1.Study of complications of spinal internal fixation surgery

Objective: To study the factors and preventive method of post-operative complications of spinal implant operation. Methods: The post-operative complication of 451 cases who had undergone spinal implant operation were analysed retrospectively. The relative factors of complications were analyzed. Results: Infection, thrombosis of deep vein and failed fusion were the most common complications. Among the factors, the age, operation time and blood lose during operation were closely related with the complication. Using antibiotic was effective to reduce the infection. Conclusion: It is effective to reduce the post-operative complication of spinal implant operation by proper pre-operative evaluation. Using antibiotic to control other diseases is also effective.
2.Hemodynamic study of the peripheral arterial Doppler flow velocity curve changes with limbs pressuring and its clinical application

Objective To observe the peripheral arterial Doppler flow velocity curve changes and elucidate the blood flow characteristics by adding extra pressure on the limbs,and to provide evidence for better diagnosis of peripheral arterial diseases by ultrasound.Methods Color Doppler ultrasound instrument was used to record the brachial artery(BA),radial artery(RA),common femoral artery(CFA) and popliteal artery(POA) Doppler flow velocity curves both at rest and under different grades of distal and proximal limb pressuring in 40 randomly selected healthy adults.The peak systolic and early diastolic reverse flow velocity(PSV,PRV) and the resistance index (RI) were measured and analyzed.Results Three-phase waveform was seen at rest.Significant changes were noticed in PRV and RI under distal pressuring,while no significant difference was seen in PSV between groups.Conclusions Normal peripheral arteries show pulsed step-by-step blood flow pattern along with the cardiac cycles.Peripheral arterial Doppler flow curves changes regularly with limb grading pressuring.
3.Effects of quiet respiration on left ventricular systolic function by echocardiography

Objective To investigate the effects of quiet respiratory intrathoracic pressure changes on ventricular dimensions and left ventricular systolic function,and to further verify our newly proposed hypothesis of the mechanism of respiratory effects on hemodynamics.Methods Twenty healthy volunteers aged 18 to 40 years average(24?7) years were included.The respiratory curve and electrocardiogram were simultaneously recorded.The interventricular septum movement,ventricular dimensions and ventricular systolic excursions were recorded by two-dimensional and M-mode echocardiography during quiet respiration.The respiratory variations of the above parameters were derived,and the parameters for assessing ventricular systolic function were calculated.Results With quiet respiration,the interventricular septum moved towards the left ventricle on inspiration,and then towards the right ventricle on expiration.The right ventricular diastolic dimension increased significantly on inspiration compared with those on expiration.The opposite changes occurred with left ventricular diastolic dimensions.The left ventricular posterior wall systolic excursion and the left ventricular stroke volume significantly increased during expiration compared with those during inspiration.No significant differences were found in left ventricular ejection fraction and fractional shortening between inspiration and expiration.Conclusions The respiratory intrathoracic pressure change is one of the factors influencing the reciprocal changes of the left and right ventricular parameters related to ventricular dimensions and systolic function.
4.The correlation between intraoperative contrast-enhanced ultrasound quantitative parameters and microvessel density of cerebral gliomas

Objective To investigate the value of intraoperative contrast-enhanced ultrasound(CEUS) in evaluating pathological grades of cerebral gliomas. Methods Intraoperative CEUS was performed in 33 patients of cerebral gliomas of different pathological grades. Real-time blood perfusion and enhance characteristics of these tumors were observed,quantitative parameters from the automatically derived time-intensity curve (TIC) were obtained, and compared with the tumor microvessel density (MVD) by immunostaining with anti-CD34. Results The time to peak was significantly shorter and the MVD was significantly higher in the high grade cerebral gliomas compared with the low grade ones ( P <0. 05). The time to peak was negatively correlated with the MVD by immunostaining (r = -0.79, P < 0. 05). Conclusions Intraoperative CEUS could be used to observe microvascular perfusion in real-time, and could indirectly reflect the information of MVD in cerebral gliomas, which is of help to grade cerebral gliomas and guide surgical resections.
