1.Determination of Active Components in Whitening and Freckle-removing Cosmetics by High Performance Liquid Chromatography

Objective To develop a high performance liquid chromatography(HPLC)method for the determination of active components,sodium ascorbic phosphate,?-arbutin,hydroquinone,niacinamide,brillian-280 and phenol,in the whitening and Freckle-removing cosmetics.Methods Whitening and freckle-removing active components were separated on a C8 column(250 mm?4.6 mm,5 ?m)by using triethanolamine solution(11.15g triethanolamine dissolved in 1 L water,pH=7.7 adjusted with 1.75 ml H3PO4)+methanol=70+30 as the mobile phase at a flow rate of 1.2 ml/min,using diode array detector(DAD)with the detection wavelengh of 273 nm and the column temperature of 25℃.Results The coefficient of variation was less than 4.7% and the recovery rates were 87.3%-112.9%.Conclusion The method introduced in this paper is simple,rapid,accurate and is suitable for the analysis of various cosmetic samples.
2.Imaging Diagnosis of Atypical Meningioma

Objective To analyse imaging features of atypical meningioma.Methods There were 17 cases with atypical meningiomas proved by pathology,7 cases were male,10 cases were female,ranged in age from 34~69 years.17 cases all examined with CT,and 10 cases with CT enhanced scanning;12 cases examined with MRI and 8 cases with MR enhanced scanning,5 cases examined with DSA.Results The major imaging features of atypical meningioma included:the borders of tumors were mostly irregular,nonhomogeneous density or signal on plain CT and MRI,nonhomogeneous enhancement on enhanced CT and MRI.Cystic and necrotic changing and the invasion of cranial bone and adjacent structure could be seen.5 cases with dural tail sign on MRI,3 of them,this sign was short,thick and irregular.Heavy staining was showed on DSA.Conclusion Atypical meningiomas are of some clinical and imaging features.
3.Predicting hemorrhagic transformation by microvascular permeability using perfusion CT in acute cerebral infarction in elderly patients

Objective To study the possibility of microvascular permeability (PS) value derived from perfusion CT (PCT) in predicting hemorrhagic transformation (HT) in acute cerebral infarction in elderly patients.Methods 52 consecutive patients with middle cerebral artery acute ischemic stroke who received thrombolytic therapy were divided into HT group and control group,and patients in HT group were further divided into hemorrhagic infarction (HI) group and parenchymal hematoma (PH) group.PCT data and Alberta stroke program early CT scores (ASPECTS) from CT angiography source images (CTA-SI) achieved within 6 hours after symptom onset of each group was retrospectively analyzed and statistically processed.Results In 52 patients,there were 22 cases developed HT,14 cases with HI,8 cases with PH.Cerebral blood flow (CBF) and cerebral blood volume (CBV) values were decreased and permeability surface area product (PS) was increased in ipsilateral in HT group and control group as compared with the contralateral (all P<0.05).PS value in ipsilateral was significantly higher in HT group than in control group (P<0.01),but CBF and CBV values in affected side showed no significant difference between the HT group and control group.The area under the receiver operator characteristic (ROC) curve of PS Az value was 0.968.When PS ≥5.77 ml · min-1 · 100 g-1,the sensitivity and specificity of predicting HT were 95.5% and 86.7% respectively.The ASPECTS was lower in HT group than in the control group (P<0.01),and ASPECTS was higher in HI group than in PH group (P<0.01).The differences in PCT results in affected side between the HI group and PH group was not significant.Conclusions The significantly increased PS value can be a reference in predicting HT risk and guiding thrombolytic therapy.
4.Preliminary studies on establishment of indicator system to evaluate basic capacities of trainers of training for community general practitioners in Shanghai

Objective To explore establishment of indicator system to evaluate basic capacities of trainers of training for community general practitioners (GPs) to standardize their capacity building and management.Methods Questionnaire survey was conducted among 190 trainers of training for community health managers and GPs who were involved in community health-care and health-care service management in 12 districts of Shanghai to investigate what basic capacities the trainers should have.Delphi expert consultation method was used to construct an evaluation indicator system.Firstly, an evaluation form was formulated based on the preceding questionnaire.Then, two rounds of expert consultation were carried out with 28 experts specialized in general practice, training for GPs, public health and health management.The last step was to set up an evaluation indicator system based on experts' opinions with statistical analysis.Results The newly-established system consisted of five first-grade indicators including comprehensive performance, capacities and personal willingness, background of education and career, personal potential for professional development, capacity in general practice and capacity in training for GPs, and 57 second-grade indicators.Activity coefficient of the experts was all 100%, authority coefficients of the experts were 0.85,and harmonious coefficient of importance of the evaluation was 0.190 and 0.225 ( all P < 0.01 ),respectively, in two rounds of Delphi consultation.Conclusions The preliminarily established indicator system to evaluate capacities of trainers of training for community GPs in diverse aspects can be used as reference for quality critria in selection of trainers of training for community GPs.
5.Legacy posterior stabilized prosthesis for valgus knee deformity:midterm follow up

BACKGROUND:Legacy posterior stabilized prosthesis has advantages in theoretic design,in vitro kinematics and abrasion, but it remains unclear whether its clinical outcomes for vaglus knee deformity are better than Legacy constrained condylar knee prosthesis. OBJECTIVE:To explore the midterm folow-up effect of Legacy posterior stabilized prosthesis in total knee arthroplasty in patients with valgus knee deformity. METHODS: From February 2006 to November 2013 in Department of Bone and Joint Surgery, Affiliated Hospital of Luzhou Medical Colege, total knee arthroplasty was used to treat valgus knee deformity in 33 cases (35 knees). Parapatelar medial approach was used. Precise osteotomy was utilized to correct limb alignment. Lateral soft tissue received selective release. Finaly, an equal type of Legacy posterior stabilized prosthesis (Zimmer, USA) was implanted and fixed with antibiotic bone cement. The differences in hospital for special surgery knee score, range of motion of knee, femoral tibial angle, and maximum flexion and extension angle, as wel as X-ray film results were compared and analyzed before and after replacement. The complications including deep vein thrombosis, peri-prosthetic infection, patelar clicking, unstable knee, and common peroneal nerve injury were recorded after surgery. RESULTS AND CONCLUSION: Al patients were folowed up for 24-50 months. The incision was healed in one-stage. No complications such as peri-prosthetic infection appeared. Three patients suffered from common peroneal nerve palsy, which was cured at half a year after surgery by expectant treatment such as trophic nerve. Two cases suffered from knee instability after replacement, which was improved at 1 month after external fixation with a brace. One case experienced deep vein thrombosis in the lower extremity, which was improved after treatment with low molecular weight heparin anticoagulation. During final folow-up, hospital for special surgery knee score increased from (51.85±4.15) preoperatively to (85.77±2.50) postoperatively (P < 0.01). There were excelent in 20 knees, good in 11 knees, with an excelent and good rate of 89%. X-ray films showed that hindlimb alignment was apparently corrected, prosthetic position was good, no loosening occurred. These data indicate that total knee arthroplasty, using parapatelar medial approach, obtained good correction outcomes through the accurate amputation to correct hindlimb alignment and selective release of the soft tissue with Legacy posterior stabilized prosthesis in the treatment of valgus knee deformity.
6.Preoperative localization of additional pulmonary nodules in suspected lung cancer patients

Objective To study CT-guided localization of additional pulmonary nodules with microcoils prior to video-assisted thoracoscopic surgery (VATS) resection in patients with suspected lung cancer.Methods Eleven patients suspected lung cancer underwent preoperative microcoils localization towards additional small pulmonary nodules.The head of microcoil was pinpointed adjacent to the target nodule while its end tail remained above the visceral pleura.VATS were performed within 24 hours, and comprehensive assessments were conducted according to surgical and pathological outcomes of primary and additional lesions, and suitable surgical processes were followed.Results All 11 localizing pulmonary nodules (4-15 mm in diameter) were successfully removed after VATS, 9 microcoils'' end tails of which were placed above visceral pleural surface.There were no serious complications related with localizing procedure.Other 16 lesions including 11 primary ones were resected.The surgical and pathological outcomes for lung lesions were utterly assessed.Conclusion Microcoil preoperative localization provides helpful orientation for complete resection and assessment of multiple pulmonary lesions in patients with suspected lung cancer.
7. Study of CT perfusion in posterior circulation cerebral ischemia and hyperacute phase of cerebral infarction

Objective:
To evaluate the diagnostic value of CT perfusion (CTP) for posterior circulation cerebral ischemia and hyperacute phase of cerebral infarction.
Methods:
CTP was performed in 184 patients with suspected posterior circulation acute ischemic stroke, and diffusion weighted imaging (DWI) of MRI was performed 24-72 hours after onset. According to the characteristics of various perfusion parameters, the perfusion defect area in CTP was divided into group Ⅰ (compensatory phase of cerebral circulation reserve), group Ⅱ (compensatory phase of cerebral metabolism reserve), group Ⅲ (hyperacute phase of cerebral infarction). The region of interest (ROI) in each perfusion defect area and the contralateral mirror perfusion normal area was delineated, and the mean values of regional cerebral blood flow (rCBF), regional cerebral blood volume (rCBV), mean transit time (MTT), and time to peak (TTP) in the ROI were recorded. The perfusion parameters of normal brain tissue were included in group Ⅳ (normal control group). One-way analysis of variance was used to compare the overall differences in CTP parameters measured in each group in each region, and the multiple comparisons were performed to assess statistical differences between the perfusion parameters of groups in all parts of the posterior circulation. The sensitivity, specificity and accuracy of CTP in evaluating the hyperacute phase of cerebral infarction in various parts of the posterior circulation were calculated by using DWI as a standard.
Results:
A total of 271 cerebral ischemia or cerebral infarction lesions were detected in 184 patients, 107 in group Ⅰ, 75 in group Ⅱ, and 89 in group Ⅲ. There were statistically significant differences in the perfusion parameters of each group and each part of the posterior circulation (
8.Nicotine dependence among rural-to-urban migrant workers in Shanghai

Objective To explore the nicotine dependence levels and the influencing factors of dependence among rural-to-urban migrant workers in Shanghai so as to provide scientific rationales for an effective implementation of tobacco control.Methods Multi-stage randomized sampling was used to select a total of 5 856 rural-to-urban migrant workers from 7 districts in Shanghai.Fagerstrom Test for Nicotine Dependence ( FTND) and logistic regression model were used to analyze the association between nicotine dependence and relevant risk factors.Results The current smoking prevalence was 23.7%.The average FTND score for current smokers , high dependence and low dependence were 3.38 ±2.49, 7.00 ±1.07 and 2.23 ±1.71 respectively.The high and low dependence rates were 21.7% and 78.3%.The results of logistic regression showed that education , duration of smoking and length of migration were associated with nicotine dependence.Lower level of education was more prone to high nicotine dependence.Those with a lower education had a higher risk of nicotine dependence ( junior high school: OR=1.742, primary school or lower: OR =1.994 vs.senior high school or higher ).High nicotine dependence increased with the duration of smoking (smoking 6-10 years:OR=3.007, 11-15 years:OR=4.076, 16-20 years:OR=5.451, 20 years or more:OR=5.726 vs.5 years or less).Length of migration over 3 years was less likely to have a high nicotine dependence (3-5 years: OR=0.602, more than 5 years: OR=0.407 vs.<1 year) .Conclusion The rate of high nicotine dependence is high among rural-to-urban migrant workers in Shanghai.Level of education , duration of smoking and length of migration are significant influencing factors of nicotine dependence.
9.Survey on mental health status of rural-to-urban migrant workers in Shanghai

Objective To survey the mental health status of rural-to-urban migrant workers in Shanghai.Methods The survey subjects were selected from migrant workers in Shanghai with a multi-stage stratified cluster sampling method.The Symptom Check List ( SCL-90 ) scores were used for evaluation of metal health status.Results The prevalence of mental health problem of 5 626 rural-to-urban migrant workers in Shanghai was 18.8% ( 1 058/5 626 ).The mean total score of SCL-90 was 114.86 ±31.21.Compulsive-obsessive, interpersonal sensitivity, hostility ranked the top three among 9 factors, with scores of 1.37 ±0.43, 1.31 ±0.43 and 1.30 ±0.41 respectively.The mean total scores and scores of 9 factors of rural-to-urban migrant workers in Shanghai were lower than those of Chinese adults norm the mean total scores(129.96 ±38.76) ( t =-15.34, -3.84 --24.08 respectively, all P <0.01) .There were significant differences in mental health among migrant workers with different age , education , marital status , living conditions and occupation (all P<0.05).Conclusion The prevalence of mental health problems in Shanghai rural-to-urban migrant workers is high, with are related to their age, education, marital status, living conditions and occupation.
10.Effect of Photodynamic Therapy for Malignant Obstruction of Biliary Tract

Objective To investigate the safety and efficacy of photodynamic therapy (PDT) for malignant obstruction of the biliary tract. Methods We retrospectively analyzed the clinical data of patients with malignant biliary obstruction treated by PDT in our medical center. On the basis of different treatment plans, the patients were categorized into the photodynamic only group and the combined treatment group, in which additional interventional operations, targeted therapy, or immunotherapy were arranged. The alterations in liver function, duration of biliary patency, and postoperative complications that occurred within one month were closely monitored in both groups. Results A total number of 19 patients were enrolled in this study. The technical success rate of PDT was 100%. The deterioration of liver function was not observed in any patients within one month after PDT. Within a maximum of 17.7 months follow-up, the patency rates of the biliary tract were 100.0%, 89.5%, 72%, and 64% at 1, 3, 6, and 12 months after the procedure, respectively.
