OBJECTIVE:To investigate the feature and tendency of the application of antihypertensive drugs in our hospital.METHODS:The utilization of antihypertensive drugs in our hospital from Jun.2004 to Dec.2006 was analyzed statistically in respect of the consumption sum and DDDs etc.RESULTS:The first line drugs used most frequently in our hospital were beta receptor antagonists,AGT Ⅱ antagonists,ACEI,a calcium antagonist etc.Leading the first place on the list of DDDs was indapamide because of its good safety and efficacy.Mesylate amlodipine,felodipine,irbesartan,perindopril,metoprolol tartrate,indapamide,bisoprolol fumarate,losartan/hydrochlorothiazide ranked among the first 10 places in either DDDs or consumption sum.CONCLUSION:The antihypertensive drugs are characterized by complicated categorization,wide variety,and widespread clinical application,and the use of which in our hospital is basically rational.

As a wide field of internet industry, the internet medical is sprouting prosperous with many advanta-ges still, namely promoting the medical resources integration and earlier disease prevention, providing more conven-ient health care services, etc. , but there are still many limitations. By analyzing the role of internet in the promotion of medical services, this article focused on the internet limitations. Ultimately, we put forward we should calmly oper-ate internet and make it as the auxiliary tool for medical care services instead of relying too much on it in order to pro-vide a reference and a basis for the prosperous development of internet medical.

ObjectivesTo investigate the effect of thyroid hormone on the expression of deiodinase and myelin basic protein in brain tissue of neonatal rats with germinal matrix-intraventricular hemorrhage (GM-IVH), and its nerve protective effect.Methods Sixty premature rabbits of gestational age 29 days were randomly divided into control, model and intervention groups. The prema-ture rabbits in model and intervention groups were given intraperitoneal injection of 50% glycerol to establish GM-IVH animal model and the premature rabbits in control group were given the same dose of saline. The GM-IVH was screened by cranial ultrasound. The premature rabbits with GM-IVH in intervention group were given intraperitoneal injection of triiodothyronine and the other two groups were given the same dose of saline. At 3 days, 7 days and 14 days after birth, the premature rabbits were killed and the brain tissue were quickly separated. The expression of type 2 iodothyronine deiodinase (D2) and type 3 iodothyronine deiodinase (D3) of periventricular zone tissue were detected by western blot method. The expression of myelin basic protein (MBP) were detected by immunohistochem-ical method.ResultsThe expression of D2 in brain tissue increased gradually in three groups. At the same time point, the expression of D2 was the lowest in the model group, the highest in the control group, and the middle in the intervention group. The differences of D2 among three groups were statistically signiifcant (allP<0.001). The expression of D3 increased gradually in the control and model groups, while it was the highest in the intervention group at day 7. At the same time point, the expression of D3 in the control group was the lowest, it was higher in intervention group than that in the model group at 3 days, then it was higher in the model group than that in the intervention group. The differences of D3 among three groups were statistically signiifcant (allP<0.001). The expression of MBP increased gradually in the control and intervention groups, and it decreased gradually in the model group. At the same time point, the MBP was the lowest in the model group, it was the highest in the control group, and it was the middle in the intervention group. The differences of MBP among three groups were statistically signiifcant (P<0.001).ConclusionsThe expressions of D2 and MBP in the brain tissues of premature rabbit with the GM-IVH increased while the expression of D3 decreased in some extent after thyroxine intervention.

Objective Feedback of 35 graduates from a Sino-Australia cooperation program was investigated at 1 -year fulfillment of clinical practice,and reflect some of the issues in the process of cultivation and utilization of nursing talents from the program. Methods 32 nursing talents from a Sino-Australia cooperation program at 1 -year fulfillment of clinical practice was selected as research object.Self made questionnaires baaed on the goal of the program were used,which included both open and close questions.The investigation results were analyzed. Results The nursing talents considered that the main advantages of the program were as followed:cultivation of their ability in active communication,improving English level,broaden international vision,and internationalization of courses. Conclusions Stratified and individualized teaching strategies need to be implemented.English study and nursing specialty study are to be appropriately combined.Routes of employment should be broadened.In addition,imported courses are to be adjusted according to domestic situation.While attaching importance in teaching,faculty should also emphasize advising.Finally,it is not to be ignored for the hospitals that they are responsible for greater utilization and cultivation of the nurses with both nursing and English ability.

Objective To evaluate the value of acoustic radiation force impulse imaging (ARFI) technique in differential diagnosis of renal tumors.Methods Totally 86 patients with 86 renal tumors underwent conventional ultrasound and ultrasound with ARFI technique.The shear wave velocity (SWV),virtual touch tissue imaging (VTI) score between tumors and the surrounding renal parenchyma,benign and malignant tumors were compared.Results In 86 patients with renal tumors,32 cases were renal benign tumors and all them were angiomyolipomas (AML),54 cases were renal malignant tumors,inculding 26 cases of renal clear cell carcinoma (ccRCC),8 cases of renal color cell carcinoma (cRCC),5 cases of renal papillary carcinoma (pRCC),15 cases of invasive urothelial carcinoma (IUC).The difference of SWV and VTI scores between lesions and the surrounding renal parenchyma were statistically significant (both P＜0.05).The SWV and VTI score renal benign tumors were lower than those of malignant tumors (both P＜0.05).The area under the ROC curve with SWV＞1.37 m/s or VTI score＞3.83 to distinguish benign and malignant renal tumors were 0.898,0.847,sensitivity were 88.9％,83.3％,specificity were 84.4％,78.1％,respectively (P＜0.05).Among renal malignant tumors,SWV and VTI score of ccRCC significantly higher than those of other malignant tumors,and the area under the ROC curve with SWV＞2.06 m/s or VTI score＞4.31 to distinguish ccRCC and other renal malignant renal tumors were 0.766,0.729,sensitivity were 65.4％,57.7％,specificity was 82.1％,78.6％,respectively (P＜0.05).Conclusion ARFI has important value in differential diagnosis of renal tumors,and can help to distinguish ccRCC with other renal malignant tumors.

Objectives By survey the head nurses or nursing instructors,to know the one year professional performance of nurses graduated from Sino-Australia cooperation program of Jinhua Polytechnic,which can help us to make the educational plan and further cultivation.MethodSelf-composed questionnaire is provided to 32 head nurses or nursing instructors from secondary and tertiary hospitals of Zhejiang province.SPSS 18 is applied for data analysis.ResultThe nurses' performance is generally consistent to the objectives ofthe Sino-Australia nursing program and the rating scores are normally distributed. There is no significant difference in the average performance of nurses comparing to a test value of 71,P≤0.08.The scores of professional quality,nursing competency,English ability,humanitarian caring,active learning,team work and adaptability to clinical responsibilities were especially well rated.ConclusionResults indicated that the school should appropriately adjust the direction for students＇ employment,consider to localize the nursing courses introduced from foreign countries.It is necessary for the hospitals to ponder on how to further cultivate and utilize the English competency of the nurses,and use the individualized strategies to retain the nurses who are proficient in English.

Purpose To investigate the relationship of infundibular bladder neck formation with pelvic floor support structure injury and urethral sphincter defect and its significance in female stress urinary incontinence.Materials and Methods The pelvic floor images of seventy-four female patients with stress urinary incontinence treated in the outpatient Department of Lanzhou University Second Hospital from April 2015 to August 2016 were analyzed retrospectively.The location of the bladder neck,posterior vesicourethral angle and the infundibular bladder neck formation were observed by the transperineal ultrasound under the resting state and the maximum Valsalva status.Meanwhile the thickness of middle urethral sphincter was measured under resting state.At the same time,eighty-one women visiting our hospital for regular physical examination were enrolled as control group.Results The infundibular urinary bladder neck formation rate (66.2％) in the stress urinary incontinence group was significantly higher than that in the control group (4.9％) under maximum Valsalva state,the difference was statistically significant (P＜0.05).The extent of the bladder neck descending and posterior vesicourethral angle in the stress urinary incontinence group were notably higher than those in the control group,both of the difference was statistically significant (P＜0.05).Stress urinary incontinence was confirmed with urethral sphincter defect by urodynamics in nine patients,in whom the infundibular bladder neck occurred.The thickness of the middle urethral sphincter in these nine patients showed no obvious difference with that in patients without sphincter defect and subjects in normal control group (P＞0.05).Conclusion The infundibular bladder neck formation,which is closely related to the pelvic floor support structure dysfunction and urethral sphincter defect,is an important indication of stress urinary incontinence.However,the assessment of urethral sphincter defect through urethral sphincter thickness need to be further studied.

Objective To investigate the effective indicators for the prognosis assessment in pa?tients with multiple myeloma (MM) by 18F?FDG PET/CT imaging. Methods A total of 36 patients(22 males, 14 females;median age 63.5 years) with MM confirmed by clinical or pathology from July 2007 to November 2014 were retrospectively reviewed. The number of lesions detected by PET/CT, the number of lesions with SUVmax>2.5, the SUVmax and MTV of each lesion were calculated. The correlation analysis was performed between the number of lesions detected by PET or CT,the number of lesions with SUVmax>2.5, the SUVmax , MTV and serumβ2?microglobulin (β2?M) , respectively. The patients were divided into differ?ent groups according to the development of lesions and the survival situation during the follow?up ( 4-92 months) . Kaplan?Meier analysis and multivariate Cox model were used to analyze the prognostic significance of the number of lesions detected by PET or CT and the number of lesions with SUVmax>2.5, the SUVmax and MTV. Results Both the number of lesions with SUVmax>2. 5 and MTV showed positive correlations with blood β2?M (r=0.776, 0.954, both P<0.001), while the number of lesions detected by PET/CT and SUVmax were not correlated with β2?M ( r=0.053, 0.063, 0.398, all P>0.05) . The number of lesions with SUVmax>2.5 and MTV in the progressive group( n=14) were significantly higher than those in the regressive group(n=22):66.57±4.59 vs 31.95±4.75, t=4.95, P<0.001;(287.54±31.94) cm3 vs (72.17±14.35) cm3, t=6.93, P<0.001. The number of lesions with SUVmax>2.5 and MTV were significantly higher in the dead group(n=15) than those in the survival group(n=21):65.73±4.32 vs 30.90±4.87, t=5.10, P<0?001;(267.28±34.89) cm3 vs (76.39±15.67) cm3, t=5.49, P<0.001. The best cutoff values for predicting pro?gression?free survival and overall survival were both 42 for the number of lesions with SUVmax>2. 5, and those were 114.74 and 105.48 cm3 for MTV, respectively. The progression?free survival rate was worse in the patients with higher index than those with lower value (χ2=18.20, 29.74, both P<0.001) , and the same re?sult was also seen for the overall survival rate (χ2=19.07, 25.34, both P<0.001) . Conclusion The number of lesions with SUVmax>2.5 and MTV on 18 F?FDG PET/CT images could predict the progression?free survival and overall survival rates of patients with MM, which may provide accurate prognosis information.

Objective To analyze the assets and liabilities of secondary public hospitals of Wuhan city in Hubei province ,in order to provide references to control the debt scale of such hospitals .Methods Twenty‐one public secondary public hospitals of Wuhan city were analyzed using official statistics on their asset and liabilities from 2012-2014 .Results Such a timeframe witnessed a year‐on‐year growth of both total assets and liabilities of these hospitals ,yet the growth rate of total assets(35.15% ) falls behind that of total liabilities (53.62% )significantly ;significantly slower growth rate of non‐current assets since 2013 ;higher current liabilities than long‐term liabilities on yearly basis ;year‐on‐year grow th of liquidity rate and quick ratio in general ;significant rise of debt‐to‐assets ratio .Conclusions The overall debt scale of such hospitals keeps growing ,a trend deserving actions and correction .

As an important part of the hierarchical medical system in China ,urban secondary hospitals play as a pivot between tertiary hospitals and community healthcare centers.By describing the overall development status of such hospitals in China ,the paper discussed the problems existing in the development of urban secondary hospitals and put forward strategies in the hope of providing reference for the transformation of these hospitals .
