Objective To explore the therapeutic strategy for aged patients (older than 70) with chest tumor. Methods To summarize and analyze the clinical data of 216 aged patients with chest tumor. Results 216 aged patients with chest tumor were involved our study,Their clinical data were summarized and analized,All patients went smoothly through perioperative period,They recovered rapidly and discharged successfully from hospital,With no death occurred,postoperative complications occurred in a total of 36 cases(16.7%), But they were cured through meticulous management. Conclusions Aged patients with chest tumor could obstain good results by strict indications,fully risk evaluation and meticulous perioperative management.

Objective To investigate the right ventricular morphology and function of patients with atrial septal defects by single beat real-time three-dimensional echocardiography.Methods Thirty-four healthy volunteers and twenty-seven ASD patients were enrolled consecutively as normal controls and the ASD group respectively.Single cardiac cycle real-time three dimensional echocardiography was performed in all to evaluate parameters concerning modality and systolic function of participants' right ventricle.The relationship of indexed right ventricular end diastolic volume (IEDVRV) and end systolic volume(IESVRV)with mean pulmonary arterial pressure measured through cardiac catheter and systolic pulmonary artery pressure calculated though the three tricuspid regurgitation were observed.Results Indexed right ventricular end diastolic volume (IEDVRv),end systolic volume(IESVRV),right ventricular stroke volume (ISVRV) and right ventrieular ejection fraction (RVEF) were greater in the ASD group than in controls (P ＜ 0.05).IEDVRv and IESVRV had positive relation with pulmonary pressure measured by cardiac catheter and systolic pulmonary artery pressure calculated though the three tricuspid regurgitation.Conclusions Single cardiac cycle real-time three dimensional echocardiography has an obvious advantage than normal test in evaluating the change of the right ventricular function and shapes,furthermore,single cardiac cycle real-time three dimensional echocardiography can indirectly determined the pressure of the pulmonary without wound.

Objective To investigate the risk factors and preventive measures for acute epidural hematoma after posterior thoracic spine surgery.Methods A retrospective study of 14 patients who developed acute epidural hematoma after thoracic spine surgery from May 2002 to May 2012 was conducted.There were 6 males and 8 females,aged from 41 to 69 years (average,61.2 years).There were 10 cases of thoracic spinal canal stenosis,3 cases of thoracic spinal meningioma,and 1 case of thoracic metastasis.About 3-14 h (average,6.6 h) after posterior thoracic spine surgery,the neurological deterioration was found,and according to the American Spinal Injury Association (ASIA) classification,there were 5 cases of grade A and 9 cases of grade B.The neurological function before evacuation of hematoma was compared with that after evacuation of hematoma and that at final follow-up.The correlations between hematoma compression time,neurological improvement rate and neurological function before evacuation of hematoma were statistically analyzed.Results After evacuation of hematoma,the ASIA classification of 14 patients was as follows:grade B in 1 case,grade C in 2 cases,grade D in 4 cases,and grade E in 7 cases.The hematoma compression time of 3 patients with grade B or C was more than 10 hours.Obvious difference of neurological function was found before and after evacuation of hematoma.The neurological improvement rate was 63.7％±23.3％ after evacuation of hematoma,which was negatively correlated with hematoma compression time and positively correlated with preoperative neurological function.The neurological function before evacuation of hematoma was significantly different from that at final follow-up.The neurological improvement rate was 86.97％±17.58％ at final follow-up,which was negatively correlated with hematoma compression time and positively correlated with preoperative neurological function.Conclusion The acute epidural hematoma after thoracic spine surgery could cause severe neurological deterioration.The neurological improvement was negatively correlated with hematoma compression time.Evacuation of hematoma must be done as soon as possible once progressive neurological deterioration is found.

Objective To summarize the etiology,classification and clinical characteristics of congenital bile duct dilatation and our experience in its diagnosis and troatment.Methods The clinical data of 37 cases of congenital bile duct dilatation were retrospectively analyzed.Results The 37 cases included TypeⅠin 31 cases,Type Ⅱ in 1 case,Type Ⅳ in 4 cases,and Type Ⅴ in 1case.All of the patients underwent operative treatment.External drainage of was done in 2 cases,Roux-en-y cystojejunostomy in 3 cases,exeisison of cyst cyst and Roux-en-y hepaticojejunostomy in 31 cases and left hepatectomy in 1case with malignant change of cyst.Five cases had postoperative complications that recovered with non-operative treatment.Thirty-two cases were cured.There were no operative deaths.Conclusions Congenital bile duct dilatation should be treated as soon as possible.Excision of the cyst and Roux-en-y hepaticojejunostomy can reduce long-term postoperative complications and is the rational treatment for congenital bile duct dilatation.

ObjectiveTo investigate the clinical features and treatment of ascending paralysis after thoracolumbar fracture.MethodsThree male patients with 2 fracture levels at T12 and one at L1 were retrospectively studied.Their mean age was 41.3 years(range,39-42 years).All 3 cases were undertaken open decompression,reduction and internal fixation.Paralysis level began to ascend at 2-5 days after injury,with 2 cases up to C2,3 and 1 case up to T7.Two patients suffered irritating pain over the paralysis level before onset of ascending.Postoperative MRI images demonstrated well reduction and no compression of spinal cord.In the early phase after ascending,MRI obviously showed swelling in spinal cord and long T1 and long T2 signals shaped patchy and stripy distribution in the central area.One patient's MRI displayed that the spinal cord shrinked 16 days after trauma with abnormal high signal in the central area.ResultsTwo cases died of respiratory muscle paralysis and 1 case suffered paraplegia with no recovery 5 years after surgery.ConclusionAscending paralysis after thoracolumbar fracture is a rare complication with very poor prognosis.MRI is available for evaluating operational effects and affected level.The exact mechanism and effective treatment are still unclear and need further investigated.

Objective To study the protective effect of Panax notoginseng saponins(PNS)and its components Rg1 and Rb1 on oxygen-glucose deprivation/reoxygenation(OGD/Reox)-induced tight junction damage. Methods Anaerobic box were used to induce OGD in HUVEC cells for 6 h followed by reoxygenation for 24 h. Transepithelial/endothelial electrical resistance(TEER)and cell permeability were detected,immunefluorescence was used to observe the ZO-1 and claudin-5 protein expression. Results PNS 20,40 mg/L and ginsenoside Rb1 significantly inhibited the OGD/Reox-induced decreased tight junction resistance,and the increased cell permeability(P< 0.05). PNS 20,40 mg/L and ginsenoside Rb1 partly restored the inter-cellular tight junctions which were regularly arranged on the cell membrane, and the cells displayed cobble stone-like arrangement. Conclusions PNS ameliorates ischemia-induced vascular endothelial cell tight junction damage via MMP-9 and VEGF/VEGFR2 signaling pathway. Rb1 is one of the effective monomer components.

Tendon injuries often need surgical treatment, which enables to repair the structure and stability of the tendons to a certain extent, whereas it is difficult to restore to their normal strength. The primary reason is that the natural healing ability of tendons is limited and the functions of the repaired tendons cannot be restored completely. As further researches on tendon healing are conducted, biological technology provides a novel orientation for tendon repair. One of the research hotspots of tendon repair currently is to facilitate tendon healing using biological auxiliaries, including tendon stem /progenitor cells(TSPCs) and growth factors. The authors review the research progress in mechanism of TSPCs and growth factors accelerating tendon healing in order to provide a reference for the biological treatment of tendon injuries.

Avian influenza virus is a new recombinant virus , which can cause severe respiratory symptoms ,such as short course,acute disease,and a high mortality rate.The purpose of this paper was to summarize the current status of this virus in terms of its epidemiology ,genetic evolution and virulence .By introducing the advancement in the research of this subtype virus, we hope to provide data and evidence ,for effective surveillance and prevention of this virus .

Objective To observe the effects of Jingning Granule on attention set shifting (AST) in rats with attention deficit hyperactivity disorder (ADHD).Methods Spontaneously hypertensive rats (SHR) were used as rat model of ADHD.Jingning granule (20.25, 10.125, 5.062 5 g/kg), Methylphenidate HCl Extended-release Tablets (Ritalin 6.75 mg/kg) and pediatric Huanglong granule (0.937 5 g/kg) were used as the intervention respectively and administered continuously for 17 days.The number of errors, burrowing times, and error rate in AST were used as the outcome measures.Results Compared with the normal control group, the number of error, error rate, and burrowing times increased in model group in Simple Discrimination (SD) phase, Compound Discrimination (CD) phase, Intra-Dimensional shift (IDS) phase, Reversal Learning (RL) phase, and Extra-Dimensional shift (EDS) phase;compared with the model group, Jingning granule reduced the frequency of error, error rate, and frequency of burrowing, and improved the attention set shifting defects in ADHD rat model.Conclusion ADHD model rats presented defected attention set shifting, and Jingning granule could effectively improve the attention set transfer ability in ADHD model rats.

Objective To use real-time three-dimensional echocardiography(RT-3DE) to compare the cardiac function related indicators between before operation and at postoperative 2 months in the patients with left ventricular aneurysm(LVA) undergoing emergency percutaneous coronary intervention(PCI),and to assess the PCI short term effect.Methods RT-3DE was applied to observe the left ventricular end-diastolic volume(LVEDV),end systolic volume(LVESV),cardiac output(CO),end-diastolic volume index(EDVI),end systolic volume index (ESVI),cardiac index (CI),left ventricular ejection fraction (LVEF) and left ventricular spherical index(SPI) before operation and at postoperative 2 months in 31 cases of acute anterior myocardial infarction(AAMI) complicating LVA.The patients were divided into the functional LVA group(A),anatomical LVA group (B) and complicating thrombus LVA group(C).The cardiac function parameters in various groups conducted the intra-group and inter group comparisons.Results Compared with before operation,LVEF at postoperative 2 months in the group A was increased and SPI was decreased(P＜0.05);CO,CI and LVEF before operation and at postoperative 2 months in the group B were increased and SPI was decreased(P＜0.05);SPI at postoperative 2 months in the group C was decreased(P＜0.05);EF and SPI before operation and at postoperative 2 months had no statistical difference among 3 groups(P＞0.05).SPI before operation and at postoperative 2 months was negatively correlated with LVEF and positively correlated with EDV,ESV,EDVI,ESVI,CO and CI.Conclusion The left ventricular morphology and overall systolic function at 2 months after PCI in the patients with functional and anatomical LVA are improved,but the curative effect in the patients with complicating thrombus LVA is unobvious.
