1.Clinical Observation of Desloratadine Citrate Disodium in the Decremental Treatment of Chronic Urticaria

OBJECTIVE:To observe the clinical efficacy and safety of the effect of desloratadine citrate disodium in the decre-mental treatment of chronic urticaria. METHODS:212 patients with chronic urticaria were randomly divided into decremental treat-ment group and control group. Decremental treatment group was treated with Desloratadine citrate disodium capsules 17.6 mg in the first week,orally,qd;8.8 mg in the second week,orally,qd;8.8 mg in the third week,orally,qod;and 8.8 mg in the fourth week,orally,q3d. Control group was treated with Desloratadine citrate disodium capsules 8.8 mg,orally,qd. The treatment course for both groups was 4 weeks. The clinic data was observed,including clinical efficacy,and serum IgE,clinical symptom scores before and after treatment,recurrence rate and incidence of adverse reactions. RESULTS:The differences were not statistical-ly significant in the total effective rate and incidence of adverse reactions between 2 groups(P>0.05). The recurrence rate in decre-mental treatment was significantly lower than control group(P<0.05). When treatment ended and after 4 weeks of drug withdraw-al,serum IgE levels in 2 groups were significantly lower than before,and decremental treatment group was lower than control group(P<0.01);the IgE level in control group was signiciantly higher after 4 weeks of drug withdrawal(P<0.01)than the end of treatment. After treatment,clinical sympton scores in 2 groups were significantly lower than before(P<0.01),however the ditferenc-es was not significant between 2 groups(P>0.05). CONCLUSIONS:Desloratadine citrate disodium have similar clinical efficacies and safety in the decremental treatment and conventional treatment of chronic urticaria. However,decremental treatment of deslo-ratadine citrate disodium is better than conventional treatment in terms of improving the level of IgE.
2.SF-36 health survey questionnaire used in patients undergoing tibial bone transport

Objective To investigate the effectiveness of the SF-36 health survey questionnaire used to investigate the quality of life (QOL) of the patients undergoing tibial bone transport.Methods Reliability analysis,correlation analysis and validity analysis were performed to evaluate the reliability and validity of the SF-36 scale.According to treatment time,94 patients who underwent tibial bone transport were divided into 3 groups:group A were 30 patients who had just started bone transport,group B 43 patients who had completed bone transport and removal of external fixation within 2 years,and group C 21 patients who had completed bone transport and removal of external fixation beyond 2 years.A mean comparison of items in the SF-36 scale was further carried out.Results The questionnaires were completed well.The Chonbach α indexes of all the items were more than 0.8,showing good reliability of the scale.Most of the correlation coefficients were almost or more than 0.5,indicating acceptable content validity of the scale.The QOL of the patients undergoing tibial bone transport was differentiated from that of normal people,proving good discriminant validity of the scale.Groups B and C obtained significantly better scores in all items of the scale than group A (P < O.05).Group C scored significantly higher in physical functioning (PF) and role-emotional (RE) than group B (P < 0.05),but there were no significant differences between the 2 groups regarding other items of the scale (P > 0.05).Conclusions With good reliability and validity,the SF-36 health survey questionnaire can be used to assess the QOL of the patients undergoing tibial bone transport.The treatment of bone transport in the tibia can significantly and durably help improve the QOL of the patients.
3.Clinical study ofQiju-Dihuang decoction combined with tears naturale on dry eye of liver and kidney

Objective To investigate the clinical effect ofQiju-Dihuang decoction combined with tears naturale(hypromellose 2910,dextran 70 and glycerol eye drops)on dry eye of liver and kidneyYin deficiency patterns.Methods Sixty patients with dry eye in Fangxian People’s Hospital and Fangxian Hospital of TCM from Jan. 2010 to Aug. 2013 were divided into a control group(30 patients)and a treatment group(30 patients) randomly according to registration order. The patients in the control group were treated with tears naturale, while patients in the treatment group were additionally treated withQiju-Dihuang decoction orally on the basis of the control group. Both groups were treated for 8 weeks. After treatment, the clinical symptoms and signs were compared and the changes of breakup time of tear film(BUT), Schirmer test I(S1T)and corneal fluorescein staining(FL)score were measured in both groups.Results The total effective rates in the treatment group and in the control group were 73.33%(22/30)and 46.67%(14/30), respectively. The difference was statistically significant(χ2=4.44,P<0.05). After treatment, the BUT, S1T and FL score of the treatment group and the control group was significantly improved than those before in both groups(t=4.200, 4.374, 9.098, 2.147, 6.627,P<0.05 orP<0.01), and the degrees of BUT, S1T score increased and FL score decreased in the treatment group were superior to those in the control group(t=2.75, 2.43, 3.05,P<0.05 orP<0.01). ConclusionsQiju-Dihuang decoction combined with tears naturale can obviously improve dry eye of liver and kidneyYin deficiency patterns, increase tear secretion, and reduce the damage of canthus and conjunctiva. Its curative effect was superior to simple tears naturale.
4.Research progress on type Ⅱ innate lymphoid cells in respiratory mucosal immunity and lung diseases

Type Ⅱ innate lymphoid cells is an emerging family of innate lymphocytes mainly distributed in mucosal tissues such as the lungs,intestines,skin,etc.These cells are capable of producing Th2-type cytokines including IL-5 and IL-13 in response to IL-25 and IL-33.ILC2s has attracted much attention for its important roles in anti-infection immunity and allergic diseases in respiratory tract.Studies have shown that ILC2s represented a crucial source of Th2-type cytokines and could regulate the adaptive immune response in allergic airway diseases.Meanwhile,ILC2s was able to fulfill vital functions in parasite clearance, anti-virus infection and subsequent tissue repair.Therefore,research in ILC2s phenotype,development and function has great practical significance.This paper reviews the definition, classification, developmental regulation of ILC2s with a particular focus on its role in respiratory mucosal homeostasis and lung-related diseases.
5.Application of three-dimensional reconstruction in pelvic and acetabular fractures

BACKGROUND:Diagnosis of pelvic and acetabular fractures is difficult with a high misdiagnosis rate. It is necessary to diagnose pelvic and acetabular fractures rapidly and accurately. And patients suffer great injury, blood loss, from traditional open pelvis and acetabulum surgery, often complicated by neurovascular injury. Correct positioning and placement of internal fixator can reduce surgical complications.
OBJECTIVE:To explore the new progress of three-dimensional reconstruction of pelvic and acetabular fractures in order to promote its clinical application.
METHODS:A computer-based online search of PubMed database, CNKI database and Vip database between September 1996 and October 2013 was performed to search related articles using the key words of“3D, three dimensional reconstruction, pelvic fracture, acetabular fracture”in English and Chinese, respectively. The duplicate research or Meta analysis articles were eliminated. Final y, 41 articles were enrol ed for further analysis.
RESULTS AND CONCLUSION:Through a three-dimensional reconstruction using a variety of post-processing techniques, such as multi-planar reformation, maximum intensity projection, shaded surface display and volume rendering, we can get high quality images at any angle. Various post-processing techniques interact with each other that can reduce missed diagnosis rate and misdiagnosis of pelvic and acetabular fractures. Moreover, three-dimensional reconstruction techniques can be used to design surgical approach, guide surgeon’s surgical program designed to simulate surgical procedures, reduce the complications caused by the dislocation of internal fixation, and reduce the surgical risk. Three-dimensional reconstruction is also used for postoperative fol ow-up review of fracture recovery.

7.Size changes of erythrocytes infected with Plasmodium malariae in thin pe-ripheral blood smears

Objective To explore the size changes of erythrocytes infected with Plasmodium malariae,so as to improve the basic?level experimenters’microscopy capabilities for P. malariae identification in thin blood smears. Methods The micro?scopic features of erythrocytes infected with P. malariae in thin peripheral blood smears were observed,and a microscope image processing software was used to measure and analyze the diameter changes of the erythrocytes infected with P. malariae. Re?sults The diameter of erythrocytes infected with P. malariae decreased significantly compared with that of the normal erythro?cytes. The three parameters in this study:the diameter of erythrocytes,the value of diameter variation,and the ratio of diameter variation varied at different developmental stages of P. malariae,and the differences were statistically significant(all P<0.01). The variances of the three parameters grouped by different cases or different developmental stages in different cases were ana?lyzed,all showing statistically significant differences(all P<0.01). Conclusion All the developmental stages of P. malariae will cause the decrease of the diameters of infected erythrocytes in peripheral blood smears,but the influence on the diameter of erythrocytes,value of diameter variation,and ratio of diameter variation varies at different developmental stages in different cases.

10.The application of digital tomosynthesis in diagnosis of occult bone fracture

Objective To evaluate the digital tomosynthesis in the diagnosis of occult bone fracture. Methods Both DR and DTS were performed in 135 cases with traumatic-induced pain. All the cases were suspected of bone fractures but with no abnormal findings in DR images. Two radiologists reviewed all the images and diagnosed in consensus. All cases were followed up and the imaging diagnoses were reviewed. Results Bone fractures or joint dislocations of 61 cases were shown in DTS images but not in DR images, including atlanto-axial fracture and subluxation in 7 cases, articular fracture of limbs in 20 cases, bone fractures of wrist, hand and foot in 11 cases, fractures of cervical, thoracic and lumbar vertebra in 7 cases, fracture of sacrococcyx in 8 cases, fracture of nasal bone in 4 cases, fractures of Sternum ,rib ,orbit and mandible in 1 case. Thirty-four cases with bone fractures were determined by using DTS in suspected cases and 17 cases were denied. Both DR and DTS images showed normal findings in 23 cases. Imaging diagnoses of all cases were consistent with the results of follow up. Conclusion DTS can depict bone fractures especially in the deep and complex bony structures, which may improve the diagnostic accuracy of bone fracture.
