This study presents a fitting method for the intensity-response curve of electroretinogram. The data of b-wave amplitudes from 10 eyes with idiopathic macular hole and their contra-lateral contrast eye (normal) were analyzed. It was observed that the Rmax de creased significantly (P=0.013) in the eyes with idiopathic macular hole.

Infection is an essential factor in chronic wound disunion. With the increasing number of patients with chronic diseases in the aging society, the number of patients with chronic wounds increases. It caused a heavy social burden. The proper cleaning method and cleaning fluid play a key role in chronic wound healing. In this paper, the concepts, classification, mechanism of different cleaning fluids, and their effects on chronic wounds were reviewed, which is expected to help health professionals to choose cleaning methods and cleaning fluids scientifically. It was supposed to provide significant guidance for chronic wound cleaning.

This article reviews the overview, measuring tool, status quo, influencing factors, and interventions of health promotion lifestyle for patients with chronic diseases, and puts forward the corresponding prospects.

BACKGROUND: Optical coherence tomography(OCT) is a new type of noncontactable, noninjurious retinal tomographic technique. Minor serous detachment in retinal neuroepithelial layer can be resoluted and shown by it. The range of inferior fluid of retinal neuroepithelail layer or pigment epithelial layer can also be quantitatively measured to evaluate the recovery of vision.OBJECTIVE: To study the applicative value of OPT in evaluating the relation of vision and neuroepithelial layer in idiopathic central serous chorioretinopathy(ICSC) and its prognosis.DESIGN: Single sample study.SETTING: Ophthalmologic center in a university hospital.PARTICIPANTS: Patients with ICSC who were primarily diagnosed from January 1999 to June 2003 in the Ophthalmologic Center of the Medical College of Jinan University were included. There were 32 males and 8 females, with an average of(36.2 ± 5.6) years old.INTERVENTIONS: Zeiss-Humphrey OCT imaging meter was used for the examination. The maximal range and height of detachment in the serous detachment region of ICSC were measured respectively, and average value and standard deviation were then calculated.MAIN OUTCOME MEASURES: Vision and the maximal range and height of serous detachment.RESULTS: Local detachment of retinal neuroepithelial layer in the center of macula retina was shown in the OCT images of the 40 eases(41 eyes) with ICSC. Detachment ranged from 705 to 5 720 μm[an average of (3 051 ± 1 338) μm], with the height from 55 to 491 μm[an average of (270 ± 114) μm] . Four eyes were complicated with serous detachment of pigment epithelial layer. And it was suggested by statistical analysis that the range and height of detachment were related with vision.CONCLUSION: OCT, as an objective noninjurious retinal tomography, its high resolution has important value and special predominee in the diagnosis, quantitative analysis and the detection and follow-up of optical functional assessment during the course of the ICSC.

Objective To evaluate the therapeutic effects and complications of modified facial mask for non-invasive ventilation (NIV) in elderly patients with respiratory failure.Methods A total of 132 elderly patients(107 males and 25 female,aged 78.5±8.6 years) treated with NIV from February 2008 to May 2011 were randomized into two groups:modified facial mask(group A,n=68,56 males and 12 females,aged 78.8±22.2 years) and control facial mask(group B,n=64,64 males and 13 females,aged 76.6±20.4 years).Duration of NIV,time in RICU(respiratory intensive care unit),length of hospital stay,risk for hospital acquired pneumonia (HAP),risk for invasive ventilation,cure rates,in-hospital mortality,NIV failure rate and cost were compared between the two groups.The complications of NIV,such as oropharyngeal dryness,skin damage of face and nose,abdominal bloating,gas leakage from mask were also compared between the two groups.Results Compared with group B,duration of NIV(12.2±2.3 d vs.18.4±3.6d),time in RICU(7.3±3.2d vs.14.6t5.4d),length of in hospital stay(16.6±4.2d vs.28.2±6.2)d,and cost(2.23±0.12 ten thousand yuan vs.4.23± 0.24 ten thousand yua) in group A were significantly decreased(t=9.72,14.91,13.08,10.81 respectively,all P＜0.05).The risk for invasive ventilation [2.9％ (2 cases) vs.43.8％(28 cases)],NIV failure rate [5.9％ (4 cases) vs.12.5％ (28 cases)] were also decreased in group A compared with group B(x2 =31.26,25.74,both P＜0.05).Compared with group B,The complications of NIV such as skin damage of face and nose[4.4％ (3 cases) vs.37.5％ (24 cases)],abdominal bloating [2.9％ (2 cases) vs.28.1％ (18 cases)],gas leakage from mask [8.8 ％ (6 cases)vs.50％(32 cases)] in group A were significantly decreased(x2 =31.26,25.74,all P＜0.05).Conclusions Modified facial mask for NIV is effective in the treatment of elderly patients with respiratory failure.The complications and in-hospital mortality are reduced with the application of modified facial mask for NIV and it is highly tolerated by patients.Modified facial mask for NIV is the first choice in the treatment in elderly patients with respiratory failure.

Objective To evaluate the effects,safety and economic cost of long-term home noninvasive ventilation (NIV) therapy in elderly patients with chronic hypercapnic respiratory failure.Methods A total of 128 elderly patients with chronic hypercapnic respiratory failure were randomly assigned to two groups:the NIV group (n=66) with conventional therapy in addition to long-term home NIV therapy,and the control group (n=62) with conventional therapy alone.Compared were parameters before and after two year follow up,which included dyspnea grade,scale for accessory muscle use,scoring for emotional disorders,mean pulmonary pressure (mPAP) by electrocardiography,arterial blood gas,the times of pulmonary infection and hospitalization rates,the duration of hospitalization invasive ventilation,the duration of in RICU and in hospital stay,tracheal intubation rates and mortality.The medical cost was calculated.Results After two years,the differences in the dyspnea grade,scale for accessory muscle use,anxiety scores,depression scores,mPAP,arterial PaCO2 and PaO2,hospitalization rates,the times of pulmonary infection,the days of hospitalization for exacerbation in the home NIV group [2.2± 0.2,2.4 ± 0.3,4 ± 1,5.3 ± 1.2,(36.6±5.2)mm Hg,(50.2±4.5)mm Hg,(63.5±4.2)mm Hg,(1.3±0.2) times/year,(2.4±0.2) times/year,(15.8 ± 4.4) days/times] were statistically significant compared to the control group [4.1±0.2,4.9±0.5,12±3,11.3±1.6,(45.2±5.2)mm Hg,(67.3±4.5) mm Hg,(48.3±4.3)mm Hg,(5.4±0.4)times/year,(8.9 ±0.3) times/year,(38.5± 6.3) days/times] (all P＜0.01).The duration of invasive ventilation,the days in RICU and in hospital stay,tracheal intubation rates on admission to the hospital were significantly decreased in the home NIV group [(8.2 ± 2.2)days,(9.6±3.1) days,(15.8±4.4) days,(2±0.2) times/two years],as compared with the control group [(15.8±3.4) days,(18.6±4.4)days,(38.5±6.3)days,(8.0±0.8) times/two years].The mortality was decreased significantly in the home NIV group (3.0 ％)compared with the control group (29.0％) (P＜0.05).The medical cost in two years was significant lower in the home NIV group [(6.4 ± 0.5) thousand yuan],compared with the control group (18.4 ±0.6) thousand yuan (P＜0.01).Conclusions Long-term home NIV therapy in patients with chronic hypercapinc respiratory failure is effective,safe and can decrease the mortality and medical cost.

Objective To observe the nutritional biochemical indicators of paraquat poisoning patients, analyze and compare the nutritional status of patients and understand the changing trend of each indicator. Methods A total of 104 patients with acute paraquat poisoning who were admitted to the emergency department of the Second Hospital of Hebei Medical University from December 2015 to December 2017 were enrolled, and divided into the cure group (patients who survived >30 days) and the death group. Nutritional biochemical indicators including serum protein (ALB, PA, TP) and serum lipids (TCh, TG, LDL) were selected for dynamic observation. The observation time points were set as follows: immediate treatment of poisoned patients (day 1 on admission), on day 4, 7, 10, 13 and 16 after admission, and on day 30 after follow-up. The nutritional biochemical indicators of the two groups on day 1 and 4 were statistically analyzed and compared by t test. The nutritional status of the patients in the cure group was analyzed, and the Repeated Measures Anova was performed to understand the trend of each indicator over time. Results In the cure group, the TP level decreased from (73.34±5.75)g/L on day 1 to (51.95±6.05)g/L on day 4, t=20.34, P<0.01; and the TCh level decreased from (4.37±0.98) mmol/L on day 1 to (3.03±1.01)mmol/L on day 4, t=7.56, P<0.01. In the death group, the TP level decreased from (72.25±8.80)g/L on day 1 to (49.07±5.48)g/L on day 4, t=12.38, P<0.01, and the TCh level decreased from (4.38±0.88)mmol/L on day 1 to (2.51±1.07) mmol/L on day 4, t=7.94, P<0.01. Compared with the cure group, serum levels of ALB, TP and TCh in the death group decreased greater from day 1 to day 4 (all P<0.05). In addition, dynamic observation of the indicators in the cure group within 16 days after admission showed that, after treatment, the levels of ALB and TP recovered slowly and were still lower than normal . While the levels of PA and lipid rose rapidly after 10 days of admission. Conclusions Paraquat poisoning seriously affects the nutritional status of patients, and the serum protein levels decline significantly and can not be recovered easily. Therefore, sufficient attention should be paid to the treatment, and timely and appropriate nutritional support should be provided.

PhaseⅡexercise rehabilitation of patients with myocardial infarction is the core stage of the whole exercise rehabilitation process, which plays a vital role in disease rehabilitation and prevention. Previous reviews are difficult to highlight the changes and focus of phaseⅡexercise rehabilitation in patients with myocardial infarction. This article provides a general introduction from the perspective of myocardial infarction patients′ phaseⅡexercise rehabilitation. It summarizes the current status and deficiencies of exercise load assessment method, the types of exercise and effect of the myocardial infarction patient′s phaseⅡexercise rehabilitation, in order to provide reference for the improvement of related studies on exercise rehabilitation.
