1.Association of HLA-DRB1 allele polymorphism and ANCA with the susceptibility to antithyroid drug-induced leucocytopenia in patients with Graves' disease

ObjectiveTo investigate the relationship of HLA-DRB1 allele polymorphism and antineutropil cytoplasmic antibody ( ANCA ) with the susceptibility to antithyroid drug ( ATD ) -induced leucocytopenia in the patients with Graves' disease (GD) in Anhui Han Chinese.MethodsThe HLA-DRB1 * 08032,DRB1 * 1501,and DRB1 * 0901 allele frequencies were determined by the polymerase chain reaction-based sequence-specific primer (PCR-SSP) method in 76 patients with Graves' disease who had ATD-induced leucocytopenia and 98 patients with Graves' disease treated with ATD,who were free from leucocytopenia.The other230 healthy subjects served as controls.Indirect immunofluorescence assay (IIF) was used to detect ANCA positive rate.Result( 1 ) Compared with the controls and the GD patients without leucocytopenia,the allele frequencies ofDRB1 * 08032 and DRB1 * 1501 in patients with ATD-induced leucocytopenia were significantly increased ( OR were 3.06,1.77,4.03,and 2.28,all P<0.05),while that of HLA-DRB1 * 0901 was decreased ( OR were 0.33 and 0.43,both P<0.05 ).(2)The ANCA frequencies were significantly increased in the GD patients with methimazole-induced leucocytopenia compared with those without leucocytopenia (x2 =4.878,P<0.05 ).( 3 ) Compared with the GD patients not carrying DRB1 * 08032,DRB1 * 1501,and DRB1 * 0901 alleles,the ANCA positive rates were significantly increased in the GD patients carrying these alleles(x2 were 5.682,5.429,4.009,and 4.549,all P<0.05).ConclusionsThe DRB1 * 08032 and HLA-DRB1 * 1501 alleles may be susceptible to ATD-induced leucocytopenia in Anhui Hans,while HLA-DRB1 * 0901 alleles may be protective or resistant gene.Immune response may be involved in the development of leucocytopenia.The occurrence of immune response is based on the genetic susceptibility.
2.Mucosa-associated lymphoid tissue malignant lymphoma of the breast:A clinicopathological study

To study clinicopathological characteristic of the breast mucosa-associated lymphoid tissue-type-lymphoma (MALT-ML). MethodsFour cases of the breast MALT-ML were studied by HE staining and immunohistochemical technique (ABC method). ResultsOf 4 cases breast MALT-ML, 2 were CCL type, 1 CCL type with blastoid transformation, monotoid Bcell type. By immunohistochemistry, all cases were monoclonity of B-cell with follicular colonization and lymphoepithelia lesions.ConclusionsBreast MALT-ML has a special pathological morphology.

The cost accounting plays central role in enhancing financial management in the hospital. With the increasing hospital competition, conventional model of cost accounting is changing, and gradually replaced with a novel model of cost accounting-total cost accounting. With the wide spread of total cost accounting in all hospitals, it is found that the general operation of hospitals is limited by the original HIS information system. EPR system feathered with the advanced data administration and potent function of financial analysis holding promise in adoption and acceptance by hospitals, serving as information collector and analyzer following HIS system.

5.Mantle cell lymphoma primarily occured in lymph nodes:A clinicopathologic study

Purpose To study the clinicopathologic and immunohistochemical characteristics of mantle cell lymphomas(MCL). Methods Six cases of MCL in lymph nodes were collected and immunohistochemical stains for CD45,CD20,CD79,CD45RO,CD30,CD68,CD43,CD5,cyclinD1,bcl-2,c-myc,IgD and IgM performed. Results According to the histological pattern, MCL could be divided into 4 subtypes, mantle zone type(1 case), nodular type (1 case), diffuse type (2 cases) and blastoid type (2 cases). The tumor cells were pan-B+, IgD+, CD43+, cyclinD1 + (5/6),CD5+(4/6). Conclusions MCL is a kind of special immunophenotypic B cell lymphoma. The different histiological subtype may have a different prognosis. The differential diagnosis includes the others B cell lymphomas, for instance, marginal zone B cell lymphomas (MZL),follicular lymphomas(FL), and choronic lymphocytic leukemia/small lymphocytic lymphomas (CLL/SLL).
6.Lymph node marginal zone B-cell lymphoma:A clinicopathological analysis

Purpose To study the pathological characteristics, diagnostic criteria and differential diagnoses of the lymph node marginal zone B-cell lymphoma (LMZL), for the clinical treatment and prognosis. Methods Histopathology and immunohistochemistry(ABC method) were used in ten cases of LMZL. Results There were six male and four female. Lymphadenopathy was the main symptom in all of the cases, of which none had hepatosplenomegaly and abnormal lymphocytes in the peripheral blood. Two of them were marginal zone type, four nodular type, two diffuse type and two blastic type morphologically. Five of them were central cell like (CCL) type, six monocytic B-cell (MBC), two of them lymphoplasmacytic and two blastic histopathologically. Immunophenotyping was done in ten cases and supported the diagnosis. Conclusions LMZL and MALT-type lymphoma have similar appearance, immunophenotype and cell origin. Based on the structural features, LMZL is a special entity.
7.Role of peritoneal transport characteristics and nutritional status in evaluation of peritoneal dialysis adequacy

Objective To evaluate the influence of peritoneal transport characteristics on nutritional status of peritoneal dialysis patients and estimate the role of peritoneal transport characteristics and nutritional status in evaluation of peritoneal dialysis adequacy. Methods Peritoneal transport properties (D/Pcr) were evaluated by Short Peritoneal Equilibration Test and then 53 cases were divided into high transport group (D/Pcr ≥0. 65) and low transport group (D/Pcr < 0. 65). Urea kinetics (Kt/v) and total clearance of creatinine (TCcr) were calcu- lated. Serum albumin (ALB), prealbumin, transferring (TF), hemoglobin (Hb), lean body mass (LBM) and LBM% were examined or calculated. Subjective global assessment (SGA) was used to evaluate the nutritional sta- tus of patients. Results There were 30 cases in high transport group and 23 cases in low transport group. D/Pcr (0. 82±0.15 vs. 0. 55±0. 08, P < 0. 01), TCcr (62. 93%±25. 98% vs. 49. 69%±16. 92% , P < 0. 05) and age were significantly higher in high transport group than those in low transport group, while ALB, TF, Hb, LBM% , and SGA were significantly lower than those in low transport group (P < 0.05). ALB was negatively cor-related with Kt/v (r= -0.2708, P<0. 05). Conclusions High peritoneal transport patients have better solute clearance but worse nourishment status compared with low transport ones. Nutritional status is one of the factors to evaluate dialysis adequacy. Nutrition management and guidance should be strengthened for the continuous ambula- tory peritoneal dialysis patients, especially for the elderly ones.

Objective To detect the expression of inducible nitric oxide synthase (iNOS) and heme oxygenase-1(HO-1) in human colon of irritable bowel syndrome(IBS), aim at finding out the role of gas messenger molecule such as nitric oxide(NO) and carbon monoxide(CO) in the pathogenesis of IBS. 9.Association between phthalate ester exposure and population obesity:A Meta-analysis

Objective:To evaluate the relationship between the phthalate ester exposure and the population obesity with Meta-analysis, and to provide a new idea for prevention and control of obesity. Methods:A comprehensive search was performed in English databases (Pubmed, Web of Science, The Cochrane Library, Elsevier Science Direct and OVID) and Chinese databases (Sinomed database, CNKI database, VIP database, Wanfang database).The studies about the relationship between phthalate ester exposure and the population obesity were retriveded.The Chinese and English studies were selected according to the inclusion criteria and exclusion criteria.Meta-analysis was performed using RevMan 5.3 software.Results:Six studies were finally obtained, involving 1259 samples.The Meta-analysis results showed that the monobutyl phthalate (MBP) level in urine of the obesity population was increased 4.1 times compared with the normal population (95%CI:1.43-6.76);while the combined effect values of dibutyl phthalate (DBP), di-2-ethylhexyl phthalate (DEHP) and diethyl phthalate (DEP) level in serum of the population in two groups were 1.17 (95%CI:0.64-1.69), 0.80 (95%CI:0.13-1.48), and 0.72 (95%CI:-0.19-1.63);the combined effect values of monoethylhexyl phthalate (MEHP) and monoethyl phthalate (MEP) levels in urine were 1.75 (95%CI:-0.45-3.96) and 2.75 (95%CI: 0.36-5.15);there were no significant differences (P>0.05).Conclusion:The elevated MBP levels in the urine may be a risk factor for obesity in the population, suggesting that MBP may contribute to obesity.
10.OBSERVATIONS ON THE NOS POSITIVE NEURONS IN HIRSCHSPRUNG DISEASE BY LIGHT AND ELECTRON MICROSCOPY

Objective To explore the etiology and pathogenesis of Hirschsprung disease(HD) and the role of nitric oxide in pathophysiology of HD. Methods The whole amount preparations of dilative and transitional segments were stained by NADPH\|diaphorase histochemistry mothod,and colonic walls were digested by dispase. Then the NOS positive nervous structure were observed by light and scanning electron microscope. Results In dilative segment, under light microscope, ganglion and neuron were bigger and more darkly stained. Intraganglionic neurons distributed mostly in peripheral area of ganglion and basal part of ganglionic fiber bundles. Under scanning electron microscope, the neurons were denser, more nerve fibers from them and connected with each other in all dimensions. And more transverse connected fibers between neurons which were arranged along muscle fibers. Myenteric plexus even connected with the submucosal plexus by nerve fibers which passed through circular muscle layer. In transitional segment, under light microscope, intraganglionic neurons cytoplasm were lightly stained and also variedly. The ganglion and neurons of the segment were smaller, and the fibers from them were thinner and paler than those of the dilative segment. Under scanning electron microscope, neurons' density was lower, the fiber connection between neurons and muscle fibers/neurons was lesser. In the transitional segment, neurons and nerve fibers were distributed linearly along the longitudinal muscle fibers. Conclusion Our results indicated that an intimate relationship existed between the development of Hirschsprung disease and abnormal distribution and metabolism of NOS positive neuron in colonic wall.
