1.Analysis of U.S.military health system reform

To curb the increase in health care costs and improve government efficiency , a series of structural and governance reforms for U.S.Military Health System(MHS) were implemented, which is the biggest structural organizational change in the MHS′s history.The reform focusing on the establishment of Defense Health Agency ( DHA) and enhancement of multi-service markets will achieve its goal by the end of 2015.This paper analyzes the major reasons for the reform and changes of the U.S.MHS before and after the reform and offer some tips for the reform of the Chinese Military Health System .

An integrated district health care model is an important field development .Since 2007, The US Armed Forces have been constructing district health care systems , integrating the medical resources in National Capital Region and San Antonio.The two new established district health care systems have been making military health care more effective while reducing costs and improving the quality of care to beneficiaries .
3.Advances in etiology of pituitary adenomas

With the development of molecular and cell biology,new concept of the etiology of pituitary adenomas was taking shape.The etiology of pituitary adenoma was considered to be related to gene mutation and transforming factors.This paper reviews the roles of gene mutations,transforming factors,expression of receptors on cells and growth factors in the etiology of pituitary adenomas.
4.Effects of Tangshentang on the expression of LOX-1 and secretion of extracellular matrix induced by ox-LDL in rat masengial cells

Objective: To investigate the effect of Tangshentang on the expression of LOX-1 and secretion of TGF-?1,FN,ColⅣ in cultured rat masengial cells(RMCs) induced by ox-LDL and to explore its molecular mechanism of preventing and treating diabetic nephropathy.Methods: The cultured RMCs were divided into control group,ox-LDL group,Tangshentang group(respectively high,medium and low group)and rosiglitazone group.The mRNA expression was tested by RT-PCR.Synthesis of TGF-?1,FN,ColⅣ in cultured RMCs were determined by ELISA methods.Results: Tangshentang drug serum significantly attenuated up-regulation of TGF-?1,LOX-1 mRNA expression and synthesis of TGF-?1,FN,ColⅣ in a dose-dependent manner,with the peak at middle concentration,in RMCs stimulated by ox-LDL(50?g/ml).Conclusion: Tangshentang drug serum may protect kidney from injury by ox-LDL via the decreased expression of LOX-1 to reduce the uptake of ox-LDL and subsequently inhibiting TGF-?1 secretion,as well as the deposition of FN,ColⅣ.

The Strategic National Stockpile (SNS) is a national repository of antibiotics, chemical antidotes, antitoxins, life-support medications , intravenous administration , airway maintenance supplies , and medical/surgical items.It is designed to supply and resupply state and local public health agencies in the event of a national emergency anywhere and at any time within the United States or its territories .This article introduces the development , emergency supplies reserve and maintenance of the SNS .
6.Screening of 10 types of Chinese herbal compounds inhibiting Abeta and their possible related mechanism in vitro.

This study is to screen the Chinese herbal compounds which could inhibit the production of Abeta and investigate the underlying mechanism. Ten types of compounds which have potential value in the treatment of AD were selected as initial screening trial. The cell models which used could overexpress Abeta and beta-secretases or Abeta and gamma-secretases. Extracellular Abeta was determined by ELISA after the cell models treated with different concentrations of compounds (0.5-100 micromol x L(-1)), separately. Then the compounds were selected which could inhibit extracellular Abeta and their best concentration ranges were decided, too. Furthermore, the cell viability and apoptosis rate, the level of intracellular Abeta, beta and gamma-secretases were determined after the cell models treated with different concentrations of selected compounds. The results showed that 4 of the 10 compounds could reduce the level of extracellular Abeta; they were cryptotanshinone, astragalosides, gastrodin and paeoniflorin, and their best concentration ranges were 0.5-5.0, 0.5-5.0, 5.0-50, 1.0-25 micromol x L(-1), respectively. Further study indicated that the 4 selected compounds were nontoxic to the cellular models and lowering intracellular Abeta were more effective compared with extracellular; of which astragalosides and gastrodin showed dose-dependent inhibition to the activities of beta and gamma-secretases, with the maximum inhibiting rates of 78.2% and 80.3%, respectively. In conclusion, cryptotanshinone, astragalosides, gastrodin and paeoniflorin could inhibit the expression and secretion of Abeta, and the underlying inhibiting mechanism of astragalosides and gastrodin were related with the reduction of the beta and gamma-secretase activities, respectively.
7.The Microtox-Based Fast Test of Comprehensive Toxicity of Safflower Injection

This study aimed at exploring a new method--microtox technology to evaluate the comprehensive toxicity of safflower injections from different drug manufacturers.In the experiment,we investigated the characteristic parameters of vibrio fischeri (CS234) under different conditions,and then locked the optimum parameters for safflower injection assay:recovery liquid volume 0.9 mL in freeze-dried vial,50 μL bacteria suspension in each sample,and the detection time of 10 mins after adding the bacteria suspension.Under this optimum detection condition,we found that the EC50 values were 3.36%,5.58% and 4.33%,being significantly different between 3 representative drug manufacturers (P < 0.05).In conclusion,it was implied that the biological detection standards of safflower injections need improving,while the microtox-based fast test of the comprehensive toxicity of safflower injection showed a favorable and prospective application to controlling the product qualities of different manufacturers.
8.Organization and implementation of aeromedical evacuation of US patients with Ebola virus infection from West Africa

In 2014,serious Ebola fever epidemic broke out in West Africa .Afterwards, the US transported two Ebola patients from Liberia by air, which aroused world wide concern .During the aeromedical evacuation , there were lots of tech-nical problems and management procedures involved .First, the US government toke emergency measures for the patients in a local hospital ,which made evacuation possible .Secondly , the US CDC and other government departments coordinated this matter during and after the evacuation .Finally,upon arrival at home the two patients received careful isolation and treat-ment.Thus, the successful aeromedical evacuation is instructive for us .
9.R & D of medicine and vaccine for Ebola hemorrhagic fever

The outbreak of Ebola hemorrhagic fever in West Africa in February 2014 is the worst one in the history of the disease.Medicines and vaccines for Ebola have attracted special attention .This study utilizes Cortellis database of Thom-son Reuters and knowledge about Ebola virus to analyze current R&D of medicines and vaccines for Ebola in order to offer references for research and development .Study findings shows that there are mainly 5 important categories:vaccines, neu-tralizing antibodies , small-molecule anti-Ebola drugs , RNAi drugs and nucleotide drugs .In each category , there are prom-ising products under development that are worthy of attention .
10.A Relevant Research on the Relationship Between TCM Syndromes of State of Evil Domination and Clinical Indicators in IgA Nephropathy

Objective To explore the relationship between traditional Chinese medicine (TCM) syndromes of state of evil domination and clinical and laboratory indicators of IgA nephropathy. Methods A prospective study was used to collect data on clinical and laboratory examination of IgA nephropathy in multi clinical centers. Patients’ TCM syndrome types were determined according to the national diagnostic criteria at the same time. Totally 266 patients with IgA nephropathy were included in the study to establish a database for analyzing the relationship between TCM syndrome of state of evil domination and clinical and laboratory indicators. Results In the relationship between syndromes of state of evil domination distribution and clinical subtypes, patients of wind-heat syndrome had more macroscopic hematuria;patients of phlegm-damp syndrome had more nephrotic syndrome;patients of damp-heat syndrome had more chronic nephritis type Ⅰ;patients of blood stasis syndrome had more chronic nephritis type Ⅱ. In the relationship between syndrome of state of evil domination distribution and clinical manifestation, the incidence of hypertension was higher in patients of blood stasis syndrome than in other three types. It was more serious for hematuresis in the patients of wind-heat syndrome. For patients of phlegm-damp syndrome, the incidence of heave proteinuria was highest. In the relationship between syndrome of state of evil domination distribution and laboratory examination, 24-hour urinary protein quantification was higher than in patients of wind-heat and damp-heat syndrome, but the level of blood albumin was lowest. For patients of blood stasis syndrome, serum creatinine level was significantly higher than in other three types;the level of eGFR was just the opposite. The levels of blood cholesterol and triglyceride in patients of phlegm-damp syndrome were higher than in other three types. The activated partial thromboplastin time (APTT) levels in patients of blood stasis and phlegm-damp syndrome were lower than in other patients, but the FIB level was the exact opposite of APTT. In the relationship between syndrome of state of evil domination distribution and the stages of chronic kidney disease (CKD), patients of wind-heat syndrome were more in the first stage of CKD;patients of blood stasis syndrome were more in the third stage of CKD. Conclusion There is relative correlation between TCM syndromes of state of evil domination and clinical and laboratory indicators in IgA nephropathy, which would provide some reference to narrow the gap in the information of the four methods of TCM with clinical and laboratory indicators to enhance accurate diagnosis of TCM syndrome.
