1.Effects of Panax quinquefolium saponin on mitochondrial membrane po-tential and cardiomyocyte apoptosis in ischemia/reperfusion rats

AIM:To investigate whether mitochondrial membrane potential (ΔΨm ) and the mitochondrial ap-optotic pathway are involved in the protective mechanism of Panax quinquefolium saponin ( PQS) against cardiomyocyte ap-optosis after ischemia/reperfusion ( I/R) injury in rat myocardium .METHODS: Ninety healthy male SD rats were ran-domly divided into sham group , I/R group, PQS (200 mg· kg -1 · d-1 ) +I/R group, cyclosporine A ( CsA) group, CsA (10 mg· kg-1 ) +I/R group and PQS +CsA +I/R group.The model of myocardial I/R injury in vivo was established by ligating the left anterior descending artery ( LAD) for 30 min followed by 120 min of reperfusion in the rats .The serum ac-tivity of lactate dehydrogenase ( LDH ) was measured by automatic chemistry analyzer .The myocardial infarct size was measured by Evans blue and 2,3,5-triphenyltetrazolium chloride (TTC) staining.Cardiomyocyte apoptosis was detected by in situ TDT-mediated dUTP nick end labeling (TUNEL).The protein levels of Bcl-2, Bax, cleaved caspase-3 and cytosol-ic cytochrome C were determined by Western blotting .ΔΨm was measured by laser scanning confocal microscopy and fluo-rescence microplate reader .RESULTS:Compared with I/R group, the serum content of LDH ,the infarction size in PQS+I/R group, CsA+I/R group and PQS +CsA+I/R group and the myocardial apoptotic index were decreased .Compared with I/R group, the fluorescence intensity of mitochondria after JC-1 staining was enhanced in PQS +I/R group, CsA+I/R group and PQS+CsA+I/R group, and the relative fluorescence units (RFU) ofΔΨm were improved in those 3 groups. In PQS+I/R group, CsA+I/R group and PQS+CsA+I/R group, the protein expression of Bcl-2 was increased, and that of Bax was decreased compared with I/R group.Moreover, in those 3 groups, the protein levels of cleaved-caspase-3 and cytosolic cytochrome C were decreased compared to I /R group, respectively.CONCLUSION:PQS attenuates myocardial injury and cardiomyocyte apoptosis during I /R, and the protective mechanisms of PQS were associated with the modulation ofΔΨm and the inhibition of mitochondrial apoptosis pathway .
2.PQS attenuates cardiomyocyte apoptosis induced by thapsigargin through inhibiting endoplasmic reticulum stress

AIM:To study the effect of Panax quinquefolium saponin (PQS) on cardiomyocyte apoptosis in-duced by thapsigargin ( TG) .METHODS:Primary cultured cardiomyocytes from neonatal SD rats were divided into con-trol group, TG group, PQS (40 mg/L, 80 mg/L and 160 mg/L) +TG group, si-PERK+TG group, and mock+TG group.The cells were treated with 1 μmol/L TG for 24 h to induce apoptosis.The PERK gene in the cardiomyocytes was knocked down by RNAi.The cell viability was detected by CCK-8 assay.Apoptosis was analyzed by flow cytometry.Wes-tern blotting was used to determine the expression of ERS molecules GRP78, CRT, ATF4 and CHOP, anti-apoptosis pro-tein Bcl-2 and pro-apoptosis protein Bax.RESULTS: Compared with control group, TG significantly and the apoptosis, reduced the cell viability (P<0.05), increased the phosphorylation of PERK and eIF2α, increased the expression of GRP78, CRT, ATF4, CHOP and pro-apoptosis protein Bax, and decreased the expression of anti-apoptosis protein Bcl-2 (P<0.05).Compared with TG group, PQS treatment (160 mg/L) significantly reduced the apoptosis and increased the cell viability (P<0.05).All the 3 different concentrations of PQS significantly increased the expression of anti-apoptosis protein Bcl-2 and reduced the expression of pro-apoptosis protein Bax (P<0.05) in a dose-dependent manner.PQS pre-treatment and knockdown of PERK both reduced the protein levels of GRP78, CRT, PERK, p-PERK, eIF2α, p-eIF2α, ATF4, CHOP and pro-apoptosis protein Bax, and increased the expression of anti-apoptosis protein Bcl-2 (P<0.05). CONCLUSION:PQS at concentration of 160 mg/L attenuated cardiomyocyte apoptosis induced by TG.PQS had the simi-lar effect as PERK knockdown on cardiomyocyte apoptosis.The mechanism may be associated with inhibiting PERK-eIF2α-ATF4-CHOP pathway of ERS-related apoptosis.
3.Research progress of TCM non-drug therapy in gastroesophageal reflux disease

Traditional Chinese Medicine (TCM) non-drug therapy for gastroesophageal reflux mainly includes appropriate TCM techniques, such as conventional acupuncture, moxibustion, fire needle, etc. Emotional therapy, such as regulating emotion, transforming the patient's spirit and change the state of qi, the five-element music therapy, etc; exercise therapy, such as Baduanjin and Zhanzhuang, can be used alone, or in combination, or in combination with oral administration of Chinese materia medica. By reducing the probability of esophageal sphincter relaxation, inhibiting gastric acid secretion, improving esophageal motility disorder, reducing visceral hypersensitivity, immune regulation and other effects, it can alleviate the symptoms of acid reflux, heartburn and pharyngeal discomfort, and help to improve the patients' anxiety, depression and other negative emotions. It has the characteristics of simplicity and fewer adverse reactions, at the same time, according to the patient's condition and compliance to choose the appropriate therapy. The mechanism of TCM non-drug therapy in the treatment of this disease needs to be further explored, so as to better guide clinical popularization and application.
4.Application progresses of new ultrasonic technologies for assisting diagnosis of BI-RADS 4 lesions

The ultrasonic manifestations of benign and malignant breast imaging-reporting and data system(BI-RADS)4 lesions overlap in some degrees,is able to result in unnecessary biopsy or untimely therapy.Accurate classifying the nature of BI-RADS 4 breast lesions can provide reliable references for clinical decision-making.The progresses of application of new ultrasonic technologies,including automated breast volume scanner,superb micro-vascular imaging,elastography,contrast-enhanced ultrasound and artificial intelligence for assisting diagnosis of BI-RADS 4 lesions were reviewed in this article.
5.Effects of case management mode combined with enhanced recovery after surgery nursing in the perioperative period of patients undergoing hip replacement

Objective:To explore the effect of case management mode combined with enhanced recovery after surgery (ERAS) nursing in the perioperative period of hip replacement patients.Methods:From January 2018 to May 2020, cluster sampling was used to select 110 patients undergoing hip replacement who were admitted to the Department of Orthopedics, Nanjing First Hospital as the research object. According to the odd and even number of admission order, patients were divided into observation group and control group. After epidural anesthesia, patients in both groups were treated with posterolateral hip incision approach. Control group carried out ERAS nursing, and observation group implemented case management mode combined with ERAS nursing for perioperative management. We observed the intraoperative blood loss, postoperative time in bed, postoperative hospital stay, hospitalization expenses of patients between two groups, and compared the Harris score and Visual Analogue Scale (VAS) of the two groups score before surgery, one day, one month and three months after surgery.Results:The intraoperative blood loss, postoperative time in bed, postoperative hospital stay and hospitalization expenses of observation group were lower than those of control group; the Harris scores of observation group before surgery, one day, one month and three months after surgery were higher than those of control group; the VAS scores of patients in observation group were lower than those of control group at one day, one month and three months after surgery; the differences were all statistically significant ( P<0.05) . Conclusions:Case management mode combined with ERAS nursing can accelerate the postoperative rehabilitation of patients undergoing hip replacement, shorten the hospital stay, reduce the hospitalization expense, and improve the hip joint Harris score of patients.
6.Experiences of LI Qiangou in Treating Laryngopharyngeal Reflux based on the Theory of "Yang Transforming Qi While Yin Constituting Form"

To summarize LI Qiangou's clinical experience in treating laryngopharyngeal reflux disease (LPRD) based on the theory of "yang transforming qi while yin constituting form". It is believed that "insufficiency of yang transforming qi and excess of yin constituting form" is the key mechanism of LPRD, in which yang deficiency of the lungs, spleen and kidneys is an important factor causing "insufficiency of yang transforming qi", while phlegm, dampness and other tangible yin turbidities are the pathological products of "excess of yin constituting form". The key treatment principle is to reinforce yang qi and dispel yin turbidity. Starting from regulating the qi transformation of the sanjiao, the treatment is based on the different pathologies of lung yang insufficiency and phlegm congestion in the upper jiao, spleen yang failing to ascend and dampness accumulation in the middle jiao, and kidney yang deficiency and phlegm-fluid retention in the lower jiao, prescribed by modified Suzi Jiangqi Decoction (苏子降气汤), Shengyang Chushi Decoction (升阳除湿汤), and Jingui Shenqi Pill (金匮肾气丸), the prescriptions could also combine with Banxia Houpo Decoction (半夏厚朴汤) to dissolve phlegm and relieve pharyngeal pains, and regulate qi to direct counterflow downward.
7.Baitouweng Tang Suppresses Colorectal Cancer HCT116 Cells by Regulating Hedgehog Signaling Pathway

ObjectiveTo explore the effect of Baitouweng Tang (BTWT) on the apoptosis of human colorectal cancer HCT116 cells and decipher the underlying mechanism based on the Hedgehog (Hh) signaling pathway. MethodHCT116 cells were treated with BTWT (25, 50, 100, 200, 500, 750, and 1 000 mg·L-1) for 24 h, and then the cell proliferation was detected by methyl thiazolyl tetrazolium (MTT) colorimetry. Five groups were designed for the treatment of HCT116 cells, including a blank control group, BTWT groups (125, 250, and 500 mg·L-1), and a positive control (5-fluorouracil, 5-FU, 40 mmol·L-1) group. The cell morphology was observed under an inverted microscope. The migration of the cells was detected by scratch test, and the apoptosis by Hoechest 33324/propidium iodide (PI) staining and flow cytometry. Western blot was employed to determine the protein levels of sonic hedgehog (SHh), GLI family zinc finger protein 1 (Gli1), smoothened (Smo), suppressor of fused (SuFu), cellular-myelocytomatosis viral oncogene (c-Myc), and the apoptosis-related proteins B-cell lymphoma-2 (Bcl-2) and Bcl-2-associated X protein (Bax). The quantitative real-time reverse transcription PCR (Real-time PCR) was employed to determine the mRNA levels of Bax, Bcl-2, SHh, Gli1, Smo, SuFu, and c-Myc. ResultCompared with the blank control group, BTWT changed the cell morphology (making the cell become round with dense nucleus), inhibited the proliferation of HCT116 cells in a dose-dependent manner, decreased the ability of migration (P<0.05, P<0.01), and increased apoptotic cells. Compared with the blank control group, BTWT (500 mg·L-1) treatment for 24 h up-regulated the protein and mRNA levels of Bax (P<0.05, P<0.01) and down-regulated the protein and mRNA levels of Bcl-2 in HCT116 cells (P<0.05, P<0.01). Moreover, the treatment down-regulated the mRNA and protein levels of SHh, Gli1, Smo, and c-Myc (P<0.05, P<0.01) and up-regulated the mRNA and protein levels of SuFu (P<0.05, P<0.01). ConclusionBTWT inhibited the proliferation and migration and induced the apoptosis of colorectal cancer HCT116 cells by down-regulating the Hh signaling pathway.
