1.Comparative Study on Multislice CT Angiography and Digital Substraction Angiography in Diagnosis of Intracranial Aneurysm

Objective To evaluate the value of multislice spiral CTA in diagnosis of intracranial aneurysm.Methods CTA data in 32 patients with intracranial aneurysms proved by operation and DSA were retrospectively analysed in comparison with the results of DSA.Results 38 aneurysms in 32 patients were detected by DSA and operation.Six cases of 32 had two aneurysms.36 and 34 aneurysms were detected by DSA and CTA respectively.4 aneurysms located at anterior communicating artery,7 at middle cerebral artery,9 at posterior communicating artery,14 at internal carotid artery and 4 at basilar artery.Conclusion MSCTA can be used as the method in screening intracranial aneurysms for surgery and interventional therapy.
2.Isolation and identification of serum exosomes in the patients with polymyositis/dermatomyositis

Objective To isolate and identify exosomes from serum samples of the patients with polymyositis / dermatomyositis (PM/ DM),and analyze their protein composition preliminarily.Methods Exosomes from serum samples of the patients with PM/DM were isolated and purified by the ExoQuickTM kit.The morphological characteristics and particle size of exosomes were determined by transmission electron microscope (TEM) and NanoSight analyzer,respectively.The surface markers of exosomes such as CD9,CD81 and Flotillin-2 were identified by western blot.The concentration and composition of exosome protein were determined by the BCA method and SDS-PAGE,respectively.Results The exosomes from serum samples of PM/DM patients displayed round or oval vesicles with membrane structure under TEM,and their diameter range was about (92 ± 67) nm.western blot showed that these exosomes expressed CD9,CD81 and Flotillin-2.The total protein concentrations of exosomes in the patients with PM/DM and healthy controls were 14.68 (6.00,32.55) μg/μL and 14.09 (8.00,23.28) μg/μL,respectively.SDS-PAGE showed that high-abundance proteins enriched in 55-70 kD in both PM/DM patients and healthy controls,and that there were different bands in 40-55 kD between them.Conclusion Exosomes are isolated from serum samples of the patients with PM/DM successfully,and their protein concentration and composition are analyzed preliminarily,which provides the experimental evidences for further finding differential proteins.

Objective To study the mutation of FUT1 and FUT2 genes in para-Bombay individual.Methods Direct DNA sequencing of FUT1 and FUT2 gene coding region were analyzed in two individuals with para-Bombay phenotype.Results One individual lost one of the three AG repeats located at nucleotides 547~552 of the FUT1 gene, whereas two of the three T repeats located at nucleotides 880~882 were deleted in the other.Conclusion Two frame-shift mutations of FUT1 gene are responsible for the H antigen deficiency
4.Misdiagnostic reason and treatment of the syndrom of splenic flexure of colon

ObjectiveTo investigate the misdiagnostic reason and treatment of the syndrom of splenic flexure of colon(SSFC). MethodsThe clonical data of 21 patients with SSFC admitted from May 1993 to May 2001 were retrospectively analysed. ResultsThese patients aged from 51 to 88 years old with a median age of 67.8years.Clinical manifestalion was repetitive stomach pain, abdominal distension, constipation, etc. Double contrast radiology of colon demonstrated that too high fixation site of colon of splenic flexure, volvulus of colon of splenic flexure, and displacement of colon usually occurred together with transverse or sigmoid colon redundant.All of them were cured by cololysis of colon of splenic flexure, redundant partial colectomy and managing other companying diseases.Postoperative pathological diagnoses were chronic colitis.Followed up was done for 6 months to 6 years, all of them released from primary symptoms. ConclusionsThe main misdiagnostic reason of SSFC is less understanding of SSFC and did not take double contrast radiology of colon. By way of cololysis of splenic flexure, redundant colon resection and managment other companying abdominal diseases, most patients with SSFC may expect satisfactory treatment effects.
5.Surgical repair of complete atrioventricular septal defect in 86 patients

Objective To investigate the early surgical outcomes of 86 patients with complete atrioventricular septal defect.Methods Between January 2007 and December 2014,consecutive 86 cases received surgical repair in our department.There were 44 male patients,and 42 female patients.Two-patch repair was performed in 69 cases,and modified single-patch repair in 17 cases.The mean age,height,and weight at the time of operation were (32.3 ± 46.5)months with a range from 1 month to 17 years,(82.1 ±27.6) cm with a range from 53 to 165 cm,and (10.8 ± 8.7) kg with a range from 4.1 to 43 kg,respectively.Rastelli A type was found in 67 cases,B type in 15 cases,and C type in 4 cases.Down's syndrome was complicated in 6 cases.Preoperative mild regurgitation of common atrioventricular valve was shown in 32 cases,moderate regurgitation in 38 cases,and moderate to severe regurgitation in 16 cases.Mild pulmonary hypertension was observed in 15 cases,moderate in 54 cases,and severe in 17 cases.Results After operation,all patients were sent into intensive care units (ICU).The mean duration mechanical ventilation,ICU stay,and hospitalization were (30.9 ± 47.7) h with a range from 2.5 to 244 h,(87.7 ± 76.8) h with a range from 14 to 306 h,and (16.4 ±9.2)d with a a range from 6 to 50 d,respectively.We encountered 4 operatively mortalities (4.7%),including 3 in two-patch repair group,and 1 in modified single-patch repair group.The cause of death was mitral regurgitation.Conclusions Modified single-patch and two-patch technique have a satisfied early outcomes.
6.Therapeutic effect of modified Shengjiang Powder acupoint application for hypertension patients with phlegm-heat syndrome

Objective:To evaluate the effect of modified Shengjiang Powder acupoint application on hypertension patients with phlegm-heat syndrome. Methods:A total of 61 patients in Shijiazhuang hospital of traditional chinese medicine (TCM) meeting the inclusion criteria from September 2018 to September 2019 were randomly divided into two groups, the treatment group was 30 cases, the control group was 31 cases. The control group was given Amlodipine Besylate Tablets orally, and the treatment group added modified Shengjiang Powder acupoint application, and both groups were treated for 4 weeks. TCM syndrome scores were performed before and after treatment, and 24 h ambulatory blood pressure were measured with an ambulatory blood pressure monitor, including daytime mean systolic blood pressure (dMSBP), daytime mean diastolic blood pressure (dMDBP), night time mean systolic blood pressure (nMSBP), and night time mean diastolic blood pressure (nMDBP), 24-hour mean systolic blood pressure (24 hMSBP), 24-hour mean diastolic blood pressure (24 hMDSBP) and blood pressure circadian rhythm detection. Results:After treatment, the nMSBP and 24 hMSBP in the treatment group were significantly lower than those control group ( t values were -2.331,-1.511, P values were 0.045, 0.033), the blood pressure circadian rhythm [(8.87 ± 8.33)% vs. (6.94 ± 6.12)%, t=1.034] of the treatment group was significantly higher than those of the control group ( P<0.05); mdizziness, headache, head wraps, vomiting, sputum salivation, upset, mouth pain, insomnia scores of the treatment group after treatment were significantly lower than those in the control group ( P<0.05). Conclusion:The modified Shengjiang Powder acupoint application can reduce the blood pressure level of hypertension patients with phlegm-heat syndrome, especially for the improvement of night blood pressure, and improve the clinical symptoms.
7.Impact of Down syndrome on the surgical treatment of congenital heart defects.

OBJECTIVE:
To evaluate the impact of Down syndrome (DS) on surgical management in patients with congenital heart defects (CHD).
METHODS:
We retrospectively analyzed the clinical data from 35 children with DS and CHD, who underwent cardiac surgery between 2004 and 2009. The data on surgical mortality, complications and follow-up results are emphasized.
RESULTS:
All of the patients underwent primary repair. One child (2.9%) with DS and complete atrioventricular septal defect (CAVSD) died early postoperatively because of pulmonary hypertension. Two patients (5.7%) had low cardiac output syndrome, and 15 (42.9%) suffered pulmonary complications. III degree atrioventricular block (AVB) occurred in 4 patients (11.5%). Thirty children who were followed up 10 months to 6 years [(3.8±1.1) years] are in NYHA class I or II. There were no reoperations or later death.
CONCLUSION
CHD in DS children can be repaired with a low risk of mortality, although a high incidence of severe infections and III degree AVB can result in a complicated postoperative course. The results of mid-term follow up are satisfactory.

8.Allergic diseases, immunoglobulin E, and autoimmune pancreatitis: a retrospective study of 22 patients

Background Autoimmune pancreatitis (ALP) is a chronic inflammatory disease of pancreas.We evaluated the clinical manifestations,imaging,and histological presentations of AlP in Chinese patients,and investigated the roles of immunoglobulin E (IgE) and allergic diseases in the diagnosis and pathogenesis of AIP.Methods The clinical records of 22 patients diagnosed with AlP were reviewed and analyzed.All patients with AlP fulfilled the 2006 revised diagnostic criteria proposed by Japan Pancreas Society or the Korean Criteria for AIP.Results Half (11/22) of AlP patients had allergic diseases.Twenty-one patients had elevated serum IgE levels,and 14 patients had IgE levels more than 3 times that of normal.There were no significant differences between the patients with higher or lower IgE,with or without allergic disease,in clinical features,laboratory tests,diffuse or focal lesions,or the choice of treatment methods; however,more complaints of body weight loss were observed in patients with higher IgE levels.Patients with higher IgE levels and with allergic diseases were more likely to have onset in March,April,May,August,September,or October.IgE levels decreased after therapy,but increased again during recurrence.Increased number of mast cells was found in the pancreatic tissue in AIP.Conclusions IgE maybe a useful marker for monitoring therapeutic response and recurrence of AlP.Allergic processes may play an important role in the pathogenesis of AIP.
9.Survey and analysis of long term care service needs for home disabled elderly people in Beiliu city

Objective To investigate the current situation of long-term care needs of the elderly with home-based disabil-ity in Beiliu City,and provide data support for the development of Internet plus care services in Beiliu City.Methods A ques-tionnaire survey method was used to design and distribute a survey questionnaire titled"Survey Questionnaire on Home Care Serv-ice Needs of Disabled Elderly People",to investigate the nursing service methods and home care needs of disabled elderly people in Beiliu City.Results Disabled elderly people mainly choose home-based nursing services,with a high demand for basic care,comfortable care,rehabilitation care,psychological care,and other aspects.Conclusion Home care is of great significance for improving the quality of life of disabled elderly people,and requires policy support such as medical insurance to promote the sup-ply of nursing services and meet the care needs of disabled elderly people.
10.Effect of tamsulosin hydrochloride sustained-release capsules combined with Huang'e capsules on sexual function indicators in patients with chronic prostatitis

Objective To explore the effect of sustained-release capsules of tamsulosin hydrochloride combined with Huang'e capsules on sexual function indicators in patients with chronic prostatitis.Methods A total of 60 patients with chronic prostatitis who visited the Department of Urology,Huzhou Central Hospital from July 2022 to October 2023 were randomly divided into control group(treated with sustained-release capsules of tamsulosin hydrochloride)and treatment group(treated with tamsulosin hydrochloride sustained-release capsules combined with Huang'e capsules)using a random number table method,30 patients in each group.Compare the symptom scores,urinary indicators,sex hormone levels,erectile function,severity of premature ejaculation,and clinical efficacy between two groups of patients with chronic prostatitis.Results Before treatment,the difference were no statistically significant between two groups(P>0.05)using the National Institute of Health-chronic prostatitis symptom index(NIH-CPSI),urinary indicators,sex hormone levels,international index of erectile function-5(IIEF-5),and Chinese index of sexual function for premature ejaculation-5(CIPE-5);After treatment,compared with control group,the NIH-CPSI score and estradiol level of patients in treatment group decreased,urinary indicators improved,testosterone level,IIEF-5,CIPE-5 score increased,and the differences were statistically significant(P<0.05).The effective rate of treatment group(96.67%)was higher than that of control group(73.33%),and the difference was statistically significant(P<0.05).Conclusion Tamsulosin hydrochloride sustained-release capsules combined with Huang'e capsules can not only significantly alleviate the symptoms of chronic prostatitis in patients,improve urinary function,but also regulate the levels of sex hormones in patients,reduce the severity of erectile dysfunction and premature ejaculation.
