1.Spermatic vein detection by color doppler ultrasound for minimally trauma varicocelectomy

Objective In order to choose the best surgical approach for minimally trauma varicocelectomy color Doppler ultrasound(CDU) was used to detect the anatomic relationships of spermatic vein in groin. Methods Sixty varicocele patients were randomly selected. Their spermatic veins were examined by CDU which beginning from superficial inguinal ring,passing the crossing point of spermatic vein and femoral artery ,and ending at the 3cm above the deep inguinal ring. The depths from skin to spennatic vein were measured and the relationships between spermatic vein and femoral artery were recorded. Results The average length of incision is 2.1cm and the average duration of operation is 22 minutes. The average depth from skin to spermatic vein was 1.1cm,1.55cm and 3.56cm respectively at the site of the superficial inguinal ring,the crossing point of spermatic vein and femoral artery,and the deep ingui-nal ring. Conclusion The best approach for minimally trauma varicocelectomy is at the crossing point of spermatic vein and femoral artery because here the spermatic vein is relative superficial and has merged into two or three vessels and the femoral artery can be easily touched by fingers.
3.Effect of TGF-β1 on epithelial-mesenchyme transition in Hela cells of human cervical cancer

Objective To investigate the effect of TGF-β1 on epithelial mesenchymal transition in Hela cells of human cervi-cal cancer.Methods Hela cells of Human cervical cancer cultured in vitro were divided into the experimental group and control group.In the control group,Hela cells were cultured in serum-free medium without TGF-β1.In the experimental group,Hela cells were treated with different concentrations of TGF-β1 (0.01,0.10,1.00,10.00 ng/mL).The morphological changes of Hela cells of human cervical cancer were stimulated by TGF-β1 at different time points observed under an inverted microscope,while the expres-sions of mRNA and protein of E-cadherin and Vimentin in Hela cells were detected by semi-quantitative RT-PCR assay and cellular immunohistochemistry respectively.Results Comparing with the control group,Hela cells stimulated by TGF-β1 for 48 h began to have morphological changes.Mesenchymal morphology changes were observed obviously after 72 h.RT-PCR analysis showed that the expression of epithelial marker E-cadherin mRNA was down regulated,while the expression of mesenchymal marker Vimentin mRNA was increased and showed a concentration dependence after the stimulation of TGF-β1,Comparing with the control group, the difference was statistically significant (P < 0.05 ).After stimulation of TGF-β1 in Hela cells,cellular immunohistochemistry showed that the concentration of TGF-β1 increased,the expression of E-cadherin protein gradually decreased,and the expression of Vimentin protein gradually increased at the same time.Comparing with the control group,the difference was statistically significant (P <0.05).Conclusion TGF-β1 may induce epithelial mesenchymal transformation in Hela cells of human cervical cancer.
4.Influence of Nape Cluster Acupuncture on Peripheral Plasma ET Content in Patients with Acute Cerebral Infarction

Objective:To investigate the mechanism of nape cluster acupuncture in the treatment of acute cerebral infarction. Methods: Eighty patients with acute cerebral infarction were selected and randomly allocated to group A and B. Group B was given routine treatment for acute phase and Group A was added with the nape cluster acupuncture based upon the treatment for Group B. The treatment was given twice daily, with 15 d as a course of the treatments, to observe the decreasing situation in the score of the nerve function defects and changes in plasma ET content. Results:There were significant differences in the score of the nerve function defects and plasma ET content before and after treatments in two groups (P<0.05), but there was significant difference between Group A and Group B after treatment (P<0.05), indicating the curative effect was better in Group A than in Group B.Conclusion: Nape cluster acupuncture can be used to effectively treat acute cerebral infarction and obviously decrease plasma ET content and reduce the score of the nerve function defects. The findings also proves that the therapeutic effect was obviously better than single medication, if nape cluster acupuncture was combined in the treatment of the acute cerebral infarction.
5.Detection of mutations of exons 5 and exons 8 of PTEN gene in oral squamous cell carcinoma

Objective:To detect mutations of exons 5 and exons 8 of PTEN gene in oral squamous cell carcinoma(OSCC),and to explore the relationship between gene mutations and development of OSCC.Methods: Mutations of exons 5 and exons 8 of PTEN gene were detected by polymerase chain reaction-single strand conformation polymorphism(PCR-SSCP).Results: The whole sequences of exons 5 and exons 8 of PTEN gene of all cases of OSCC were expanded.There was no mutation in exons 5 and exons 8 of PTEN gene of all cases of OSCC.Conclusion: There is no mutation in exon 5 and exon 8 of PTEN gene in OSCC.This might show that there is no correlation between mutations in exon 5 and exon 8 of PTEN gene and the development of OSCC.
6.Study on expression of serum tumor markers spectrum and chemokine protein in lung cancer and its predictive value

Objective To analyze the expression and predictive value of serum tumor markers spectrum and chemokine protein in lung cancer and predictive value.Methods One hundred and fifty patients with lung cancer were selected as the observation group and contemporaneous 150 individuals undergoing physical examination served the control group.The levels of ProGRP,CEA,SCC and Cyfra21-1 were measured by chemiluminescence microparticle immunoassay.The chemokine protein was determined by multiple immunofluorescent assay.Results The levels of CCL28,LIF,LIGHT and GRO in the observation group were significantly higher than those in the control group (P<0.05).The levels of CCL28,LIF,LIGHT and GRO in the observation group were higher than those in the control group.The levels of CCL28,NAP-2 and MDC in the observation group were lower than those in the control group (P<0.05).Conclusion The regular detection of serum tumor markers spectrum and chemokine protein can predict the treatment prognosis and evaluate the clinical curative effect.
7.Effects of different depth of anesthesia on ScvO2 and ABL in elderly patients undergoing gastrointestinal surgery

Objective To investigate the effects of different depth of anesthesia on ScvO2 and ABL in elderly patients with gastrointestinal cancer. Methods Totally 55 elderly patients undergoing gastrointestinal sur-gery were randomly divided into deep anesthesia group(BIS40-49)(group D,n=27)and light anesthesia group (BIS50-59)(group S,n = 28). While SVV value was fixed,fluid management was conducted while monitoring CVP. HR,MAP,the change of central venous oxygen saturation and lactic acid index were recorded in different time points. Results Propofol dosage in group D and group S indicated statistical significance(P<0.05). ScvO2 in group S in T3~T5 was higher than that in T0 and the difference was statistically significant(P < 0.05). Com-pared with ScvO2 in T3 in group D,that in group S was significantly higher and the difference was statistically sig-nificant(P<0.01),but no significant difference was found in other time point in 2 groups(P>0.05). There was no significant difference between and within ABL groups in each time period(P>0.05). There was no significant difference in terms of other indicators between 2 groups. Conclusions For elderly patients with gastrointestinal surgery,shallow anesthesia can improve tissue oxygenation and reduce the amount of anesthetics.
8.Emergency damage control treatment for the severe abdomen trauma of 192 cases

Objective: To explore the application experiences and effects of emergency damage contro(lDC)treatment for severe abdomen trauma. Methods: The clinical data of 192 patients with severe abdomen trauma were selected as the object of this study. A retrospective analysis was done on therapy condition and effects of emergency damage control treatments. Results: Pre-hospital emergency care was performed in all 192 patients, damage control operations (DCO)were done in 186 cases, 184 cases entered surgical intensive care unit (SICU)for resuscitation, and 177 cases were adopted corresponding reoperations after SICU resuscitation. The abnomal DCO indices recovered gradually in the 72 hours after DCO. The cure rate was 90.7% with 174 cases, and the incidence of complication was as low as 2.3% with 4 cases after reoperations, and the 4 cases all healed by conservative treatments. Mortality rate was 6.2% with 12 cases, and it caused by associated organs injuries, severe primary disease and secondary infection of other organs. Conclusion: Emergency damage control treatment modus should be chosen positively and eligibly under indication to severe abdomen trauma patients. According to the difference of location and degree of injury, eligible therapies by stages are the key managements.
9.Fluoroquinolone Resistance and Mutation Patterns in gyrA and parC Genes in Neisseria gonorrhoeae Isolates from Shanghai,China

In order to study the resistance of Neisseria (N.) gonorrhoeae to the fluoroquinolone and detect mutation patterns of quinolone resistance-determining regions (QRDRs) of clinical isolates in Shanghai,China,a total of 80 clinical isolates of N.gonorrhoeae were consecutively collected from Shanghai.The MIC of fluoroquinolone for the isolates was examined by using the agar dilution method and the mutation profiles of the QRDRs of gyrA and parC were analyzed by sequencing and restriction fragment length polymorphism (RFLP).Chi-square test was used for comparison of the mutation patterns.The results showed that:(1) High percentages of the 8 isolates were resistant to ciprofloxacin (95.0%),ofloxacin (95.0%) and lomefloxacin (97.5%),only one strain was susceptible to the ciprofloxacin.(2) Sensitive strains had a substitute of Asp95→Ala in the gyrA,and all isolates that were resistant or intermediated to the ciprofloxacin,had a double mutation in the gyrA (Set91,Ala 92 and Asp95).Some strains also had a mutation in the parC.(3) The MICs of these isolates were significantly associated with the mutation patterns in the gyrA and parC.A double mutation of gyrA combined with parC87 mutation was a predominant pattern in Shanghai and could mediate high level resistance to ciprofloxacin.It suggests that mutations in the QRDRs of gyrA and parC .may be re-sponsible for the fluoroquinolone resistance.And fluoroquinolone could not be used as the first line antibiotics for gonorrhea treatment any more in Shanghai,China.
10.A 3D tomography technique based study on event-related potential in attention.

In this paper the event-related potential (ERP) of selective attention to the left and right visual field is investigated with 3D tomography technique. The results show that compared with the scalp surface ERP data, the current source intensity derived from tomography can better reflect the enhanced effect of P1 and N1, especially N1. Moreover, C1 component can be observed in the results, which cannot be identified in the scalp data.
