Objective:To study the influence of Yizhiping Capsules on hemorrheology of experimental rats with hyperlipoidemia and its embryonic toxicity, teratogenicity. Methods:① The rat experimental hyperlipoidemia models were established by feeding with high lipid diet for 10 days. Meanwhile Yizhiping was taken orally at dosages of 75、150mg?kg -1 , some indices of hemorrheology markers were observed. ②According to the procedure of toxicology, the rat in the experimental groups were administered with Yizhiping capsules by gavage at a dose of 250、500、1000mg?kg -1 respectively, the embryonic toxicity and teratogenicity were observed. OBJECTIVETo investigate the hemodynamics evidence of the descending branch of lateral circumflex femoral artery in a reversed way. To explore the clinical result of using the reversed descending branch of the lateral circumflex femoral artery as the receipt artery for free flaps for reconstruction of the leg soft-tissue defect.

METHODSFrom October 2005 to February 2012, 38 patients with severe leg soft-tissue defects were treated. The proximal antegrade and retrograde mean artery pressure of the descending branch of the lateral circumflex femoral artery in 16 of 38 patients were recorded during operation. All wounds had osteomyelitis, bone and tendon exposure requiring coverage reconstruction. And there was no recipient artery in the injured lower leg for free flaps in all 38 patients. Reversed descending branches of lateral femoral circumflex arteries were used as recipient arteries for free flaps (free latissimus dorsi flap, free thoracoumbilical flap, and free anterolateral thigh flap) in all patients. The flap donor site was closed directly or with the skin graft.

RESULTSThe proximal antegrade mean artery pressure of the descending branch of lateral circumflex femoral artery was(81.6 +/- 12.4) mmHg. The proximal retrograde pressure was(48.2 +/- 10.7) mmHg. The proximal retrograde mean artery pressure was 59.07 percent of the proximal antegrade pressure. The donor skin graft survived and wound healed primarily. After operation, 2 flaps had distal partial necrosis and healing was achieved after dressing change. All the other flaps survived completely without vascular problems. All the patients were followed up for 11 months to 2.5 years (mean, 1.6 years). The flap appearance was satisfactory. The texture and color of flaps in all cases were good.

CONCLUSIONSThe reverse descending branch of lateral circumflex femoral artery is a reliable recipient artery for the free flaps. It is an easy and simple technique that can be used for reconstruction of the defects in the lower leg, with the reversed descending branch of lateral circumflex femoral artery as recipient artery.

Objective To explore the effect of trauma control concept on severe multiple injuries.Methods From March 2016 to March 2018,74 emergency patients with severe multiple injuries were selected in Yiwu Central Hospital.The patients were divided into the observation group and the control group according to the digital table method,with 37 cases in each group.The control group was treated with one-time surgical treatment,and the observation group was given treatment measures for severe multiple trauma in emergency department under the concept of trauma control.The recovery time of body temperature,the recovery time of prothrombin (PT),the time of shock correction,the occurrence of complications,and the therapeutic effect were compared between the two groups.Results The body temperature recovery time,PT recovery time and shock correction time in the observation group were (6.73 ± 3.29) h,(5.18 ± 1.89) h and (5.84 ± 3.14) h,respectively,which were shorter than those in the control group [(9.85 ± 1.62) h,(9.86 ± 2.41) h,(8.45 ± 0.87) h],and the differences were statistically significant (t =5.175,9.295,4.873,all P ＜0.05).The incidence rate of complications in the observation group was 8.11％ (3/37),which was lower than that in the control group [32.43％ (12/37)],the difference was statistically significant(x2 =6.773,P ＜ 0.05).The cure rate of the observation group was 97.30％ (36/37),which was higher than that of the control group [91.89％ (34/37)],but there was no statistically significant difference (P ＞ 0.05).Conclusion The treatment effect of severe multiple injuries under the concept of trauma control is obvious.It is an effective and feasible method,which can effectively reduce the complications and fatality rate.
