[Objective]To evaluate the outcomes of treatment of unstable pelvic fractures with complex fixation.[Method]Twenty-eight cases of patients with unstable pelvic fracture from January 2000 to June 2007 were treated and followed-up.Eighteen cases were male,10 cases were female,aged from 22 to 68 years old,with the average of 38-years-old.The types of fracture were classified by Tile classification:Type B2 9 cases,Type B3 8 cases,Type C1 6 cases,Type C2 2cases,Type C3 3 cases.The 28 cases had different complicated injury,all the patients were treated with complex fixation.[Result]After being evaluated by the Matta scale and the functional outcome,the outcomes of reduction were excellent in 18 cases,good in 7,fair in 2 and poor in 1,with an excellent and good rate of 85%.[Conclusion]External fixation can win the time for the rescue,choose internal fixation after stable condition,the complex fixation technology is a good way to treat unstable pelvic fractures.

Poly (N-isopropylacrylamide) (PNIPAAm), a temperature-responsive polymer, can be potentially applied to replace enzymes or cell scrapers to recover attached cells. Taking full advantage of this unique function of PNIPAAm, cells can be protected from enzymatic hydrolysis and mechanical treatment, thereby to provide ideal seed cells with high quality for biomedical fields. In this review we describe the method to facilitate cell effective adhesion and rapid detachment on thermoresponsive two dimensional surfaces, including selecting special substrate, introducing hydrophilic group, adjusting reactant ratio, controlling polymer thickness/density, providing appropriate external force, so as to effectively improve adherent cell adaptability to thermoresponsive surfaces, depress the risk of bacterial contamination and reduce the effect of low-temperature treatment on the cells. The three dimensional cell culture systems involved in temperature-sensitive microcarriers, scaffolds and gels were briefly discussed. The application based on the platforms for cell culture was also presented.

Objective To study the correlation between osteoporosis and degeneration of intervertebral disc in the aging rat.Methods Fifty healthy SD rats were fed for twenty-two months in order to build the model of aging rats,the histology of femoral bone,tibial bone and lumbal vertebra was used to observe,then we evaluated the indexes which reflect the degeneration of intervertebral disc including the quantity of collagen type Ⅱ,Ⅹ of intervertebral disc by the immunohistochemistry methods,the relative area of vascular bud,the thickness of calcified layer and un-calcified layer on cartilage endplate and the indexes which reflect the osteoporosis including the thickness ratio of cortical bone,the relative area of bone trabecula,the bone density,the max stress and straining of bone.Results There was moderate or strong negative correlation among the indexes of the degeneration of intervertebral disc including the relative area of vascular bud,the ratio of the un-calcified and the calcified layer,the quantitation of collagen type Ⅱand the indexes of osteoporosis including the thickness ratio of cortical bone,the relative area of bone trabecula,the density of femoral bone and vertebral body bone,the max stress and straining of bone.However,there was moderate or strong positive correlation with the quantitation of collagen type Ⅹ,of which the bone density of vertebral body had the strongest dependablity with collagen type Ⅹ.And the indexes of bone biomechanics including the max stress and straining had no correlation with the indexes of degeneration of intervertebral disc.Conclusion Osteoporosis has negative correlation with the degeneration of intervertebral disc,which is not distributed to the environment factors;but the indexes of bone biomechanics including the max stress and straining have no correlation with the indexes of degeneration of intervertebral disc.

To study the effect of sphingosine-1-phosphate (S1P) on the cardiomyogenic differentiation of human umbilical cord mesenchymal stem cells (UC-MSCs) and human adipose-derived mesenchymal stem cells (AD-MSCs), we seeded the cells in the culture plates and used cardiomyocyte culture medium (CMCM) combining with different concentration of S1P to induce UC-MSCs and AD-MSCs in vitro for 7, 14 and 28 days. Cardiomyogenic differentiations were identified through immunofluorescence staining, and the results were observed with fluorescence microscopy and confocal microscopy. The effects of S1P and CMCM on cell activity were evaluated by the methyl thiazolyl tetrazolium assay. The functional characteristic similar to cardiomyocytes was evaluated through detecting calcium transient. Our results showed that cardiomyogenic differentiation of UC-MSCs or AD-MSCs were enhanced with S1P concentration increasing, but cell activities declined. Results showed that the suitable differentiation time was 14 days, and the optimal concentration of S1P was 0.5 micromol/L. When working together with CMCM, S1P could promote the differentiation of UC-MSCs or AD-MSCs into functional cardiomyocytes, giving rise to specific electrophysiological properties (the calcium transient). Taken together, our results suggested that S1P could promote the differentiation of UC-MSCs or AD-MSCs into functional cardiomyocytes when being cultured in CMCM.

Objective To investigate the effect of prevascularized tissue-engineered bone graft on regeneration of femoral bone defects in rats.Methods Models of femoral bone defect were created at the bilateral hind limbs of 20 healthy female 10 week-old rats which were divided into 2 even groups randomly (n =10).In group A,conventional tissue-engineered bone grafts were transplanted into the femoral bone defects;in group B,tissue-engineered bone grafts and vascular bundles were implanted into the femoral defects.At 1,4 and 8 weeks after operation,3 rats were sacrificed each time in each group to harvest samples.The remaining one in each group served as a spare animal.Regeneration of bone defects and degradation of scaffolds were assessed by radiologic modality and hematein eosin staining.Results At week 1,the new bone ratio (BV/TV) was 5.47％ ± 1.90％ in group A and 8.49％ ± 1.26％ in group B,showing no significant difference (P ＞ 0.05);at weeks 4 & 8,the BV/TV were 17.54％ ±2.04％ and 39.73％ ± 4.01％ in group A,significantly lower than those in group B (25.32％ ± 2.15％ and 53.22％ ± 2.94％) (P ＜ 0.05).At weeks 1 & 4,the scaffold degradation ratios (RSV/SV) were 97.33％ ± 2.52％ and 80.60％ ±4.00％,showing no significant differences from those in group B (95.67％ ±3.51％ and 75.22％ ±6.20％) (P ＞ 0.05).At week 8,the scaffold degradation ratio in group A (65.46％ ±4.51％) was significantly higher than that in group B (50.19％ ±4.91％) (P ＜ 0.05).At week 8,hematein eosin staining showed better integration of scaffolds with the femur,faster degradation of the interior scaffolds and greater osteogenetic activity in group B.Conclusion Prevascularization of tissue-engineered bone graft may increase new bone volume and scaffold degradation rate,promoting repair of femoral bone defects in rats.

Objective To understand the knowledge,attitude and behavior on health hazards of haze and secondhand smoke exposure among middle school students in Beijing,and provide us with guidance to carry out effective health education for middle school students.Methods A total of 1 294 students were selected randomly using stratified cluster sampling method;1 085 valid questionnaires were recovered.Among them,739 were junior high school students and 346 were senior high school students.The questionnaire included questions on middle school students' knowledge on haze and secondhand smoke exposure,their attitudes towards the judgement of severity of haze and secondhand smoke exposure and their utilization of protective measures when they are exposed to haze or secondhand smoke.Pearson's chi-squared test was performed to assess differences in knowledge,attitude and behavior between junior middle school students and senior high school students,boys and girls.Results Middle-school students' awareness rate on "the haze and exposure to secondhand smoke may cause respiratory disease" was the highest,which was 93.55％ and 95.76％ respectively.The awareness rate of the fact that haze,secondhand smoke exposure can cause cardiovascular disease was 38.99％ and 55.85％ respectively;while the rate of their awareness on the haze and exposure to secondhand smoke can lead to cancer was the lowest,which was 54.10％ and 69.95％ respectively.The differences was significant in the awareness rate of the fact that respiratory disease,cardio-cerebrovascular disease and cancer can be caused by the haze (x2=732.539,P＜0.05),likewise the differences were significant in the awareness rate of the above three kinds of diseases can be caused by secondhand smoke exposure (x2=460.291,P＜0.05).In the comparison between the severity of haze and smoking hazards,35.94％ of students believe that haze hazards is heavier than tobacco;48.02％ of the students think that the dangers of smoking is greater than haze;16.04％ of the students cannot determine the extent of damage of the haze and smoke.The proportion of students who think smoking was more harmful than haze was higher in junior high school group than in senior high school group.The two groups had a significant difference in the understanding of the severity comparison between haze and smoking (x2=13.014,P＜0.05).The differences between boys and girls in the view of the severity comparison between haze and smoking are also statistically significant,more boys believed smoking was more harmful than haze (x2=9.650,P＜0.05).In the aspect of protection from haze and secondhand smoke exposure,86.36％ of the students would like to protect themselves from haze,while 55.94％ of the students would take measures to prevent the secondhand smoke exposure initiatively,the junior high school students performed better than the senior high school students in this two aspects(x2=17.125,P＜0.05;x2=45.785,P＜0.05).Conclusion While middle school students have a certain understanding of the impact of haze and secondhand smoke exposure on health hazards,their understanding is limited.They have not paid enough attention to the harm of secondhand smoke exposure and their awareness of active refusal to secondhand smoke exposure needs to be improved.Schools should strengthen the health education for students and educate them not only to stay away from haze but also should be far away from secondhand smoke exposure.

When the size of a neurosphere cultured in vitro reaches a certain critical value, a necrotic core will appear inside the neurosphere because of the limitation of oxygen or other nutrients transport from medium to the cells in the neurasphere. Large necrotic core will greatly reduce the expansion of NSCs. The cellular automaton (CA) model is applied in this article to model the growth of NSCs in sphere state. The appearance and enlargement of the necrotic core in a neurosphere is calculated by coupling the CA model with the nutrient diffusion analysis in bioreactors. The calculation results indicate that the culture conditions, such as seeding density, the concentration of nutrients in medium and the mass transfer coefficient between a neurosphere and medium, have some effects on the appearance of the necrotic core. However, the necrotic core mainly depends on the inner diffusion. It will certainly appear if the size of the neurosphere is large enough even the outside mass transfer is in a good condition in bioreactors. Additionally, the appearance of the necrotic core resulting from the shortage of oxygen is earlier than that caused by the limitation of glucose. And the growth of the necrotic core is very fast after its appearance, and the whole neurosphere may become necrotic. The model developed with cellular automaton and mass transfer is a good qualitative representation of NSCs growth in bioreactors.

With the enormous resources having been invested in oncology drugs development in China in recent years, the Center for Drug Evaluation (CDE) of National Medical Products Administration has been issuing a number of technical guidelines to further standardize the requirements on implementation and registration of domestic oncology clinical trials. As data is the cornerstone of clinical trials, data integrity and quality will directly decide the outcome of clinical studies. Given the specific characteristics of oncology therapeutic clinical trials, and combined with the clinical data standards established by the Clinical Data Interchange Standards Consortium (CDISC) and the issued industrial guidelines, this article introduces the general considerations of clinical data management for oncology clinical trials, with the aim of emphasizing normative data collection and timely data monitoring to ensure the data quality and reliability of results of the study. This article discusses the impact of complex study design on CRF, design CRF according to CDASH, develop DVP scientifically, rolling submissions and data cut-off.

PURPOSE: c-Met and its ligand, hepatocyte growth factor (HGF), play a critical role in oncogenesis and metastatic progression. The aim of this study was to identify inhibited enzymogram and to test the antitumor activity of SIM-89 (a c-Met receptor tyrosine kinase inhibitor) in non-small cell lung cancer. MATERIALS AND METHODS: Z′-LYTE kinase assay was employed to screen the kinase enzymogram, and mechanism of action (MOA) analysis was used to identify the inhibited kinases. Cell proliferation was then analyzed by CCK8 assay, and cell migration was determined by transwell assay. The gene expression and the phosphorylation of c-Met were examined by realtime-PCR and western blotting, respectively. Finally, the secretion of HGF was detected by ELISA assay. RESULTS: c-Met, activated protein kinase (AMPK), and tyrosine kinase A (TRKA) were inhibited by SIM-89 with the IC₅₀ values of 297 nmol/L, 1.31 µmol/L, and 150.2 nmol/L, respectively. SIM-89 exerted adenosine triphosphate (ATP) competitive inhibition on c-Met. Moreover, the expressions of STAT1, JAK1, and c-Met in H460 cells were decreased by SIM-89 treatment, and c-Met phosphorylation was suppressed in A549, H441, H1299, and B16F10 cells by the treatment. In addition, SIM-89 treatment significantly decreased the level of HGF, which accounted for the activation of c-Met receptor tyrosine kinase. Finally, we showed cell proliferation inhibition and cell migration suppression in H460 and H1299 cells after SIM-89 treatment. CONCLUSION: In conclusion, SIM-89 inhibits tumor cell proliferation, migration and HGF autocrine, suggesting it's potential antitumor activity.

BACKGROUNDDendritic cell (DC)-based immunotherapy is a new approach and effective for some malignant tumors. The aim of this study is to observe the efficacy and toxicity of immunotherapy with carcinoembryonic antigen (CEA) peptide-pulsed DCs in patients with refractory advanced lung cancer.

METHODSLung cancer patients with high CEA expression were enrolled into this project. Autologous DCs were generated from patients' plastic-adherent peripheral blood mononuclear cells and loaded with CEA 5 days later. Cytokine-induced killer cells (CIK) were cultured from non-adherent peripheral blood mononuclear cells. DCs and CIK were transfused to patients. Responses and toxicities were observed.

RESULTSA total of 22 patients with lung cancer received DCs immunotherapy. DCs doses were 2.5×10⁶-9.6×10⁷ (5.03×10⁶). CIK doses were 3.4×10⁸-46×10⁸. CD3, CD8, NK and IFN-γ levels obviously increased after treatment (P < 0.05). The 1-year survival rate was 68.2% (15/22). Main toxicities were fever and rash.

CONCLUSIONSDCs-based immunotherapy is feasible and safe to patients with lung cancer.
