Forensic entomology applies knowledge about insects associated with decedent in crime scene
investigation. It is possible to calculate a minimum postmortem interval (PMI) by determining the age
and species of the oldest blow fly larvae feeding on decedent. This study was conducted in Malaysia
to identify maggot specimens collected during crime scene investigations. The usefulness of the
molecular and morphological approach in species identifications was evaluated in 10 morphologically
identifi ed blow fl y larvae sampled from 10 different crime scenes in Malaysia. The molecular
identifi cation method involved the sequencing of a total length of 2.2 kilo base pairs encompassing
the ‘barcode’ fragments of the mitochondrial cytochrome oxidase I (COI), cytochrome oxidase II
(COII) and t-RNA leucine genes. Phylogenetic analyses confi rmed the presence of Chrysomya megacephala, Chrysomya rufifacies and Chrysomya nigripes. In addition, one unidentified blow fly species was found based on phylogenetic tree analysis.

Even though the combined use of ultrasound (US) and alpha-fetoprotein (AFP) is recommended for the surveillance of hepatocellular carcinoma (HCC), the utilization of AFP has its challenges, including accuracy dependent on its cut-off levels, degree of liver necroinflammation, and etiology of liver disease. Though various studies have demonstrated the utility of protein induced by vitamin K absence II (PIVKA-II) in surveillance, treatment monitoring, and predicting recurrence, it is still not recommended as a routine biomarker test. A panel of 17 experts from Asia-Pacific, gathered to discuss and reach a consensus on the clinical usefulness and value of PIVKA-II for the surveillance and treatment monitoring of HCC, based on six predetermined statements. The experts agreed that PIVKA-II was valuable in the detection of HCC in AFP-negative patients, and could potentially benefit detection of early HCC in combination with AFP. PIVKA-II is clinically useful for monitoring curative and intra-arterial locoregional treatments, outcomes, and recurrence, and could potentially predict microvascular invasion risk and facilitate patient selection for liver transplant. However, combining PIVKA-II with US and AFP for HCC surveillance, including small HCC, still requires more evidence, whilst its role in detecting AFP-negative HCC will potentially increase as more patients are treated for hepatitis-related HCC. PIVKA-II in combination with AFP and US has a clinical role in the Asia-Pacific region for surveillance. However, implementation of PIVKA-II in the region will have some challenges, such as requiring standardization of cut-off values, its cost-effectiveness and improving awareness among healthcare providers.

Objective To determine independent factors correlated with clinical effects of DK crush and classical crush technique with drug-eluting stents on bifurcation lesions.Methods 311 patients with bifurcation lesions were randomized to classical(C,n=156)or double kissing(DK)crush(n=155)stent implantation group.The primary endpoints included major adverse cardiac events(MACE).Results Final kissing balloon inflation(FKBI)success rate was 76%in C and 100%in DK groups(P<0.001).Dkcrush procedure was characterized by lower unsatisfactory FKBI rate(27.6%VS.6.3%,P<0.01).Clinical follow-up was available in 100%and angiographic follow-up in 82%patients.The overall restenosi srate was 32.3%in C and 20.3%in DK groups(P=0.01).respectively.Cumulative 8-month MACE was 35.9%in without-FKBI and 19.7%in with-FKBI sub-groups,and 11.4%in DK group(P=0.02).The incidence of stent thrombosis was 3.2%in C group (5.1%without VS.1.7%with FKBI)and 1.3%in Dkgroup(P>0.05).The predictive factors of MACE included minimal side branch stent lumen diameter and lack of DK crush technique.Conclusion DK crush technique is an alternative of double stenting techniquesin terms of improvement of restenosis and clinical outcomes.
8.Recommendations to promote breast milk feeding and enhance nutritional care for preterm infants in the Asia-Pacific region: highlights from a roundtable discussion of key opinion leaders

Introduction: Preterm infants are vulnerable to nutritional deficiencies, thus
optimal nutrition is crucial in promoting growth among these infants. However,
socio-cultural complexities and limited resources in the Asia-Pacific demands
a judicious approach in implementing nutritional care that is pragmatic to align
with current evidence-based recommendations. Methods: A roundtable meeting
was held in Jakarta in 2017 for key opinion leaders in neonatology from the Asia-
Pacific to discuss issues when delivering nutritional care in this region and the
unique circumstances encountered. Results: Priority areas discussed include: (i)
breast milk feeding, (ii) donor milk bank/sharing, (iii) human milk fortification, and
(iv) nutrient-enriched breast milk substitutes. Socio-cultural practices impeding
breastfeeding, insufficient maternity leave, the religious issue of milk kinship, and
limited availability of specialty nutritional care products were among the most
challenging factors. Conclusion: The group proposed recommendations to enhance
breastfeeding uptake, accessibility to a complete portfolio of specialty nutritional
care products, and encouraging more active collaborations to engage policy makers
in addressing these contemporary issues.
