1.Detection of peripheral NGAL and MMP9 levels and its clinical significance in primary hepatic carcinoma

Objective To explore the changes of matrix metalloproteinase 9(MMP9) and neutrophil gelatinase associated li-pocalin(NGAL) in primary hepatic carcinoma and its clinical significance .Methods Enzyme-linked immunosorbent assay (ELISA) was used to detect the serum level of MMP9 and NGAL in 47 patients with primary hepatic carcinoma and 19 healthy controls .The association between serum MMP9 and NGAL levels with the clinicopathological features of hepatic carcinoma were evaluated .ROC curves were used to analyze the influence of MMP9 and NGAL on the diagnostic value .Kaplan-Meier method was used to evaluate the relationship between the expressions of NGAL and MM P9 and the prognosis of patients with HCC .Results Serum MMP9 and NGAL levels in hepatic carcinoma patients are significantly higher compared with normal controls (P ≤ 0 .05) .The serum level of MMP9 and NGAL in primary hepatic carcinoma was positively correlated with TNM stage (P≤ 0 .05) .The sensitivity and specifici-ty of MMP9 and NGAL for primary hepatic carcinoma were 70 .2% and 83 .0% .The sensitivity of the joint detection were 94 .9% . Kaplan Meier survival curves showed that cumulative survival rates of patients with positive expression of NGAL were significantly lower than those of patients with negative expression .Conclusion MM P9 and NGAL might play an important role in gastric cancer generation and development .Monitoring of serum MMP9 and NGAL levels might be helpful for diagnosis and evaluation of progno-sis for primary hepatic carcinoma .
2.Relation between expression of BCSCs associated genes from swelling axillary lymph nodes of breast cancer with recurrence and metastasis of tumor

Objective To explore the relation between the expression of breast cancer stem cells(BCSCs) related gene CD44 , OCT4 and Twist1 in breast cancer tissues and swelling axillary lymph nodes with the recurrence and metastasis of breast cancer . Methods The genes and antigen expressions of BCSCs relative factors CD44 ,CD24 ,OCT4 and Twist1 in th breast cancer tissue and axillary lymph nodes were detected by the immunohistochemical method and RT‐PCR .Results Totally 14 cases of preopera‐tively and intraoperatively diagnosed breast cancer had axillary lymph nodes metastasis of cancer cells .The over expression of BC‐SCs relative factors CD44 ,OCT4 and Twist1 antigen positive in the breast cancer tissue and axillary lymph nodes were detected by the immunohistochemical method ,while CD24 was lowly expressed;the gene expressions of BCSCs relative factors CD44 ,OCT4 and Twist1 in the breast cancer tissue and axillary lymph nodes were enhanced by the RT‐PCR detection (P<0 .05) ,while CD24 gene was lowly expressed(P>0 .05) .Conclusion The genes and antigen expression of BCSCs related factors CD44 ,OCT4 and Twist1 in the patients with breast cancer complicating swelling axillary lymph nodes are significantly increased ,which might indicate that the BCSCs related factors are related with easily early metastasis and recurrence of breast cancer cells .
3.The effect of modified pericardial devascularization on esophageal gastric varices of liver ;cirrhosis with portal hypertension

Objective To explor the operative method of modified pericardial devascularization and compare the effects of modified pericardial devascularization and traditional pericardial devascularization on esophageal gastric varices of liver cirrhosis with portal hypertension. Methods The clinical data of 35 patients receiving modified pericardial devascularization (MED group) and 35 patients receiving traditional pericardial devascularization (ED group) were retrospectively analyzed . Results The operating time in MED group was significantly lower than that in ED group:(182 ± 30)min vs.(220 ± 30) min, P<0.05. The blood loss in MED group was significantly lower than that in ED group:(200 ± 15) ml vs. (300 ± 100) ml, P<0.05. Early complication rate in MED group was significantly lower than that in ED group:5.7%(2/35) vs. 22.9%(8/35), P<0.05. The esophageal gastric varices of patients who were followed up in MED group and ED group were almost equal (P>0.05). Conclusions The operating time, blood loss and early complication rate in modified pericardial devascularization is better than traditional pericardial devascularization. There is no significant difference in long-term efficacy between two methods.
4.Intervention in vitro of miR30a on tumor stem cell related gene expression and invasiveness of breast cancer swollen axillary lymph nodes

Objective To investigate the micrometastatic lesion of tumor stem cells in the axillary swollen lymph nodes of breast cancer patients and the influence of miR30a on its invasive ability ,and to explore the feasibility of miRNAs anti‐breast cancer treatment .Methods The tumor stem cell‐like breast cancer cells were separated from the axillary swollen lymph nodes in breast cancer patients and cultured .miR30a oligonucleotide fragment was synthesized and transfected into human primary generation breast cancer cells by using adenovirus ,meanwhile the breast cancer cell line MDA‐MB‐231 was taken as the experimental control and the transfection efficiency was assessed by the fluorescence microscopy .The changes of tumor cell proliferation and invasiveness before and after transfection were detected by the Transwell chamber in vitro invasion assay .Western blot was used to detect the ALDH1 ,Vimentin and N‐Cadherin protein expression .Results The Transwell chamber in vitro invasion assay showed that the pri‐mary generation breast cancer cell had more strong invasive ability than MDA‐MB‐231 cell line ,their invasion indexes were (75 .3 ± 3 .2)% and (58 .4 ± 2 .8)% respectively ,the difference was statistically significant (P<0 .05) ,and after transfecting miR30a ,the in vitro invasiveness ability in these two kinds of cells were significantly weakened and their invasion indexes were (21 .4 ± 1 .9)% and (28 .2 ± 2 .3)% respectively ,the difference compared with the control group showed the statistical significance (P<0 .05) .Conclu‐sion The ALDH expression in partial axillary hyperplasia and swollen lymph nodes in the patients with breast cancer is increased , and the tumor micrometastasis may exist ,which should be completely cleaned in operation .miR30a inhibits the expression and inva‐sive ability of tumor stem gene .
5.Survey on knowledge of home blood pressure monitoring in community physicians of Zhejiang province

Objective To survey on knowledge of home blood pressure monitoring (HBPM) in community physicians of Zhejiang province.Methods One hundred and thirty eight community physicians from Xiacheng District of Hangzhou Municipality were assigned in intervention group (n =70) or control group (n =68).The knowledge of home blood pressure monitoring was surveyed by using the China HBPM questionnaire in both groups.Results Total 132 valid questionnaires were returned,including 70 from intervention group and 62 from control group.The results showed that 80.0% (61/70) physicians in intervention group and 62.9% in control group frequently suggested hypertensive patients to have HBPM in out-patient clinics (P < 0.05).Physicians who correctly instructed patients to use upper arm electronic sphygmomanometer,to measure blood pressure in morning and night,to repeat 2-3 times each time,to record result 3 times,and to take ≥ 135/85 mmHg(1 mmHg =0.133 kPa) as criteria of hypertension in intervention and control groups were 67.1% (47/70) versus 32.3% (20/62),5.7% (39/70) versus 29.0% (18/62),92.9% (65/70)versus 66.7% (40/62),38.6% (27/70)versus 6.4% (4/62)and 57.1% (40/70) versus 38.7% (24/62),respectively,and the differences between two groups were all statistically significant (P < 0.05).As for measuring frequency,92.9% (65/70) physicians in intervention group and 67.7% (42/62) in control group considered that blood pressure should be measured every day for poorly controlled patients ; 81.4% (61/70) versus 14.5 % (9/62) considered that blood pressure should be measured every week for well-controlled patients; 87.1% versus 35.5% (22/62) considered that blood pressure should be measured every year at least for people with normal blood pressure; there were significantly differences between two groups (P < 0.05).The community physicians generally considered that HBPM can be used to improve the awareness of hypertension for patients and to increase the blood pressure control rate in community.
6.Peripheral lung adenocarcinoma versus squamous cell carcinoma: evaluation with first-pass perfusion imaging using 64-detector row CT.

The aim of this study was to elucidate the characteristics of time attenuation curve and CT perfusion parameters for pulmonary adenocarcinomas and squamous cell carcinomas. 58 cases of pulmonary adenocarcinomas and 27 cases of squamous cell carcinomas underwent first pass CT perfusion imaging with 64-row MDCT. Data were analyzed using commercial software to generate time attenuation curve (TAC) and CT perfusion parameters, including perfusion, peak enhanced (PE), time to peak (TTP), and blood volume (BV). For TAC, there were 36.2% of type I and 63.8% of type II in adenocarcinomas, while there were 22.2% of type I and 77.8% of type II in squamous cell carcinomas. There was not significant difference (P>0.05). Perfusion, PE, TTP and BV of adenocarcinomas were 63.2 +/- 45.4 ml x min(-1) x ml(-1), 60.2 +/- 46.6 Hu, 34.8 +/- 10.2 s and 34.3 +/- 23.6 ml x 100 g(-1), respectively, while 54.3 +/- 50.2 ml x min(-1) x ml(-1), 48.5 +/- 34.9 Hu, 36.1 +/- 11.2 s and 27.6 +/- 21.7 ml x 100 g(-1), for squamous cell carcinoma, respectively. No significant differences were found between groups (P>0.05). No significant differences in TAC and CT perfusion parameters were found between adenocarcinomas and squamous cell carcinomas.

7.Expression of HIF-2α and VEGF in colorectal cancer and its relationship with prognosis

Objective To study the expression of HIF-2α and VEGF in colorectal cancer and to investigate the relationship between them and clinicopathologic parameter.Methods Immunohistochemistry staining was conducted to detect the expression of HIF-2α and VEGF protein in 67 samples of colorectal tumor tissues and 67 samples of normal adjacent tissue.Results The expression of HIF-2α and VEGF in colorectal cancer tissues was significantly higher than that in adjacent tissues.The expression of HIF-2α and VEGF increased with the clinical stage and lymph node metastasis.The expression of HIF-2α increased with the tumor volume.The expression of HIF-2α and VEGF was not related to the age,sex,tumor location and tumor differentiation of the patients.HIF-2α was positively correlated with VEGF expression.Kaplan-Meier survival analysis showed that HIF-2α expression was associated with survival,that is,the higher expression of HIF-2α the worse of prognosis was obtained.Conclusion HIF-2α is involved in the process of growth,invasion and metastasis of colorectal cancer.This process may be related to the regulation of VEGF expression.
