Objective To assess the quality of life(QOL)in patients with stroke at the acute stage,and to determine whether all the dimensions of the Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36)are applicable to the stroke patients.Methods Fifty stroke patients at the acute stage were studied.The SF-36 was employed to assess the quality of life of the patients,and a self-designed questionnaire was used to record the general information of them.The data obtained from the patients was compared with the Chinese norm reported in literature. Objective To observe the efficacy of multifactor intensive nursing intervention on chronic heart failure patients with outside hospital treatment,including self-management ability and related clinical indicators.Methods By application of prospective study,a total of 215 patients with chronic heart failure discharged from hospital were randomized into the usual care group (105 cases) and the intensive nursing intervention group (1 10 cases).Patients were followed up for 12 months,and 205 patients completed the follow-up.The usual care group only received conventional outpatient follow-up,the intensive intervention group patients received telephone counseling,more detailed clinical follow-up and regular health education.The intervention effect was compared between two groups.Results Compared with the usual care group,the passing rate of sodium/water limit and daily monitoring of weight was higher in the intensive intervention group.Left ventricular ejection fraction and six-minute walking test were better in the intensive intervention group,while the level of NT-pro-BNP was lower in the intensive intervention group.Compared with the usual care group,re-hospitalization rate and incidence of cardiovascular events were lower in the intensive intervention group.Conclusions Multifactor intensive nursing intervention is helpful on improving self-management ability,alleviate cardiac function,reduce re-hospitalization rate and incidence of cardiovascular events for chronic heart failure patients.

Objective To present a method to treat soft tissue defects with bone exposure after the distal tihial fracture.Methods Twenty-six patients who had suffered from the distal tibial fracture complications leading to exposure of the bone or fixation material sought medical help in our department.The internal fixation material was removed and radical revision of dead and infected tissue was carried out in all cases.Five of the 26 cases were treated using a free microvascular muscle flap and the mesh skin graft by one stage.Twenty-one of them were re- paired with the same method by two-stage.Results Follow-ups were 10 to 24 months.The microvascular re- construction was successful in 22 patients.Two of the 26 cases had partial necroses of the skin graft,and the other two cases had delayed wound healing.All the fractures united.Conclusion Microvascular free muscle flap re- construction of the leg is regarded as a reliable method to treat legs with massive soft-tissue defects or defects with bone exposure and chronic infection or fistulation at the distal leg.

Objective To compare application in Beijing population using the diagnostic criterias for metabolic syndrome(MS)according to the criterias of the International Diabetes Federation(IDF),Third Report of the National Cholesterol Education Program(NCEP)Expert Panel on Detection,Evaluation,and Treatment of High Blood Cholesterol in Adults[ATP Ⅲ(revised)]and Chinese Diabetes Society(CDS), respectively.Methods This study was based on an investigation on the risk factors of cardiovascular diseases involving 4 628 cases.MS was diagnosed according to these three definitions respectively and prevalence derived from these three definitions was compared.Results The prevalence of MS were 40.3 %, 44.3% and 35.7% according to the IDF,ATP Ⅲ(revised)and CDS respectively.The agreement in the diagnosis of MS using IDF and CDS,ATP Ⅲ(revised)and CDS,IDF and ATP Ⅲ(revised)were 81.9%, 83.7% and 96.0% respectively.There were 4.0% subjects among the MS patients diagnosed by ATP Ⅲ (revised)who can not be screened by IDF criteria.The prevalence of obesity was 19.6% according to ATP Ⅲ,much lower than the prevalence using CDS and IDF,whereas this rate increased to 58.6% using ATP Ⅲ(revised)criteria,showing diagnostic agreement with CDS.The coincidence rate between ATP Ⅲ (revised)and CDS was 80.6% Conclusion This investigation shows good agreement in the diagnosis of MS using IDF,CDS and ATP Ⅲ(revised)among Beijing population and these criterias can be used in China.

OBJECTIVE To study the risk factors and clinical characteristics of nosocomial infection in senile patients with acute myelogenous leukemia who received chemotherapy.METHODS Ninety-one cases of senile patients with acute myelogenous leukemia received 304 times of chemotherapy with etiological examination,to analyze the relationship between nosocomial infection and absolute neutrophil count in peripheral blood,the cellularity of the marrow and chronic systemic disorder.RESULTS All indicators of senile patients with acute myelogenous leukemia were higher than those of younger patients(P0.05).CONCLUSIONS Age,different stage of chemotherapy,the neutrocytopenia level and prolongation,and hypocellularity of bone marrow are related to the nosocomial infection in senile patients with acute myelogenous leukemia,and the chronic systemic disorder is not a risk factor.

Objective: To observe the effect of acupuncture plus thunder-fire moxibustion on the expressions of matrix metalloproteinase-3 (MMP-3), tissue inhibitor of metalloproteinase-1 (TIMP-1) and transforming growth factor-β1 (TGF-β1) in cartilage of knee osteoarthritis (KOA) rats, and to explore the mechanism of acupuncture plus thunder-fire moxibustion in the treatment of KOA. Methods:Thirty Sprague-Dawley (SD) rats were randomly divided into a blank control group, a model group and an acupuncture-moxibustion group by random digits table, 10 rats in each group. Rats in the model group and the acupuncture-moxibustion group were injected with papain in the right posterior knee joint to prepare the models. The levels of MMP-3 and TIMP-1 in rat synovium of each group were measured by enzyme-linked immunosorbent assay (ELISA) after 2 weeks of treatment. The level of TGF-β1 was determined by Motic B5 Micro-camera system. Results:The levels of MMP-3 and TIMP-1 in the cartilage of the model group were significantly higher than those in the blank control group (allP<0.01); the levels of MMP-3 and TIMP-1 in the acupuncture-moxibustion group were lower than those in the model group, and the between-group differences were statistically significant (all P<0.05). The levels of MMP-3 and TIMP-1 in the acupuncture-moxibustion group were higher than those in the blank control group, and the differences were statistically significant (all P<0.05). The level of TGF-β1 in cartilage tissues of the model group was significantly lower than that in the blank control group (P<0.01); the level of TGF-β1 in the acupuncture-moxibustion group was higher than that in the model group (P<0.05), but it was lower than that in the blank control group, and the between-group difference was statistically significant (P<0.05). Conclusion: Acupuncture plus thunder-fire moxibustion can effectively recover the abnormal expressions of MMP-3 and TIMP-1 in KOA model rats and somewhat up-regulate TGF-β1, which may be one of its mechanisms of acupuncture plus thunder-fire for KOA.
