3.Antibiotic therapy of pediatric sepsis
Chinese Pediatric Emergency Medicine 2012;19(5):470-472
Sepsis is systemic inflammatory response syndrome associated with infection.Early recognition of severe sepsis or septic shock, early hemodynamic resuscitation and the immediate initiation of effective intravenous anti-infective therapy are the cornerstones in the management of patients with sepsis. The infectious microorganisms associated with sepsis in pediatric patients vary with the patient's age and immune status.Initial empirical antimicrobial therapy should include one or more drugs that have activity against all likely pathogens and that penetrate in adequate concentrations into the presumed source of sepsis.Empirically therapy should be de-escalated to the most appropriate single therapy as soon as the susceptibility agent is known.
4.Discussion on humanistic education among clinical medical postgraduates
Chinese Journal of Medical Education Research 2012;11(9):900-902
The combination of medical humanistic spirit and medical scientific spirit is the sign of the maturity of medicine. The modern medical mode of ' biology-psychology-society-environment' sets new requirements for medical humanistic education.Clinic medical postgraduates, the front -line medical workers, play a very important role in medical work, but they show huge deficiency in interpersonal communication, humanistic knowledge and humanistic possession nowadays. The paper tried to draw some lessons from the successful education mode of George Washington university and discussed practicable methods to improve humanistic education among medical postgraduates aiming at cultivating qualified medical workers who have both noble medical ethics and perfect medical technology and ability.
5.Effects of recombinant human bone morphogenetic protein 2 on integrin beta 1 expression during mandibular distraction
Chinese Journal of Tissue Engineering Research 2009;13(50):9818-9821
BACKGROUND:Integfin levels can affect cellular function, regulate bone formation and bone resorption. However, this process need activated by other factors, whether recombinant human bone morphogenetic protein 2 (rhBMP-2)has this effect is poorly understood.OBJECTIVE:To study the effects of the different concentration of rhBMP-2 on the expression of inergrin β_1 during the mandibular dfScraction in rabbits.DESIGN,TIME AND SETTING:The randomized, controlled.animal grouping experiment of histological observation was performed at the Animal ExperimentaI Center and Immunohistochemistry Center of Liaoning Medical University from May 2006 to May 2007.MATERIALS:A total of 45 Japanese white rabbits were used.rhBMP-2 was supplied by Beijing Academy of Military Medical Sciences.METHODS:The rabbits were prepared for mandibular distraction models and randomly assigned into control,1.5 mg rhBMP-2 and 3.0 mg rhBMP-2 groups.with 15 animals in each group.Collagen sponge and collagen binding proteins were implanted into mandibles of rabbits, respectively, and distraction 0.4 mm per time, twice per day for 5 days. The mandibles were lengthened 4 mm.MAIN OUTCOME MEASURES:The expression of integrin 131 was detected using immunohistochemistry at days 1,3,7,14,and 28 after distraction.RESULTS:All rabbits were included in the final analysis. The integdn β_1 mainly expressed at 7 days after distraction. At the same time point.when concenrtation rhBMP-2 was increased, the positive integrin β_1 rate and positive stained area percentage were increased(P<0.05).Compared to the same concentration, the positive integrin β_1 rate and positive stained area percentage were gradually increased as time prolonged (P<0.05).CONCLUSION:The integdn β_1 expression can be promoted by rhBMP-2 at the anaphase of mandibular distraction osteogenesis in a concentration-dependent manner.
6.Clinical evaluation of superior thyroid artery hemodynamic characteristics by color Doppler ultrasound on 131I treated Graves'hyperthyroidism
Chinese Journal of Ultrasonography 2008;17(9):782-784
Objective To investigate the hemodynamic characteristics of superior thyroid artery and its corresponding clinical significance with patients of Graves'hyperthyroidism. Methods Thirty patients with Graves'hyperthyroidism received Doppler ultrasonography.Patients were grouped with different serologic indices at 4 month after radioiodine therapy as relief(R),uncured(U)and hypothyroidism(H).The relationships between thyroid volume(V),superior thyroid artery peak systolic velocity(PSV),end diastolic velocity(EDV),RI,PI and systolic accelerating time(SAT)and the 4-month outcome were analyzed respectively. Results Besides the effect of 24 h-131I uptake rate on patients'short-term outcome,V,PSV and RI were of predominate prognosis value(P<0.01).PSV and V of group R were positively correlated with 131I dose respectively(r1=0.825,r2=0.697;P<0.001),while SAT of group R was negatively correlated with 131I dose(r=-0.489,P<0.05).Conclusions Patients with high V,PSV and RI maybe still suffering hyperthyroidism after regular dosage of 131I therapy.Doses of 131I should be added appropriately for patients with high PSV and V,but short SAT to achieve complete remission, while it should be reduced for patients in opposite condition to avoid occurrence of hypothyroidism.
7.Clinical significance of lymphocyte subsets changes in the active stage of ulcerative colitis: a retrospective study
Chinese Journal of Clinical Nutrition 2010;18(4):219-223
Objective To observe the change of peripheral blood lymphocytes subsets in patients with active ulcerative colitis (UC) and explore its clinical significance. Methods The clinical data and lymphocyte subsets of 40 active UC patients who admitted to the Department of Gastroenterology, Peking University First Hospital, from June 2007 to January 2010 were retrospectively analyzed. Patients who had previously used immunosuppressants or tumor necrosis factor monoclonal antibody were excluded. Seventy-nine subjects with health examination served as controls. Peripheral blood lymphocyte subsets detected included total T cells, CD4 + T cells, CD8 + T cells, B cells, and NK cells. Only the first detection results of UC patients were used for analysis. Results The proportion of total T cells for UC patients (73.60% ± 8.35% ) was significantly higher than the controls (69. 76% ±7.37%) (P =0.012). CD8+T cell ratio (35.53% ± 10.99%) was significantly higher than the controls ( 30. 56% ± 6. 75 % ) ( P = 0. 011 ). When the UC patients were stratified according to inflammatory involvement, the total T cells, CD8 +T cell, and NK cell ratio were significantly different among pancolitis, non-pancolitis, and the controls ( all P < 0. 05 ). When the UC patients were stratified according to the disease course, the total T cells and CD8 + T cell ratio were significantly different among chronic recurrent/persistent, onset, and the controls ( both P < 0.05 ). When the UC patients were stratified according to the extraintestinal manifestations, the total T cells and CD8 + T cell ratio were significantly different among those with extraintestinal manifestations, those without extraintestinal manifestations, and the controls ( both P < 0.05 ). Conclusions The proportion of total T cells and CD8 + T cells increase in UC patients. Such immune abnormalities are even more distinctive in those UC patients who suffer from pancolitis or chronic recurrent/persistent type or those with extraintestinal manifestations.

Chinese Journal of Tissue Engineering Research 2005;9(15):220-222
BACKGROUND: Disturbance of lipid metabolism is responsible for many kinds of diseases, and to screen safe and effective prescription for lipid reduction has clinical significance.OBJECTIVE: To study the influence of puerarin-arginine composition on lipid metabolism in rats.DESIGN: Comparative observing study based on animal model.SETTING: Experimental animal center and department of biochemistry in a military hospital of Chinese PLA.MATERIALS: This study was carried out at the Experimental Animal Center and Department of Biochemistry of the General Military Hospital of Chinese PLA between May and August 2004. Eighty Wistar rats were randomly divided into high dosage, middle dosage, low dosage group and control group with 10 male and 10 female rats each.METHODS: Rats took food freely, and experimental groups were supplemented with puerarine-arginine composition of different dosage (0, 0.93,1.39 and 1.85 g/kg) for observing the influence on rats.MAIN OUTCOME MEASURES:① blood parameters; ② change of body mass and utilization rate of food; ③ parameters of vital organs;④histopathological changes.RESULTS: From onset of the second week, body mass of both male and female rats in experimental groups were found obviously lower than those in control group. The levels of blood cholesterol in female rats were(1.63 ±0.20) and (1.29 ±0.23) mmol/L in 0.93 g/kg dosage group and control group respectively, and were(1.75 ±0. 13) and(1.31 ±0. 19) mmol/L in male rats, and both have statistical significance( F = 6.43, 6. 58, P < 0. 05).Fatty parameters of the three experimental groups were found obviously lower as comparing with those of control group.CONCLUSION: Such composition has been proved to be able to exert function in lipid reduction by decreasing body mass, and improving utilization rate of food and proportion of cholesterol and organic fat.
10.Applications of ultra-wide-field fluorescein angiography in the diagnosis and treatment of retinal diseases
Chinese Journal of Ocular Fundus Diseases 2017;33(1):90-92
Ultra-wide-field fluorescein angiography (UWFA) is a novel breakthrough in ocular fundus imaging technology, which can capture a single, high-resolution, 200° wide image of the ocular fundus that traditional fluorescein angiography cannot reach. This technology has important impacts on the screening, diagnosis, staging, treatment and follow-up of vascular diseases involving peripheral retina (such as diabetic retinopathy, age-related macular degeneration, retinal vein occlusion, uveitis and so on).
