re-sults FXR KO mice showed significantly increased immobility time in FST (P <0.01 ),and it showed in-creased tendency to enter the open arms in EMPT (P<0.01 ).The number of probing the open arms by FXR KO mice was more than that of the controls.Mo-reover,in SIT,FXR KO mice had remarkably in-creased sniffing interactions with the stranger mouse in the same cage (P <0.01 ).But in PAT,the latency for FXR KO mice to enter the dark chamber on the test day and the number of FXR KO mice to enter the dark chamber didn’t differ from those wild-type mice.In hippocampus,the contents of GABA,Glu,and NE were decreased prominently in FXR KO mice (P <0.05,P <0.05 and P <0.01 ,respectively)as well as the ratio of GABA to Glu (P <0.05).But in pre-frontal cortex,none of the neurotransmitters examined showed any difference between FXR KO mice and their controls.Conclusion FXR may be involved in main-tenance of the homeostasis of neural transmission in the central nervous system,thereby influences the emotion-al and social behavior in animals.
9.Influence of olfactory bulbectomy on mouse neurobehaviors,hippocampal neurotransmission and depression relevant gene expressions

Aim To investigate the changes of behav-iors,neurotransmitters and depression related gene ex-pressions in hippocampus of C57BL/6 mice after olfac-tory bulbectomy (OB).Methods Forced swimming test (FST),tail suspension test (TST),open filed test (OFT),and elevated plus maze test (EPMT) were used to evaluate the behavioral changes 18 days after surgery. LC-MS/MS method was employed to measure the hippocampal neurotransmitters.Quantita-tive PCR approach was established to determine the hippocampal gene expressions associated with depres-sion.Results OB mice were hyperactive in OFT (P<0.05 ) accompanied with increased time spent in open arm of EPMT (P<0.01).Meanwhile,the sur-gery led to the reduction of the ratio of 5-HIAA/5-HT (P <0.01 ) but the increase of Glu/GABA (P <0.01 ) in hippocampus.Moreover,OB elevated the gene expressions of BDNF,Trkb,GDNF,CD1 1 b and TNF-αbut down-regulated that of TPH2 in hippocam-pus (P<0.05 ).Conclusion Behavioral alternation of OB mice was a result of comprehensive effect of the changes of neurotransmission and depression related genes,which call us special attention in using OB as an animal model for research and development of anti-depressants.
10.Relations of job satisfaction to emotional intelligence and emotional labor in bus drivers

Objective:To explore the relationship among emotional intelligence,emotional labor and job satisfaction.Methods:Totally 257 bus drivers [158 males and 99 females,aged 17 to 50 years,mean age (32 ± 7)years] in Jinan were surveyed with the Emotional Intelligence Scale (EIS),the Emotional Labor Scale (ELS) and the Minnesota Satisfaction Questionnaire (MSQ).Bootstrap was used to test the mediating effect.Results:The scores of EIS and ELS were both positively correlated with the MSQ scores (r =0.28,0.23,Ps <0.01) in the bus drivers.The surface acting (SA) scores were negatively correlated with the MSQ scores (r =-0.18,P < 0.01),and the deep acting (DA) score were positively correlated with the MSQ scores (r =0.40,P < 0.01).Mediating effect test showed that the indirect effect of surface acting was 0.07 (95% CI:0.01-0.16),and the indirect effect of deep acting was 0.10 (95% CI:0.03-0.21).Correspondingly,the direct effect of emotional intelligence on job satisfaction respectively was 0.35 (95% CI:0.20-0.58) and 0.33 (95% CI:0.12-0.53).Conclusion:These findings support that the bus drivers with higher emotional intelligence may be more satisfied with their job.Emotional intelligence is found to indirectly influence job satisfaction through surface acting and deep acting,and it may affect the bus drivers more effectively choose and use emotion regulation strategies.
